Effects of Korean Medical Treatments Combined with Foot Orthotics on Patients' Center of Gravity with Flat Foot : a retrospective study (후향적 연구를 통한 맞춤형 족부보조기와 한방치료를 병행한 편평족 환자의 인체중심선 변화에 대한 연구)

  • Objectives : The purpose of this study is to find out the effect of Korean Medical treatments combined with Orthotics on patients's center of gravity with flat foot. Methods : We reviewed the medical records of 24 patients diagnosed with flat foot. They had received Korean medical treatments combined with Foot Orthotics. To estimate the efficacy in rebalancing of body, we analyzed the changes in center of gravity. Results : Among the 24 patients, 13 were male and 11 were female. There was no statistically significant change in frontal angle at the time of treatment. But side angle was changed statistically significant(P=0.009). There was no difference according to sex of male and female. Conclusion : In this study, We found that Korean Medical treatments with Foot Orthotics was effective for correction of patients's center of gravity with flat foot.

PULPOTOMY IN PRIMARY MOLAR TEETH USING ELECTROSURGERY AND MTA : A RETROSPECTIVE STUDY OF SURVIVAL RATES (유구치에서 electrosurgery와 MTA를 이용한 치수절단술 : 성공률에 대한 후향적 연구)

  • This study investigates the success rate of pulpotomy using electrosurgery operated on a primary molar in caries. The pulpotomy using electrosurgery was done on 253 primary molars of 111 young patients from 2 to 9 years old during the period of the first day of January 2011 to the last day of December 2015. After the amputation of pulp and hemostasis of primary molar were done using electrosurgery, MTA as pulp capping material was applied to the primary molar and the tooth was restored with the stainless steel crown. The follow up period after the treatment ranged from 4-46 months. The clinical and radiographic success rate ranged from 92.1 - 94.3%. Which is comparable to formocresol and ferric sulfate pulpotomy. Due to its non-pharmacological characteristic, electrosurgery can minimize harmful effect on the pulp tissue. Its fast bleeding control makes it easy and safe to use in pediatric and disabled patients in comparatively simple manner. Electrosurgery can be an alternative for pulp therapy considering the side effects of pharmacological ways.

Five Year's Follow-up of the Cardiovascular Disease Risk Factors among the Low Level Organic Solvent Exposure Workers (저농도 유기용제 노출 사업장 근로자의 심혈관질환 위험인자 변화 추적(2008~2012))

  • Purpose: The objective of the present study was to evaluate the effects of occupational exposure to low dose organic solvents on the prevalence of cardiovascular risk factors. Methods: Study design was retrospective cohort study subjected on 191 exposures and 118 controls working in a petrochemical manufacture company. The eight indicators related to CVD risk were followed up for five years from 2008 to 2012. The risk level was compared during the follow up years and subject's characteristics, and the change of risk level were analyzed with repeated measures ANOVA and multiple logistic regression analysis. Results: At the start year 2008, the rate over cutoff value (ROCV) of BS (p<.001) and mean systolic BP (p=.017) were higher in organic solvent exposure group and the others showed no difference. And by the subject's characteristics, odds ratio of the ROCV of BS were higher in organic solvent exposure group and work shift group as 2.51 and 3.07. Comparing the results in 2012 to those of 2008, cardiovascular disease risk in organic solvent exposure group was about 1.5 times higher than that of in the control group. Conclusion: Gradual increase in the CVD risk was identified in organic solvent exposure group. However, the risk might be influenced by shift work and bad behaviors rather than organic solvent exposure.

Comparison of Antifebrile Effects of Hypothermia Blanket and Rectal Antipyretics for Infants after Open Heart Surgery (개심술 후 환아에게 적용한 저온담요와 직장용 해열제의 해열효과 비교)

  • Purpose: The purpose of this study was to compare the effectiveness for infants of antifebrile therapy using a hypothermia blanket or rectal antipyretics following open heart surgery. Methods: This was a retrospective study and 174 infants who had open heart surgery at P University Hospital, and whose body temperature body temperature exceeded $37.2^{\circ}C$ were included in the study. The assessment tool was composed of 32 items was used for assessment of fever therapy, physiological indexes and antifebrile duration. Physiological indexes included systolic blood pressure, diastolic blood pressure, heart rate, pH, $PaCO_2$, $PaO_2$, $HCO_3{^-}$, $SaO_2$, and $K^+$ and the antifebrile duration was minutes from having a fever until BT returned to normal levels. Results: The antifebrile duration with the hypothermia blanket was shorter than with rectal antipyretics. There were significant differences in the physiological indexes with either type of antifebrile therapy, but drop in BT was greater with the hypothermia blanket than rectal antipyretics. Conclusion: The results of this study indicate that a hypothermia blanket is a non-invasive, non-drug and safe antifebrile therapy. Therefore, a hypothermia blanket can be applied to infants with a fever following open heart surgery.

Effects on the Antimicrobial Use of Clinical Decision Support System for Prescribing Antibiotics in a Hospital (항생제 처방 지원 프로그램이 항생제 처방과 사용량에 미치는 효과)

  • Objective: This study was to define the clinical effect on the clinical decision support system (CDSS) for prescribing antibiotics integrated with the order communication system in a National Hospital. Method: We extracted data collected before integrating the CDSS of 4,406 adult patients in 2007 and data collected after integrating the CDSS of 4,278 adult patients in 2009. These patients were 50.4% and 45.2% of all patients admitted in 2007 and 2009, respectively. The clinical effect was defined as the proportion of prescribed antibiotics, the length of antibiotics use, and the DDDs (defined daily doses) of antibiotics per 1,000 patient-days using these retrospective data. Results: There were a significant change in the proportion of patient prescribed penicillins with extended spectrum (OR=0.55, p=001), penicillins included beta-lactamase inhibitors (OR=0.75, p<.001), 3rd cephalosporin (OR=1.47, p<.001). The mean of the length of antibiotics use was decreased statistically from $6.09{\pm}5.48$ to $5.85{\pm}5.51$ days (p=.003). The DDD of glycopeptides was decreased from 24.43 DDD to 19.55 DDD per 1000 patient-days. The DDD of 3rd cephalosporins was also decreased from 15.88 to 11.65. Conclusion: Therefore, the clinical decision support system for prescribing antibiotics was effective for the clinical outcomes.

Effects of the Obesity Therapy with Korean Herbal Medicine on Menstrual Cycle Disorder: Case Series (한방비만치료가 월경주기 이상에 미치는 영향: 증례보고)

  • Obesity is associated with menstrual cycle disorder. The aim of this study was to investigate the clinical effect of the obesity therapy with Korean herbal medicine (KHM) on obese female patient with oligomenorrhea or amenorrhea. This is a retrospective observational study performed on obesity patients with oligomenorrhea or amenorrhea. Nine patients were treated with KHM and lifestyle management at least 2 months. During the treatment, body weight, body mass index (BMI), percent body fat (PBF) and menstrual cycle length were checked afterwards. Body weight, BMI, and PBF of the patients were decreased during treatments. Five of 9 subjects recovered to normal menstrual cycle. Two subjects improved menstrual cycle. The other subjects remained at oligomenorrhea or amenorrhea. This study shows that the obesity therapy including KHM could be effective to improve menstrual cycle on obese patient. Further large scale studies are necessary for more evidences.

The Effects of Nafamostat Mesilate on a Bleeding Risk as an Anticoagulant During Use as a Continuous Renal Replacement Therapy: Systematic Review

  • Objectives: In the past, the pharmaceutical drug heparin was mostly used as the anticoagulant for continuous renal replacement therapy (CRRT), but the duration time is long to have the risk of a bleeding adverse effect, and in that case the drug therapy Nafamostat mesilate was utilized instead, as it is more safe in this case, with a short half-life and is increasing in use to permit lower concerns for bleeding incidents. However, there are insufficient number of large-scale studies on the comparison of Nafamostat mesilate and heparin. Methods: In this study, a systematic review are used to compare the bleeding risk of Nafamostat mesilate and Heparin, as subjected to patients and procedures for measuring risks performed with a CRRT, and the filter life span is to be evaluated as well in this patients. Results: As a result of literature review search, a total of 6 studies were included in systematic review. The reducing risk of bleeding and filter life span was analyzed. The retrospective cohort studies confirm that Nafamostat mesilate is less at risk of bleeding than heparin. And a cohort study confirms that Nafamostat mesilate is longer filter lifespan than heparin and randomized controlled trial studies show that Nafamostat mesilate is longer filter lifespan than not using the anticoagulants. Conclusion: Nafamostat mesilate is considered to be a good therapeutic option because it has a longer filter life span as well as the advantage of reducing bleeding.

Factors Affecting Tibial Tuberosity-Trochlear Groove Distance in Recurrent Patellar Dislocation

  • Background: The tibial tuberosity-trochlear groove (TT-TG) distance is used to determine the necessity of tibial tubercle osteotomy. We conducted this study to determine the extent to which each of the tibial tuberosity lateralization, trochlear groove medialization, and knee rotation angle affects the TT-TG distance in both normal and patella dislocated patients and thereby scrutinize the rationale for tuberosity transfer based on the TT-TG distance. Methods: Retrospective analysis of rotational profile computed tomography was done for patella dislocated and control group patients. Femoral anteversion, tibial torsion, knee rotation angle, tuberosity lateralization, and trochlear groove medialization were assessed in all patients. Relationship of these parameters with the TT-TG distance was investigated to evaluate their effects on the TT-TG distance. Results: We observed that the patellar dislocation group, compared to the control group, had increased TT-TG distance (mean, 19.05 mm vs. 9.02 mm) and greater tuberosity lateralization (mean, 64.1% vs. 60.7%) and tibial external rotation in relation to the femur (mean, $7.9^{\circ}$ vs. $-0.81^{\circ}$). Conclusions: Tuberosity lateralization and knee rotation were factors affecting patellar dislocation. These factors should be considered in addition to the TT-TG distance to determine the need for tibial tubercle osteotomy in patients with patellar dislocation.

Effect of Korean Medicine Treatments for Pain Reduction in Patients with Hip Fracture : A Retrospective Observational Study (고관절을 이루는 골격의 골절 환자의 통증 감소에 대한 한의학적 치료 효과 : 후향적 관찰연구)

  • The aim of this study was to observe the effectiveness of Korean Medicine treatments on pain reduction of patients who were diagnosed hip fracture - Acetabulum and Proximal Femur. We reviewed the 17 patients' records who had admitted to Daejeon Korean Medical hospital and diagnosed fracture of acetabulum or proximal femur bones from January 1, 2011 to April 30, 2020 during the admission retrospectively. Statistical analysis was done using the IBM SPSS statistics 25 program. We measured Numeric Rating Scale to evaluate the treatment's efficacy. Korean Medicine treatments reduced median of Numeric Rating Scale reduced statistically significantly from 6.00 to 3.00(p<0.05). During the admission, patients didn't experience the side effects of Korean Medicine treatments. In conclusion, these results implied that Korean medicine treatments showed a positive effect on pain reduction of hip fracture patients. Further research is required to confirm the effectiveness of Korean Medicine treatments.

Effects of Trauma-Related Shock on Myocardial Function in the Early Period Using Transthoracic Echocardiography

  • Purpose: The present study aimed to analyze the effect of trauma-related shock on myocardial function in the early stages of trauma through transthoracic echocardiography (TTE) findings. Methods: We performed a retrospective review and analysis of the medical records of patients aged ≥18 years who were evaluated by TTE within 2 days of admission for trauma-related shock (n=72). Patients were selected from a group of 739 patients admitted with trauma-related shock between January 2014 and December 2016. Results: The incidence rate of myocardial dysfunction in the left ventricle (LV) was 6.8% (5/72), with rates of 7.7% (4/52) in the thoracic injury group and 5.0% (1/20) in the non-thoracic injury group. In the diastolic function of LV, relaxation abnormality was present in 55.8% (29/52) of patients in the thoracic injury group and 50% (10/20) of patients in the non-thoracic injury group. Conclusions: This study may suggest that traumatic shock without thoracic injury may influence myocardial function in the early stages after trauma. Therefore, evaluation of myocardial function may be needed for patients experiencing shock after trauma, regardless of the presence of thoracic injury.
