A Study on the Measurement of Respiratory Rate Using a Respirator Equipped with an Air Pressure Sensor

  • In order to measure the respiratory rate, one of the major vital signs, many devices have been developed and related studies have been conducted. In particular, as the number of wearers of respirators increases in the COVID-19 pandemic situation, studies have been conducted to measure the respiratory rate of the wearer by attaching an electronic sensor to the respirator, but most of them are cases in which an air flow sensor or a microphone sensor is used. In this study, we design and develop a system that measures the respiratory rate of the wearer using an air pressure sensor in a respirator. Air pressure sensors are inexpensive and consume less power than the other sensors. In addition, since the amount of data required for calculation is small and the algorithm is simple, it is suitable for small-scale and low-power processing devices such as Arduino. We developed an algorithm to measure the respiratory rate of a respirator wearer by analysing air pressure change patterns. In addition, variables that can affect air pressure changes were selected, and experimental scenarios were designed according to the variables. According to the designed scenario, we collected air pressure data while the respirator wearer was breathing. The performance of the developed system was evaluated using the collected data.

  • 기계적인 순환보조장치는 급성호흡부전이나 심부전에서 생명을 구할 수 있는 방편의 하나이다. 현재 여러 가지 형태의 순환 보조장치가 있으며 Twin-Pulse Life Support (T-PLS) system은 박동형 펌퍼의 한 종류다. 3예의 심한 심폐기능부전 상태 환자에게 순환보조장치로 T-PLS system을 사용하였다. 적응증은 인공호흡기와 강심제를 최대한 사용함에도 불구하고 지속되는 호흡부전이나 심부전이었다. 감염에 의한 호흡부전환자가 2예, 심근염에 의한 심부전 환자가 1예였다. 각각 한 명의 호흡부전환자와 신부전 환자가 생존하였는데 보조장치의 사용시간은 각각 3일과 5일이었다. T-PLS system은 순환보조장치로 유용하게 사용할 수 있으며 다발성 장기부전이 오기 전에 적용하여야 할 것으로 생각된다.

호흡동조 방사선치료를 위한 호흡 움직임 신호 측정 (Measurement of Respiratory Motion Signals for Respiratory Gating Radiation Therapy)

  • 호흡 움직임은 종양에 대한 방사선치료의 부정확한 방사선조사가 유도되도록 복부 및 흉부에서 움직임을 야기한다. 그러므로 치료시 이러한 움직임에 대한 정확한 계산은 정상조직에는 저 선량을 조사되도록 CTV의 마진을 줄일 수 있고 방사선치료에 발생되는 부작용을 줄일 수 있다. Intrafraction motion을 고려하는 기술로는 호흡 멈춤, 호흡동조, 4차원 또는 종양추적 기술이 있다. 호흡동조 방법은 환자의 호흡 신호가 호흡주기의 일정한 범위에 위치할 때 주기적으로 빔을 조사하고 그 범위를 벗어날 때는 빔을 조사하지 않는 방법이다. 이러한 기술은 내부장기 움직임과 상관관계가 있는 호흡 움직임 신호의 획득이 요구된다. 호흡동조 방사선치료를 위한 예비연구로 본 연구자들은 호흡 움직임 신호 측정을 수행하였다. 환자의 호흡 움직임 신호 측정을 위해서, 호흡측정시스템을 3 가지 센서, 4 채널 데이터 획득 시스템, 신호처리용 컴퓨터로 구성하였다. 2명의 환자를 대상으로 본 연구자들은 호흡측정시스템을 가지고 호흡주기 및 파형을 평가하였다. 그 결과 호흡주기는 시간의 함수에 따라 일반적으로 정확한 대칭 형태는 아니지만 주기적인 형태로 측정되었다. 호흡측정 시스템은 기대했던 만큼 환자의 호흡을 잘 추적하였으며 실험에 적용하기 위해 쉽게 컨트롤 할 수 있었다.

Feasibility Study of Robotics-based Patient Immobilization Device for Real-time Motion Compensation

  • Intrafractional motion of patients, such as respiratory motion during radiation treatment, is an important issue in image-guided radiotherapy. The accuracy of the radiation treatment decreases as the motion range increases. We developed a control system for a robotic patient immobilization system that enables to reduce the range of tumor motion by compensating the tumor motion. Fusion technology, combining robotics and mechatronics, was developed and applied in this study. First, a small-sized prototype was established for use with an industrial miniature robot. The patient immobilization system consisted of an optical tracking system, a robotic couch, a robot controller, and a control program for managing the system components. A multi speed and position control mechanism with three degrees of freedom was designed. The parameters for operating the control system, such as the coordinate transformation parameters and calibration parameters, were measured and evaluated for a prototype device. After developing the control system using the prototype device, a feasibility test on a full-scale patient immobilization system was performed, using a large industrial robot and couch. The performances of both the prototype device and the realistic device were evaluated using a respiratory motion phantom, for several patterns of respiratory motion. For all patterns of motion, the root mean squared error of the corresponding detected motion trajectories were reduced by more than 40%. The proposed system improves the accuracy of the radiation dose delivered to the target and reduces the unwanted irradiation of normal tissue.

협동동물병원의 유우질병에 대한 연간조사분석 (Analysis of Dairy Cow Diseases at Hyup-dong Animal Hospital)

  • The number of dairy cows diagnosed and treated at Hyup-dong Animal Hospital from July 1983 to June 1984 was amounted to 3,073 heads of Holstein breed and the result of analysis of diseases was as follows : 1. In the topographic diagnostic categories, 41.23% of all cases involved the urogenital system and were observed most prominent. Next were digestive system (22.10%), general and regional (14.55 %), respiratory system (8.20%) and musculoskeletal system (7.06%) in decreasing order. 2. In urogenital system 50.20% of all diagnosis involved metritis and this were most prominent disease. Next were repeat breeder(15.31%), retained placenta (12.79%) and ovary disfunction(9.40 %) in decreasing order. 3. In digestive system 36.82% of all diagnosis involved enteritis and were most prominent. Next were abomasal displacement (29.31%), lumen indigestion (20.32%) and lumen bloat (9.57%) in decreasing order 4. In general and regional diseases, 52.80% of all diagnosis involved ketosis and were most prominent. Next were milk fever (25.50%), downer cow (5.82%) and omphalitis (4.47%) in decreasing order. 5. In respiratory system, 82.54% of all diagnosis involved upper respiratory tract infections and were most prominent. Next were pneumonia (15.87%). 6. In musculoskeletal system, 49.31% of all diagnosis involved foot rot and were most prominent. 7. In milk secreting system, 61.64% of all diagnosis involved mastitis and ere most prominent. Next were stenosis of teat canal (16.35%). 8. Among the individual disease entities, endometritis marked 20.7%, enteritis 8.13%, ketosis 7.68% upper respiratory tract infection 6.77%, repeat bheeder 6.31%, retained placenta 5.27%, abomasal displacement 4.69%, and lumen impaction 4.49%, respectively of total 3,073 cases in decreasing order.

평진건비탕(平陳健脾湯)으로 호전된 만성기침 2예(例) (Two cases of chronic cough treated with Pyeongjingeonbi-tang)

  • Cough is an important defensive reflex of the respiratory tract needed to clear and protect the upper airways, but it may become exaggerated and interfere quality of life. Gastroesophageal reflux disease (GERD) is one of the three most common causes of chronic unexplained cough. Diagnosing GERD related cough is challenging since many patients do not have esophagitis or an increased esophageal acid exposure during 24 h esophageal pH-metry. We treated two patients who had chronic cough, these subjects were diagnosed as Sikjeoksu (Shijisou, 食積嗽) and was administered Pyeongjingeonbi-tang (Pingchenjianpi-tang, 平陳健脾湯). First case had stuffiness below the heart and acid regurgitation with cough, but second case only had cough. After treatment, cough and other symptoms were improved. It showed that we must consider GERD on chronic cough patients.

금은화에 대한 최근 10년의 연구 동향 (Research trends of Lonicera japonica over the last 10 years)

  • Objective : The purpose of this report was to provide the information about Lonicera japonica published in Korea during lately 10 years. Methods : Domestic papers related to Lonicera japonica were reviewed and analyzed. These papers were then classified by year, experimental method and subject. Results : The following results were obtained in this study. 1. Thirty nine papers were searched. The studies were completely focused on experimental models. 2. The studies of Lonicera japonica continuously increased during 10 years. 3. The subjects of studies were anti-inflammation, toxicology & liver injury, components analysis & taxology, respiratory disease, dermatology, anti-viral & anti-biotic, anti-oxidant, anti-cancer etc. Conclusion : Lonicera japonica is being used in various ways. However, mechanism study should be conducted at the molecular biology level and more clinical studies on the efficacy of Lonicera japonica are needed.

A Study on the Design of Real-Time Monitoring System Using IoT Sensor in Respirator

  • A lot of research has been conducted on a system that collects and observes patients' health information in real time using Internet of Things (IoT) technology, and cares for and supports patients based on this. However, most studies have focused on underlying diseases such as diabetes or cardiovascular disease, and research on IoT systems to cope with respiratory infectious diseases such as COVID-19 is still insufficient. In a COVID-19 situation, the purpose of using an IoT respirator may vary depending on the user. In this paper, we design a system that can adequately cope with respiratory infectious diseases such as COVID-19 by applying IoT technology to respiratory protection. We categorize IoT respirator wearers into patients, medical staff, and self-quarantine persons, and define the purpose and use case of the IoT respirator system according to each classification. The proposed IoT respirator system was designed to achieve each purpose. We developed a prototype system consisting of a smart sensor, a communication module, and a non-motorized hooded respirator to show that the proposed IoT respirator system works.

삼복고의 상기도감염 예방 효과에 대한 후향적 관찰연구 (A Retrospective Review of the Preventive Effect of Sambok-go on Upper Respiratory Tract infection)

  • Objectives: The aim of this study was to investigate the preventive effects of Sambok-go for upper respiratory tract infection. Methods: Sambok-go was performed to outpatients which had visited Daejeon university oriental medicine hospital. The number of participants who received Sambok-go at least once were 85, 21 patients did not receive Sambok-go three times were excluded first. Participants who received treatment three times were 64 patients. After the 2011 winter, 2012.05 we follow-up by telephone, except eight, 56 patients were analyzed. Sambok-go is composed of 8 kinds of medicinal herbs. Each Sambok-go weighs about 1.1g and attatchment surface diameter is about 1.5 cm. Sambok-go was attached to both side of 폐유(肺兪)(BL13), 심유(心兪)(BL15), 폐유(膈兪)(BL17), patients under the age of 15 were attached for an hour, patients over 15 years age were attatched for 3 hours. Results: Among the finally analyzed 56 participants, we compared the number of flu outbreak during winter before and after treatment. Before treatment measured $6.14{\pm}6.57$, after treatment measured $1.57{\pm}1.14$. In conclusion, the number of flu outbreak was significantly decreased(p<0.001). To investigate the effect of worsening factors on the pre-existing upper respiratory diseases, we checked whether the patients had pre-existing upper respiratory diseases. The patients who already had old upper respiratory diseases were 34. The increase in the frequency of deterioration in 5 patients, 4 patients no change, decreased in 24 cases, one patient was unresponsive. After evaluating the satisfaction of The patients who were treated with the Sambok-Go, the 29 cases were very satisfied, the 23 were satisfied, None of the paitents showed dissatisfaction or bad dissatisfaction. Conclusion: Sambok-go seems to be effective in the prevention of URI. Further studies are recommended to confirm this effect.

방수 스프레이 흡입 노출로 인한 급성 호흡기 중독 사례 및 원인 고찰 (Comprehensive Review of Acute Respiratory Failure Following Inhalation Exposure to Waterproofing Agents)

  • Background: In Korea, a healthy 36-year-old man developed acute interstitial pneumonitis soon after inhaling a waterproofing spray which he had applied at home to his outdoor jacket. Objectives: The objectives of this study were to review cases of varying degrees of respiratory toxicity and poisoning in connection with the use of waterproofing spray and summarize major reasons for cases of poisoning. Methods: We searched articles reporting on a combination of a waterproofing agent and/or respiratory symptoms, including acute respiratory syndrome, lung injury, pneumonia, pulmonary toxicity, and respiratory disease. Results: We reviewed a number of cases of varying degrees of respiratory toxicity and poisoning resulting from inhalation of waterproofing spray containing fluorocarbon co-polymer, solvents and propellants reported in a variety of countries. The literature searches concluded that among the ingredients of waterproofing agents, fluorinated polymer may cause acute respiratory health effects. Conclusion: Environmental policy should be implemented in order to prevent consumers from using household and industrial products including waterproofing agents. In addition, a national surveillance system should be created to collect cases of poisoning caused by the use of consumer products.
