A Case of Giant Primary Liposarcoma of the Mediastinum (종격동 거대 원발성 지방육종 1예)

  • Primary mediastinal liposarcoma is very rare, and only a few cases have ever been reported. A 25 year-old man who had coughing and mild fever for a month is reported. The simple delete X-ray and computed tomography of the chest revealed a huge mass in the right thoracic cavity that originated in the anterior mediastinum. Cytologic examination of the needle aspiration biopsy confirmed the diagnosis of a primary mediastinal liposarcoma. Through an exploratory right thoracotomy and a median sternotomy, the huge mass, lobulated and weighing about 2,500 gm delete, was extirpated. Mass was attached to a portion of the pericardium and diaphragm, but there was no invasion or metastasis to the pleura and lung. The patient was discharged 12 days after the operation. He is continuously receiving radiotherapy.

Non-infected and Infected Bronchogenic Cyst: The Correlation of Image Findings with Cyst Content

  • We hereby report a case on bronchogenic cyst which is initially non-infected, then becomes infected after bronchoscopic ultrasound (US)-guided transesophageal fine-needle aspiration (FNA). The non-infected bronchogenic cyst appears to be filled with relatively echogenic materials on US, and the aspirate is a whitish jelly-like fluid. Upon contrast-enhanced MRI of the infected bronchogenic cyst, a T1-weighted image shows low signal intensity and a T2-weighted image shows high signal intensity, with no enhancements of the cyst contents, but enhancements of the thickened cystic wall. The patient then undergo video-assisted thoracic surgery 14 days after the FNA. The cystic mass is known to be completely removed, and the aspirate is yellowish and purulent. To understand the image findings that pertain to the gross appearance of the cyst contents will help to diagnose bronchogenic cysts in the future.

A study about the relation between elev ated transaminase lev el and severity of viral lower respiratory tract infection in children (바이러스성 하기도 감염 환자에서 간기능 이상과 중증도와의 연관성에 관한 연구)

  • Purpose : Viral respiratory tract infection is most common cause for admission to hospital in children. There are many cases with elevated transaminase level in patients with viral lower respiratory tract infection (LRTI). The aim of this study was to compare indexes of disease severity such as duration of assisted ventilation, length of hospital stay and Respiratory Distress Assessment Instrument (RDAI) score in children with viral LRTI with and without elevated transaminase levels and to determine the etiology related to elevated transaminase levels in this patients group. Methods : Virological analysis was done from respiratory specimens obtained from patients with LRTI admitted to Kangnam Sacred Heart Hospital from Jan. 2003 to Jun. 2005. Viral diagnosis was made by isolation of viruses employing HEp-2 cell culture from nasopharyngeal aspiration. Medical records of children were reviewed retrospectively. We compared age, sex, RDAI score, Respiratory Rate (RR) score and mean duration of hospital stay between patients with elevated transaminase levels (Patient Group) and patients with normal transaminase levels (Control Group). Results : Viruses were isolated from 181 children with LRTI. 16 cases were excluded according to criteria. 28 cases (17.0%) had elevated transaminase levels (Patient group) and 137 cases (83.0%) had normal transaminase levels (Control group). There were no significant difference in duration of fever, RR score, RDAI score, incidence of $O_2$ inhalation and duration of hospital stay between patient group and control group. We found 17 (60.7%) cases of RSV, 4 cases (14.3%) of parainfluenza, 4 cases (14.3%) of influenza B virus, 3 cases (10.7%) of adenovirus and 1 case (3.6%) of influenza A virus infection in patient group and 78 cases (56.9%) of RSV, 28 cases (20.4%) of parainfluenza virus, 13 cases (9.5%) of influenza A virus, 9 cases (6.6%) of influenza B virus, 6 cases (4.4%) of adenovirus and 3 cases (2.2%) of coxsackie virus infection in control group. Conclusion : There were 28 cases (17.0%) with elevated transaminase level among patients with virus isolated LRTI. There was no relation between elevated transaminase level and severity of disease. The viral etiologies in two groups were not significantly different. There was no significant difference of age distribution between two groups.

  • Background and Objectives: Most of tracheal stenosis is resulted from longstanding endotracheal tube insertion. Treatments of tracheal stenosis are divided conservative and reconstructive treatment. The propose of this study was to evaluate the effect of prosthetic tracheal T-tube insertion on tracheal stenostic patients who can not be operated invasive surgery. Subjects and Method : Nine prosthetic tracheal T-tube insertion were studied from 9 patients from January 2002 to April 2007. The effect of silastic T-tube was analyzed according to the factors that were respiratory difficulty, oxygen saturation, phonation, aspiration and significant complications. Results: Four patients were good for respiration and no complication. But five patients occur various complications. A successful group did not have cartilagenous lesions but failed group had catilagenous lesions, infection and necrosis. Conclusion: A silastic T-tube insertion was good for palliative treatment in patients without catilagenous lesions.

Clinical evaluation of primary lung cancer: analysis of 138 cases (폐암의 임상적 고찰)

  • As of today, the frequency of primary lung cancer is one of the improved problems in modern medicine and is increasing rapidly year by year. This study dealed with 138 cases of primary lung cancer proved by histopathologic examination in Thoracic & Card iovascular Surgery Dept. of N.M.C. from Sept. 1966 through June 1981. The majority of patients belong to 5th and 6th decade. Ratio between male and female was 3.7: 1. Initial symptoms were cough, blood tinged sputum, hemoptysis, chest pain, dyspnea and duration of symptoms before admission was within 6 months [60%] and 12 months [78%]. Histopathologically, 64 cases [61%] of them were the squamous cell carcinoma, 21 cases [15%] were the adenocarcinoma, 20 cases [14.8%] were the anaplastic carcinoma. Fifty-six cases were resectable: 43 cases were subjected to pneumonectomy and 13 cases were to lobectomy. The remaining 82 cases were nonresectable, but exploratory thoracotomy was performed in 22 cases of them. Among 56 resected cases, 33 cases were radically operated and 17 cases were subjected to palliative operation and 6 cases were subjected to extended operation. [Concomitant pericardium resection in 1 case and concomitant chest wall resection in 5 cases]. Surgical mortality was 10.7% and causes of death were aspiration of contralateral lung, respiratory insufficiency, postop. empyema with B.P.F, cardiogenic failure. This study analysed the cancer stage between preop clinical T.N.M. stage and postop. T.N.M. stage in 78 cases; resectable 56 cases and non-resectable 22 cases.

Open Lung Biopsy Procedure for Diffuse Infiltrative Lung Disease -Collective Review of 50 Cases- (미만성 폐침윤 질환에서 개흉폐생검)

  • Open lung biopsy still has important roles for the marking of diagnosis of diffuse infiltrative lung disease even though transbronchial bronchoscopic lung biopsy and percutaneous needle aspiration biopsy gain popularity nowadays. This is clinical retrospective review of the 56 patients with diffuse infiltrative lung disease undergoing open lung biopsy by minithoracotomy from 1984 to Dec. 1992 in the Department of Thoracic & Cardiovascular Surgery of Catholic University Medical College. 27 men and 29 women, aged 17 to 73 year [mean 49 year , were enrolled & divided into 2 groups;Group A consisted of patients with immunocompromised state [n=19 , Group B patients with non-immunocompromised state[n=38 . Pathologic diagnosis was made in 54 cases[96.4% of these two groups and as follows: infectious; 12 patients[21.4% , Neoplastic; 10 patients[17.9% , granulomatous; 4 patients[7.1% , interstitial pneumonia; 12 patients[21.4% , Pulmonary fibrosis; 8 patients[14.3% , others; 3 patients[5.4% , nonspecific; 5 patients[8.9% , and undetermined; 2 patients[3.6% . Therapeutic plans were changed in 39 patients[69.6% after taking of tissue diagnosis by open lung biopsy. Group B has higher incidence of infectious diseases and change of therapeutic plan than the Group A. The postoperative complications developed in 8 cases[14.3% ,and there is no difference of incidence between the 2 groups. 4 patients belongs to group A, died of respiratory distress syndrome [2 and sepsis [2 which were not related with open lung biopsy procedure. In conclusion, open lung biopsy is a reliable method to obtain a diagnosis in diffuse pulmonary infiltrates and can be performed safely, even in acutely ill, immunosuppressed patients.

  • Epidural emphysema and pneumoscrotum with subcutaneous emphysema are rare in a child past the neonatal period. Their most common causes are bronchial asthma and respiratory infection. Here, we report an 18-month-old boy who was presented with severe air leak, consisting of epidural emphysema, pneumoscrotum, subcutaneous emphysema, and pneumomediastinum, complicated by a bronchial foreign body. The air leak was resolved dramatically after removing the foreign body.

  • An electromyographic device was used to investigate the relationship between a linguistic hypothesis of tense-lax distinction and muscular activity. Muscle action potentials of the orbicularis oris muscle and the depressor anguli oris muscle were obtained from four subjects using CVCVCV and CVCVC words in English and VCV and CVC words in Korean. Findings: The hypothesis that the speaker may select at least one of muscles involved in the articulation of a phoneme so that the selected muscle could be activated for tense-lax distinction, and either a timing variable or an amplitude variablethe and/or both from the selected muscle distinguish(es) /p/ from /b/ in English and /$p^{h},\;p^{l}$/ from /p/ in Korean, with the English /p/ and the Korean /$p^{h},\;p^{l}$/ being tense, and the Korean unaspirated /p/ and the English /b/ lax, has been verified, except for the case with subject 2 in stressed syllables in English. (2) Thus, the linguistic hypothesis of tense-lax distinction was strongly supported by the muscular activities during the Korean bilabial stops, with /$p^{h}\;and\;p^{l}$/ being tense and /p/ lax. (3) Considering the intermuscle compensation and the interspeaker variabilities in the choice of a muscle or muscles, in English the usability of the feature 'tensity' appeared to be positive rather than negative although further investigations with more subjects remain to take on the muscles associated with the onset/offset of the labial closure, including the respiratory muscles related with the aspiration. The phoneme-sensitive EMG manifestations of stress and possible reasons for the interspeaker variabilities are discussed.

Nontraumatic Pneumothorax -A Review of 56 Cases- (비외상성 기흉의 임상적 고찰)

  • We observed 56 cases of nontraumatic pneumothorax clinically and statistically, which had been experienced at the deparment of chest surgery. St. Mary`s Hospital,Catholic Medical College in theserecent years. 1] In the underlying pathology of spontaneous pneumothorax, nontuberculous origin [60.7%], especially due to pulmonary emphysema or blebs[17.8%], especially due to pulmonary emphysema or blebs[17. 8%], tended to increase as the reports of foreign countries, but tuberculous origin was still high in our country[39.3%]. Considering the 14 cases, unknown underlying pathology, the most of them might have scattered blebs which were not revealed in chest Roentgen films. 2] The principle treatment done in our clinic was as follows; The patients, below 20% lung collapse were treated by bed rest and abdominal respiration. The patients, between 20% and 40% lung collapse were treated by repeated pleural aspiration or closed thoracotomy followed. The cases,over 40% lung collapse were treated by closed thoracotomy initially. 3] The average duration of indweIling catheter was 3 to 4 days in the closed thoracotomy. We used to not remove the indwelling catheter early to promote pleural adhesion. 4] Sometimes, the closed thoracotomy drainage induces bronchial irritation and asthmatic attacks, especially in old age group accompanying pulmonary emphysema. In these cases, respiratory difficulties and acidosis should be prevented and controlled with medical treatment including steroid therapy.

Transthoracic Fine Needle Aspiration Biopsy in Localized Pulmonary Lesions: Diagnostic Accuracy and Complications (국소적 흉부 병변에서 세침 흡입 생검의 진단 성적 및 합병증)

  • Introduction: Transthoracic fine needle aspiration biopsy(TNAB) has shown to be a resonably safe, simple, and accurate procedure in diagnosis of intrathoracic lung lesions. We reviewed the results of 1,005 TNAB of chest lesions performed on 930 patients with 20 or 22-gauze needles over a period of 10 years. Methods: From November 1983 to June 1995, 1,005 cases in 930 patients with an undiagnosed lung lesion underwent TNAB at the Hanyang University Hospital: 66% were men and 34% were women. Most of the patients were 40~60 years old and the youngest patient was 3 years of age. Result: 540 patients had various malignant chest lesions and 322 patients had benign pulmonary lesions. The diagnostic accuracy of TNAB was 96.1 percent in malignant diseases with one false positive result and 90.1% in benign diseases. A definitive diagnosis was not obtained in the remaining 68 patients. The most common diagnoses among 519 malignancy chest lesions with TNAB were the following: squamous cell lung carcinoma, 31.7%; adenocarcinoma, 24.7%; small cell lung carcinoma, 16.7%; metastatic cancer, 14.2%; large cell lung carcinoma, 6.2% and so on. Complications included pneumothorax in 12.3% necessitating chest tube drainage in 0.6%. Minor hemoptysis occurred in 3.6%. There was no death directly attributable to the procedure. Conclusion: We concluded that TNAB permits a direct approach to all kinds of localized lung lesions with a high degree of accuracy and without major complications.
