Objectives : To investigate the prevalence and utilization pattern of complementary and alternative medicine (CAM) in individuals with type 2 diabetes. Methods : We conducted a nationwide survey and a total of 535 individuals with type 2 diabetes were asked about their demographic, diabetes-related characteristics and CAM use. We used multiple logistic regression modeling to determine independent predictors of CAM use. Results : Of the 535 subjects, 417 (77.9%) used CAM in last 12 months. The most commonly used modality was red ginseng (17.9%). CAM use was independently associated with being married, northern area residence, higher education and diabetes duration of 3-5 years. More than half(58.3%) of the CAM users used it expecting a better outcome in addition to conventional therapy. Information sources about CAM were mostly friends (36.0%) and family (31.9%), only 28.1% of diabetic patients seek counsel from medical doctors or oriental medical doctors about CAM. Many people perceived CAM as effective (62.7%) and reported side effects only rarely (1.2%). Conclusions : Many diabetic patients used CAM and found it effective, which urges clinicians to pay attention to CAM use of their diabetic patients. Also, further studies are required regarding efficacy and safety about CAM.

Background: Hepatocellular carcinoma (HCC) is the first cause of death in cirrhotic patients, mostly due to viral hepatitis with HCV or HBV infection. This study was performed to estimate the true prevalence of viral hepatitis-related HCC and the demographic and clinical-pathological associations with the two virus types. Materials and Methods: This cross sectional observational study enrolled clinical data base of 188 HCC patients and variables included from baseline were age, sex, area of residence, clinical-pathological features such as underlying co-morbidity, presence or absence of liver cirrhosis, macrovascular involvement, tumor extension and metastasis, liver lobes involved, serum alpha-fetoprotein level, and hepatitis serologies. Results: Overall prevalence of HCV- and HBV-related HCC was 66.0% and 34.0%, respectively. Patients with HCV were more likely to develop HCC at advanced age ($52.4{\pm}11.9$ vs. $40.7{\pm}12.09$ years), with highly raised serum AFP levels (${\geq}400ng/ml$) 78.2% (HBV 67.1%), large tumor size (HCV-66% >5 cm, HBV-59.3%), and presence of portal vein thrombosis (8.06%, HBV 1.56%). A binominal multivariate analysis showed that HCV-HCC group were more likely to be cirrhotic (OR=0.245, 95%CI: 0.117, 0.516) and had more than two times higher rate of solitary macrovascular involvement (OR=2.533, 95%CI: 1.162, 5.521) as compared with HBV associated HCC. Conclusions: Statistically significant variations were observed from baseline to clinical-pathological characteristics in HCV vs HBV associated HCC. Our study suggests prompt and early screening for high risk patients so that the rate of progression of these chronic viral diseases to cirrhosis and cancer can be decreased.
Objectives: The purpose of the study was to investigate the association between denture wearing and dementia using a Mini-Mental State Examination (MMSE-K) in the community dwelling elderly. Methods: The study subjects were 184 community dwelling elderly using Korean version of mini-mental state examination(MMSE-K). The variables included the general characteristics of the subjects, denture earing, number of mastication tooth, subjective mastication ability, and MMSE(dementia). Denture wearing was divided into two groups of the natural dentition including the fixed prosthesis and removable denture including the complete and/or partial denture. The data were analyzed by ${\chi}^2$ test, t-test, and binary logistic regression using SPSS version 22.0 program. Logistic regression was used to analyze the association of MMSE-K score and denture wearing, and 95% confidence interval was calculated. The subjective mastication ability was measured by Likert 5 points scale. Dementia was measured by MMSE-K. Results: There was a significant association between MMSE-K score and denture wearing. The odds ratio(OR) of denture waering was 6.01(95% CI: 2.824-12.784). After adjusting the age, gender, residence and education, OR was 5.53(95% CI: 2.364-12.935). Conclusions: This study showed a significant association between MMSE-K score and denture wearing in the Korean elderly.
Objectives: The study sought to identify the perception of schoolchildren's parents concerning an approval on the expansion implementation of community water fluoridation program in Gimhae, Korea. Methods: Questionnaires were distributed to schoolchildren's parents in Gimhae in 2008. Responses were collected from 1,703 parents. The standard questionnaire of Ministry of Health and Welfare was used and items were socioeconomic variables, water used for drinking and cooking, awareness on the fluoridation program and approval on the expansion implementation of fluoridation program at the region of Myeongdong water plant. Significance was analyzed by chi-square test and multiple logistic analysis. Results: Of parents, 1.9% used plain tap water and 39.8% used boiled tap water for drinking and 50.0% used tap water for cooking. 58.0% of parents were aware of fluoridation program with 27.1% being aware of the implementation of the program in Gimhae. 74.5% approved the expansion implementation of the program to the region of Myeong-dong water plant. Significant variables associated to approving the expansion implementation of program were age, gender, residence period in Gimhae, occupation and awareness of the program. Conclusion: The expansion implementation of fluoridation program in Gimhae is desirable for caries prevention because of the majority of schoolchildren's parents' approval.
Background: Dietary polyunsaturated fatty acids (PUFA) are thought to modify systemic inflammation. The present study aimed to evaluate the relationship between PUFA intake, lung function, and health-related quality of life (HRQoL) in patients with chronic obstructive pulmonary disease (COPD). Methods: In this study, we used the dataset of 6th Korea National Health and Nutrition Examination Survey, in which, a total of 22,948 individuals including 573 participants with a high probability of developing COPD were enrolled. Participants with missing data for the investigated variables were excluded. Linear regression analyses were used to evaluate the association between PUFA intake (omega-3 [N3], omega-6 [N6], and total) with lung function, and HRQoL. HRQoL was determined according to the European Quality of Life-5 Dimensions (EQ-5D). Subgroup analysis of older patients was performed. Age, sex, body mass index, smoking, alcohol, education, residence, total calorie intake, and predicted FEV1% were adjusted in all analyses. Results: Although lung function was not associated with PUFA intake, EQ-5D index was remarkably associated with N3, N6, and total PUFA intake in a dose-dependent manner. This association was more pronounced in elderly COPD patients. Mean levels of N3, N6, and total PUFA intake were significantly higher in patients having better HRQoL with respect to mobility, self-care, and usual activities. Conclusion: Our results suggest that N3, N6, and total PUFA intake are associated with HRQoL in COPD patients. This association may be attributed to mobility, self-care, and usual activities. Further longitudinal study is required to clarify this relationship.
본 연구는 휴양림 개발에 따른 개발 인접 지역의 주민 태도를 알아보기 위해 수행하였다. 기존 관광 영향 연구 문헌을 참조하여 20개 주민 태도 항목을 선발하였다. 본 연구를 위해 2개의 휴양림지역이 선정되었다. 1995년 휴양림 지역 인근에 거주하고 있는 204명의 지역 주민을 대상으로 직접방문조사가 실시되었다. 응답은 5 point 척도를 이용하였다. 주민 태도 분석결과 대부분의 응답자들은 현 휴양림 개발에 대해 경제적 혜택이 적고 지역 환경에 피해를 주고 있다는 부정적 시각을 갖고 있었다. 지각 태도는 응답자들의 사회 경제적 특성에 따라 유의한 차이가 있었다. 특히 휴양림 방문객으로부터의 수입 여부와 거주기간 정도에 따른 유의적 차이가 현저하였다. 본 연구결과 휴양림 개발지역은 정부가 의도하고 있는 긍정적 측면이 지각되지 못하고 있음을 보여주고 있었다. 현재 및 계획된 휴양림 개발 효과에 대한 지역 영향 연구가 수행될 필요가 있었다.
Background: Hardcore smoking is represented by a subset of daily smokers with high nicotine dependence, inability to quit and unwillingness to quit. Estimating the related burden could help us in identifying a high risk population prone to tobacco induced diseases and improve cessation planning for them. This study assessed the prevalence and associated factors of hardcore smoking in three South-East Asian countries and discussed its implication for smoking cessation intervention in this region. Materials and Methods: Global Adult Tobacco Survey (GATS) data of India, Bangladesh and Thailand were analyzed to quantify the hardcore smoking prevalence in the region. On the basis of review, an operational definition of hardcore smoking was adopted that includes (1) current daily smoker, (2) no quit attempt in the past 12 months of survey or last quit attempt of less than 24 hours duration, (3) no intention to quit in next 12 months or not interested in quitting, (4) time to first smoke within 30 minutes of waking up, and (5) knowledge of smoking hazards. Logistic regression analysis was carried out using hardcore smoking status as response variable and gender, type of residence, occupation, education, wealth index and age-group as possible predictors. Results: There were 31.3 million hardcore smokers in the three Asian countries. The adult prevalence of hardcore smoking in these countries ranges between 3.1% in India to 6% in Thailand. These hardcore smokers constitute 18.3-29.7% of daily smokers. The logistic regression model indicated that age, gender, occupation and wealth index are the major predictors of hardcore smoking with varied influence across countries. Conclusions: Presence of a higher number of hardcore smoking populations in Asia is a major public health challenge for tobacco control and cancer prevention. There is need of intensive cessation interventions with due consideration of contextual predictors.
Purpose: This study was to evaluate the utilization of health care service and to provide supportive data for health care policy making in one urban area in Korea. Method: This study tested the significance of public health service using the database of an university hospital and public health center from Feb. 2000 to Dec. 2004. Data were analyzed by multidimensional analysis and data mining technique and produced the information on the classification of utilization characteristics by main disease and the total cost of use and disease association with the users of the public health center. Results: The Results were as follows: 1) Top 10 diseases in the area accounted for 22.4% of total frequency for the most recent 5 years in university hospital, while 59.0% in public health center. 2) There were significant correlations between university hospital and public health center user's insurance type and place of residence: It showed higher use of public health center for free service beneficiaries residing in Seoul than residents in nearby or local area. The medical insurance types for hospital users were more various than those for public health center users. 3) The use of hospital for patients of hypertension, diabetes mellitus and hyperlipidemia was tended to concentrate in mostly autumn and winter since August 2000, while the cost of using public health center for those patients has been steadily reduced since July 2000. 4) As a result of cluster analysis, there were classified into three homogeneous groups according to the total cost of using public health service, age, and the frequency of use. 5) The association analysis on patients with chronic disease in public health center produced a detailed information on accompanying diseases related to the incidence rate of disease of high frequency due to aging, information on drug abuse and immune disease. Conclusion: The health care policy for local community should be evaluated continuously. And the policy to build an integrated data warehousing by public health indicator system and to enhance the faithfulness of data is required.
This study was descriptive survey research. The main purpose of this study is to examine the daily activity of hospitalized children from two month years old to twelve years old and to identify needs or responses of mother who has hospitalized children. The subjects for the study were 179 mothers who have hospitalized children at pediatric ward two hospitals attached to a university in Seoul. The data was collected by two researchers and two assistants using structured open questionnaire for interview. The data was analyzed by using SPSS/PC. The results of the study were as follows ; 1. The daily activity of hospitalized children was mainly play activity except for treatment or nursing activity. It was limited activity at sickbed and various according to developmental stage of children. 2. The common responses of mothers on intravenous injection. blood sampling and fretful children were heartache. crying. empathy and guilty feeling. 3. The responses of mothers on disease progress were comfort. aspiration. anxiety. gloominess. critique and a serene state of mind. 4. The responses of mothers on medical personnel were kindness. carefulness. comfort. satisfaction. calmness and unkindness. 5. The responses of mothers on another hospitalized children were mainly empathy and sympathy. 6. The responses of mothers on patient clothes were comfort and deny. 7. The responses of mothers on residence with child were comfort. inevitable duty. laborious. exhaust. annoyance and worried about another family member. 8. The requirements of mothers were mainly convenience facility and play place. The results of this study indicate that hospital life were indifferent growth and development of children. Nurses need to identify hospitalized children and mothers have hospital adjustment problems and intervene as soon as possible to promote normal growth and development of theses children.
The purpose of this study is to analyze the job of the security and secretary, to offer useful basic data throughout human resources management by withdrawing the ability required from the security and secretary based on the analyzed results. In this regard, this study intended to look into the job of the security and secretary, the necessary ability for the job of the security and secretary. To achieve the research goals, the study selected the research participants, composed of 5 secretaries for security with more than 5-year careers after graduating from a security and secretarial service department in a college and conducted an in-depth interview with them concerning their jobs. The in-depth interview data from the 5 participants was applied with a classification analysis used by Spradley (1980). In order to enhance the dependability and validity of the research, the study held an expert meeting composed of 2 persons with a doctoral degree in securities service studies and 1 person with a master's degree in secretarial information studies, twice. Also, the study drew results concerning the job of the security and secretary and the necessary ability of the job. Though the above process, the following conclusions were drawn; the job of the security and secretary includes the areas of job safety, a housing residence job, a health care job, an interpersonal relationships job, an assistant' s job, document and office works, general affairs, and an education job. The necessary ability for the job of the security and secretary involves martial arts abilities, risk management ability, the strict keeping of secrets, decision-making ability, information processing ability, foreign language proficiency, understanding other cultures, communications skills and office work ability.
