Advances in the use of dried blood spots on filter paper to monitor kidney disease

  • Patients with kidney disease require frequent blood tests to monitor their kidney function, which is particularly difficult for young children and the elderly. For these people, the standard method is to evaluate serum creatinine or cystatin C or drug levels through venous sampling, but more recently, evaluation using dried blood spots has been used. This narrative review reports information from the literature on the use of dried blood spots to quantify the main markers used to detect kidney diseases. The ScienceDirect and PubMed databases were searched using the keywords: "dried blood on filter paper," "markers of renal function," "renal function," "creatinine," "cystatin C," "urea," "iohexol," and "iotalamate." Studies using animal samples were excluded, and only relevant articles in English or Spanish were considered. Creatinine was the most assessed biomarker in studies using dried blood spots to monitor kidney function, showing good performance in samples whose hematocrit levels were within normal reference values. According to the included studies, dried blood spots are a practical monitoring alternative for kidney disease. Validation parameters, such as sample and card type, volume, storage, internal patterns, and the effects of hematocrit are crucial to improving the reliability of these results.

Evaluation of response to stereotactic radiosurgery in patients with radioresistant brain metastases

  • Purpose: Renal cell carcinoma (RCC) and melanoma have been considered 'radioresistant' due to the fact that they do not respond to conventionally fractionated radiation therapy. Stereotactic radiosurgery (SRS) provides high-dose radiation to a defined target volume and a limited number of studies have suggested the potential effectiveness of SRS in radioresistant histologies. We sought to determine the effectiveness of SRS for the treatment of patients with radioresistant brain metastases. Materials and Methods: We performed a retrospective review of our institutional database to identify patients with RCC or melanoma brain metastases treated with SRS. Treatment response were determined in accordance with the Response Evaluation Criteria in Solid Tumors. Results: We identified 53 radioresistant brain metastases (28% RCC and 72% melanoma) treated in 18 patients. The mean target volume and coverage was 6.2 ± 9.5 mL and 95.5% ± 2.9%, respectively. The mean prescription dose was 20 ± 4.9 Gy. Forty lesions (75%) demonstrated a complete/partial response and 13 lesions (24%) with progressive/stable disease. Smaller target volume (p < 0.001), larger SRS dose (p < 0.001), and coverage (p = 0.008) were found to be positive predictors of complete response to SRS. Conclusion: SRS is an effective management option with up to 75% response rate for radioresistant brain metastases. Tumor volume and radiation dose are predictors of response and can be used to guide the decision-making for patients with radioresistant brain metastases.

음곡(陰谷)에 시술한 동규인약침이 LPS로 유도된 흰쥐의 신장염에 미치는 영향 보고 (Herbal-acupuncture at $KI_{10}$ on LPS-induced Nephritis in Rats)

  • Objectives : This study aimed to evaluate the effects of Malvae Semen herbal-acupuncture (MS-HA) at$KI_{10}$($Eumgok$) on nephritis induced by lipopolysaccharide (LPS) in rat. Methods : Rats with nephritis induced by LPS, were treated with Malvae Semen herbal-acupuncture(MS-HA) injection at $KI_{10}$. Two control groups, N.P group and Saline group, were treated with 26 gauge needle at $KI_{10}$, 3 times a week. In Saline group, normal saline was injected at $KI_{10}$. To evaluate the effects of Malvae Semen Herbal-acupuncture at $KI_{10}$ on nephritis in rats, WBC, Neutrophils in blood, BUN, Creatinine, TNF-a, CINC-1 in serum, urinal volume, creatinine, total protein in urine and renal TNF-a, renal tissue were analyzed. Results : MS-HA at $KI_{10}$ significantly reduced WBC and neutrophil in blood, TNF-${\alpha}$ in serum of LPS-stimulated rats. MS-HA at $KI_{10}$ significantly reduced total protein level in urine. MS-HA at $KI_{10}$ reduced accumulation of neutrophil in renal tissue of LPS-stimulated rats. Conclusions : Taken together, MS-HA at $KI_{10}$ has a therapeutic effect on nephritis in LPS-stimulated rat. Therefore, it is suggested that MS-HA at $KI_{10}$ may be an useful therapeutics for nephritis in clinical field.

호도약침(胡桃藥鍼)이 Glycerol에 의한 급성신부전(急性腎不全) 유발(誘發)시 요농축능(尿濃縮能)의 장애(障碍)에 대한 영향(影響) (The Effect of Juglandis Semen Aquacupuncture on Urine Concentrating Defect in Glycerol-induced Acute Renal Failure)

  • Objective : The present study was carried out to determine if Juglandis Semen Aquacupuncture(JSA) exerts beneficial effect against the urine concentrating defect induced by glycerol injection in rabbits. Methods : In order to test the effect of JSA, rabbits were acupunctured with $0.5m{\ell}$ of 1%JSA for 7days at both sides of $Sh{\grave{e}}ns{\bar{u}}$(BL23) before the glycerol injection. The other animals were pretreated with an equal volume of saline instead of JSA. Results : The urine flow was reduced, but the urine osmolality was significantly lower than the basal period in glycerol-injected animals, indicating that glycerol causes the urine concentrating defect. The fractional excretion of Na and K was increased in glycerol-treated animals. The Free-water clearance and fractional water excretion(V/GFR) were increased in animals treated with glycerol. Conclusion : These results indicate that glycerol injection resulted an impairment in the urine concentrating ability in rabbits. Such changes were prevented by JSA. JSA may be used as a method to treat and prevent glycerol-induced acute renal failure.

황연해독탕 합 육미지황탕 가 조구등이 자발적 고혈압 백서의 혈압 및 현장 기능에 미치는 영향 (Effects of Hwangryunhaedok-tang plus Yukmijihwang-tang add Chogudeung on Blood Pressure and Renal Function in Spontaneously Hypertensive Rats)

  • The present study examined the effects of water extract of Hwangruynhaedoktang plus Yukmijihwangtang add Chogudeung (HYC) on the blood pressure and renal function in spontaneously hypertensive rats (SHR). The HYC water extract blocked increase of systolic blood pressure (SBP) in SHR. The urinary volume (UV), sodium (UNaV), potassium (UkV), and chloride (UclV) excretions were significantly increased in SHR administered with high dose of water extract of HYC, but urinary osmolality was not changed. The creatinine clearance (Ccr), which is index of glomerular filtration rate (GFR), was increased by administration of HYC, while solute-free water reabsorption (TcH2O) was not changed. Taken together, these results suggest that the depressor effects of HYC, at least in part, caused by diuretic effect due to increase of glomerular filtration rate.

  • A study on the mechanism of biliary and urinary excretion of chloramphenicol has been performed in the dog. 1) Chloramphenicol administered intravenously to dogs with ligated renal pedicle, readily appeared in bile greater than in plasma. 6.9% of a 50mg /kg i. v. dose of chloramphenicol were excreted into bile within 100 minutes. During the same periods of above experiment, the bile/plasma concentration ratios(B/P ratios) were 46 to 87. 2) Chloramphenicol injected into the vein of dog was rapidly excreted into urine. 18% of the administered dose were excreted into urine within 70 minutes. In the same periods of this experiment, Ccm/Ccr ratios were greater than 1.0 in most cases. 3) In experiment of simultaneous measurement of biliary and urinary excretion of chloramphenicol, Ccm/Ccr ratios were less than 1.0 and B/P ratios were 50 to 52. 4) In experiment measured simultaneously biliary and urinary excretion both Ccm/Ccr and $C^Hcm$(hepatic clearance) were significantly declined by probenecid, but not affected by 2,4-DNP and aminophylline although 2,4-DNP increased only bile flow and aminophylline both bile and urine volume. 5) Ccm/Ccr and $C^Hcm$ were increased in proportion to increment of plasma concentration ranging from 3.3 to 30 mg% of chloramphenicol. But when plasma concentration were increased to 70mg %, Ccm/Ccr were not increased and $C_Hcm$ were reduced about 30% in comparison with values obtajned at 30mg% of chloramphenicol. 6) Free/Bound(free to bouid from) ratios ranging from 1.0 to 90.0mg% of chloramphenicol were 76.2+3.72% $(mean{\pm}S.E.)$ Above results suggest that chloramphenicol is excreted into bile by a process of active trasport, that excretion of chloramphenicol into urine was made up with dual process, reabsorption and secretion, and that renal secretion was attained by active trasport process although renal reabsorption process could not understand.

황연해독탕(黃連解毒湯)이 자발적(自發的) 고혈압(高血壓) 백서(白鼠)의 혈압(血壓) 신장(腎臟) 기능(機能)에 미치는 영향(影響) (Effect of Hwangryunhaedok-tang on Blood Pressure and Renal Functions in Spontaneously Hypertensive Rats)

  • The present study designed to investigate whether hwangryunhaedok-tang show an anti-hypertensive effect and elucidate its possible mechanism in spontaneously hypertensive rats. The systolic blood pressures (SBP) were significantly decreased as an oral administration of hwangryunhaedok-tang compared with their control group. The urine volume was significantly increased by administration of hwangryunhaedok-tang but urinary sodium (UNaV), potassium (UKV), chloride excretion (UCIV) were not remarkably affected. The urinary creatinine excretion rate (UcrV) was significantly increased in rats administered with hwangryunhaedok-tang in association with increase of creatinine clearance (Ccr). The urine osmolality (Uosmol) was significantly decreased in SHR administered with hwangryunhaedok-tang without being changed in solute-free water reabsorption ( TcH20). The expressions of Aquaporin 2 (AQP-2). 3 and ${\alpha}\;1$, ${\beta}\;1$ subunits of Na.K-ATPase were determined by Western blot analysis to assess the role of these proteins in association with changes of renal functions in SHR administered with hwangryunhaedok-tang. The expression of AQP-2 and 3 protein was significantly down-regulated in the kidney of SHR administered with hwangryunhaedok-tang compared with those in control rats without being altered expression of ${\alpha}\;1$, ${\beta}\;1$ subunits of Na,K-ATPase. In the in vitro assay, Angiotensin converting enzyme (ACE) was inhibited by hwangryunhaedok-tang in a dose-dependent manner. Berberine and/or palmatine, which are well known as a main components of hwangryunhaedok-tang, also have an ACE inhibitory effects in a dose-dependent manner. Taken together, these results suggest that hwangryunhaedok-tang lowered blood pressure through the increase of diuresis caused by down-regulation of water channels and the inhibition of Angiotensin converting enzyme.

Gamma Knife Surgery for Brain Metastasis from Renal Cell Carcinoma : Relationship Between Radiological Characteristics and Initial Tumor Response

  • Objective : The authors have speculated that metastatic brain lesions from renal cell carcinoma (RCC) show diverse radiological patterns and tumor responses after Gamma knife surgery (GKS), and have hypothesized that these can be predicted from tumor radiological characteristics. The goal of the current study was to identify the radiological characteristics of RCC brain metastases and the predictors of initial radiosurgical response after GKS. Methods : A retrospective analysis was performed on 48 lesions in 18 patients with RCC brain metastasis treated by GKS. The radiological characteristics of these lesions in magnetic resonance images (MRI) were classified into 3 categories according to enhancement patterns in T1-weighted images and signal intensity characteristics in T2-weighted images. Responses to GKS were analyzed according to these categories, and in addition, other potential predictive factors were also evaluated. Results : MRI findings in the three categories were diverse, though numbers of the lesion were comparable. At 2-month MRI follow-ups after GKS, response rate was 54% and the local tumor control rate 83%. T2 signal intensity was found to be the principal predictive factor of response to GKS, namely negative predictive factor. Other variables such as age, sex, tumor volume, dose, duration from initial diagnosis to GKS, and previous systemic therapies failed to show significant relationships with treatment response by multivariate analysis. Conclusion : Careful evaluation of the radiological characteristics of brain metastases from RCC is important prior to GKS because MRI heterogeneity has predictive value in terms of determining initial tumor response.

  • The aim of this experiments was to investigate the effect of Croton. Tiglii. semen water extract on the renal function, plasma renin activity, plasma levels of atrial natriuretiu peptide and aldosterone in rats. The results of this study were as follows: 1. Water balance was not changed significantly after the administration of Croton Tiglii semen water extract. 2. Urine volume decreased significantly after the administration of Croton Tiglii semen water extract $80{\mu}l/200g$. 3. Urinary excretion of sodium increased significantly after the administration of Croton Tiglii semen water extract $40{\mu}l/200g$, but decreased significantly after the administration of Croton Tiglii semen water extract $80{\mu}l/200g$. 4. Urinary excretion of potassium decreased significantly after the administration of Croton Tiglii semen water extract $80{\mu}l/200g$. 5. Urinary excretion of chloride was not changed significantly after the administration of Croton Tiglii semen water extract. 6. Free water clearance was not changed significantly after the administration of Croton Tiglii semen water extract. 7. Urinary excretion of creatinine increased significantly after the administration of Croton Tiglii semen $40{\mu}l/200g$. 8. Plasma renin activity was not changed significantly after administration of Croton Tiglii semen water extract. 9.
