A Study on the Mobile Medical Service Program -Based on the Community Diagnosis of a Remote Farm Area- (순회진료사업(巡回診療事業)의 문제점(問題点)과 개선방향(改善方向) (일부(一部) 무의지역에 대(對)한 지역사진단(地域社診斷)을 중심(中心)으로))

  • The mobile medical service has been operated for many years by a number of medical schools and hospitals as a most convenient means of medical service delivery to the people residing in such area where the geographical and socioeconomic conditions are not good enough to enjoy modern medical care. Despite of official appraisal showing off simply with numbers of outpatients treated and medical persons participated, however, as well recognized, the capability (in respect of budget, equipment and time) of those mobile medical teams is so limitted that it often discourages the recipients as well as medical participants themselves. In the midst of rising need to secure medical service of good quality to all parts of the country, and of developing concept of primary health care system, authors evaluated the effectiveness of and problems associated with mobile medical servies program through the community diagnosis of a village (Opo-myun, Kwangju-gun) to obtain the information which may be halpful for future improvement. 1. Owing to the nationwide Sae-Maul movement powerfully practiced during last several years, living environment of farm villages generally and remarkably improved including houses, water supply and wastes disposal etc. Neverthless, due to limitations in budget time and lack of knowledge (probably the most important), these improvements tend to keep up appearances only and are far from the goal which may being practical benefit in promoting the health of the community. 2. As a result of intensive population policy led by the government since 1962, there has been considerable advances in understanding and the rate of practicing family planning through out the villages and yet, one should see many things, especially education, to be done. Fifty eight per cent of mothers have not received prenatal check and the care for most (72%) delivery was offered by laymen at home. 3. Approximately seven per cent of the population was reported to have chronic illness but since only a few (practically none) of the people has had physical check up by doctors, the actual prevalence of chronic diseases may reach many times of the reported. The same fact was observed also in prevalence of tuberculosis; the patients registered at local health center totaled 31 comprising only 0.51% while the numbers in two neighboring villages (designated as demonstration area of tuberculosis control and mass examination was done recently) were 3.5 and 4.0% respectively. Prevalence rate of all dieseses and injuries expereinced during one month (July, 1977) was 15.8%. Only one tenth of those patients received treatment by physicians and one fifth was not treated at all. The situation was worse as for the chronic patients; 84% of all cases either have never been treated or discontinued therapy, and the main reasons were known to be financial difficulty and ignorance or indifference. 4. Among the patients treated by our mobile clinic, one third was chronic cases and 45% of all patients, by the opinion of doctors attended, were those who may be treated by specially trained nurses or other paramedics (objects of primary care). Besides, 20% of the cases required professional managements of level beyond the mobile team's capability and in this sense one may conclude that the effectiveness (performance) of present mobile medical team is quite limitted. According to above findings, the authors would like to suggest following for mobile medical service and overall medicare program for the people living in remote country side. 1. Establishment of primary health care system secured with effective communication and evacuation (between villages and local medical center) measures. 2. Nationwide enforcement of medical insurance system. 3. Simple outpatient care which now constitutes the main part of the most mobile medical services should largely be yielded up to primary health care unit of the village and the mobile team itself should be assigned on new and more urgent missions such as mass screening health examination of the villagers, health education with modern and effective audiovisual aids, professional training and consultant services for the primary health care organization.

  • This paper presents the content regarding electronic medical examination chart and data processing for efficient medical examination and fast treatment by realizing remote medical examination system of mutual conversation type among 3 parties(patient, doctor, pharmacist) on internet base, and establishment of database enabled system integration for efficient data processing in both on-line and off-line mode by interconnecting ASP and SQL on IIS 4.0 web server, consultation between patient and doctor, medical examination on off-line mode, transmission of prescription sheet to the pharmacist designated by patient, preparation of medicine, semieternal storage of medical examination data owing to storage and check of medical examination data, more accurate medical examination and prescription using this medical examination data by patient and doctor, and so on. And, data processing between doctor and pharmacist is differently performed based on class such as general member and charge member, and service access right pursuant to this is endowed, so that certification of each member must follow by all means.

Association between lone work and self-rated health status: using the 5th Korean Working Conditions Survey

  • Background: Lone workers are generally defined as individuals who work alone without supervision, including self-employed people. While lone workers are considered a vulnerable group in some countries, there is a lack of research on their health status in domestic studies. Globally, the number of lone workers has been increasing, and this trend has been further accelerated since the coronavirus disease 2019 (COVID-19) pandemic with the rise of remote work. Methods: The study analyzed data from 44,281 participants, excluding unpaid family workers, soldiers, and those with missing data. Lone workers were defined as individuals who reported having no colleagues with the same job at their current workplace. Self-rated health status was categorized as "good" or "poor." Results: This study found a statistically significant higher number of lone workers among women compare to men. The largest occupational category for lone workers was service and sales workers, followed by agriculture and fisheries workers. A majority of non-lone workers reported working 40 hours or less per week, while the majority of lone workers reported working 53 hours or more per week. In addition, lone workers had significantly poorer health status evaluations compared to non-lone workers (odds ratio: 1.297; 95% confidence interval: 1.165-1.444). Conclusions: Further research is needed to investigate the causal relationship between lone work and health, using data collected after the COVID-19 pandemic.

Dose Characteristics by the Co-60 Source Oscillations in High Dose Rate After Loading Irradiations (고선량율 원격강내조사의 코발트-60 이동선원에 의한 선량특성)

  • Dose distributions around Co- 60 moving source in high dose rate remote afterloading unit, Buchler 3K unit, were experimented with X-omat V films and calculations. In our experiments, film dosimetries have achieved to evaluated the axial dose distributions for source oscillations were 0, 3.5, 5.0 and 6.0 cm in periodically, In results, the dose distributions in axial of source movement showed apparently higher than in transverse direction caused by source locations, dwelling time and air gap in the applicator. In the calculations, the dose rate was derived by using the inverse square law, filteration corrections and Meisberger constant for scatter corrections as source movings. In our experiments and calculations, the average dose uncertainties were showed -2.1$\pm$1.9% in fixed sourdce, -2.9$\pm$1.8%, -7.4$\pm$6.1% and -6.7$\pm$4.6% at 3.5 cm, 5.0 cm and 6.0 cm source oscillations, but the calculations have showed very close to experimental dose rate within 4 cm distance from source.

  • The purpose of this paper is to take a closer look at an area having shorted emergence facilities and to determine optional candidate sites instead of vulnerable area by using GIS spatial analysis. Newly determined new candidate is performed by concerning spatial efficiency and spatial equity for a public service. It was determined through using the analyzing of the physical accessibility measure, the Location-Allocation, sort of classic model in spatial statistics and general network analysis. The area of this research has been used in administrative boundary of Young-Dong in Gangneung including 13 emergency, medical hospitals, 46 fire-stations and sub-fire stations. In general terms, what all this show is that the way we are approached for geographical view from using GIS spatial analyzing technique of determined location and allocation problem by the social, economical, political factor and simple administrative discrimination at the meantime. At the same time, with problem occurred in the space it is possible to make an Effective proposal or means, policy, decision for new candidate location-allocation suggesting optimum model.

  • Hemangiopericytomas, which used to be classified as the subtype of the meningiomas, like the angioblastic meningiomas or hemangiopericytic meningiomas, are rare in the central nervous system with an incidence of <1% of all central nervous system(CNS) tumors. In spite of a total surgical removal, hemangiopericytomas have a tendency of high local recurrence and distant metastasis. The authors report a case of hemaniogipericytoma recurred at the remote area from the primary site. The tumor recurred in the meninges of right temporal area after total removal of the tumor in the right parasaggital region including the falx.

  • In this paper, the module for measuring SpO2 and PPG(Photo Plethysmo Graphy) signal of human body and the module for wireless communication of these bio-signals to PC were developed. The program was developed to display transmitted SpO2 and PPG signal by various type data and graph without information loss during a emergency transfer. This system can be utilized as appropriate for remote medical care and a new market is expected, to be created according to revision of medical law.

  • It is essential to investigate the structure and the main characteristics of BSN (Bio-Sensor Network) platform in built smart healthcare environment while designing healthy housing facilities. For this study, WSN (Wireless Sensor Network) data transmission technologies have been employed with medical sensors, and optimal medical devices would provide various Web 2.0 services by connecting to the WiBro network. The BSN platform normally recognizes in surroundings of WBAN (Wireless Body Area Network) or WPAN (Wireless Personal Area Network), and it is possible to manage sensor nodes by utilizing SOAP (Simple Object Access Protocol) and REST (REpresentational State Transfer). In addition, the feature of SNMP (Simple Network Management Protocol) for mobile gateway is also included for being adapted to huge network structure. Finally, BSN platform will play a role as important clues for developing personal WSN service models for smart healthy housing properties.

  • Picture archiving and communications systems(PACS) for digital image distribution, archiving and transmission, represent the modern hospital. PACS is the medical image management system within a hospital, whereas teleradiology system is the medical image communication system between remote sites. PACS and teleradiology systems usually consists of mainly four parts such as image acquisition part, image storage and database part, Image communication network part, and image display workstation part. Among these components comprised In the PACS, workstations are the point of contact between a PACS and the radiologist or referring physician. Therefore, the acceptance of PACS is highly dependent on workstation functionality, performance and easy of use compared with the conventional film-based radiology. This paper describes overall configurations and some requirements of PACS and teleradiology.
