노인의 일상 활동 범주에 따른 활동 보유 수준 비교 (Comparison of Retained Activity Level according to Activity Areas of the Elderly)

  • 목적 : 본 연구는 노인의 일상 활동 범주에 따라 보유하고 있는 활동 수준을 확인해보고 일상 활동범주에 따른 차이를 비교해보고자 하였다. 연구방법 : 지역사회에 거주하면서 경제 활동을 하는 노인 30명과 경제 활동을 하지 않는 노인 30명, 시설에 거주하면서 활동을 할 수 있는 노인 30명을 대상으로 한국형 활동분류카드(Korea Activity Card Sort; K-ACS)를 사용하여 활동 보유 수준을 비교하였다. 또한 노인의 활동 수준과 상관이 있는 일반적 특성과 일상 생활 특성을 확인하였다. 결과 : 대상자의 일상 활동 범주에 따른 활동 보유 수준은 통계적으로 유의한 차이가 있었다(p<.001). 수단적 활동에는 각 활동의 범주에 따라 차이를 보이는 반면 여가활동과 사회활동에서는 세 그룹이 비슷한 양상을 나타내었다. 특히 여가 활동에서는 세 그룹 모두 휴식하기와 텔레비전 보기를 가장 많이 하고 있어 우리나라 노인의 여가 활동이 매우 제한적인 것을 확인할 수 있었다. 또한 일반적 특성 중에는 학력, 결혼상태, 종교와 인지 수준이, 일상 활동 특성 중에는 받고 있는 치료와 규칙적인 운동 및 정기 모임 횟수가 노인의 활동 영역과 상관성이 있는 것으로 분석되었다. 결론 : 노인의 일상 활동 범주에 따라 활동 보유 수준은 차이가 있었으며 일반적 특성 및 일상 활동 특성이 노인의 활동 영역과 상관이 있음을 알 수 있었다. 이는 노인 작업치료 시 노인의 일상 활동에 영향을 미치는 특성과 활동보유 수준을 고려하여 클라이언트 중심의 중재 계획을 수립하고 치료적 활동을 계획하기 위한 기초자료가 될 수 있을 것이라 사료된다.

일부 요양시설 노인들의 사회적 지지와 우울수준과의 관련성 (Relationship Between Social Supports and Depression Symptoms Among the Elderlies at Care Facilities)

  • 본 연구는 요양시설 노인의 사회적 지지와 우울수준과의 관련성을 규명하고자 시도하였다. 조사대상은 D광역시에 소재하고 있는 15개소의 요양시설에 입소하고 있는 65세 이상 노인 328명을 대상으로 하였으며, 자료 수집은 2018년 3월 1일부터 5월 31일까지의 기간 동안에 조사원이 조사대상 요양시설을 직접 방문하여 면접조사에 의하였다. 연구 결과, 조사대상자에서의 우울군의 분포는 사회적지지 점수가 낮은 군일수록 유의하게 높았으며, 우울수준 점수(CES-D)는 사회적지지 점수와 유의한 음의 상관관계를 보였다. 로지스틱 회귀분석 결과, 우울군에 속할 위험비는 사회적지지 점수가 가장 높은 군(Q4)보다 높은 군(Q3)에서 2.7배, 중간 군(Q2)에서 5.4배, 낮은 군(Q1)에서 10.1배 유의하게 증가하였다. 다중회귀분석 결과, 우울수준에 영향을 미치는 요인으로 종교유무, 자녀와의 관계, 주관적 건강상태, 신체의 부자유유무, 흡연상태, 아침식사여부 및 사회적지지가 유의한 변수로 선정되었으며, 투입된 변수들의 설명력은 41.1%이었다. 위와 같은 연구결과는 요양시설 노인의 우울수준은 사회적 지지와 유의한 관련성이 있음을 시사하고 있다.

자연장 활성화를 위한 서비스 개선방안 연구 (A Study on the Improvement of Service for the Revitalization of Natural Burial)

  • 장사 방법의 선택은 생의 마지막 순간에 필요한 결단이며, 이를 위해 우리는 몇 가지를 판단기준으로 삼는다. 우리의 장례방법에는 자연이 아닌 조상숭배 문화와 종교가 지배적이었다. 인간적인 시각에서 자연을 수단화하여 이용했을 뿐 자연에 대한 배려나 자연과의 공생과 같은 자연장의 방법이 등장한 건 최근이다. 최근 자연장의 국민 선호도가 높은 것은 오늘날의 강력한 시대정신, 자연 친화적 가치이다. 2021년 통계 기준, 우리나라 전국 화장률 92%를 상회 했으며, 불과 20년 전 화장률 20% 미만에 견주면 우리의 장사방법은 급변했다. 90년대 초반부터 시작된 화장장려 운동과 정부시책이 체계적으로 전개되면서 봉안시설이 우리 곁에 자리 잡았다. 하지만 이 또한 국토훼손이라는 비난의 굴레를 면하지 못하던 중, 2008년 자연장이라는 장법이 제도권에 도입되었고, 약 15년이 흘렀지만 자연장 활성화는 예상보다 더디다. 발전 정체의 원인 중 하나는 "(한번 숲으로 돌아가면) 숲에 영원히 잠든다"라는 자연장의 기본 정신을 잊고, 마치 묘지의 분묘처럼 생각(改葬을 인정)한 데서 출발했다고 본다. 이에, 본 연구에서는 자연장의 도입배경과 현재 운영실태를 파악하고 자연장이 국민들로부터 사랑받는 추모 공간으로 거듭기 위한 추모서비스 개선을 위한 발전방안을 제시하고자 한다, 또한 국내외 우수 사례와 고품격이면서도 친환경적인 선진국 자연장의 조성사례를 통해 시사점과 적용 가능성을 검토한다.

농촌지역의 일차보건사업 개발을 위한 기초조사 연구 - 경기도 여주군 금사면 산북부락을 중심으로 - (A Baseline Survey on Development of Primary Health Care in the Rural Korea -Sanpuk Village, Kumsa-Myun, Yuju- Gun, Kyunggi-Do-)

  • It is widely recognized that primary health care in the community is one of the most important and effective health measures in these days. However, it is reality that unsatisfactory health care system, ineffective utilization of health care by the community people in the rural area are hampering better understanding for primary health care. Therefore promoting health for the rural people and increasing understanding about primary health care, the baseline survey in the community focused in examination for safe community water supply was carried out. The survey was conducted through August 25-31, 1986 in order to find out health problems and relevant factors and to define the demographic characteristics of $^*$Sanpuk village, Kumsa-Myun, Yuju-Gun, Kyunggi-Do, Korea. Household survey was carried out for every home by trained interviewers. The major results are found out as follows : 1) 84.2%(400 houses) of total households were surveyed because 15.8%(75 houses) were unable to survey due to either refusal against interview or absence of family. These 400 households were composed of 1,697 residents(male:830, female 867). Educational level of respondents showed 34.1% as elementary school graduated. Religion distribution showed Buddism(23.8%) as the most dominant. 50.7% of respondents married in the area. 2) Most households(91.5%) have lived in their own house in Sanpuk area. Average family size showed 4.3. More than half of residents(64.2%) have used public supplied water tap. Only 1.5% of the households had a flush toliet. The rest of households have still used primitive insanitary latrines. 3) 32.5% of residents have used gas burner for cooking and for heating in the house, and the coal briquet were used for boiler. Lack of convenient public transportation was the chief complaint for their day life. 4) Each household occupied 1,990 pyungs of rice paddy and 1,170 pyungs of ordinary field in average. Beside farming products, mushroom was the highest product. 5) Sixth percent of households in the survey area regularly participated in community meeting one hand and on the other hand 39.5% never participated. Most of respondents closely contacted with their neighbours and they seemed very friendly each other. 6) The prevalence rate of illness and injury during recent 15 days showed 48.3 per 1,000. The prevalence rate of chronic illnesses during the past one year showed 74 per 1,000. Injury and accident lead the higher portion(22.0%) in the former and in the latter pain(arthritis, back-pain) showed 27.0% as the dominant sickness. 87.8% of the ill residents in the former received medical treatment. As the most frequently utilized medical facility, the clinic or hospital were counted. Among the residents suffering from chronic illnesses, 77.3% in Sanpuk area get some kind of medical treatments and they rarely utilized the clinic or hospital. The reason why the patient did not receive any medical care was found out the fact that symptoms of illness was light or mild and economic problems was serious. 7) Average age of marriage showed 21.6 years old in the women and the average duration of marital period was shown for 15.1 years. The married woman in reproductive age in Sanpuk area had experienced pregnancies 4 times in the aver-age including 0.7 time of pregnancy in average were interrupted by induced abortion and 0.3 time by spontaneous abortion respectively. The practicing rate of the family planning of the married woman during reproductive ages showed 70.7% and the tuballigation was found out as the most frequently used contraceptives. 8) Among woman who has children under 2 years old, 70.0% had received the prenatal care for the last pregnancy. However, the average number of prenatal care visitis per woman showed 3.3 times. Fifty-two % of woman who received the postnatal care for the last delivery showed only 37.5%. 9) Immunization rate of the children under 2 years old showed relatively high and looked successful. The breast feeding for these children showed dominantly in the most. Most of the mothers in Sanpuk area had started the supplementary diet during weaning period of their infants of 6th and 7th month after birth. * : Sanpuk area is a demstration area for community development which has been supported by the Community Development Foundation during the part 10 years. The village is relatively closer to urban area such as Seoul, However, it has a similar characteristics shown as a remote village because of geographical location and inconvenient transportation at present.

요양시설의 조직구성원의 임파워먼트가 직무만족에 미치는 영향에 관한 연구 (Study the impact on job satisfaction in the care facility of the empowerment of the members of the organization)

  • 최근 우리나라 의료복지 분야는 인구의 고령화, 만성질환의 증가, 교통사고, 각종 산업재해, 약물 오 남용, 의학의 발달, 국가복지재정의 악화 등 다양한 환경변화에 둘러싸여 있으며, 새로운 형태의 의료복지와 관련된 새롭고 다양한 서비스 개발의 필요성이 부각되고 있다(조만복, 2010). 국가의 경제적 수준이 높아질수록 사회복지에 대한 사회적 관심은 증가하는 추세이나 복지에 대한 국가의 역할 축소와 사회복지정책에 민간참여의 확대 등 사회복지 여건의 변화는 요양시설 사회복지사들에게 시설서비스의 효과성과 효율성 제고를 위해 다양한 새로운 지식과 기술 습득, 그리고 스스로 최고의 성과를 달성하고자 노력하는 정신자세의 정립을 요구하고 있다. 요양시설에서 서비스 제공자인 사회복지사는 시설클라이언트의 삶의 질에 직접적인 영향을 미치며 서비스의 효과성에 매우 중요한 영향을 미치게 된다. 그러나 양질의 서비스 제공이라는 중요한 책무성을 지니는 장애인 및 노인요양시설은 조직 및 직무환경에 있어서 직원개발 기회의 부족, 조직보상체계의 미흡, 조직의 폐쇄성, 직원자율성의 결여, 전문성의 부족 등의 다양한 문제를 경험하고 있다(이문휘, 2007; 신현석, 2006; 박정양, 2003; 김철용, 2009). 본 연구는 실천현장에서 요양시설조직 내에서 요양시설 사회복지사의 임파워먼트 영향요인인 직무의 특성, 조직의 특성 요인에 대한 인식과 임파워먼트 수준 그리고 직무만족도에 관해서 알아보고 이러한 변인들이 사회인구학적 특성 변인별로 차이가 있는가에 대해서 살펴보고자 한다. 또한 요양시설 사회복지사의 임파워먼트에 미치는 영향력을 분석하고, 직무의 특성, 조직의 특성 그리고 임파워먼트 수준이 직무만족에 미치는 영향을 분석하는데 연구 목적이 있다.

"삼국사기(三國史記)"에 기록된 의약내용(醫藥內容) 분석 (Study on Medical Records In ${\ulcorner}$the Historical Records of the Three Kingdoms${\lrcorner}$)

  • We tried to observe the features of ancient medical practice by analysing the records related to medicine in the book, ${\ulcorner}$the Historical Records of the Three Kingdom${\lrcorner}$ of which content includes the features of medicine in mythology, plague, delivery of twins, drugs, medical system, shamanism, constitutional medicine, psychiatry, forensic medicine, deformity, a spa, medical phrase, health and welfare work, religion, death. physiological anatomy, Taoist medicine, acupuncture, the occult af of transformation and etc. Our initial concern was about where to draw line as of medical field and we defined medicine in more broad meaning. The book ${\ulcorner}$the Historical Records of the Three Kingdoms${\lrcorner}$ describes the world of mythology by way of medicine which is not clearly a conventional one. There appears records of birth of multiple offsprings 7 times in which cases are of triplets or more. Delivering multiple offsprings were rare phenomenon though such fertility was highly admired. This shows one aspect of ancient country having more population meant more power of the nation. Of those medical records conveyed in that book includes stories of childbirth such as giving birth to a son after praying, giving birth to Kim Yoo-shin after 20 months after mother's dream of conception, and a song longing for getting a laudable child. Plagues were prevalent throughout winter to spring season and one can observe various symptoms of plagues in the record. Of these epidemic diseases, cold type might have been more common than the heat one. Appearance of epidemic diseases frequently coincided with that of natural disasters that this suggests a linkage between plague and underlying doctrine on five elements' motion and six kinds of natural factors. There exists only a few names of diseases such as epidemic disease, wind disease, and syndrome characterized by dyspnea. Otherwise there appeared only afflictions that were not specified therefore it remains cluless to keep track of certain diseases of prevalence. Since this ${\ulcorner}$Historical Records of the Three Kingdoms'${\lrcorner}$ wasn't any sort of medical book, words and terms used were not technical kind and most were the ones used generally among lay people. Therefore any mechanisms of the diseases were hardly mentioned. Some of medicinal substances such as Calculus Bovis, Radix Ginseng, Gaboderma Luciderm, magnetitum were also in use in those days. 53 kinds of dietary supplies appears in the records and some of these might have been used as medicinal purpose. Records concerning dicipline of one's body includes activities such as hunting, archery, horseback riding etc. In Shilla dynasty there were positions such as professor of medicine, Naekongbong(內供奉), Kongbong's doctor(供奉醫師), Kongbong's diviner(供奉卜師). As an educational facility, medical school was built at the first year of King Hyoso's reign and it's curricula included various subjects as ${\ulcorner}$Shin Nong's Herbal classic${\lrcorner}$, ${\ulcorner}$Kabeul classic of acupuncture and moxbustion${\lrcorner}$, ${\ulcorner}$The Plain Questions of the Yellow Emperor's Classic of Internal Medicine${\lrcorner}$, ${\ulcorner}$Classic of Acupuncturer${\lrcorner}$, ${\ulcorner}$The Pulse Classic${\lrcorner}$, ${\ulcorner}$Classic of Channels and Acupuncture Points${\lrcorner}$ and ${\ulcorner}$Difficult Classic${\lrcorner}$. There were 2 medical professors who were in charge of education. To establish pharmacopoeia, 2 Shaji(舍知), 6 Sha(史), 2 Jongshaji(從舍知) were appointed. In Baekje dynasty, Department of Herb was maintained. Doing praying for the sake of health, doing phrenology also can be extended to medical arena. Those who survived over 100 years of age appear 3 times in the record, while 98 appears once. The earliest psychiatrist Nokjin differentiated symptoms to apply either therapies using acupuncture and drug or psychotherapy. There appears a case of rape, a case of burying alive with the dead, 8 cases of suicide that can characterize a prototype of forensic medicine. Deformity-related records include phrases as follow: 'there seems protrudent bone behind the head', 'a body which has two heads, two trunks, four arms.', 'a body equipped with two heads' In those times spa can be said to be used as a place for he리ing, convalescence, and relaxation seeing the records describing a person pretended illness and went to spa to enjoy with his friends. Priest doctors and millitary surgeons were in charge of the medical sevice in the period of the Three Kingdoms by the record written by Mookhoja(墨胡子) and Hoonkyeom(訓謙). Poor diet and regimen makes people more vulnerable to diseases. So there existed charity services for those poor people who couldn't live with one's own capacity such as single parents, orphans, the aged people no one to take care and those who are ill. The cause of affliction was frequently coined with human relation. There appeared the phenomenon of releasing prisoners and allowing people to become priests at the time of king's suffering. Besides, as a healing procedure, sutra-chanting was peformed. There appears 10 cases of death related records which varies from death by drowning, or by freezing, death from animals, death from war, death from wightloss and killing oneself at the moment of spouse's death and etc. There also exist certain records which suggest the knowledge of physiology and anatomy in those times. Since the taoist books such as ${\ulcorner}$Book of the Way and Its Power(老子道德經)${\lrcorner}$ were introduced in the period of Three Kingdoms, it can be considered that medicine was also influenced by taoism. Records of higher level of acupuncture, records which links the medicine and occult art of transformation existed. Although limited, we could figure out the medical state of ancient society.
