Factors Associated with Nurses' Activities for Hospital Fall Prevention (간호사의 병원낙상 예방활동에 영향을 미치는 요인)

  • Purpose: The purpose of this study was to identify the factors affected with nurses' prevention activity against hospital fall. Methods: The data were collected from 325 nurses at C University Hospital in G City by using the structured questionnaires from February 21, 2011 to March 12, 2011. The data were analyzed by stepwise multiple regression. Results: The main factor associated with prevention activity against hospital fall was the attitude towards hospital fall (${\beta}$=.26, p<.001), the next one was the educational level (${\beta}$=.16, p=.002), and the last one was the frequency of fall prevention education (${\beta}$=.14, p=.009). The all factors could explain 11.1% of the variance in the nurses' prevention activities against hospital fall. Conclusion: Hospital managers need to make hospital culture to enhance the nurse's positive attitude about hospital fall prevention. In addition, educators need to develop educational programs including hospital fall prevention through academic curriculum and continuing education.

The Effect of the Team-based Cardiac Rehabilitation Program on the Health Behavior Performance, Anxiety, and Quality of Life in the Group of Ischemic Heart Disease with PTCA (팀접근 심장재활 교육이 건강행위 이행, 불안과 삶의 질에 미치는 효과)

  • This study was designed to determine the outcomes of the team-based cardiac rehabilitation programs for patients with Ischemic Heart Disease(IHD) on their level of health behavior performance, anxiety, and quality of life. A total of 19 in-patients with IHD who have had PTCA treatment at G.Hospital in Inchon were selected as subject to data collection from 1 May 1998 to 30 Oct. 1998. The experimental group, consisting of 11 patients participated in the Hospital's cardiac rehabilitation program was compared with the non-rehabilitation control group of 8 patients. The experimental group has been trained on the cardiac rehabliltation programfor 120-150minute during hospitalization by a team consisted of four specialists, those are cardiologist, head nurse, dietian, and the author. Also individual follow-up training and interview were performed for the members of experimental group at their every visit to the Hospital after discharge. For all subjects, scores on health behavior performance, anxiety, and quality of life were collected two days and 12 weeks after PTCA treatment by questionnaire. The collected data were analyzed with the SPSS. The results of this study were summarized as follows : 1) The experimental group shows significant increase in health behavior performance score compared to the control group(-2.27, p=.022). 2) There was no significant difference between two group's anxiety scores (-.24, p= .803) and quality of life scores(-1.86, p= .061). 3) The correlation between health behavior performance and anxiety ( .07, p= .771), and that between health behavior performance and quality of life( .12, p= .621) respectively were not significant statistically. But the correlation between anxiety and quality of life were significant reversely ( .49, p= .032).

The Effects of A Day Care Rehabilitation Program on Physical and Emotional Function of Patients with Job-related Injury (주간재활 프로그램이 산업재해 환자의 신체적, 정서적 기능에 미치는 효과)

  • Purpose: The purpose of this study was to examine the effects of a day care rehabilitation program managed by nurses on physical and emotional function of patients with job-related injuries. Method: A one group pre-test and post-test quasi-experimental design was used. Thirty patients participated in a day care rehabilitation program and 9% of those were unable to complete the 16 weeks program due to absence. The physiotherapist, occupational therapist, and clinical psychologist offered the day care rehabilitation program, 5 times a week for 16 weeks. Outcome measures included physical and emotional function. Results: The program participants had significantly greater motor function (t=-2.85, p=.008) and activity of daily living (t=-5.34, p<.001), and lower depression (t=5.20, p<.001), state anxiety (t=4.71, p<.001), and trait anxiety (t=4.40, p<.001). Conclusion: The nurse managed day care rehabilitation program significantly improved physical and emotional function in patients with job-related injuries. The program should be further tested in a larger sample to validate the findings.

The Difference of the Nurses' Perception on the Importance and Performance of Rehabilitation Nursing Service (산업장 재활간호서비스의 중요도에 대한 간호사의 지각과 수행의 차이)

  • Purpose: The purpose of this study was to compare the rehabilitation nursing services what nurses recognize to be important and what they do in practice of the industrial workers. Method: The subjects were 150 nurses who were working at the industrial hospital. The questionnaire was made up with 43 items about the perception on importance and performance of rehabilitation services. Result: The highest services in the perception on importance and performance subscale were physical and therapeutic services. The highest difference between perception on importance and performance was the social and occupational services. The perception on importance of rehabilitation services had positive correlation with performance. The group take care of general patients had a higher level of perception on importance than the other group, and level of performance had significant difference with age and position. Conclusion: We need to develop the strategy for promoting the performance of services that recognize it will be important services, but not to do. Further study on system for promoting the nurse practitioner who responsible for rehabilitation nursing.

Development of Korean Patient Classification System for Critical Care Nurses (한국형 중환자간호 분류도구 개발)

  • Purpose: This study was performed to develop a valid and reliable Korean Patient Classification System for Critical Care Nurses (KPCSC). Methods: Tertiary and general hospitals with various levels of ICU nurse staffing were included. To verify interrater reliability, data collectors and staff nurses of 15 ICUs in 11 hospitals classified 262 patients. To verify construct validity, the staff nurses classified 457 patients according to KPCSC comparing difference by medical department and type of stay in ICU. For conversion index, 195 patients from 10 ICUs in 7 hospitals were classified and nursing time was measured by 174 nurses, 7 head nurses, 18 charge nurses, 37 nurse aids and 1 secretary. Results: The developed KPCSC has 11 categories, 82 nursing activities and 115 criterias. Reliability was found to have high agreement (r=.96). Construct validity was verified by comparing differences in medical department and type of stay in ICU. According to scores, four groups in the KPCSC were identified. One score on the KPCSC indicates 6.12 minutes of nursing time. Conclusion: The findings show that the KPCSC can be used to measure new and complex nursing demands including rehabilitation and the safety of ICU patients.

A Study on the Needs and Understanding about the Home Physical Therapy System (가정 물리치료제도에 대한 인식 및 필요에 대한 조사연구 -가정간호수습생을 대상으로-)

  • The purpose of this study was to investigate the need and understanding about the physical therapy. The subjects of this study were 101 licensee nurses who was taking the home nursing trainers. The results were as follows: 1. The home nursing trainers(97.1%) were required the need for home physical therapy service. The reasons which requires the home physical therapy were 'the increased elderly patients(97.0%)', 'the early leaving of the hospital(96.0.%)', 'the lack of caretakers(96.0%)', and 'the increased chronic patients(96.0%)'. 2. The ratio of affirmation of home physical therapy system was 97% of the home nursing trainers, the reasons which were helped the patients was 'the saved time(97.0%)'. 'the decreased psychological stress(95.0%)' 'the continuing treatment after discharge(93.0%)' and 'the teaching patients and caretakers(93.0%)'. 3. The main center which was organized the home physical therapy service was 'the department of home physical therapy arid the home nurse(42.6%)', 'the department of home nurse(22.8%)', and 'the department of home physical therapy(1l.9%)'. 4. The home nursing trainers wanted to learn the home physical therapy methods for chronic patients(95.0%). The contents of home physical therapy methods was 'the therapeutic exercise for preventing deformity(15.4%)', and 'maintenance of normal posture and howe to change body position(12.4%)'. 5. Statistically significant difference in the teams (the home physical therapy, the home nurse, and the home physical therapy with the home nurse) was found between the age groups, and statistically significant difference in the teams was found between the educational degree groups.

The Effects of Rehabilitation Hospital Nurses' Professional Autonomy and Self-Efficacy on Professional Self-Concept (재활전문병원 간호사의 전문직 자율성과 자기효능감이 전문직 자아개념에 미치는 영향요인)

  • The aim of this descriptive correlation research was determine the factors affecting the professional self-concept of nurses working at rehabilitation hospitals. The data were collected from 192 nurses working at three rehabilitation hospitals with less than 300 beds in B City and at two in D City from May 1 to 30, 2015, and then analyzed using the SPSS Win 18.0 program. They scored 3.03 for professional autonomy, 3.43 for self-efficacy, and 3.25 for professional self-concept on average; professional autonomy was positively correlated with the professional self-concept and self-efficacy was strongly positively correlated with the professional self-concept. The professional self-concept generally accounted for 46.9% and the self-efficacy had the greatest impact on the professional self-concept, accounting for 33%. Based on these results, it is necessary to help rehabilitation hospital nurses improve their self-efficacy and develop standardized professional education programs to give them pride and dignity.

Influence of Job Stress and Professional Self-concept on Job Satisfaction among Nurses in Rehabilitation Units (재활병동 간호사의 직무 스트레스와 전문직 자아개념이 직무만족에 미치는 영향)

  • Purpose: The purpose of this study was to define the relationships between job stress, professional self-concept, and job satisfaction among nurses in rehabilitation units and to identify factors that influence job satisfaction. Methods: This study surveyed 240 nurses working at the rehabilitation units of 10 hospitals located in G City. To collect the data, nurses' job stress, the professional self-concept of nurses instrument (PSCNI), and the Korean-Minnesota Satisfaction Questionnaire (K-MSQ) were used. The data were analyzed through t-tests, ANOVA, Pearson's correlation coefficient, and multiple regression using the SPSS/WIN 18.0 program. Results: The scores for job stress, professional self-concept, and job satisfaction were $3.34{\pm}0.61$, $72.36{\pm}8.64$ and $3.17{\pm}0.39$, respectively. Job satisfaction and job stress showed a low-level negative correlation with statistical significance (r=-.13, p=.042), whereas a medium-level positive correlation was found between job satisfaction and professional self-concept (r=.46, p<.001). Job stress, professional self-concept, total career length and size of working hospital explained approximately 25.8% of job satisfaction (F=17.52, p<.001). Conclusion: Considering the results in this study, it is necessary to develop a practical educational program to establish positive professional self-concept for nurses in rehabilitation units. Such efforts must be supported by clinical nursing administrations through policies aimed at reducing job stress and increasing job satisfaction.

Effects of the Nurse-led Discharge Education on Symptom Experience and Self-Care Compliance in Patients with Chronic Obstructive Pulmonary Disease (일대일 퇴원교육이 만성폐쇄성 폐질환자의 증상경험 및 자가간호수행에 미치는 효과)

  • Purpose: This study was conducted to examine the effects of the nurse-led one-on-one discharge education on the levels of symptom experience and self-care compliance among patients with chronic obstructive pulmonary disease (COPD) who had not previously participated in an education or respiratory rehabilitation program. Methods: This study used a quasi-experimental pre-and post-test design with nonequivalent control group. Sixty seven hospitalized patients (34 for experimental and 33 for control groups) were recruited from in-patient units of a general hospital from August to November, 2010. The one-on-one education session of symptom management and daily self-care was administered to experimental group for 50 minutes along with a follow-up phone call. Self-administered questionnaire was administered at the pretest and at 1 month after discharge. Results: The subjects were 67.43(${\pm}10.24$)years old in average. The mean differences of symptom experience (t=3.39, p=.001) and self-care compliance (t=-38.13, p<.001) in the experimental group was significantly higher than those of the control group. Conclusion: Nurse-led one-on-one discharge education was effective in reducing the level of symptom experience and enhancing self-care compliance at home within one month after discharge. Therefore, this form of one-on-one education provided by nurses might be applicable effectively for COPD patients who do not have access to respiratory rehabilitation programs.

Effects of a Discharge Education Program using Computerized Animation Video for Post-operative Colon Cancer Patients (대장암 수술 후 퇴원 환자에게 적용한 컴퓨터 기반 애니메이션 동영상 교육의 효과)

  • Purpose: This study was to identify the effects of a nurse-led education program using computerized animation video for post-operative colon cancer patients. Methods: a total of 163 patients and 51 nurses were participated in this study. With a non-equivalent control group post-test design, patients were divided into three groups (77 got traditional education, 46 were applying brochure, 40 were watching video). Twelve-item animation video and brochure about the management after discharge for post-operative colon cancer patients were developed based on patient survey and the items of Korea Healthcare Accreditation. Results: The computerized video watching group had better satisfaction than the others, but there was no significant difference about comprehension. When video was applied, satisfaction, usefulness, application, and perceived patients' comprehension of nurses were all increased. Conclusion: This video education program was developed by nurses and it had a special thing for patient to access the same program even after discharge using the authorization system. It would be helpful for nurses to be more concentrated on the direct care for hospitalized patients as well as for patients to provide self-care at home. This program would be adjusted into more various diseases and settings.
