Infectious Cholecystitis and Concurrent Endocarditis in a Dog: Rare but Important Association (개에서 감염성 담낭염과 동시 발병한 감염성 심내막염 1례)

  • A 12-year-old castrated male miniature Schnauzer had anorexia, vomiting, lethargy, and fever for four days. The dog had leukocytosis, thrombocytopenia, hypoglycemia, hypoalbuminemia, hyperbilirubinemia, increased hepatic enzymes on hematologic and biochemical examinations. On abdominal ultrasonographs, there was an enlarged gallbladder with hyperechoic irregular wall thickening. Echocardiography showed vegetative change and regurgitation of aortic and mitral valves. E.coli was identified through ultrasound guided percutaneous cholecystocentesis and bile culture. Bacterial cholecystitis and concurrent endocarditis were diagnosed. The dog was recovered from sepsis with antibacterial and aggressive supportive therapy.

Renal Amyloidosis in a Beagle (비글견에서 신장 아밀로이드증 증례)

  • An eight-year-old intact male Beagle had anorexia, vomiting, depression for two days. The dog had hypoalbuminenia and mild azotemia on hematologic and clinical chemistry examinations. Severe proteinuria was identified on urinalysis. On abdominal ultrasonographs, there were small amount of ascites and bilateral renomegaly with severe hyperechoic renal cortex and hyperechoic medullary rim sign. Renal biopsy and histopathology revealed renal amyloidosis. The quality of life in this dog was satisfactory with aggressive supportive care for three months. Euthanasia was performed due to deteriorated azotemia and nephrotic syndrome.

  • Supernumerary teeth that are present in the molar region may be evident based on crowding and impaction, but most cases are asymptomatic and discovered as incidental findings during routine radiological examinations. This article reports the case of a 29-year-old woman who presented with a severe feeling of pressure in the region of the maxillary third molars that had been increasing in intensity for weeks. A clinical examination revealed crowding of the maxillary anterior teeth despite the completion of orthodontic treatment and an erupted third molar with localized gingivitis in the second quadrant. A radiographic examination revealed bilateral supernumerary maxillary fourth and fifth molars, so cone-beam computed tomography was performed to locate the supernumerary teeth precisely for a preoperative diagnosis and comprehensive treatment planning. This report presents the radiological and surgical case management of a rare case of bilateral supernumerary molars and reviews the literature regarding epidemiology and treatment options.

Application of the Orally Inserted Guiding Device for the Improved Motion Artifacts of the Cervical Spine MRI (경추부 자기공명영상의 움직임 인공물 개선을 위한 구강내 삽입 유도 기구의 적용)

  • This study aims to suggest and test methods using an orally inserted guiding device in order to improve a motion artifact by involuntary oral motor such as removing one's dentures and swallowing saliva clinically structured cervical spine scan and to make the optimal image by minimizing motion artifact. A cervical spine test was conducted with 30 patients who wore dentures among those who had a cervical spinal disease from January 1, 2014 through June 30, 2014. As for testing methods, after removing denture, T1-TSE-Sagittal, T2-TSE-Sagittal, T1-TSE-Axial and T2-TSE-Axial were obtained in a normal position and a supine position; the orally inserted guiding device was inserted in patients' mouth; and then T1-TSE-Axial and T2-TSE-Axial were retested. As a result, in SNR, T1-TSE-Axial before inserting an orally inserted guiding device was $22.33{\pm}8.59$; T1-TSE-Axial after inserting the orally inserted guiding device was $25.21{\pm}7.93$; T2-TSE-Axial before inserting the orally inserted guiding device was $14.49{\pm}5.74$; and T2-TSE-Axial after inserting the orally inserted guiding device was $16.61{\pm}6.72$. In CNR, T1-TSE-Axial was measured at $0.23{\pm}0.01$ while T2-TSE-Axial at $0.21{\pm}0.01$. As a result of the qualitative analysis, T1-TSE-Axial before inserting the orally inserted guiding device was $3.49{\pm}0.11$; T1-TSE-Axial after inserting the orally inserted guiding device was $3.95{\pm}0.14$; T2-TSE-Axial before inserting the orally inserted guiding device was $3.25{\pm}0.18$; and T2-TSE-Axial after inserting the orally inserted guiding device was $3.68{\pm}0.09$. As a result of using an orally inserted guiding device, the resolution and contrast of the images improved as the patients' involuntary artifact decreased because of removing dentures and swallowing saliva, and it was found that the interpretation of the images and identification of the diseases improved.

The Optimum of Respiratory Phase Using the Motion Range of the Diaphragm: Focus on Respiratory Gated Radiotherapy of Lung Cancer (횡격막의 움직임을 이용한 최적화된 호흡 위상의 선택: 폐암의 호흡 동기 방사선치료 중심)

  • This study was to analyze quantitatively movement of planning target volume (PTV) and change of PTV volume through movement of diaphragm according to breathing phase. The purpose of present study was to investigate optimized respiration phase for radiation therapy of lung cancer. Simulated breathing training was performed in order to minimize systematic errors which is caused non-specific or irregular breathing. We performed 4-dimensional computed tomography (4DCTi) in accordance with each respiratory phase in the normalized respiratory gated radiation therapy procedures, then not only defined PTVi in 0 ~ 90%, 30 ~ 70% and 40 ~ 60% in the reconstructed 4DCTi images but analyzed quantitatively movement and changes of volume in PTVi. As a results, average respiratory cycle was $3.4{\pm}0.5$ seconds by simulated breathing training. R2-value which is expressed as concordance between clinically induced expected value and actual measured value, was almost 1. There was a statistically significant. And also movement of PTVi according to each respiration phase 0 ~ 90%, 30 ~ 70% and 40 ~ 60% were $13.4{\pm}6.4mm$, $6.1{\pm}2.9mm$ and $4.0{\pm}2.1mm$ respectively. Change of volume in PTVi of respiration phase 30 ~ 70% was decreased by $32.6{\pm}8.7%$ and 40 ~ 60% was decreased by $41.6{\pm}6.2%$. In conclusion, PTVi movement and volume change was reduced, when we apply a short breathing phase (40 ~ 60%: 30% duty cycle) range. Furthermore, PTVi margin considered respiration was not only within 4mm but able to get uniformity of dose.

Research on the Actual Condition of the radiation Safety Management(RSM) for the Educated Training of the Dental Diagnostics X-ray Generators (교육용 치과 엑스선 발생장치에 대한 방사선 안전 관리 실태 조사)

  • The purpose of this study is to offer data base for establishment of dental training x-ray generator based safety usage through surveying real radiation safety management state of radiation worker's in plan of operations that have dental training x-ray generators and use it. For it, comprehensive references were surveyed referring reports of current state of regulation technique development and domestic radiation safety evaluation and nuclear related legislation regarding radiation safety management of dental training x-ray generators. On the basis of it, questionnaires were filled in about respondent's general characteristic radiation safety manager's status current state of radiation safety management and the level of knowledge & consciousness. For the study, the survey was conducted to 224 people of radiation safety managers and university graduates training assistants and full-time professors who can treat dental training x-ray generators in education center. through this survey 95 questionnaires were used as analysis materials except the insufficient and omitted responses. As a method of analysis, the frequency and percentage were figured out with the general characteristics and safety manager's status. Chi-square test for frequency and correlation per question analysis and Pearson correlation analysis for crosslevel correlation were done with current state of radiation safety management and knowledge & consciousness level. As a result, running dental training x-ray generators was dealt with by 20's to 40's who have high education level over post undergraduate degree and major in dental hygienic. In addition, female have higher consciousness level for radiation safety management than male. A Study on the Measurement of the Pollution Level of Bacteria and Disinfection of Table and IP Cassette (촬영 테이블과 IP Cassette의 세균 오염도 측정 및 소독에 관한 연구)

  • Purpose : For the number of microbes and the pollution level of bacteriology of IP Cassette and Table by laboratories, after identifying the bacteria before and after using alcohol and tissue including disinfectant and statistically testing, this research was intended to provide the basic data for the prevention and the right disinfection guideline for infection management of hospitals in diagnostic radiology. Subject and Method : The subject of this research was the general room of diagnostic radiology of a university hospital in Daejeon City. The research was conducted from Apr 5 to Apr 12, 2007. The number of microbes and the pollution level of bacteriology of IP Cassette and Table by laboratories were tested before and after using alcohol and tissue for disinfection including disinfectant. In order to collect specimens exactly, they were collected with the nurse who specialized in infection management of the hospital, and statistical processing was done with SPSS V13.0. To compare the results before and after using alcohol and tissue, T-test was implemented, and post-hoc test was conducted. Results : Bacteria were detected in 19 cases of 24 subjects(79.2%), however, they were not detected in 5 cases(20.8%). 7kinds of bacteria were detected as isolated bacteria, of which Methicillin Resistant coagulase-negative Staphylococci(MRCNS) were detected in 15 cases(62.5%), which was most, Methicillin Resistant Staphylococcus Aureus(MRSA) in 6 cases(16.7%), Enterococcus Faecium(EFM) in 5 cases(20.8%), Acinetobacter baumannii(ABA) in 2 cases(8.3%), and Bacillus sp, Coagulase-negative Staphylococci(CNS), and Enterococcus sp(ENT) in 1 case, respectively(4.2%). In all bacteria except ABA, Gram positive bacillia were detected in 30 cases(97%), and Gram negative bacilli were detected only in 1 case(3%). As for the kinds of bacteria and the number of groups before and after using 70% Alcohol by Groups, when the bacteria were identified after disinfecting IP Cassette and Table with 70% Alcohol, all the bacteria became extinct in both IP Cassette and Table Group(100%). As for the kinds and number of bacteria before and after using Tissue Cleaner by Groups, the bacteria in Tissue Cleaner Group became completely extinct only in 10 Groups(71.2%), and in 4 Groups, there was much decrease in bacteria, however, they were still detected. The extinction rate of all the bacteria was 91.5%. That is, though the other bacteria became extinct(100%), that of MRCNS bacteria was lowest(83.6%), followed by MRSA(95%). Conclusion : As a result of comparing the mean of the bacteria which were detected before and after using 70% Alcohol and Tissue Cleaner, there was statistically significant in the significant level of 5% in both of them.

Evaluation of the Usefulness of Assist Device for Rosenberg View Test (Rosenberg View 검사를 위한 보조기구의 유용성 평가)

  • Due to the nature of the Rosenberg Method, the patient needs to maintain posture for a certain period of time, and the joint space is observed in various forms depending on the angle of knee flexion, which causes difficulties in examination. In order to solve these problems, Image quality was evaluated in order to evaluate the usefulness of the assistive device by making the assistive device itself. SNR and CNR analysis about the presence or absence of an assistive device using the extremity phantom and the angle of the assistive device itself were not statistically significant(p < 0.05). As a result of measuring the distance between the right and left edges of the medial condyle based on the presence or absence of an assist device, and the absence of assist device (96.00±40.6 mm) and presence of an assist device (134.86±17.68 mm) were statistically significant (p <0.05). To find the aLDFA relationship about the femur and tibia, we measured the right and left aLDFA based on the presence or absence of assist device. As a result, the absence of the right-side aLDFA assist device (74.63°±4.87) and the presence of assist device (79.64°±3.65) were statistically significant (p <0.05). The absence of the left-side aLDFA assist device (76.39°±4.62) and the presence of assist device (79.64°±3.65) were statistically significant (p < 0.05). but, As a result of measuring the distance of the overlapping parts of the right and left proximal tibiofibular joint and the lateral condyle, There were no statistically significant differences between the right and left sides. In conclusion, we confirmed that we can obtain Diagnostically valuable images with a constant knee-to-knee spacing using an assist device we self-created. In addition, we could learn through aLDFA relationship between femur and tibial that the smaller the angle, the more medial condyle overlaps with JSW, We also confirmed the significance by deriving similar values on the normal range values of aLDFA using assist devices. However, it is considered necessary to pay attention to internal and external rotations in order to obtain good quality images by evaluating the distance of overlapping parts between proximal tibiofibular joint and lateral condyle.

Utility Evaluation of Supportive Devices for Interventional Lower Extremity Angiography (인터벤션 하지 혈관조영검사를 위한 보조기구의 유용성 평가)

  • The purpose of this study is to evaluate the effectiveness of supportive devices which are for minimizing the patient's movement during lower extremity angiography and to verify image quality of phantom by analyzing of Mask image, DSA image and Roadmap image into SNR and CNR. As a result of comparing SNR with CNR of mask image obtained by DSA technique using the phantom alone and phantom placed on the supportive devices, there was no significant difference between about 0~0.06 for SNR and about 0~0.003 for CNR. The study showed about 0.11~0.35 for SNR and 0.016~0.031 for CNR of DSA imaging by DSA technique about only water phantom of the blood vessel model and the water phantom placed on the device. Analyzing SNR and CNR of Roadmap technique about water phantom on the auxiliary device (hardboard paper, pomax, polycarbonate, acrylic) and water phantom alone, there was no significant difference between 0.02~0.05 for SNR and 0.002~0.004 for CNR. In conclusion, there was no significant difference on image quality by using supportive devices made by hardboard paper, pomax, polycarbonate or acryl regardless of whether using supportive devices or not. Supportive devices to minimize of the patient's movement may reduce the total amount of contrast, exam-time, radiation exposure and eliminate risk factors during angiogram. Supportive devices made by hardboard paper can be applied easily during angiogram due to advantages of reasonable price and simple processing. It is considered that will be useful to consider cost efficiency and types of materials and their properties in accordance with purpose and method of the study when the operator makes and uses supportive devices.

Evaluation of MTF Image by Target/Filter Combined of X-ray Tube Using Mammography (유방촬영용 X선관 target/filter 조합에 따른 MTF영상평가에 관한 고찰)

  • It is important to consider the contrast of object in Mammography because an absorption gap between tissues of body and breast in breast is low. This study is to evaluate MTF image with resolution chart according to change of combination of target and filter. The results were as follows : 1. There were significant differences in X-ray energy according to combination of filter(Mo/Mo, Mo/Rh. Mo/Al, Rh/Rh, Rh/Al) and acrylic thickness(2 cm, 3 cm, 4 cm). 2. The value of lp/mm on MTF to 0.5 showed that the sharpness in MTF curve was 2.4 compared to Mo/Mo and 2cm acryl, 2.63 in Mo/Rh and 4 cm acryl, and 2.9 in Rh/Rh and 6cm acryl. 3. The value of lp/mm on MTF showed that the resolution in MTF curve was 6.0 compared to Mo/Mo and 2 cm acryl, 4.60 in Rh/Al and 4cm acryl, and 6.03 in Rh/Al and 6 cm acryl. 4. The value of MTF on 2.5 lp/mm distinguishable visually was 0.48 compared to Mo/Mo and 2 cm acryl, 0.53 in Mo/Rh and 4cm acryl, and 0.59 in Rh/Rh and 6cm acryl. 5. For the evaluation of an image of the mammo-phantom, the score of Mo/Mo was 12 points, Mo/Rh 11, Rh/Rh 10.5, Mo/Al 10, Rh/Al 9.0, respectively.
