Breast Magnetic Resonance Imaging Indications in Current Practice

  • Although mammography is the primary imaging modality for the breast, it has its limitations especially with dense breast parenchyma. Breast magnetic resonance imaging (MRI) has evolved into an important adjunctive tool as it is currently the most sensitive technique for breast cancer detection. Despite this high sensitivity, overlap in the appearances of some benign and malignant breast lesions results in additional unnecessary intervention with negative results. These false positives, in addition to high cost and limited availability, necessitate establishing proper indications for breast MRI. The literature was here reviewed for recent clinical trials, meta-analyses and review papers which have studied this important subject. PubMed; the US national library of medicine, was utilized to review the literature in the last twenty years. Using the obtained information, current uses of breast MRI are discussed in this paper to determine the indications which are relevant to clinical practice.

Multiple intraosseous cervical pneumatocysts: A case report of a rare incidental finding on cone-beam computed tomography

  • This report presents a case of cervical pneumatocysts as an incidental finding on cone-beam computed tomography. Pneumatocysts are gas-containing lesions of unknown etiology. They usually present in the ilium or sacrum, adjacent to the sacroiliac joint. In the literature, 21 case reports have described cervical pneumatocysts. Cervical pneumatocysts should be differentiated from other lesions, such as osteomyelitis, osteonecrosis, and neoplasms, as well as post-traumatic and post-surgical cases. Computed tomography, cone-beam computed tomography, and magnetic resonance imaging are appropriate tools to diagnose cervical pneumatocysts.

방사선작업종사자 및 방사선관계종사자의 현황 분석과 교내 실습 만족도 조사를 통한 방사선(학)과의 규제에 대한 고찰 (A Study on Regulations Through Analysis of the Status of Radiation Workers and Related Workers and Satisfaction Survey in the Radiology Department)

  • 본 연구에서는 방사선(학)과 재학 중 교내 실습의 만족도에 대한 설문과 방사선관계종사자 및 방사선작업종사자의 현황 등을 조사해 방사선(학)과에 대한 원자력안전법의 규제에 대한 실효성 및 타당성에 대한 기초 연구에 목적을 두었다. 방사선(학)과 재학 중 수시출입자로 지정되어 교내 실습 중 방사선 발생장치를 취급 및 조작하지 못한 종사자의 실습 만족도는 만족하지 못한다가 34.62%로 나타났다. 반면 재학 중 방사선작업종사자로 지정되거나 원자력안전법의 규제 이전에 재학하여 방사선 발생장치를 취급 및 조작한 종사자의 실습 만족도는 만족한다가 50%로 나타났다. 또한 교육기관의 방사선작업종사자의 연간 피폭선량은 0.05 mSv 이하로 나타났다. 방사선작업종사자와 방사선관계종사자의 추이를 보면 방사선(학)과를 졸업한 학생들은 의료기관 중에서도 방사선관계종사자로 등록된 진단용 방사선 발생장치를 취급하는 분야로 가장 많은 취업을 한다는 것을 알 수 있다. 따라서 현재의 원자력안전법의 규제를 완화하거나 의료법 및 진단용 방사선 발생장치의 안전관리에 관한 규칙 등을 개정하여 의료기사 등에 관한 법률에서 정한 방사선(학)과 학생들의 학습권을 보장하고 실무 중심의 교육이 이루어질 수 있도록 하는 것이 필요하다고 사료된다.

  • Computed tomography (CT) plays an important supplementary role in the evaluation of patients with heart disease. CT can be used to evaluate the aorta, pulmonary artery, pulmonary vein, cardiac chambers, coronary artery, valves and systemic veins (superior vena cava, inferior vena cava and hepatic veins). The "Learning Objectives" describe the normal anatomy and typical pathological conditions seen on axial scans and reformatted images from CT in patients with heart disease, focusing focus on frequent, fatal, and rare but characteristic diseases encountered in routine practice.

  • In orthodontics and orthognathic surgery. cephalogram has been routine practice in diagnosis and treatment evaluation of craniofacial deformity. But its inherent distortion of actual length and angles during projecting three dimensional object to two dimensional plane might cause errors in quantitative analysis of shape and size. Therefore, it is desirable that three dimensional object is diagnosed and evaluated three dimensionally and three dimensional CT image is best for three dimensional analysis. Development of clinic necessitates evaluation of result of treatment and comparison before and after surgery. It is desirable that patient that was diagnosed and planned by three dimensional computed tomography before surgery is evaluated by three dimensional computed tomography after surgery. too. But Because there is no standardized normal values in three dimension now and three dimensional Computed Tomography needs expensive equipments and because of its expenses and amount of exposure to radiation. limitations still remain to be solved in its application to routine practice. If postoperative three dimensional image is constructed by pre and postoperative lateral and postero-anterior cephalograms and preoperative three dimensional computed tomogram. pre and postoperative image will be compared and evaluated three dimensionally without three dimensional computed tomography after surgery and that will contribute to standardize normal values in three dimension. This study introduced new method that computer-simulated three dimensional image was constructed by preoperative three dimensional computed tomogram and pre and postoperative lateral and postero-anterior cephalograms. and for validation of new method. in four cases of dry skull that position of mandible was displaced and four patients of orthognathic surgery. computer-simulated three dimensional image and actual postoperative three dimensional image were compared. The results were as follows. 1. In four cases of dry skull that position of mandible was displaced. range of displacement between computer-simulated three dimensional images and actual postoperative three dimensional images in co-ordinates values was from -1.8 mm to 1.8 mm and 94% in displacement of all co-ordinates values was from -1.0 mm to 1.0 mm and no significant difference between computer-simulated three dimensional images and actual postoperative three dimensional images was noticed(p>0.05). 2. In four cases of orthognathic surgery patients, range of displacement between computer­simulated three dimensional images and actual postoperative three dimensional images in coordinates values was from -6.7 mm to 7.7 mm and 90% in displacement of all co-ordinates values was from -4.0 to 4.0 mm and no significant difference between computer-simulated three dimensional images and actual postoperative three dimensional images was noticed(p>0.05). Conclusively. computer-simulated three dimensional image was constructed by preoperative three dimensional computed tomogram and pre and postoperative lateral and postero-anterior cephalograms. Therefore. potentiality that can construct postoperative three dimensional image without three dimensional computed tomography after surgery was presented.

Role of Balloon Guide Catheter in Modern Endovascular Thrombectomy

  • Proximal flow control achieved with a balloon guide catheter (BGC) during endovascular treatment of acute ischemic stroke is reviewed in this article. In clinical practice, BGCs offer a multi-faceted approach for clot retrieval by creating proximal flow arrest, reducing embolic burden, and shortening procedure time. Evaluation of frontline thrombectomy procedures with BGCs revealed advantages of combined use over the conventional guide catheter (CGC), notably in the significant reduction of distal emboli to both the affected and previously unaffected territories. Recently, new measures of early and complete reperfusion at first thrombectomy pass have been identified as independent predictors of improved outcomes, which were consistently demonstrated with use of BGC as a safe and effective option to minimize number of passes during intervention. Prior randomized controlled trials reported the positive correlation between BGC-treated patients and a lower risk of mortality as well as shortened procedure time. While BGC use is more common in stent retriever-mediated mechanical thrombectomy, preliminary data has shown the potential benefit of device application during contact aspiration thrombectomy to achieve successful recanalization. However, the question of which major endovascular strategy reigns superior as a frontline remains to be answered. Along with clinical case assessments, BGC performance during in-vitro simulation was analyzed to further understand mechanisms for optimization of thrombectomy technique.

정상인의 Gastric Emptying Rate 측정 (Measurement of Normal Gastric Emptying Rate Using Chicken Liver Labeled with $^{99m}Tc-Colloid$)

  • Measurement of gastric emptying rate has been performed by a variety of techniques. Nuclear medicine method is a major advance in the quantitative evaluation of gastric function and also of pharmacologic intervention. Normal gastric emptying rate was measured in 48 healthy volunteers using live chicken liver labeled with $^{99m}Tc-Tin$ Colloid as a solid phase marker. In vitro studies were performed to evaluate the labeling efficiency and stability in hydrochloric acid and in human gastric juice of intracellularly labeled chicken liver. Anterior image counts only were compared with the geometric mean of anterior and posterior counts in 12 healthy volunteers who were studied by both anterior count and posterior count. The results were as follows: 1) The labeling efficiency in gastric juice and hydrochloric acid were $95.5{\pm}1.23%,\;95.7{\pm}1.15%$, respectively. 2) Half gastric emptying time by anterior count only was $126{\pm}21$ minutes 3) Although standard deviation of geometric mean method was smaller than anterior count method, gastric emptying curves from both method were similar. In daily practice, anterior count method may be useful alternative to geometric mean method in evaluation of gastric emptying rate.

방사선(학)과 학생들의 전공만족도분석 (An Analysis on the Satisfaction of Radiology Students with their Major)

  • 방사선(학)과 학생들의 전공만족의 요인을 파악하고 효율적인 교육과 진로 교육 등의 기초자료를 제공하고자 본 연구를 수행하였다. 그 결과, 교수의 역할에 대한 만족도는 높았으나, 전반적으로 교과과정에 대한 수치는 낫게 나타났으며, 자기주도 학습의 인식도와 실천도의 사이의 차이가 높은 것으로 분석되었다. 또한 상관관계분석결과에서도 교수의 역할이 매우 중요한 요인으로 작용함을 알 수 있었다. 이에 본 연구를 토대로 교수의 역할(수업 및 국시준비지도, 학생들의 질문 등)을 중심으로 방사선(학)과 학생들의 전공만족을 향상시킨다면, 학과 및 학교 적응에 도움을 줄 수 있으며, 향후 전문직업인으로서의 긍정적인 인성이 형성될 것으로 사료된다.

Insights into Smoking and its Cessation among Current Smokers in India

  • Background: Initiation, perpetuation and cessation of smoking are all multifactorial. It is essential to explore interactions among various parameters influencing smoking and its cessation for effective smoking cessation interventions. Objectives: To obtain insights into smoking and its cessation among current smokers in India. Materials and Methods: The present study was conducted among current smokers visiting the Department of Oral Medicine and Radiology, Manipal College of Dental Sciences (MCODS), Manipal University, Mangalore. Knowledge, attitudes, behavior, worksite practices towards smoking and its cessation, barriers to smoking cessation and socio-demographic variables were explored using a structured, pretested, self-administered questionnaire. Results: A total of 175 current smokers participated in the study. Mean knowledge, attitude, worksite practice and barrier scores were $15.2{\pm}5.67$ (66.1%), $57.5{\pm}7.67$ (82.1%), $4.18{\pm}2.02$ (41.8%) and $57.4{\pm}12.37$ (63.7%) respectively. Correlation analysis revealed: association of knowledge with education, occupation and religion; attitude with education and occupation; worksite practices with occupation; knowledge with attitude; and barriers negatively with worksite practices. The majority (85.7%) of respondents intended to quit smoking and this was associated with higher attitude scores, whereas actual quit attempts were associated with high knowledge, attitudes, worksite practices and low barrier scores. Conclusions: Various socio-demographic factors associated with smoking and its cessation were identified. The present study highlights the importance of identifying and targeting these interactions while framing guidelines and interventions for effective tobacco cessation in a developing country like India.

일반 방사선검사의 소요 시간 실태조사 (Investigation of the Time Required for General Radiography)

  • In this study, by analyzing the examination time for each procedure, the appropriate workload of radiologic technologist is analyzed based on the actual examination time in the current clinical setting by comparing with the examination time in the radiology field setting of the health insurance review and assessment service. In addition, this result is introduced into the calculation of relate value units; it was attempted to provide accurate and objective evidence in the field of radiology. From May 2020 to December 2021, the study retrospectively investigated the examination times recorded in the electronic medical record and picture archiving and communication system at 5 tertiary general hospitals and 1 general hospital. The total of 16 examination parts are applied in this study, including the head, sinuses, chest, ribs, abdomen, pelvis, cervical, thoracic, lumbar, shoulder, elbow, wrist, hip, femur, knee, and ankle. The minimum number of images that could be obtained per radiation generator was 3.6 images for one hour, and the maximum was 6.4 images. When 50% median of procedure time is calculated, the minimum number of images that could be obtained was 16.7 images and maximum was 35.3 images; in addition, minimum examination time is 1.7 minutes, and maximum time is 3.6 minutes. In conclusion, it is judged that there will be insufficient explanation time for basic infection instructions such as hand hygiene during the examinations in current clinical practice. It is believed that radiologic technologists will contribute to providing higher-quality of radiation examination services to the public by complying with guidelines for work and setting appropriate workload on their own.
