Acute Acquired Metabolic Encephalopathy Based on Diffusion MRI

  • Metabolic encephalopathy is a critical condition that can be challenging to diagnose. Imaging provides early clues to confirm clinical suspicions and plays an important role in the diagnosis, assessment of the response to therapy, and prognosis prediction. Diffusion-weighted imaging is a sensitive technique used to evaluate metabolic encephalopathy at an early stage. Metabolic encephalopathies often involve the deep regions of the gray matter because they have high energy requirements and are susceptible to metabolic disturbances. Understanding the imaging patterns of various metabolic encephalopathies can help narrow the differential diagnosis and improve the prognosis of patients by initiating proper treatment regimen early.

Small Solitary Ground-Glass Nodule on CT as an Initial Manifestation of Coronavirus Disease 2019 (COVID-19) Pneumonia

  • The 2019 novel coronavirus (2019-nCoV) outbreak in Wuhan, Hubei Province, China in 2019 led to large numbers of people being infected and developing atypical pneumonia (coronavirus disease 2019, COVID-19). Typical imaging manifestations of patients infected with 2019-nCoV has been reported, but we encountered an atypical radiological manifestation on baseline computed tomography (CT) images in three patients from Wuhan, China infected with the 2019-nCoV. Surprisingly, the only similar CT finding was a solitary sub-centimeter ground-glass nodule adjacent to bronchovascular bundles, which could be easily overlooked. In addition, the follow-up images in these patients showed how COVID-19 pneumonia evolved from these small nodules. The radiologic manifestation of the three cases will expand contemporary understanding of COVID-19.

  • 소아와 청소년에서 서혜부와 음낭 질환들은 비교적 흔하며, 영상은 이들 질환의 진단과 감별진단에 매우 유용하다. 때문에 이 질환들의 영상 소견을 알고 있는 것이 중요하다. 이 논문에서는 이 질환들을 크기가 작은 고환, 잠복고환, 남아있는 초상 돌기, 급성 고환통, 외상, 종양, 그 외로 분류하고 이 질환 들의 특징적인 소견에 대해 기술하고자 한다.

  • 수막종은 중추신경계에서 흔한 종양이지만, 부비동, 특히 상악동에 위치한 일차성 두개외 수막종은 매우 드물다. 본 연구에서는 상악동에서 발생한 일차성 섬유질형 수막종의 증례를 보고하고, 문헌의 섬유질형 수막종의 영상 소견과 함께 고찰하고자 한다.

  • 후복막강 출혈은 항응고 치료의 치명적 합병증일 수 있으며, 나이 및 산과력과 상관없이 난소동맥이 출혈 병소일 수 있다. 저자들은 폐경기 여성에서 항응고 치료 중에 발생한 난소동맥 출혈을 경도관 혈관조영술 및 색전술을 통해 정확한 진단 및 효과적인 지혈이 가능했던 증례를 소개하고자 한다.

  • Nonalcoholic fatty liver disease, characterized by excessive accumulation of fat in the liver, is the most common chronic liver disease worldwide. The current standard for the detection of hepatic steatosis is liver biopsy; however, it is limited by invasiveness and sampling errors. Accordingly, MR spectroscopy and proton density fat fraction obtained with MRI have been accepted as non-invasive modalities for quantifying hepatic steatosis. Recently, various quantitative ultrasonography techniques have been developed and validated for the quantification of hepatic steatosis. These techniques measure various acoustic parameters, including attenuation coefficient, backscatter coefficient and speckle statistics, speed of sound, and shear wave elastography metrics. In this article, we introduce several representative quantitative ultrasonography techniques and their diagnostic value for the detection of hepatic steatosis.

  • 심낭오목은 흉부 컴퓨터단층촬영을 받는 환자에서 종종 확인이 된다. 심낭은 종종 정상 변이나 질환과 오인될 수 있어 심낭의 정상 해부학 구조를 알고 있는 것이 중요하다. 그러므로 이 논문에서는 심낭오목의 해부학 및 위치 그리고 심낭오목과 관련된 다양한 질환에서 보일 수 있는 특징적인 소견에 대해 기술할 것이다.

  • Transrectal ultrasound (TRUS)-guided systematic biopsy, the current gold standard for the detection of prostate cancer, suffers from low sensitivity for clinically significant cancer. The use of diagnostic multiparametric MRI has increased the relevance of targeted biopsy techniques such as MRI-TRUS fusion biopsy and direct (in-bore) MRI-guided biopsy, which have higher detection rate for clinically significant cancer. Although primarily used in patients who remain at high clinical suspicion for prostate cancer despite a negative systematic biopsy, with the increasing use of upfront diagnostic MRI, these biopsies are expected to replace routine systematic biopsies. This pictorial essay aims to enhance our understanding of the concepts of these biopsy techniques so that they can be performed safely and provide maximum diagnostic yield.

Troponin-Positive Non-Obstructive Coronary Arteries and Myocardial Infarction with Non-Obstructive Coronary Arteries: Definition, Etiologies, and Role of CT and MR Imaging

  • In approximately 10% of patients with acute myocardial infarction (MI), angiography does not reveal an obstructive coronary stenosis. This is known as myocardial infarction with non-obstructive coronary arteries (MINOCA), which has complex and multifactorial causes. However, this term can be confusing and open to dual interpretation, because MINOCA is also used to describe patients with acute myocardial injury caused by ischemia-related myocardial necrosis. Therefore, with regards to this specific context of MINOCA, the generic term for MINOCA should be replaced with troponin-positive with non-obstructive coronary arteries (TpNOCA). The causes of TpNOCA can be subcategorized into epicardial coronary (causes of MINOCA), myocardial, and extracardiac disorders. Cardiac magnetic resonance imaging can confirm MI and differentiate various myocardial causes, while cardiac computed tomography is useful to diagnose the extracardiac causes.

  • 후복막 자궁외 임신은 자궁외 임신 중에서도 매우 드물게 발생한다. 후복막 자궁외 임신은 발생률이 낮고 비특이적 증상을 보여 첫 내원 시 진단이 매우 어렵다. 주로 방사선 위험이 없는 초음파, 자기공명영상을 통해 진단이 이루어지며 전산화단층촬영을 촬영하는 경우는 드물다. 우리는 전산화단층촬영을 통하여 진단된 드문 후복강 자궁외 임신 1예를 보고하고자 한다.
