The Influence of the Change of Patient Radiation Exposure Dose Distribution on the Grid Condition and Detector Acquisition Dose on the Exposure Distance in the Use of Amorphous Silicon Thin Film Transistor Detector with AEC (자동노출제어장치를 이용한 비정질 실리콘 평판형 검출기에서 격자의 조건에 따른 환자선량 변화와 촬영 거리의 변화가 검출기 획득선량에 미치는 영향)

  • This study attempts to propose an appropriate method of using digital medical imaging equipments, by studying the effects of automatic exposure control(AEC), grid ratio and the change of radiography distance on the patient dose and detertor acquisition dose during the procedure of acquiring image through a digital medical imaging detector. The change of dose following the change of grid ratio's exposure and radiography distance was measured, by using an abdominal phantom organized with tissue equivalent materials in an amorphous silicon thin film transistor detecter installed with AWC. The case to use grid ratio 12 : 1, focal distance 180cm to radiography distance 110cm in AEC, the patient dose increased rather when we used grid ration 10 : 1, focal distance 110cm. When AEC was not used,the dose necessary for image acquisition decreased as the grid ratio became higher and the distance became further. but detector acquisition dose was not reduced when in applied AEC. When purchasing digiral medical imaging equipments, optional items such as AEC and grid shall be accurately selected to satisfy the use of the equipments. Radiography error made by radiation technologist and unnenessary patient dose can be reduced by selecting equipments with a radiography distance marker equipment when it did not apply AEC. These equipments can also be helpful in maintaining high imaging quality, one of the merits of digital detectors.

Evaluation of Articular Cartilage using 3D FFE-PROSET Technique in 3.0 T Knee MR Imaging : Comparison with 2D TSE - SPIR Technique (3.0T 무릎자기공명영상에서 3차원 FFE-PROSET 기법을 이용한 관절연골평가 : 2차원 TSE-SPIR 기법과 비교)

  • The purpose of this study is to know a clinical usefulness for delineation of articular cartilage compared with 2D TSE-SPIR and 3D FFE-PROSET technique. From January 2013 to september 2013, a total of 30 normal volunteers(12 men and 18 women aged between 35 and 55 years; mean 49.48 years) were studied on a philips 3.0T MRI scanner. As a quantitative analysis, SNRs and CNRs were evaluated by using two methods for delineation of articular cartilage. As a qualitative analysis, image quality was evaluated by special radiological technologist of MRI for image delineation on a three grade. As a results, SNRs and CNRs for articular cartilage were significantly greater for the 3D FFE-PROSET(SNRs: 8.40, 114.02, 9.53, CNRs: 104.49, 139.49) technique compared to 2D TSE-SPIR(SNRs: 4.41, 71.63, 7.34, CNRs: 64.30, 58.41) technique, image quality also was higher for evaluation of 3D FFE-PROSET(2.40) technique(p=0.0021). In conclusion, this study showed that a 3D FFE-PROSET MRI has improved SNRs and CNRs for evaluating of the articular cartilage, these conclusions in the future will be provided useful information in diagnosis of articular cartilage.

Infection Control of Computed Radiography Portable in Radiology (영상의학과 이동촬영장비의 감염 관리)

  • The purpose of this study is to prevent infection in the hospital by computed radiography portable and to provide basic data on infection-related education by investigating bacterial contamination level of computed radiography portable equipment using IP cassette. The results suggest that IP cassette No. 1 is infected with CNS and VRE, no. 2 with CNS, No. 3 with CNS and Pseudomonas aeruginosa, No. 4 with CNS, No. 5 with CNS and Bacillus sp., and No. 6 with enterococcus faecium. Enterococcus faecium and bacillus sp. were detected from the IP reader and Acinetobacter baumannii was detected on the mobile handle; Bacillus sp. on the control buttons, CNS and Bacillus sp. from the irradiation control handle, Acinetobacter baumannii on the x-ray generation switch, and CNS on the barcode scanner. In addition, Bacillus sp. Acinetobacter baumannii was found on the IP cassette mobile table and CNS and bacillus sp. were found on the lead apron. Acinetobacter baumannii and CNS were detected from the medical gloves worn by a radiological technologist during radiography. This suggests that IP cassette should be sterilized after use as it can hand over bacteria to IP reader and IP mobile table. Medical gloves that are in direct contact with patients should also be replaced after using them once and other supplies such as x-ray generation switch and lead apron should thoroughly be sterilized to prevent infection due to radiography as they are in a lot of contact with patients.

The Image Distortion Analysis of Levin-tube tip by Patient position and Incidence Angle when taking Mobile Chest AP Projection (Mobile Chest AP 검사 시 환자자세와 입사각도에 따른 Levin-tube tip의 영상왜곡 분석)

  • This study's purpose is improve image quality to keep accurate tube angle in order to recognize distortion degree conditions by patient's position or tube angle and to provide exact clinical informations when taking chest AP projection for patient which have L-tube in stomach. The experimental equipment was ELMO-T6S by SHIMADZU corporation, then we put L-tube which attached 1 mm gap scales ruler on chest phantom surface. The experiment set by 90 kVp, 4 mAs, 120 cm distance. Each phantom position which changed supine, 30degree, 45degree, 60degree on the table exposured direct, ${\pm}5degree$, ${\pm}10degree$, ${\pm}15degree$ to head and feet directions. As a result, L-tube tip's position was changed by patient's position and tube angle. When patient's position is supine, tip's position change was lower than 30degree, 45degree, 60degree. We have to adjust patient's position or tube angle in order to occur image distortion by fault tube angle when confirming correct position L-tube tip through chest x-ray. Also, Radiological technologist try to make accurate evaluation index for satisfied L-tube insertion.

A Study on the Radiation Dose Managements in the Nuclear Medicine Department (핵의학과에서 방사선 피폭관리 실태에 대한 조사 연구)

  • After administration of a radiopharmaceutical, the patient remains radioactive for hours or even days, representing a source of potential radiation exposure. Thus, including the personnel who are occupationally exposed to ionizing radiation, radiation exposure must be managed for members of the public, in particular for people accompanying patients. In this study we investigated radiation exposure dose management in the nuclear medicine departments at seven general hospitals. Two of them had no radiation safety considerations for patient transporters, sanitation workers and the like. And they all were careless of radioprotection for people accompanying patients. The average dose rate to people accompanying patients from radioactive patients just before a bone scan was 25.60 ${\mu}$Sv h-1. This is higher than 20 ${\mu}$Sv $h^{-1}$which is the annual public dose limit for temporary use. Therefore radiation dose measurement and risk assessment of patient transporters, sanitation workers and the like should be performed. And the nuclear medicine technologist should provide advices on the radiation safety to patient transporters, sanitation workers, people accompanying patients and so on. To ensure the radiation safety for people accompanying patients, it is required to restrict the patient's access to his relatives, friends and other patients or isolate patients.

Effects on Patient Exposure Dose and Image Quality by Increasing Focal Film Distance in Abdominal Radiography (복부 일반촬영시 초점-필름간거리 변화가 피폭선량 및 화질에 미치는 영향)

  • We can and must improve the diagnostic images using available knowledge and technology. At the same time we must strive to reduce the patient's integral and entrance radiation dose. Reducing the integral dose to the patient during the radiologic procedure is a primary concern of the patient, especially the pediatric patient, the radiologist and the technologist. A 100cm focal film distance generally is used for most over-table radiography. The early x-ray tubes and screen film combinations required long exposures, which often resulted in motion artifacts. But nowaday, we have the generators and x-ray tubes that can deliver the energy necessary in a very short time and the receptors that can record the information just as rapidly. And, we performed this studies to evaluate the patient exposure dose and the image quality by increasing focal film distance in diagnostic radiography. There are many factors which affected to exposure factor, but we studied to verify of FFD increase, only. Effect of increasing the focal film distance to a 140 cm distance was tested as follows; 1. The focal film distances were set at 100, 120, and 140cm. 2. A 18cm acryl(tissue equivalent) phantom was placed on the table top. 3. An Capintec 192 electrometer with PM 05 ion chamber was placed at the entrance surface of the phantom, and exposure were made at each focal film distances. 4. The procedure was repeated in the same manner as above except the ion chamber was placed beneath the phantom at the film plane. 5. Exit exposure were normalize to 8mR for each portions of the experiment. Based on the success of the empirical measurements, a detailed mathematical analysis of the dose reduction was performed using the percent depth dose data. The results of this study can be summerized as followings ; 1) Increasing FFD from 100 cm to 140 cm, we would create a situation that would have a significant effect on the overall quality of radiograph and achive the 17.42% reduction of entrance dose and the 18.95% reduction of integral dose that the patient receives. 2) Thickness of Al step wedge for equal film density increased with the long distance. 3) Increasing FFD, Magnification of image was lowered. 4) Resolution of image also increased with the FFD. As the results described above, we strongly recommend using the long FFD to provide better information for our patients and profession in abdomen radiographic studies.

The Utility of Chatbot for Learning in the Field of Radiology (방사선(학)과 분야에서 챗봇을 이용한 학습방법의 유용성)

  • The purpose of this study is to investigate the utilization of major learning tools among radiology science students and assess the accuracy of a conversational artificial intelligence service program, specifically a chatbot, in the context of the national radiologic technologist licensing exam. The survey revealed that 84.3% of radiology science students actively utilize electronic devices during their learning process. In addition, 104 out of 140 respondents said they use search engines as a top priority for efficient data collection while studying. When asked about their awareness of chatbots, 80% of participants responded affirmatively, and 22.9% reported having used chatbots for academic purposes at least once. From 2018 to 2022, exam questions from the first and second periods were presented to the chatbot for answers. The results showed that ChatGPT's accuracy in answering first period questions increased from 48.28% to 60%, while for second period questions, it increased from 50% to 62.22%. Bing's accuracy in answering first period questions improved from 55% to 64.55%, and for second period questions, it increased from 48% to 52.22%. The study confirmed the general trend of radiology science students utilizing electronic devices for learning and obtaining information through the internet. However, conversational artificial intelligence service programs in the field of radiation science face challenges related to accuracy and reliability, and providing perfect solutions remains difficult, highlighting the need for continuous development and improvement.

Usefulness of applying Macro for Brain SPECT Processing (Brain SPECT Processing에 있어서 Macro Program 사용의 유용성)

  • Purpose: Diagnostic and functional imaging softwares in Nuclear Medicine have been developed significantly. But, there are some limitations which like take a lot of time. In this article, we introduced that the basic concept of macro to help understanding macro and its application to Brain SPECT processing. We adopted macro software to SPM processing and PACS verify processing of Brain SPECT processing. Materials and Methods: In Brain SPECT, we choose SPM processing and two PACS works which have large portion of a work. SPM is the software package to analyze neuroimaging data. And purpose of SPM is quantitative analysis between groups. Results are made by complicated process such as realignment, normalization, smoothing and mapping. We made this process to be more simple by using macro program. After sending image to PACS, we directly input coordinates of mouse using simple macro program for processes of color mapping, adjustment of gray scale, copy, cut and match. So we compared time for making result by hand with making result by macro program. Finally, we got results by applying times to number of studies in 2007. Results: In 2007, the number of SPM studies were 115 and the number of PACS studies were 834 according to Diamox study. It was taken 10 to 15 minutes for SPM work by hand according to expertness and 5 minutes and a half was uniformly needed using Macro. After applying needed time to the number of studies, we calculated an average time per a year. When using SPM work by hand according to expertness, 1150 to 1725 minutes (19 to 29 hours) were needed and 632 seconds (11 hours) were needed for using Macro. When using PACS work by hand, 2 to 3 minutes were needed and for using Macro, 45 seconds were needed. After applying theses time to the number of studies, when working by hand, 1668 to 2502 minutes (28 to 42 hours) were needed and for using Macro, 625 minutes (10 hours) were needed. Following by these results, it was shown that 1043 to 1877 (17 to 31 hours were saved. Therefore, we could save 45 to 63% for SPM, 62 to 75% for PACS work and 55 to 70% for total brain SPECT processing in 2007. Conclusions: On the basis of the number of studies, there was significant time saved when we applied Macro to brain SPECT processing and also it was shown that even though work is taken a little time, there is a possibility to save lots of time according to the number of studies. It gives time on technologist's side which makes radiological technologist more concentrate for patients and reduce probability of mistake. Appling Macro to brain SPECT processing helps for both of radiological technologists and patients and contribute to improve quality of hospital service.

A Study of Decrease Exposure Dose for the Radiotechnologist in PET/CT (PET-CT 검사에서 방사선 종사자 피폭선량 저감에 대한 방안 연구)

  • Positron emission tomography scan has been growing diagnostic equipment in the development of medical imaging system. Compare to 99mTc emitting 140 keV, Positron emission radionuclide emits 511 keV gamma rays. Because of this high energy, it needs to reduce radioactive emitting from patients for radio technologist. We searched the external dose rates by changing distance from patients and measure the external dose rates when we used shielder investigate change external dose rates. In this study, the external dose distribution were analyzed in order to help managing radiation protection of radio technologists. Ten patients were searched (mean age: $47.7{\pm}6.6$, mean height: $165.5{\pm}3.8cm$, mean weight: $65.9{\pm}1.4kg$). Radiation was measured on the location of head, chest, abdomen, knees and toes at the distance of 10, 50, 100, 150, and 200 cm, respectively. Then, all the procedure was given with a portable radiation shielding on the location of head, chest, and abdomen at the distance of 100, 150, and 200 cm and transmittance was calculated. In 10 cm, head ($105.40{\mu}Sv/h$) was the highest and foot($15.85{\mu}Sv/h$) was the lowest. In 200 cm, head, chest, and abdomen showed similar. On head, the measured dose rates were $9.56{\mu}Sv/h$, $5.23{\mu}Sv/h$, and $3.40{\mu}Sv/h$ in 100, 150, and 200 cm, respectively. When using shielder, it shows $2.24{\mu}Sv/h$, $1.67{\mu}Sv/h$, and $1.27{\mu}Sv/h$ in 100, 150, and 200 cm on head. On chest, the measured dose rates were $8.54{\mu}Sv/h$, $4.90{\mu}Sv/h$, $3.44{\mu}Sv/h$ in 100, 150, and 200 cm, respectively. When using shielder, it shows $2.27{\mu}Sv/h$, $1.34{\mu}Sv/h$, and $1.13{\mu}Sv/h$ in 100, 150, and 200 cm on chest. On abdomen, the measured dose rates were $9.83{\mu}Sv/h$, $5.15{\mu}Sv/h$, and $3.18{\mu}Sv/h$ in 100, 150, and 200 cm, respectively. When using shielder, it shows $2.60{\mu}Sv/h$, $1.75{\mu}Sv/h$, and $1.23{\mu}Sv/h$ in 100, 150, and 200 cm on abdomen. Transmittance was increased as the distance was expanded. As the distance was further, the radiation dose were reduced. When using shielder, the dose were reduced as one-forth of without shielder. The Radio technologists are exposed of radioactivity and there were limitations on reducing the distance with Therefore, the proper shielding will be able to decrease radiation dose to the technologists.

Usefulness Comparative Experimental Study of the CT and MR Imaging in the Dog Clonorchiasis (잡견 간흡충증의 전산화단층촬영과 자기공명영상의 유용성에 관한 실험적 연구)

  • Purpose : Be aware of clinical possibilities on image quality by comparison of contrast-enhanced dynamic CT and MR imaging applied of MIP technique after the experimentally induced clonorchasis infection in dogs. Materials and Method : Twenty mongrel dogs prepared in zoo-laboratory were followed up with serial CT scans and MR imaging for 13 weeks after the experimental infection in liver. Two-phase helical CT was acquired in the supine position with the following scanning parameters. After the injection of contrast material, the arterial phase was initiated using a bolus-racking method. The portal phase scan was started 15 seconds after the arterial phase scan. CT protocol was determined after single level dynamic scans. MR imaging used the CP body coil and images get a 2D image using HASTE, FLASH, TSE pulse sequence. Bile duct MR imaging were obtained in three plans. Then each image was post processed by using target MIP algorithm. Two experimentation above, as a method of evaluation, one pathologist, three radiologist and five radiological technologist were analyzed visually for evaluation of following findings, enhancement of the bile duct wall, dilatation of bile duct tip, liver parenchyma, background suppression. Results : Five dogs was died of a disease after the infection, the rest one else shows the chronic dilatation of the intrahepatic bile duct with CT and MR imaging. Contrast administration of CT shows the contrast-enhanced of the bile duct walls with live parenchyma. MR imaging calculated of CNR and CR from pulse sequence for comparative evaluation and shows the pattern of the intrahepatic bile duct, dilatation of bile duct tip using MIP technique. CNR of the clonorchiasis, HASTE was $16{\pm}0.83$, TSE $7.06{\pm}3.0$, FLASH $1.19{\pm}0.2$ and CR, HASTE was 73.3%, TSE 62.3%, FLASH 6.4%. Conclusion : CT and MR imaging is very usefulness in diagnosis of dog clonorchiasis.
