Gross-β Level in Dental Ceramic Materials (치과용 세라믹 재료의 전β 방사능 준위)

  • In all of dental ceramics, these materials contained the radio-isotopes with natural abundance ratio. After dental treatment, remained dental ceramics in the oral cavity is caused for human internal dose. In this study, the gross beta activity levels were measured in dental materials including 22 dental ceramics, 1 resin, and 2 cements for estimation of human internal dose. In dental ceramic samples, the obtained results showed that the gross beta activity level varied between 1.317 ~ 2.935 Bq/g and the gross beta activity level was 2.379 Bq/g. And the same level for dentine, opacious dentine, translucent and enamel were 2.479 Bq/g, 2.491 Bq/g, 2.470 Bq/g and 2.069 Bq/g, respectively. The gross beta activity level of temporary resin and cements were negligible, compare to the same level of dental ceramics. The high gross beta activity level observed in opacious dentine code OD-A4 is 2.935 Bq/g thus mainly ascribable to 40K. The reduction of the radiation level of natural radio-isotopes and the improvement of the dental ceramic materials should be required for internal dose reduction.

A Study on the Artifact Reduction Method of Magnetic Resonance Imaging in Dental Implants and Prostheses (치아 임플란트와 보철에서 발생하는 자기공명영상의 인공물 감소방안 연구)

  • Although magnetic resonance imaging without linear hardening of CT is recognized as a method of obtaining high contrast of tissue and excellent resolution image in brain disease and head and neck examination, magnetic susceptibility artifact is generated in case of metal implants in the oral cavity, which is an obstacle to image diagnosis. Therefore, an effort was made in this thesis to find a method to reduce artifacts caused by dental implants and prosthesis in MRI. Implant-induced artifacts in magnetic resonance imaging showed that the signal size increased with shorter TE in GE technique and was inconsistent with water temperature change. In SE technique as well, the signal size of water was generally higher than that of air, but the signal to noise ratio (SNR) was not different by air and temperature. In EPI technique, images with fewer artifacts were obtained quantitatively and qualitatively when there was more water than air, and the signal to noise ratio was measured the highest, especially at water temperatures of 20° and 30°. In conclusion, when examining using the EPI technique rather than the SE or the GE technique, obtaining brain diffusion using a 20° and 30° water bag reduces the magnetic susceptibility artifacts caused by implants and prosthesis, suggesting that it may provide images with high diagnostic value.

3.5 mm T-shaped LCP (Locking Compression Plate) Fixation for Unstable Distal Clavicular Fractures (3.5 mm T형 잠김 금속판을 이용한 원위 쇄골 골절의 치료 결과)

  • Purpose: To review the clinical and radiological results after an open reduction and internal fixation with a T-shaped LCP for unstable distal clavicle fractures. Materials and Methods: From February 2005 to June 2006, ten patients with distal clavicle Neer type II fractures were treated with an open reduction and internal fixation with a T-shaped LCP. Bony union was identified by plain radiography. The clinical results were analyzed according to the UCLA scoring system. Results: The mean time to fracture union was 9weeks and union was achieved in all cases. The mean UCLA score was 33.4 (30-35); excellent in 8 cases and good in 2 cases. In one case, loosening of one distal screw was occured and mild AC joint subluxation was observed in another case. Conclusion: 3.5 mm T-shaped LCP fixation is a useful technique for treating unstable distal clavicle fractures. This procedure provide stable fixation with no further AC joint injury.

The Surgical Treatment of Chronic Avulsion Fracture of the Anterior Cruciate Ligament (진구성 전방 십자 인대 견열 골절의 수술적 치료)

  • Objective : The purpose of this study was to evaluate the clinical and radiological results after treatment of old ACL(anterior cruciate ligament) avulsion fracture of tibia. Materials and Method : 11 cases who were followed up at least 2 years after treatment in chronic ACL avulsion fracture of tibia were included in this study. The average age of patients was 26(9-66) years and the mean period of follow up was 53(24-131) months. After evaluation of the lesion and treatment of associated lesion under arthroscope, open reduction and internal fixation or fragment removal was performed. Pull-out suture was performed in 8 cases, screw fixation in 2 cases. Results : The Lysholm knee score was 64.3 in average preoperatively and improved to 96.2 average at follow up. On the Lachman test, there were mild(+) instabilities in 7 cases, moderate(++) in 4 cases. On the postoperative Lachman test, there were no instabilities in 9 cases, mild(+) instabilities in only 2 cases. According to Meyers and Mckeever's evaluation protocol, 9 cases $(81.8\%)$ were excellent, 2 case $(18.2\%)$, good. On preoperative instrumented anterior laxity test with $Telos^{\circledR}$, side to side difference in 201b was 7.8(10-4) min in average and 2.1(6-0) mm in average at follow-up. Conclusion : In chronic ACL avulsion fracture, it is thought that combination of arthroscopic evaluation of associated injury and open reduction and fixation and fragment removal can bring about satisfactory results.

RADIOLOGICAL EVALUATION OF FRACTURE LINE STABILITY DURING FUNCTIONAL LOADING AFTER MINIPLATE FIXATION OF MANDIBULAR ANGLE FRACTURES (하악 우각부 골절의 소형금속판 내고정 후 기능시 골접합선의 안정도에 관한 방사선학적 평가)

  • After miniplate fixation of mandibular angle fractures, fracture line stability during functional loading was evaluated. Using panoramic radiographs, 15 mandibular angle fracture patients who were treated by open reduction and one miniplate fixation along the external oblique ridge, were evaluated at postoperative 1, 4 and 8 weeks. At each time, 2 radiographs were taken: one taken during maximum biting of hardened silicone sheet on the affected side molar area and the other on the non-affected side. The distraction gap of inferior border of mandible at each time and each side was measured and these data were analysed statistically with clinical findings. The differences of inferior border distraction gap during hardened silicone sheet biting on the affected side molar area and on the non-affected side molar area at 4 week radiographs were smaller than those of 1 week's except one case. At 8 week's radiographs, the fracture lines were so stabilized that it was almost impossible to find the gap differences except one case and there were increased radiopacity along the entire fracture lines. Clinically, bony union was confirmed in all cases during plate removal performed at postoperative 6 month. By statistical analysis(paired t-test), the inferior border distraction gap during biting of hardened silicone sheet on the affected side was significantly reduced during 1 and 4 week interval(p<0.01). The differences of inferior border distraction gap during biting on the affected side molar area and on the non-affected side molar area were also significantly reduced at 1 and 4 week interval(p<0.01). But the inferior border distraction(compression) gap during non-affected side biting was not significantly changed. From these findings, it could be concluded that fracture line stability during functional loading after one miniplate fixation of mandibular angle fractures stems mainly from reduction of inferior border distraction gap during affected side biting on time interval. According to these radiographic and clinical findings, the clinical superiority of one miniplate fixation technique in mandibular angle fracture treatment could be confirmed.

A CLINICAL STUDY OF FRONTAL BONE FRACTURE (전두부 골절 환자의 임상적 연구)

  • Fracture of frontal bone is infrequent, but may have serious complications because of their proximity to the brain, eyes and noses. Fractures of the frontal area range from 5% to 15% of all facial bone fracture and include supraorbital rim and frontal sinus. As frontal bone fractures most frequently occur in the multiply injured patient, a thorough clinical and radiological examination of the patient is required before diagnosis and treatment plans are established. Sometimes coorperative treatment with other department is required. It is specially considered that incision for access to frontal region and surgical methods for open reduction, cranialization, cannulization, sinus obliteration. After surgical or conservative treatment, it may have complication. Complication of frontal bone injury vary in severity and may occur at several years after the incidents. The major types of complications are those that occur directly at the time of injury, infection and chronic problems. This is clinical study on 31 patients with frontal bone fracture, at department of oral and maxillofacial surgery in dental hospital of Wonkwang university during past ten years. The results were as follows; 1. The sex ratio of all patients is 29 (94%) male to 2 (6%) female, the average age is 33 and the prominent groups are 2nd, 3rd decade age. 2. The causative factors are mostly traffic accident 22 cases (70%) and fall dawn, industrial accidents, so on. 3. The 17 cases has shown alert mental status, but neurologic problems is in 14 (45%) cases in initial accessment. 4. Associated facial bone fractures are prominent in the maxilla (42%) and panfacial fracture (39%). 5. Involved general problems are in department of neurologic surgery problems (65%), orthopaedic problems (23%) and ophthalmologic problems (19%) in order. 6. Open reduction has done in 15 cases and 16 cases with conservative management. 7. Postoperative complications are chronic headache (42%), esthetic problems (39%) and ophthalmologic problems (35%)in order.

Reduction of Radiation Dose according to Geometric Parameters from Digital Coronary Angiography (디지털 심혈관조영장치의 기하학적 특성에 따른 선량 감소)

  • This study aims to find out geometric parameters which practitioner adjustable to reduce dose in coronary angiography. We take fluoroscopy and cine exposure by use of phantom, and got dose use the dose-area product(DAP) meter of angiography device, than convert DAP to effective dose. As results, Cine exposure shows higher dose measurement about 6-7 times than fluoroscopy. Dose in frame per second(FPS) mode could be decrease down to 70%, as lower FPS. In view of X-ray tube angle, LAO $45^{\circ}$+Caudal $30^{\circ}$ shows highest dose measurement. More use of Collimator, lower dose measurement. Source-image intensifier distance(SID) get longer to 10cm, dose of each fluoroscopy and cine exposure increase up to 25-30%. Image magnification of field of view(FOV) could increase dose up to 1.21-2 times. Also table-image intensifier distance get longer to 10cm, dose increased 1.11-1.25 times. Practitioner can adjust several geometric parameters, as FPS mode, tube angle, Collimation, SID, table-image intensifier distance, FOV. And each factors can reduce radiation dose in coronary angiography.

Correlation Analysis of between Patient and Equipment Factors and Radiation Dose in Chest Low Dose and Abdominal Non-contrast CT (흉부 저선량 및 복부 비조영 CT 검사에서 환자 및 장비 인자와 선량과의 상관관계 분석)

  • This paper is to establish a basis for a dose reduction strategy by confirming correlations with the factors that may affect the radiation dose based on the dose records in low-dose chest CT and abdominal non-contrast CT. In order to find out the causes of unnecessary exposure, the correlation between seven factors (age, gender, height, weight, BMI, patient status [inpatient and outpatient], and use of dose modulation) and CT dose were identified. Logistic regression was used as the statistical analysis for correlation verification. In the low dose chest CT, as the higher values of height and BMI and dose modulation off were associated with lowering the risk exceeding Diagnostic Reference Levels(DRL) (odds ration<1, p<0.05). However, as woman compared to man and the higher values of weight were associated with highering the risk exceeding DRL (odds ration>1, p<0.05). In the abdomen CT, as dose modulation off were associated with lowering the risk exceeding DRL (odds ration<1, p<0.05). Therefore It is necessary to conduct research on the relationship between various factors affecting radiation exposure and patient radiation dose for reducing the dose.

Artifact Reduction in Sparse-view Computed Tomography Image using Residual Learning Combined with Wavelet Transformation (Wavelet 변환과 결합한 잔차 학습을 이용한 희박뷰 전산화단층영상의 인공물 감소)

  • Sparse-view computed tomography (CT) imaging technique is able to reduce radiation dose, ensure the uniformity of image characteristics among projections and suppress noise. However, the reconstructed images obtained by the sparse-view CT imaging technique suffer from severe artifacts, resulting in the distortion of image quality and internal structures. In this study, we proposed a convolutional neural network (CNN) with wavelet transformation and residual learning for reducing artifacts in sparse-view CT image, and the performance of the trained model was quantitatively analyzed. The CNN consisted of wavelet transformation, convolutional and inverse wavelet transformation layers, and input and output images were configured as sparse-view CT images and residual images, respectively. For training the CNN, the loss function was calculated by using mean squared error (MSE), and the Adam function was used as an optimizer. Result images were obtained by subtracting the residual images, which were predicted by the trained model, from sparse-view CT images. The quantitative accuracy of the result images were measured in terms of peak signal-to-noise ratio (PSNR) and structural similarity (SSIM). The results showed that the trained model is able to improve the spatial resolution of the result images as well as reduce artifacts in sparse-view CT images effectively. Also, the trained model increased the PSNR and SSIM by 8.18% and 19.71% in comparison to the imaging model trained without wavelet transformation and residual learning, respectively. Therefore, the imaging model proposed in this study can restore the image quality of sparse-view CT image by reducing artifacts, improving spatial resolution and quantitative accuracy.

The Usability Evaluation According to the Application of Bismuth Shields in PET/CT Examination (PET/CT 검사에서 비스무스(bismuth) 차폐체의 적용에 따른 유용성 평가)

  • Purpose: Recently with CT developed, various studies for reduction of exposure dose is underway. Study of bismuth shields in these studies is actively underway, and has already been applied in the clinical. However, the application of the PET/CT examination was not activated. Therefore, through this study, depending on the application of bismuth shields in the PET/CT examination, we want to identify the quality of the image and the impact on the SUV. Materials and Methods: In this study, to apply to the shielding of the breast, by using the bismuth shields that contains 0.06 mmPb ingredients, was applied to the PET/CT GEMINI TF 64 (Philips Healthcare, Cleveland, USA). Phantom experiments using the NEMA IEC Body Phantom, images were acquired according to the presence or absence of bismuth shields apply. Also, When applying, images were obtained by varying the spacing 0, 1, 2 cm each image set to the interest range in the depth of the phantom by using EBW-NM ver.1.0. Results: When image of the PET Emission acquires, the SUV was in increased depending on the use of bismuth shields, difference in the depth to the surface from deep in the phantom increasingly SUV increased (P<0.005). Also, when using shields, as the more gab decreased, SUV is more increased (P<0.005). Conclusion: Through this study, PET/CT examination by using of bismuth shields which is used as purpose of reduction dose be considered. When using shields, the difference of SUV resulting from the application of bismuth shields exist and that difference is more decreased as gab of shields and surface is wider. Therefore, setting spacing of shield should be considered, if considering the reduction of the variation of SUV and image quality, disease of deep or other organs should be a priority rather than superficial disease. Through this study, when applying identified to clinical examination, the reduction of unnecessary exposure is considered.
