A 8-year-old spayed female Korean short-haired cat was presented with respiratory distress. CBC, serum chemistry analysis, plain radiography, and abdominal ultrasonography were performed. Besides hypertrophic cardiomyopathy (HCM) suspected by the thoracic radiograph, a tubular, tortuous soft tissue structure was detected at the region of the left retroperitoneal cavity on the abdominal radiograph. On the abdominal ultrasonography, a shunt vessel is identified caudo-lateral to the left kidney region. These findings are consistent with spleno-systemic shunts in cats. Furthermore, portal hypertension and diffuse hepatic lesion were also identified. Although the cause of a shunt vessel is not easy to diagnose, it is important to include spleno-systemic shunt into differential diagnosis list, when convoluted, tubular soft tissue opacity is seen on the digital radiography (DR). This report will allow clinicians to raise awareness of complications of portosystemic shunt (PSS) and better treat PSS suspected feline patients when the advanced modalities such as computed tomography and magnetic resonance imaging are not available.
One day to one week-old 3 female Korean native calves were referred to the Veterinary Teaching Hospital, Chonbuk National University, with atresia ani. The authors performed radiographic and ultrasonographic diagnosis with physical examination, general hematology and blood chemical examination as investigation. The atresia ani were type I, II, III in each 3 calves. In abdominal radiography and ultrasonography, there were ventral displacement of descending colon(gas and feces filled) and typical enlargement of the blind end of colon in all cases. Operation site was determined by radiographic and ultrasonographic findings in these cases. Surgical treatment, two cases were translocation of the colon to the body wall exiting as a colostomy and one case was corrected by making a circumcision through the skin covering the site of the anus.
The purpose of this study was to determine the success rate of air reduction as the primary treatment of intussusception and whether the success of air reduction could be predicted by plain x-ray. The authors reviewed the medical records of 54 consecutive patients diagnosed with intussusception from Jan 2005 to Dec 2007 at the Department of Surgery, Masan Samsung Hospital. The natures of symptoms and findings of plain abdominal radiography performed in the emergency department (ED) were reviewed. Air reduction failed more frequently (26.3 %) in patients who visited ED more than 24 hours after symptom onset (p=0.009). The mean duration of symptom for operated patients was longer than air reduction group (p=0.01). Also, 3/4 of patients having localized distension of small bowel in the left upper quadrant abdomen had unsuccessful air reduction (p=0.002). In conclusion, the time interval from symptom onset to arrival at ED and localized distension of small bowel in the left upper quadrant abdomen significantly increased the failure rate of air reduction.
These studies performed to establish the diagnostic methods for the luminal adhesions to abdominal wall with radiography, ultrasonography and electromyography, and to evalute sodium carboxymethylcellulose(SCMC) for prevention of the abdominal adhesions in goats. The adhesion sites were not detected by rediography and ultrasonography at non-pneumoperitoneum, but detected after pneumoperitoneum in goats with experimentally induced ruminal abhesions to abdominal wall. Electromyography revealed only the presence of intraabdominal adhesions. In fibriongen values and total leucocytes, there were no alterations between SCMC treated and non-treated group. The average gross scores of adhesions in SCMC treated and non-treated were 0.3 and 2.7, respectively. It was concluded that radigraphy, electromyography and ultrasonography could be available for the diagnosis of the ruminal adhesions to abdominal wall and the use of SCMC was useful to prevent the intraabdominal adhesions in ruminants.
Cystic endometrial hyperplasia (CEH) and uterine dilation (hydro-, muco-, hemato- and pyometra) are common uterine diseases in intact female dogs. The aim of the present study was to assess the usefulness of digital abdominal radiography in diagnosing uterine disease in intact female dogs. Two hundred and thirteen intact female dogs were included, and were classified into four groups on the basis of radiographic uterine visibility (visible/invisible) and ultrasonographic findings (normal uterus/abnormal uterine condition including CEH and uterine dilation). For each dog, the ratio of the maximum uterine diameter to the height of the fifth lumbar vertebral body (U/L5 ratio) was calculated on radiographs. There were 78 and 135 dogs in the normal and abnormal groups, respectively; 34.6% normal and 53.5% abnormal uteri were visible on abdominal radiographs. Our results suggested that a mean U/L5 ratio of $1.18{\pm}0.53$ ($mean{\pm}2SD$) indicated a normal radiographic uterus diameter, and that a value of > 1.60 should be used as an indicator of uterine disease in clinical practice. However, because false negative results were noted, radiography cannot replace ultrasonography for assessment of the uterus.
Purpose: Ultrasound is of proven accuracy in abdominal and thoracic trauma and may be useful for diagnosing extremity injury in situations where radiography is not available, such as disasters and military and space applications. However, the diagnosis of fractures is suggested by history and physical examination and is typically confirmed with radiography. As a alternative to radiography, we prospectively evaluated the utility of extremity ultrasound performed by trained residents of emergency medicine (EM) one patient with wrist and ankle extremity injuries. Methods: Initially, residents of EM performed physical examinations for fractures. The emergency ultrasound (EM US) was performed by trained residents, who used a portable ultrasound device with a 10- to 5-MHz linear transducer, on suspected patients before radiography examination. The results of emergency ultrasound and radiography and the final diagnosis were recorded, and correlation among them were determined by using Kappa s test Results: Thirty-nine patients were enrolled in our study. The average age was $36.6\;{\pm}\;19.3$ years. There were radius Fx. (n=21), radius-ulna Fx. (n=1), ulna Fx. (n=1), and contusion (n=2) injuries among the wrist injury and lat.-med. malleolar Fx. (n=13), lat. malleolar Fx. (n=6), and med. malleolar Fx. (n=3) injuries among the ankle injury. Comparing EM US with radiography, we found the sensitivity, specificity, positive predictive value, and negative predictive value of EM US for Fx. diagnosis to be 100%, 66.7%, 97.3%, 100% and those of radiography to be 97.2%, 100%, 100%, and 75%, respectively. Kappa s test for a correlation between the Fx. diagnosis of EM US and the final diagnosis of Fx was performed, and Kappa's value was 0.787 (P = 0.004).Conclusion: EM US for Fx. can be performed quickly and accurately by EM residents with excellent accuracy in remote locations such as disaster areas and in military and aerospace applications. EM US was as useful as radiography in our study and had a high correlation to the final diagnosis of Fx. Therefore, ultrasound should performed on patients with extremity injury to determine whether extremity evaluation should be added to the FAST (focused abdominal sonography trauma) examination.
A 7-year-old male poodle dog with abdominal distension and dysuria was referred to the Veterinary Teaching Hospital at Jeju National University. Intraabdominal mass was detected by radiography and ultrasonography. Surgical excision was performed by laparotomy. After the operation, this intraabdominal mass was diagnosed as seminoma based on histopathologic examination.
Purpose: This was conducted to assess the need for conventional radiographs of the thoracolumbar spine for routine screening of trauma patients who revealed no spinal trauma on chest and abdominal computed tomography (CT). Methods: We performed a retrospective review of the medical records of trauma patients who underwent conventional radiographs of the thoracolumbar spine following a chest and abdominal CT that revealed no spinal trauma. Results: Two hundred seventy-five trauma patients underwent conventional radiographs of the thoracolumbar spine following a chest and abdominal CT that revealed no spinal trauma. In 274 of the cases, the thoracolumbar spine series was also negative. Conclusion: CT of the chest and abdomen is an adequate evaluation of the thoracolumbar spine in trauma patients who require routine thoracolumbar spine screening, making subsequent conventional radiographs of the thoracolumbar spine unnecessary.
A 6-year old, Greyhound bitch was presented with vaginal hemorrhage and dystocia. Physical examination revealed severe vaginal hemorrhage, abdominal pain, pale mucous membranes and the presence of solid structures to abdominal palpation. A hematological test revealed a marked hemorrhagic anemia, and abdominal radiography and ultrasonographic examination showed two dead fetuses in the uterus. Median laparotomy revealed a rupture of the left uterine horn adjacent to the bifurcation, region of weakened uterine wall in the right uterine horn, blood clots and uterine fluids in abdominal cavity without septic peritonitis. The bitch underwent ovariohysterectomy and recovered without complication.
A 6-year-old, intact female mixed dog was referred to Veterinary Medical Teaching Hospital of Gyeongsang National University due to vomiting, anorexia, depression of 1 month's duration. On abdominal radiography, numerous irregularly marginated mineralized branching structures were present within the liver. On ultrasonography, irregular hyperechoic spots with an strong acoustic shadow was identified, but evidence of ductal dilatation was absent. Based on radiography and ultrasonography, intrahepatic gallstones were diagnosed incidentally.
