Surgical Outcomes of Radiographically Noninvasive Lung Adenocarcinoma according to Surgical Strategy: Wedge Resection, Segmentectomy, and Lobectomy

  • Background: The aim of this study was to evaluate the outcomes of surgical resection in patients with radiographically noninvasive lung adenocarcinoma according to the surgical strategy. Methods: A retrospective study was conducted of 128 patients who underwent pulmonary resection for ground-glass opacity (GGO)-dominant nodules measuring ${\leq}2cm$ with a consolidation/tumor ratio ${\leq}0.25$ based on computed tomography between 2008 and 2015. The 5-year disease-free survival (DFS) rate and 5-year overall survival (OS) rate were analyzed. Results: Among the 128 patients, wedge resection, segmentectomy, and lobectomy were performed in 40 (31.2%), 22 (17.2%), and 66 patients (51.6%), respectively. No significant differences were found among the groups in the mean size of tumors (p=0.119), the rate of pure-GGO nodules (p=0.814), the consolidation/tumor ratio (p=0.695), or the rate of invasive adenocarcinoma (p=0.378). Centrally located tumors were more common in the lobectomy group (21.2%) than in the wedge resection (0%) or segmentectomy (0%) groups (p=0.001). There were no significant differences in the 5-year DFS rate (100%, 100%, 92.7%, respectively; p=0.76) or 5-year OS rate (100%, 100%, 100%; p=0.223) among the wedge resection, segmentectomy, and lobectomy groups. Conclusion: For radiographically noninvasive lung adenocarcinoma measuring ${\leq}2cm$ with a consolidation/tumor ratio ${\leq}0.25$, wedge resection and segmentectomy could be comparable surgical options to lobectomy.

  • A 29-year-old male patient was referred to Dept. of Dentistry due to halitosis and food impaction, who had been diagnosed as progressive systemic sclerosis at Dept. of Dermatology, SNUH. Oromaxillofacially lips were slightly thin, rigid and shiny. Pale gingiva and pinched nose were found also. Radiographically, uniform extreme widening of periodontal ligament space was found.

  • Pulmonary bullae were diagnosed in 2 dogs with respiratory distress at Veterinary Medical Teaching Hospital, Seoul National University. Radiographically, thin-walled, fluid-gas leveled bullae in standing lateral positioning and moderately thick-walled bullae in lateral positioning were identified in case 1 and 2, respectively.

  • An abnormal female Holstein calf with ectrodactyly of right hindlimb was observed macroscopically and radiographically, and the results were summarized as follows. 1. The central and fourth tarsal bones in the left hindlimb were separated. 2. The tarsus in the right hindlimb consisted of five pieces; the talus, calcaneus, central and first tarsal bones were separated, and the second, third and fourth tarsal bones were fused. 3. The large metatarsal bone in the right hindlimb was fused rudimentary fourth metatarsal bone and well developed third metatarsal bone. 4. The phalanges of the lateral(fourth) digit were absent in the right hindlimb.

  • The author observed a rare case of radicular dentin dysplasia associated with enamel hypoplasia in a 11-year-old boy with a complaint of gum boil formation. 1. Clinically. yellowish-brown colored teeth with severe attrition and several gum boils were observed. 2. Radiographically, obliteration of pulp chamber and root canal, multiple periapical radiolucencies without obvious cause and blunt roots were observed. 3. Systemically, scalp hair and eyebrows were loose and short. And saddle nose could be also seen.

Pericoronal radiolucency associated with incomplete crown

  • The author experienced 8 cases of pericoronal radiolucency involving an incomplete tooth crown that had not developed to form the cemento-enamel junction, and the underdeveloped crown sometimes appeared to be floating within the radiolucency radiographically. The first impression was that these cystic lesions had odontogenic keratocysts, but half of them turned out to be dentigerous cysts histopathologically. There has been no report concerning odontogenic cysts involving an incompletely developed crown. The purpose of this paper is to report that dentigerous cysts may develop before the completion of the cemento-enamel junction of a developing crown.

  • Hiatal hernia is a rare disease in Korea compared to western countries. It is even rarer to find the one that requires operation after unsuccessful medical treatments. We operated three cases of hiatal hernia with gastroesophageal reflux by Belsey-Mark IV procedure. One case developed paraesophageal hernia postoperatively and we performed laparotomy to correct the complication. Postoperatively, all three cases showed satisfactory results clinically and radiographically.

A radiographical study on the changes in height of grafting materials after sinus lift: a comparison between two types of xenogenic materials

  • Purpose: The performance of implant surgery in the posterior maxilla often poses a challenge due to insufficient available bone. Sinus floor elevation was developed to increase the needed vertical height to overcome this problem. However, grafting materials used for the sinus lift technique eventually show resorption. The present study radiographically compared and evaluated the changes in height of the grafting materials after carrying out maxillary sinus elevation with a window opening procedure. This study also evaluated the difference between two xenogenic bone materials when being used for the sinus lifting procedure. Methods: Twenty-one patients were recruited for this study and underwent a sinus lift procedure. All sites were treated with either bovine bone (Bio-$Oss^{(R)}$) with platelet-rich plasma (PRP) or bovine bone (OCS-$B^{(R)}$)/PRP. A total of 69 implants were placed equally 6-8 months after the sinus lift. All sites were clinically and radiographically evaluated right after the implant surgery, 7-12 months, 13-24 months, and 25-48 months after their prosthetic loading. Results: Changes of implant length/bone length with time showed a statistically significant decreasing tendency (P<0.05). There was no significant change in the Bio-$Oss^{(R)}$ group (P>0.05). In contrast, the OCS-$B^{(R)}$ group showed a significant decrease with time (P<0.05). However, no significant difference was observed between the two groups (P>0.05). Conclusions: The results showed that there was significant reduction in comparison with data right after placement, after 7 to 12 months, 13 to 24 months, and over 25 months; however, reduction rates between each period have shown to be without significance. No significant difference in height change was observed between the Bio-$Oss^{(R)}$ and the OCS-$B^{(R)}$ groups.

Calcaneo-stop Procedure for Management of Pediatric Symptomatic Flexible Flatfoot (증상이 동반된 소아 유연성 편평족 치료에서의 Calcaneo-stop 술식)

  • Purpose: The aim of the current study is to report on the clinical and radiographic results after calcaneo-stop procedure in Korean children with symptomatic flexible flatfoot. Materials and Methods: Twenty-two children suffering pain along the medial aspect of midfoot with flexible flatfoot whose symptoms did not improve with conservative measures and therefore underwent calcaneo-stop procedure were identified retrospectively. Clinically, American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot scale and visual analogue scale (VAS) were evaluated. Radiographically, standing anteroposterior and lateral radiographs of the foot and Saltzman's alignment views were taken and talonavicular coverage angle, lateral talo-first metatarsal angle, and hindfoot alignment angles were measured and analyzed. Results: Clinically, AOFAS ankle-hindfoot scale improved from $70.3{\pm}5.6$ to $97.3{\pm}2.5$ and VAS improved from $6.4{\pm}1.6$ to $0.2{\pm}0.4$. Radiographically, talonavicular coverage angle improved from $28.3^{\circ}{\pm}12.3^{\circ}$ to $10.9^{\circ}{\pm}8.1^{\circ}$, lateral talo-first metatarsal angle improved from $-19.3^{\circ}{\pm}9.0^{\circ}$ to $-2.4^{\circ}{\pm}8.1^{\circ}$, and hindfoot alignment angle improved from valgus $11.9^{\circ}{\pm}10.0^{\circ}$ to $3.5^{\circ}{\pm}4.3^{\circ}$ at minimum 2-year follow-up. No complications occurred postoperatively. Conclusion: Calcaneo-stop procedure is a simple and very effective procedure for management of pediatric symptomatic flexible flatfoot that does not respond to conservative treatment.
