본 논문에서는 쥐 뇌의 감각 피질을 자극하기 위해 변조신호를 이용하여 경두개 자기자극(TMS) 동물실험을 수행하였다. 제안한 TMS 시스템은 인버터, 변압기, 커패시터, 가변 인덕터, 자극 코일로 구성되어 쥐 뇌의 감각피질에 약 1.5 mT의 자기장이 생기도록 시스템을 설계하였다. 자극 신호는 구형파 변조되었으며, 반송 주파수는 85~91 kHz의 범위로 변화시키며, 자극 효과를 관찰하였다. 자극 결과 반송 주파수가 89 kHz 미만일 때에는 자극의 효과를 볼 수 없었고, 반송 주파수가 89 kHz 이상일 때에는 압력 자극에 반응하는 역치 값이 낮아지는 효과를 볼 수 있었다.
In this paper, the study was carried out to design and implement the medical compound stimulator based on the preexisting individual medical stimulators with Low frequency, High frequency and cooling stimulation. The proposed equipment is designed to compound all the functions including a cooling stimulation with a range of $0{\sim}20^{\circ}C$ and the verified low and high frequencies of 250Hz, 500Hz, 1KHz and 1MHz respectively from the previous clinical experiment with a consideration of its credibility and efficiency. Also, it was constructed by using a new technique, thermoelectricity semiconductor with a consideration of miniaturization and stability. In accordance with patients' treatment purpose, the hand piece of low frequency/cooling stimulation and High frequency/cooling stimulation were separately designed for convenience. The frequency, accuracy and other factors of implemented medical compound stimulator was satisfied according to its measurement. It was also tested by Korean Testing Laboratory (KTL) for its stability and efficacy and it confirmed that the medical compound stimulator is suitable for use as it fits in with the medical equipment standards.
The purpose of this study was to demonstrate the effects of silver spike point (SSP) low frequency electrical stimulation on plasma ${\beta}-endorphin$ activities measured by radio- immunoassay from normal volunteer and the effects of ${\beta}-endorphin$ on 5-hydroxytryptamine (5-HT, serotonin)-induced contraction investigated by isometric tension methode in rats. The current of 3 Hz continue type, but not 100 Hz continue type, of SSP low frequency electrical stimulation significantly increased in plasma ${\beta}-endorphin$ from normal volunteer. The endothelial cell-dependent 5-HT-induced contractions were inhibited by ${\beta}-endorphin$$1{\mu}M$. These results suggest that the ${\beta}-endorphin$ regulates nociceptive-like substance, such as 5-HT, in part and that the SSP low frequency electrical stimulation, specifically current of low frequency of 3 Hz continue type, significantly increases plasma ${\beta}-endorphin$ from normal volunteer.
Objectives : The objectives of this study is to compare the effects of electroacupuncture(EA), warm needling(WN) and Radio Frequency warm needling(RFWN) stimulations on the acupoints at the artificially damaged ankles of Sprague-Dawley rats, which could be classified as the Grade 3. Methods : The foot weight bearing force ratio(FWBFR) of ankle sprain was measured first at 24 hours after without any other stimulations. Pain Recovery Index(PRI) represents the analgesic level, and modified Pain Recovery Index(mPRI) shows the accumulated recovery level. PRI was measured at 2 hours after each stimulus on GB34, GB39 and GB42, and mPRI was during 7 days. Results : EA stimulation of GB34 and GB39 acupoint in grade 3 ankle sprain showed a marked analgesic and recovery effect. RFWN of GB42 exhibited significant analgesic and pain recovery effect. RFWN of GB34 resulted in pain recovery effect but not analgesic effects, and RFWN of GB39 resulted in analgesic effect 2 hours after but not pain recovery. However, WN did not affect the pain recovery among three acupoints at all. Conclusions : In the Grade 3 ankle sprain model, the difference of analgesic effects were explained by the acupoints and the stimulation methods according to the accumulated recovery effects during 7 days. It is insufficient to determine that a certain acupoint has a specific analgesic effect depending on the stimulation method by the results of this study. Therefore, the effects of each stimulation on the acupoints in any other meridians at the sprained ankle should be compared and analyzed.
Objectives : The purpose of this study was to investigate the analgesic effect of electroacupuncture(EA) and radio-frequency warm needling(RFWN) stimulation in acupoint combination on ankle sprained pain in rats. Methods : The lateral ligaments of the Sprague-Dawley rats ankle were injured surgically resulting in sprain, of which was divided into EA, RFWN treatment groups and control group without treatment. The level of pain was measured through foot weight bearing force ratio followed by calculating pain relief. To stimulate proximal or distal area in ankle sprain, combination of proximal acupoints(GB34-GB39) and distal acupoints(GB39-GB42) from sprain area were applied, respectively, to either EA or RFWN stimulation. In addition, naltrexone or phentolamine was injected intraperitoneally before the stimulation to observe the pathway of analgesic effects. Results : In the proximal combination of GB34-GB39, EA and RFWN significantly increased pain relief compared to the control group (p<0.05). However, in distal combination with GB39-GB42, both EA and RFWN stimulation did not relieve pain due to ankle sprains. In the combination of GB34-GB39, the analgesia of EA was inhibited by blockade of the ${\alpha}$-adrenoceptor receptor. The analgesia of RFWN was inhibited by blockade of the ${\alpha}$-adrenoceptor receptor as well as ${\mu}$-opioid receptor. Conclusions : We observed that the proximal combination was effective in relieving pain when the treatment by acupoint combination was applied to the ankle sprain pain. Also, it was confirmed that this analgesia was also related to the pathways of ${\mu}$-opioid receptors and/or ${\alpha}$-adrenoceptors.
Functional electrical stimulation (FES) has developed over the last 35 years to become a scientifically, technologically and clinically recognized field of interest in clinical medicine. FES has been applied to locomotion, grasping, ventilation, incontinence, and decubitus healing. However, all of these achievements illustrate the initial applications of FES; its true potential has not yet been realized. Recently, FES systems, which are miniaturized stimulation devices, have been utilized in the clinical setting. However, because the stimulating electrodes of the current FES devices are percutaneous electrodes, which are susceptible to wire breakage, and skin infection an implantable FES stimulating electrode has been introduced in the U.S. and Japan. In the present study, an external power supply method using radio frequency (RF) coupling and data transmission was developed for the control of the implantable FES device. In addition, we review the current understanding of FES devices and their application in clinical medicine.
본 논문에서는 뜸의 열적 효과를 구현할 수 있는 심부 열 자극 시스템을 제작하고 뜸 자극에 의한 온도변화와 유사한 자극 프로토콜을 설계하고 평가하였다. 뜸은 질병에 대한 면역력을 높이고 질병을 치유하는데 사용하지만, 체표면에 화상을 입히는 부작용 및 열 자극의 세기를 조절하기 힘든 한계점이 있다. 이런 한계점을 극복하기 위해 자극의 세기 조절이 용이한 고주파 심부 열 자극 시스템을 개발하고, 심부에 뜸과 동일한 열 자극을 전달할 수 있는 프로토콜을 제안하여 뜸의 심부온도와 비교 하였다. 심부의 온도측정은 고주파의 영향을 받지 않는 적외선 열 센서를 사용해 측정하였고, 적외선 열 카메라 및 thermometer를 이용해 온도를 측정하고 비교하였다. 뜸과 심부 열 자극 시스템을 이용하여 심부에 자극을 인가한 후 측정한 온도변화를 비교한 결과, 표피에 가까울수록 유사한 온도 변화 패턴을 보여준 반면 심부로 내려갈수록 심부 열 자극 시스템의 열적효과가 뜸에 비해 더 효과적인 것을 알 수 있었다. 이를 통해 제작한 심부 열 자극 시스템과 열 자극 프로토콜의 유용성을 확인 할 수 있었다.
고령사회의 진입과 인간의 평균 수명이 연장되고, 여성의 사회적 진출 및 남성들의 외모에 대한 관심증가, 미디어 매체를 통한 K-문화가 전 세계적으로 관심을 받으면서 자연스럽게 관심은 K-Bueaty에 집중되고 있다. 최근, 의료관광 분야의 점유를 보면 성형 및 피부과와 같은 피부미용 의료관광의 경우 중국, 일본 등 아시아뿐만 아니라 북미, 유럽 등에서 인기를 누리고 있다. 사람의 노화를 가장 먼저 외면적으로 확인할 수 있는 부위는 바로 얼굴의 피부 주름이다. 깨끗하고 주름이 없으면서 탄력성 있는 건강한 피부는 대부분의 사람들이 원하는 바람이다. 대표적으로 집속형초음파자극(HIFU:High Intensity Focused Ultrasound)와 저주파, 고주파(RF:Radio Frequency), 미세전류를 이용한 갈바닉 테라피, 급속 냉각을 이용한 크라이오 테라피 등 피부의 컨디션과 상태에 따라 관리하는 방식이 달라지며, 같은 기전을 이용한 의료기기 및 피부미용 기기의 시술 역시 출력 및 자극 부위 등에 따라 시술의 효과가 차이가 난다. 본 연구는 수많은 피부미용 의료기기 및 미용기기 중 마이크로니들을 이용한 침습형 고주파 피부 의료기기를 개발하고자 국제규격인 IEC 60601-2(의료기기개별기준규격)및 MFDS(Ministry of Food and Drug Safety : 식품의약품안전처)에서 고시한 고주파 자극기 기준 규격을 준수한 고주파 출력 장치를 설계 및 개발하였다. 회로 설계는 Class-A Topology를 이용한 증폭장치(AMP:Amplifier)와 Half-Bridge Topology를 이용한 전원장치로 이루어져 있다. 개발된 고주파 출력 장치를 측정한 결과 평균 63.86%의 효율을 얻었으며, 최대출력은 116.7W, 50.67dBm으로 측정되었다.
This study has been conducted on the nonequivalent control group pretest-posttest design in quasi experimental basis and newly born premature infants from intensive care unit of G Medical University Hospital in Inchon Metropolitan were selected in two groups of 21 infants each. The first group for experimental and the other for control. Data has been collected form October 30, 1997 to August 29, 1998. For the experimental group tactile and kinesthetic stimulation was applied 2 times a day for 10 days(10 : 00~11 : 00 hours in the morning and 17 : 00~18 : 00 in the afternoon). As a weight weighing instrument. electronic indicator scale(Cas Co. korea) was used. To determine urine cortisol concentration level in stress hormone, radio immune assay method was used. And high performance liquid chlomatography was used to determine urine norepinephrine, concentration level To determine behavior status, tools developed by Anderson et at(1990) and remodeled by Kim Hee-Sook(1996) were used. Collected data were analyzed with the SAS program using x$^2$-test, student t-test, repeated measures ANOVA and paired t -test. The result were as follow. 1. As for the daily weight gain. the experimental group showed first change in weight and this group also showed higher weight in the average weight than the control group. Statistically, however, there was no significant factor between the two group. 2. The cortisol concentration in urine showed decrease in the experimental group norepinephrine concentration in urine showed increase in both experimental and control groups. No statistical significance was shown between the two groups. 3. In the aspect of behavior status. the experimental group showed statistical significance by showing inactive in the state of alert and conversion to a positive state than the control group. In conclusion, the sensory stimulation in this study showed a positive aspect through there was no statistical significance in the weight gain and urine stress hormone concentration. In the behavior status, there was statistical significance in the frequency of staying inactive in the state of alert and conversion to a positive state.
