Purpose : To determine the optimal radiation dose in a localized non-Hodgkin's lymphoma of the head and neck in the treatment setting for combined chemoradiotherapy. Materials an Methods :Fifty-three patients with stage I and II diffuse large ceil non-Hodgkin's lymphoma of the head and neck, who were treated with combined chemoradiotherapy between 1985 and 1998 were retrospectively reviewed. The median age was 49 years, and the male-to-female ratio was approximately 1.6. Twenty-seven patients had stage 1 disease and 26 had stage II. Twenty-three patients had bulky tumors $(\geq5\;cm)$ and 30 had non-bulky tumors (<5 cm). The primary tumors arose mainly from an extranodal organ $(70\%)$, most cases involving Waldeyer's ring $(90\%)$. All patients except one were initially treated with $3\~6$ cycles of chemotherapy, which was followed by radiotherapy. Radiation was delivered either to the primary tumor area alone $(9\%)$ or to the primary tumor area plus the bilateral neck nodes $(91\%)$ with a minimum dose of 30 Gy $(range\;30\~60\;Gy)$. The failure patterns according to the radiation field were analyzed, and the relationship between the dose and the in-field recurrence was evaluated. Results : The 10-year overall survival and the 10-year disease free survival rates were similar at $75\%\;and\;76\%$, respectively. A complete response (CR) after chemotherapy was achieved in 44 patients $(83\%)$. Subsequent radiotherapy showed a CR in all patients. Twelve patients $(23\%)$ had a relapse of the lymphoma after the initial treatment. Two of these patients had a recurrence inside the radiation field. No clear dose response relationship was observed and no significant prognostic factors for the in-field recurrences were identified because of the small number of in-field recurrences. However, for patients with tumors <5 cm in diameter, there were no in-field recurrences after a radiation dose 30 Gy. The 2 in-field recurrences encountered occurred in patients with a tumor $\geq5\;cm$. Conclusion .A dose of 30 Gy is sufficient for local control in patients with a non-bulky (<5 cm), localized, diffuse large cell non-Hodgkin's lymphoma when combined with chemotherapy. An additional boost dose in the primary site is recommended for patients with bulky tumors $(\geq5\;cm)$.

Purpose : The aim of this study is to conform the possibility of the liquid type EPID as a QC tools to clinical indication and of replacement of the film dosimetry. Aditional aim is to describe a procedure for the use of a EPID as a physics calibration tool in the measurements of radiation beam parameters which are typically carried out with film. Method & Materials : In this study we used the Clinac 2100c/d with EPID. This system contains 65536 liquid-filled ion chambers arranged in a $256{\times}256$ matrix and the imaging area is $32.5{\times}32.5cm$ with liquid layer thickness of 1mm. The EPID was tested for different field sizes under typical clinical conditions and pixel values were calibrated against dose by producing images using various thickness of lead attenuators(lead step wedge) using 6 & 10MV x-ray. We placed various thickness of lead on the table of linear accelerator and set the portal vision an SDD of 100cm. To acquire portal image we change the field size and energy, and we recorded the average pixel value in a $3{\times}3$ pixel region of interest(ROI) at field center was recorded. The pixel values were also measured for different field sizes in order to evaluate the dependence of pixel value on x-ray energy spectrum and various scatter components. Result : The EPID, as a whole, was useful as a QA tool and dosimetry device. In mechanical check, cross-hair centering was well matched and the error was less than ?2mm and light/radiation field coincidence was less than 1mm also. In portal dosimetry the wider the field size the the higher the pixel value and as the lead thickness increase, the pixel value was exponentially decreased. Conclusions : The EPID was very suitable for QA tools and it can be used to measure exit dose during patients treatment with reasonable accuracy. Purpose: Radiotherapy of the neck is known to cause carotid artery stenosis. We compared the carotid artery dose received between volumetric modulated arc therapy (VMAT) and conventional fixed-field intensity-modulated radiotherapy (IMRT) plans in patients with early glottic cancer. Materials and Methods: Twenty-one early glottic cancer patients who previously underwent definitive radiotherapy were selected for this study. For each patient, double arc VMAT, 8-field IMRT, 3-dimensional conformal radiotherapy (3DCRT), and lateral parallel-opposed photon field radiotherapy (LPRT) plans were created. The 3DCRT plan was generated using lateral parallel-opposed photon fields plus an anterior photon field. VMAT and IMRT treatment plan optimization was performed under standardized conditions to obtain adequate target volume coverage and spare the carotid artery. Dose-volume specifications for the VMAT, IMRT, 3DCRT, and LPRT plans were calculated with radiotherapy planning system. Monitor units (MUs) and delivery time were measured to evaluate treatment efficiency. Results: Target volume coverage and homogeneity results were comparable between VMAT and IMRT; however, VMAT was superior to IMRT for carotid artery dose sparing. The mean dose to the carotid arteries in double arc VMAT was reduced by 6.8% compared to fixed-field IMRT (p < 0.001). The MUs for VMAT and IMRT were not significantly different (p = 0.089). VMAT allowed an approximately two-fold reduction in treatment delivery time in comparison to IMRT (3 to 5 minutes vs. 5 to 10 minutes). Conclusion: VMAT resulted in a lower carotid artery dose compared to conventional fixed-field IMRT, and maintained good target coverage in patients with early glottic cancer.
Ornithine decarboxylase(ODC) is an indicate enzyme in carcinogenesis. We divided Sprague Dawley rats into six groups: control, electric field exposure, X-ray only irradiation, X-ray irradiation with electric field exposure group, Sr-90 injected group and Sr-90 and electric field complex exposure group. The ODC activity was measured in rat's bone marrow cell every week. The results were summarized as follows: The ODC activitied was increased in X-ray irradiated, Sr-90 injected and Sr-90 and electric field complex exposed group as compared with that of control(p<0.05). The ODC activity was increased comparing that of control neither in X-ray and electric field complex exposed group nor electric field only exposed group. This result suggests that the electric field doesn't have myeloid carcinogenicity and myeloid cancer incidence caused by ionized radiation is suppressed by electric field exposure.

The IMRT is proper implement to get high dose deliver to tumor as its shape and selective approach in radiation therapy. Since the IMRT is performed as modulated the radiation fluence by the MLC created the open shapes and its irradiation time, the dose of segment of radiation field effects on the cumulated portal dose. The accurate output factor of small and step shape of segment is important to improve the determination of deliver tumor dose as it is directly proportional to dose. This experiment performed with the 6 MV photon beam of Clinac Ex(Varian) from $3{\times}3cm^2$ to $0.5{\times}0.5cm^2$ small field size for collimator jaw in MLC free and/or for MLC open field in fixed collimator jaw $10{\times}10cm^2$ using the CC01 ion chamber, SFD diode, diamond detector and X-Omat film dosimetry. As results of normalized to the reference field of $10{\times}10cm^2$ of MLC, the output factor of $3{\times}3cm^2$ showed $0.899{\pm}0.0106$, $0.855{\pm}0.0106$ for $2{\times}2cm^2$, $0.764{\pm}0.0082$ for $1{\times}1cm^2$ and $0.602{\pm}0.0399$ for $0.5{\times}0.5cm^2$. The output factor of MLC open field has shown a maximum 3.8% higher than that of the collimator jaw open field.

Purpose : Film has been the primary tool in coincidence testing between the light field and the radiation field, which constitutes the quality assurance list of a linear accelerator. But there is a great chance of errors being different among the observer when using film. Thus this study set out to use the BIS(Beam Image System) in addition to film in comparing and evaluating coincidence results between the two fields and in searching for the improvement measures. Materials & Methods : Photon beam of 6 and 15MV was exposed to film and the BIS using a linear accelerator. The light and radiation fields were each $50{\times}50,\;100{\times}100,\;and\;200{\times}200mm^2$. The gantry angle was $0^{\circ}$ when using film and $0^{\circ}\;and\;270^{\circ}$ when using the BIS. The devices adopted to test coincidence between the two fields were a ruler and film scanner when using film. With the BIS, the width of the scanned light and radiation fields was measured for errors with setting the X and Y axis. Results : The visual measurements of the observer with film resulted that the radiation field was bigger than the light field and that their maximum error was 1.9mm. The results were the same with the measurements using the film scanner except for the average error, which was less than 1.9mm. On the contrary, the measurements using the BIS showed that the light field was bigger than the radiation field at the gantry angle of $0^{\circ}\;and\;270^{\circ}$. The maximum error was 0.96mm, and the error range was $<{\pm}2mm$ both in the X and Y axis. The average error of ${\Delta}X$, Y was the smallest in the order of the visual film measurements, film scanner measurements, and BIS measurements Conclusion . This requires a careful measurement for accurate quality assurance since errors are much different according to each observer that tests coincidence between visual fields with film. And an observer needs to use another image device or develop a measuring device of his own if it seems necessary for accurate measurements.

Exposure to ionizing radiation induces free radicals in human nails. These free radicals generate a radiation-induced signal (RIS) in electron paramagnetic resonance (EPR) spectroscopy. Compared with the RIS of tooth enamel samples, that in human nails is more affected by moisture and heat, but has the advantages of being sensitive to radiation and easy to collect. The fingernail as a biological sample is applicable in retrospective dosimetry in cases of localized hand exposure accidents. In this study, the dosimetric characteristics of fingernails were analyzed in fingernail clippings collected from Korean donors. The dose response, fading of radiation-induced and mechanically induced signals, treatment method for evaluation of background signal, minimum detectable dose, and minimum detectable mass were investigated to propose a fingernail-EPR dosimetry protocol. In addition, to validate the practicality of the protocol, blind and field experiments were performed in the laboratory and a non-destructive testing facility. The relative biases in the dose assessment result of the blind and field experiments were 8.43% and 21.68% on average between the reference and reconstructed doses. The results of this study suggest that fingernail-EPR dosimetry can be a useful method for the application of retrospective dosimetry in cases of radiological accidents.
Concrete-filled steel tubular (CFST) members have been widely used in engineering, and their tube diameters have become larger and larger. But there is no research on the thermal field of large-diameter CFST structure. These studies focused on the thermal field of the large-diameter CFST structure under solar radiation. The environmental factors and the actual placement position were considered, and the finite element model (FEM) of the thermal field of CFST members under solar radiation (SR) was established. Then the FEM was verified by practical experiments. The most unfavorable temperature gradient model in the cross-section was proposed. The testing results showed that the temperature field of the large-diameter CFST member section was non-linearly distributed due to the influence of SR. The temperature field results of CFST members with different pipe diameters indicated that the larger the core concrete diameter was, the slower the central temperature changed, and there was a significant temperature difference between the center and the boundary. Based on the numerical model, the most unfavorable temperature gradient model in the section was proposed. The model showed that the temperature difference around the center of the circle is small, and the boundary temperature difference is significant. The maximum temperature difference is 15.22℃, which appeared in the southern boundary area of the specimen. Therefore, it is necessary to consider the influence of SR on the thermal field of the member for large-diameter CFST members in actual engineering, which causes a large temperature gradient in the member.

In this study, the influence of the near field, far field and radiation directivity of microstrip patch antenna when is used mushroom EBG(Electromagnetic Band Gap) as ground is investigated. Using characteristic of dispersion diagram of mushroom EBG, we calculated forbidden band(2.36GHz-2.85GHz) given mushroom type EBG microstrip antenna(2.45GHz) having 2-layer EBG that is operating within forbidden band. In oder to conform performance enhancemen of antenna using EBG ground, we have compared with the antenna using PEC(Perfect Electric Conductor) ground. We could know about 2.74dB increment of the radiation directivity, because EBG can suppress surface wave that is generated at interfaces of the dielectrics-conductor.

Purpose: Even if the wedge filter is widely used for the radiation therapy to modify the photon beam intensity, the wedged photon beam dose calculation is not so easy. Radiation therapy planning systems (RTPS) have been used the empirical or semi-analytical methods such as attenuation method using wedge filter parameters or wedge filter factor obtained from measurement. However, these methods can cause serious error in penumbra region as well as in edge region. In this study, we propose the dose calculation algorithm for wedged field to minimize the error especially in the outer beam region. Materials and Method: Modified intensity by wedge filter was calculated using tissue-maximum ratio (TMR) and scatter-maximum ratio (SMR) of wedged field. Profiles of wedged and non-wedged direction was also used. The result of new dose calculation was compared with measurement and the result from attenuation method. Results: Proposed algorithm showed the good agreement with measurement in the high dose-gradient region as well as in the inner beam region. The error was decreased comparing to attenuation method. Conclusion: Although necessary beam data for the RTPS commissioning was increased, new algorithm would guarantee the improved dose calculation accuracy for wedged field. In future, this algorithm could be adopted in RTPS.
