Concurrent Chemoradiotherapy in Nasopharyngeal Carcinoma (비인강암의 동시 항암화학방사선치료)

  • Purpose:This is a retrospective study to evaluate the results of concurrent chemoradiotherapy in nasopharyngeal carcinoma. Material and Methods:From Mar 2000 to June 2005, 18 patients with nasopharyngeal carcinoma completed planned concurrent chemoradiotherapy. Stages were I in 1 patients, II in 2 patients, III in 7 patients and IV in 8 patients. Pathologic type was squamous cell carcinoma(WHO type 1) in 2 patients, non-keratinizing type(WHO type 2) in 8 patients and undifferetiated carcinoma(WHO type 3) in 8 patients. The follow up period ranged from 30 months to 95 months with a median of 56 months. Follow up was possible in all patients. Results:Response to concurrent chemoradiation therapy was a complete response in all patients. Patterns of failure were as follows:local recurrence in only one patient(5.6%) and distant metastases in three patients with N3 diseases(16.7%). The overall 5 year survival rates were 88.5%, the 5 year disease free survival rate was 77% and these were very good results. There were no significant differences in the local control and survival rates between the clinical stages and pathologic types. Conclusion:The outcome of the nasopharyngeal carcinoma treated with concurrent chemoradiotherapy was very good, even though most of the patients(15/18=83.3%) were in stage III and IV diseases. We concluded that concurrrent chemoradiotherapy in nasopharyngeal carcinoma showed the good local control and survival rates without significant complications. In the patients with N3 disease, we have to consider the more effective and strong chemotherapeutic regimens to prevent distant metastases.

Kanamycin Concentration for Selection of 'Mcintosh Wijcik' Transgenic Apple (사과 'McIntosh Wijcik' 형질전환체 선발을 위한 Kanamycin 농도)

  • Effects of kanamycin concentration on regeneration and rooting of transgenic 'McIntosh Wijcik' were investigated to establish the efficient Agrobacterium-mediated transformation system. Relatively high regeneration frequency of explants appeared even at the high concentration of $150mg{\cdot}L^{-1}$ kanamycin, but the regeneration frequency and the number of normal shoots decreased significantly at a concentration of higher than $100mg{\cdot}L^{-1}$ kanamycin in the gelrite-gellifying medium. Rooting response varied with the transgenic lines in the agar-solidifying medium supplemented with the different concentrations of kanamycin and they were grouped with the inhibition level at $30mg{\cdot}L^{-1}$ concentration. No correlation between copy number and root response was observed. The optimum concentrations of kanamycin for the regeneration of 'McIntosh Wijcik' apple in the medium gellified with gelrite and for indirect-selection of putative transformants in the rooting medium solidified with agar were found to be $100mg{\cdot}L^{-1}$ and $30mg{\cdot}L^{-1}$ respectively.

Statistical Behavior of RC Cooling Tower Shell due to Shape Imperfection (철근콘크리트 냉각탑의 형상불완전에 의한 확률론적 거동)

  • For the large scale reinforced concrete cooling tower shells, the shape imperfection can be introduced due not only to mistakes in the process of construction but also to the long term behavior of concrete. The shape imperfection evokes the additional responses such as displacements and stresses in addition to the design values. In this study, the statistical behavior of the RC cooling tower shell due to the shape imperfection is investigated using the Monte Carlo simulation. The radius of cooling tower and the shell thickness are adopted as the parameters which cause the shape imperfection. The shape imperfection is modeled as a stochastic field rather than the local one of axisymmetric or bulge type of imperfection. The randomness in the radius is shown to be more affecting the structural responses than the randomness in the shell thickness. In addition to the geometrical randomness, the effect of randomness in the modulus of elasticity on the structural response is also investigated and compared with that of the geometrical ones.

Structural Damage Assessment Using Transient Dynamic Response (동적과도응답을 사용한 구조물의 손상진단)

  • A damage detection and assessment algorithm is developed by measuring accelerations at limited locations of a structure under forced vibrations. The developed algorithm applies a time-domain system identification (SI) method that identifies a structure by solving a linearly constrained nonlinear optimization problem for optimal structural parameters. An equation error of the dynamic equilibrium of motion is minimized to estimate optimal parameters. An adaptive parameter grouping scheme is applied to localize damaged members with sparse measured accelerations. Damage is assessed in a statistical manner by applying a time-windowing technique to the measured time history of acceleration. Displacements and velocities at the measured degrees of freedom (DOF) are computed by integrating the measured accelerations. The displacements at the unmeasured DOF are estimated as additional unknowns to the unknown structural parameters, and the corresponding velocities and accelerations we computed by a numerical differentiation. A numerical simulation study with a truss structure is carried out to examine the efficiency of the algorithm. A data perturbation scheme is applied to determine the thresholds lot damage indices and to compute the damage possibility of each member.

Metastatic Cervical Lvrnphadenopathy from Uterine Leiornyosarcorna with Good Local Response to Radiotherapy and Chemotherapy (자궁 평활근육종의 경부림프절 전이와 방사선치료 및 화학요법에의 반응)

  • The metastasis of uterine leiornyosarcorna to the neck node has not been reported previously and the radiotherapy has been rarely used for the metastatic lesion of the other sites. We report a case of neck metastasis from a uterine leiornyosarcorna, which developed 10 months after surgery and postoperative pelvic radiotherapy. It also involved the parapharyngeal space, adjacent spine, and spinal canal. The metastatic neck mass was inoperable, and was treated by neck radiotherapy (6,000 cGy) and chemotherapy including taxol and carboplatin. The mass has regressed progressively to a nearly impalpable state. She has never developed spinal cord compression syndrome, and has maintained good swallowing for eight months since the neck radiotherapy and chemotherapy. Since the extensive metastatic neck mass showed good local response to high dose radiotherapy and chemotherapy, both treatments may be considered for an unresectable metastatic leiornyosarcorna.

Optimization of Gamsulgi Prepared with Persimmon Peel Powder Using Response Surface Methodology (반응표면분석법에 의한 감설기 제조조건의 최적화)

  • To enhance gamsulgi quality, the optimal conditions for persimmon peel powder (PPP) addition were evaluated with PPP content(10, 50), sugar content(5, 25) and water content(10, 50%) by reponse surface methodology (RSM). The statistical significanes of sugar element in gamsulgi preperation did not show. Optimal conditions for the content of PPP and water, predicted for each corresponding sensory parameter of gamsulgi, were 10∼40% and 18∼50% for consistency, 10∼32% and 11∼50% for after swallowing, 24∼50% and 14∼39% for color, 10∼40% and 31∼50% for chewiness, 14∼38% and 22∼47% for moistness, 10∼38% and 22∼50% for texture, and 10∼43% and 18∼50% for overall quality. The overall optimal conditions, which satisfied with all sensory properties of gamsulgi, were 25% for PPP content and 35% for water content.

Optimization of Submerged Culture Conditions for the Production of Ginseng Root Using Response Surface Method (반응표면분석법을 이용한 인삼 Root 액체배양조건의 최적화)

  • To develop the production of ginseng root using plant tissue culture technology, submerged culture conditions were optimized by means of the fractional factorial design with 4 factors and 3 levels by a RSM computer program. The ginseng (Panax ginseng C. A. Meyer) roots induced by plant growth regulators were cultured on SH medium and the effects of various pH of medium, sucrose concentration, nitrogen concentration and phosphate concentration on fresh weight of the ginseng root were investigated. The fresh weight of ginseng root increased with a decrease in nitrogen concentration and fresh weight of ginseng root varied from 1.00 to 2.33g under various conditions. The optimum pH of medium and sucrose concentration determined by a partial differentiation of the model equation, nitrogen and phosphate concentration were pH 5.6, sucrose 3.8%, nitrogen 50 mg/L and phosphate 80.7 mg/L, respectively. Under these conditions, the predicted growth of ginseng root was estimated to be 2.36g.

  • We report three patients with pulmonary hypertension in Takayasu's arteritis, who showed long-term favorable response, clinically and hemodynamically, to the nitric oxide donor, molsidomine. In these patients, the inhaled nitric oxide was effective in reducing pulmonary artery pressure (PAP) and pulmonary vascular resistance (PVR) as was shown in the acute vasodilator response test using the invasive hemodynamic monitoring. Molsidomine (single oral dose of 4 mg) was also effective in reducing PAP and PVR in the acute test, but nifedipine was not. With 4 mg of molsidomine three times daily, their dyspnea, exercise capacity and hemodynamic parameters were improved. These favorable responses have lasted during the 1st and 3rd month follow-up in all patients.

  • Physiological response of Panax ginseng var. atropurpureacaulo (purple stem variety, Pg) to light was reviewed through old literatures and recent experiments. Canopy structure, growth, pigment, leaf anatomy, disease occurence, transpiration, photosynthesis (PS), leaf saponin, photoperiodism and nutrient uptake were concerned. P. ginseng var. xanthocarpus (yellow berry variety, Px) and Panax quinquefolius(Pq) were compared with Pg if possible. Compensation point(Cp) increased with increase of light and ranged from 110 to 150 at $20^{\circ}C$ but from 140 to 220 at $30^{\circ}C$ with 4 to 15 Klux indicating occurence of light and temperature-dependent high photorespiration. Characteristics of Korea ginseng to hate high temperature was well accordance with an observation 2000 years ago. Korea ginseng showed lower Cp and appeared to be more tolerant to high light intensity and temperature than American sheng although the latter showed greater PS, stomata frequency and conductance, chlorophyll and carotenoids. Px showed lower PS than Pg probably due to higher Cp. Total leaf saponin was higher in leaves grown under high light. Ratio or diol saponin and triol saponin(PT/PD) decreased with increase of light intensity during growing mainly due to decrease of ginsenoside $Rg_1$ but increase of ginsenoside Rd. Leaves of Pg and Px had $Rg_1$ but no $Rb_3$ which was only found as much as $20\%$ of total in Pq leaves, and decreased with increase of light intensity. Re increased in Pg and Px but decreased in Pq with increase of light. PT/PD in leaf ranged 1.0-1.5 in Pg and Px but around 0.5 in Pq. Korea ginseng has Yang characteristics(tolerant to high light and temperature), cultured under Eum(shade) condition and long been used for Yang efficacy (to build up energy) while Pq was quite contrary. Traditional low light $intensity(3-8\%)$ for Korea ginseng culture appeared to be strongly related to historical unique quality. Effect of light quality and photoperiodism was not well known. Experiences are long but scientific knowledge is short for production and quality assessment of ginseng. Recent scientific knowledge of ginseng should learn wisdom from old experiences.

Effects of the Instability of International Financial Market on Port Import from China in Korea (국제금융시장의 불안정성이 한국의 대중국 항만 수입에 미치는 영향)

  • This paper examines the relationship between port import from China and macroeconomic variables such as international financial crisis, exchange rate, and industrial production during the period 2000-2009. I employ GPH cointegration methodology since the model must be stationary to avoid the spurious results. The empirical results show that our model is stationary as well as mean-reverting. This paper also applies impulse-response functions to get additional information regarding the responses of the port import to the shocks economic variables such as financial crisis, exchange rate, and industrial production. The results show that the response of port import to exchange rate and financial crisis declines at the first and dies out slowly.
