종양간호사와 일반간호사의 흡연 중재 비교 조사 연구 (A Comparison on the Tobacco Control Intervention, Barriers, and Facilitators between Oncology Nurses and General Nurses in Clinical Practice)

  • Purpose: This study was conducted to 1) find out the frequency of tobacco control intervention, barriers, and facilitators. 2) compare the differences in tobacco control intervention, barriers, and facilitators between oncology nurses and general nurses. Method: A sample was composed of 96 oncology nurses and 284 general nurses. The survey questionnaire was mailed out to nurses who were working at the randomly selected hospitals throughout the country. The questionnaire was adopted from the study of national survey on oncology nurse's tobacco interventions in United States by Sarnar, et al.(2000). Results: Oncology nurses were found to provide tobacco control interventions more frequently comparing to the general nurses. "Patient not motivated to quit smoking", "Lack of time", "Lack of recognition/rewards", were the most commonly identified barriers. "Patient wants to quit", "Adequate time", "Confidence in ability help people to stop smoking", were the most commonly identified facilitators. Conclusions: Although oncology nurses are in an important position in delivering tobacco interventions and providing resources, their participation in consistent delivery of a tobacco control intervention was less than desirable. To help nurses participate in the assessment of tobacco use and interventions for cessation, the development of educational program is necessary.

일부 보건계열 대학생의 흡연실태 및 금연에 대한 태도에 관한 조사 (Health care students smoking status and attitude towards smoking cessation)

  • This study aimed to investigate the actual smoking status, knowledge of oral health related to smoking, and attitude toward smoking cessation in 161 health students of the University in Jeonnam. This was done to examine the role of smoking cessation guidance as health care professionals after graduation. The collected data were analyzed using SPSS version 18.0. The significance level was α=0.05. Of those surveyed, 33.3% had previously smoked, and 30.4% were current smokers. The high school period had the highest smoking rate, at 59.2%. A daily smoking rate of 20 or more cigarettes was the highest at 30.4%. Of the respondents, 68.3% had received smoking cessation, while 10.6% said that they were encouraged by others to quit, such as friends. As a result of a correlation analysis between major satisfaction, smoking-related oral health knowledge, and attitudes toward smoking cessation, major satisfaction showed a negative correlation(r=-.394, -.337*) with smoking-related oral health knowledge and attitude towards smoking cessation. Smoking-related oral health knowledge showed a positive correlation(r=.546**) with attitude towards smoking cessation. As a result of a regression analysis, the knowledge of oral health related to smoking was the primary variable that affected the attitude towards smoking cessation(t=6.799, p<0.001). Based on these results, it is suggested that education on smoking cessation and the risk of smoking is delivered so that health care workers can be better prepared and educated for their future roles.

정상성인에서 흡연, 일반적 특성과 혈청지질과의 상관관계 (The Relationship between Smoking, General Characteristics and Serum lipids in the Normal Adult)

  • The purpose of this study was examine the relationship between smoking, general characteristics, and serum lipids in the normal adult. The subjects consist of 6,800 persons (male ; 4,309(63.4%), female ; 2,491(36.6%)) in A hospital in Kyungki-Do. The collected data were analyzed using T-test, and Multiple Regression by SPSS/WIN program. The result were following ; 1. Cigarette smoking was associated with decreased serum HDL cholesterol level (p<.001). 2. Serum LDL cholesterol level was not statistically correlated with smoking tend to level by 5.77mg/dl respectively. 3. Cigarette smoking was associated with elevated serum Triglyceride level (p<.001). 4. By using regression, HDL cholesterol level was negatively correlated with obesity and alcohol.. HDL cholesterol level was positively correlated with exercise. LDL cholesterol level was positively correlated with diet and obesity. Triglyceride level was positively correlated with obesity and alcohol. In conclusion, Smoking was the most correlated with Triglyceride among the serum lipid and Serum lipid was the most correlated with obesity. Thus, it is necessary to quit smoke and weight control for prevention from disease such as coronary heart disease and lung cancer.

Effect on Smoking Quit Rate of Telling Smokers their Health Risk Appraisal in Terms of Health Age: A Randomized Control Trial

  • Background: We evaluated whether providing health risk appraisal for Koreans (KHRA) in terms of 'health age' during smoking cessation program would effectively help smokers quit smoking or not. Materials and Methods: A total of 332 male smokers aged between 30-65 years old, registered for a smoking cessation program in a public health center in a city, were recruited and underwent a baseline survey from January 2010 to February 2011. They were then prospectively randomized to a conventional counseling group (n=165) or a KHRA group (n=167), and received conventional counseling or KHRA-based counseling for six months. Abstinence rates were identified through carbon monoxide measurement (at the $4^{th}$ and $24^{th}$ weeks) or urinary cotinine level (at the $12^th$ week). Results: The abstinence rate confirmed by exhaled carbon monoxide was significantly higher in the KHRA group (61.1%) than the control group (49.1%) at the $4^{th}$ week (absolute difference 12.0%, 95% CI: 1.4%-22.6%). However, there was no difference in abstinence rates between the two groups at the 12th and 24th weeks. The predicting factors of 24 week's smoking cessation success were age, older than 50 years old (OR 2.02, 95% CI: 1.16-3.52), lower Fagerstr$\ddot{o}$m Test for Nicotine Dependence score less than 4 (OR 1.84, 95% CI: 1.03-3.29), and higher Self Efficacy/Temptation score (OR 1.79, 95% CI: 1.05-3.06). Conclusions: Smoking cessation counseling with KHRA could be effective compared to conventional counseling in the short period of smoking cessation. Further study is needed to evaluate the long-term efficacy of KHRA in tobacco dependence treatment and to establish the indication and target population of this tool.

일 지역 고등학생의 금연의도 영향 요인 (Factors Influencing Intention to Quit Smoking of High School Students in a Community)

  • Purpose. This study uses Ajzen's theory of planned behavior to investigate predictors associated with the intention of smoking cessation in smokers among high school students in a community. Method. The study population consisted of 138 smokers from high school students in a community. Between May to April 2015, Data was collected through questionnaires. Data analysis was performed using the PASW 21.0 program, and one-way ANOVA, Pearson correlation coefficients, and stepwise multiple regression analysis. Results. The mean intention of smoking cessation of this study was 3.98/5. The significant correlates of the intention of smoking cessation were attitudes toward smoking cessation (${\beta}=.36$, p=.003), self efficacy for smoking cessation (${\beta}=.44$, p<.001), satisfaction of school life (${\beta}=.22$, p=.009), number of friends smoking (${\beta}=.76$, p<.001), and previous experience of smoking cessation (${\beta}=.92$, p<.001). These variables explained 75.6% of variance in the intention of smoking cessation. Conclusion. The study suggests that various factors including previous experience of smoking cessation, number of friends smoking, self efficacy for smoking cessation and attitudes toward smoking cessation were important variables in explaining the intention of smoking cessation among high school students. Therefore, these variables are crucial in determining the promotion and intention of smoking cessation among high school students.

인터넷과 모바일 금연교육프로그램의 효과 비교분석 (Evaluation of an Internet and a Mobile-Based Smoking Cessation Program)

  • The potential contribution of the internet and mobile phone to smoking cessation seems huge, given that a majority of Koreans now use internet and have mobile phones. The Korean Ministry of Health and Welfare developed and has been offering an internet-based smoking cessation program from the January, 2002 and a mobile-based program from the April, 2004. Despite the anticipated effectiveness of internet and mobile-based smoking cessation programs, there is little empirical evidence regarding the efficacy of both types of programs. Therefore, this study intended to evaluate the effectiveness of an internet and a mobile-based smoking cessation program and to provide the basic database for the development of effective smoking cessation strategies for the health promotion for the whole nation. Cessation and satisfaction data were obtained from the 434 participants in both programs. The data were analysed for the comparison of quit-smoking rate and the degree of satisfaction between an internet smoking cessation program and a mobile smoking cessation program. The main results of this study were as follows: 1. For the internet program participants, there were statistically significant differences in cessation rate by gender(P<0.05) and age(P<0.01). In contrast, none of general characteristics provided statistically significant differences in cessation rate for the mobile program participants. 2. For the internet program participants, smoking related characteristics(average smoking amount and the reasons for smoking) didn't have an effect on smoking cessation. But, for mobile program participants, there was a statistically significant difference in cessation rate by the reasons for smoking(P<0.05). 3. The cessation rate was 31.9%(abstinence for the previous 30 days) for internet program participants and 17.8% for mobile program participants.

병원기반 금연중재가 뇌경색 환자의 퇴원 후 12개월 지속 금연율에 미치는 효과 (The effect of a hospital-based smoking cessation intervention on the continuous abstinence rate from smoking over 12 months in patients with cerebral infarction)

  • Objectives: The aim of this study was to investigate the effectiveness of a hospital-based smoking cessation intervention for increasing continuous abstinence rate from smoking in patients with cerebral infarction. Methods: One-hundred and two smokers with cerebral infarction who decided to quit smoking were enrolled in the smoking cessation intervention from December 2012 to February 2015. The smokers underwent six consecutive times of individual intervention with nurse specialist on smoking cessation including education on behavioral modification, counseling for withdrawal symptoms, and anti-smoking advice over a 12-month period. Results: Among the total participants, the continuous abstinence rate from smoking changed from 79.4% at 1 month to 60.8% at 12 months after discharge. The continuous abstinence rate from smoking after 12 months was 88.5% in participants who completed the entire program (6 times), while 51.3% in participants who did not complete the entire program (${\leq}5$ times) (P=0.001). After adjustment for general and smoking-related characteristics, complete implementation of hospital-based smoking cessation intervention was significantly associated with continuous abstinence from smoking after 12 months (odds ratio: 5.93; 95% confidence interval: 1.45-24.22). Conclusions: The hospital-based smoking cessation intervention might be effective for smoking cessation in patients with cerebral infarction, especially when the intervention was implemented thoroughly.

금연에 관한 성인남성의 인식 및 금연동기 조사 (Understanding Male Adult's Awareness and Motivation of Non-smoking)

  • 본 연구는 흡연자와 비 흡연자의 금연결심에 대한 인식구조를 알아보고, 금연 교육기관과 금연 교육자의 선택 및 흡연자의 금연 동기부여 관련사항을 알아보고자 시행된 연구이다. 흡연자와 비 흡연자는 금연 정책 인식에 차이가 있었고, 금연 교육기관의 선택에서는 보건소가 높았으며, 금연 교육자는 내과의사의 비중이 높았으나 인식차이는 없었다. 일반적 사항에 따른 금연 동기부여 간 관련성에서는 폐암사진, 치과치료, 담배 값 인상, 금연 구역지정, 금연 광고 등이 흡연자의 금연 결심 원인(동기)와 금연의 동기부여 간의 결과에서 통계적으로 유의하게 나타났다[P=.000].

Effectiveness of a Brief Physician Counselling Session on Improving Smoking Behaviour in the Workplace

  • Background: Brief physician counselling has been shown to be effective in improving smokers' behaviour. If the counselling sessions can be given at the workplace, this would benefit a larger number of smokers. This study aimed to determine the effectiveness of a ten-minute physician counseling session at the workplace in improving smoking behaviour. Materials and Methods: This prospective randomised control trial was conducted on smokers in a factory. A total of 163 participants were recruited and randomised into control and intervention groups using a table of random numbers. The intervention group received a ten-minute brief physician counselling session to quit smoking. Stages of smoking behaviour were measured in both groups using a translated and validated questionnaire at baseline, one month and three months post intervention. Results: There was a significant improvement in smoking behaviour at one-month post intervention (p=0.024, intention to treat analysis; OR=2.525; CI=1.109-5.747). This was not significant at three-month post intervention (p=0.946, intention to treat analysis; OR=1.026; 95% CI=0.486-2.168). Conclusions: A session of brief physician counselling was effective in improving smokers' behaviour at workplace, but the effect was not sustained.

흡연 남자대학생의 스트레스, 금연에 대한 자기효능감 및 흡연유혹이 니코틴 의존도에 미치는 영향 (Influence of Stress, Self-efficacy for Smoking Cessation, Smoking Temptation and Nicotine Dependency in Male College Students who Smoke)

  • Purpose: The purpose of this study was to explore the levels of stress, self-efficacy for smoking cessation, smoking temptation, and nicotine dependency, and to identify factors influencing nicotine dependency among male college students who smoke. Methods: In this study, a cross-sectional survey design was adopted for 283 male college students who smoke in D city, Korea. Data was analyzed for descriptive statistics, Pearson's correlation, and multiple regression using the SPSS 20.0 program. Results: The mean score of stress was $27.00{\pm}6.28$, self-efficacy for smoking cessation was $25.61{\pm}6.71$, smoking temptation was $53.87{\pm}11.02$, and nicotine dependency was $3.63{\pm}1.96$. There was a significant positive correlation between stress and nicotine dependency (r=.58, p<.001) and between smoking temptation and nicotine dependency (r=.59, p<.001). There was a significant negative correlation between self-efficacy for smoking cessation and nicotine dependency (r=-.59, p<.001). The significant factors influencing nicotine dependency were stress (${\beta}=.357$, p<.001), self-efficacy for smoking cessation (${\beta}=.359$, p<.001), and smoking temptation (${\beta}=.297$, p<.001). This model explained 60.3% of variance in nicotine dependency (F=105.59, p<.001). Conclusion: The results suggest that an intervention program is needed to reduce the perception of stress and smoking temptation, and to increase the ability of self-efficacy for smoking cessation among male college students who smoke with the intention to quit smoking.
