요양병원의 서비스 질 평가 영역 수립을 위한 질적 연구 (Quality Dimension of Long Term Care Hospital)

  • Purpose: This is a qualitative study to identify dimensions of long-term care hospital care quality that provide high-level medical services for long-term care patients in Korea. Methods: Service consumers and providers were interviewed, and collected data were analyzed into thesis, type and dimension. The focus group method was applied to two provider groups and individual interview was applied to two persons who had experienced a long-term care hospital. Results: The results of analyzing the consumers and providers was integrated into 8 dimensions: physical environment, staff, clinical care and nursing, multiplicity of activity program, atmosphere, interaction with family, nutrition, and quality improvement system. Conclusion: The dimensions of long-term care hospital care quality from this study can be used as a basis of quality indicators. Quantitative studies to test these dimensions are required for establishing quality management systems.

장기요양시설 노인의 기능상태에 따른 간호서비스 필요도 예측 (Predictors of Nursing Service Need for Nursing Homes Residents)

  • Purpose: The purposes of this study were to explore the functional status of elderly residents and to analyze time use, and finally identify factors to predict nursing care needs in relation to functional status and health related variables. Methods: In this study a descriptive-correlational design was used. Functional status of participants was obtained through interviews, and nursing care time was examined using a 1 min time-motion study with a standardized instrument developed by Korea Long-Term Care Planning Committee (2005). Results: The mean total functional score was 65 (range 28-125) and mean total nursing care time was 144.15 min per day. There were significant positive relationships between total nursing care time, marital status, back pain, dementia, and vision impairment. Multiple regression analyses showed that a liner combination of number of illnesses, types of primary disease, ADL, IADL, cognitive function, nursing demand, and rehabilitation demand explained 42.8% of variance of total nursing time. ADL (${\beta}$=-.533) was the most significant predictor of nursing service need. Conclusion: Identifying factors that result in variations of service need has implications for adequate nursing service, estimation of optimum nurse to patient ratio, quality of care and patient safety.

급성기병원에서의 간호 · 간병통합서비스 운영 전후 낙상 및 욕창 발생 변화 (Performance of Comprehensive Nursing Care Service in an Acute Care Hospital: Focusing on Accidental Falls and Pressure Injuries)

  • Purpose: This study aimed to investigate whether the comprehensive nursing care service positively affected accidental falls and pressure injuries. Methods: This study was a retrospective study that analyzed the accidental falls and pressure injuries cases in an acute care hospital located in Seoul and compared the rates of accidental falls and pressure injuries before and after the comprehensive nursing care service was operated. Results: Comparing the accidental fall incidence rates per 100 person-months between a comprehensive nursing care ward and a general ward, it showed fewer accidental falls by 0.44 in comprehensive nursing care wards, but the result was not statistically significant. In the case of pressure ulcers, the incident rate per 100 person-month was 6.17 in general wards and 4.77 in comprehensive nursing care wards, which showed that the number of pressure ulcer patients was lower in comprehensive nursing care wards, however it was also not statistically significant. Conclusion: It is not confirmed that the operation of the comprehensive nursing care service contributes to the reduction of accidental hospital falls or pressure injuries. Follow-up studies are recommended to determine the effectiveness of comprehensive nursing services in quality indicators.

보건소 방문 간호 사업의 질보장을 위한 연구 (A study for quality assurance of visiting nurses service of a public health center)

  • The purpose of the study is to evaluate the visiting nurses service of a public health center. Data were collectd from the 36 clients who received services from a public health center. In terms of the process evaluation, the tool is composed 4 parts, 27 items such as assessment planning, implementation, and evaluation. It was measured through the health records by 2 peer review. In terms of the outcome evaluation, the level of client satisfaction was measured by self report or interview by 2 supervisor. The result were as follows: 1. 30% of 36 health records showed narsing process was not and out of them, nursing care plann including spectific activities were rarely established or unclear. 2. The lack of systematic data collection' showed and nursing diagnosis was not adressed in health records review. 3. Client satisfaction score was 32, 97, out of maximum score 36. 4. The lack of sufficent objective data, care plan, record of client's health status change, and evaluation was founded therefore quality assurance for visiting nurses service and in-service education are required and the development of standardized record system need.

간호·간병통합서비스병동과 일반병동 간호사의 피로, 수면의 질, 대인관계 스트레스 비교 (Comparison of Nurses's Fatigue, Sleep Quality and Interpersonal Relationship Stress between Comprehensive Nursing Care Service Wards and General Wards)

  • Purpose: The purpose of this study was to compare the differences in fatigue, sleep quality and interpersonal relationship stress levels between nurses in comprehensive nursing care wards and in general wards. Methods: Data were collected using self-reported structured questionnaire from 14 August to 17 September 2018. The participants were 220 nurses from 6 hospitals in Seoul and Gyeonggi-do. The data were analyzed using the IBM SPSS 23.0 program, descriptive statistics, t-test, ${\chi}^2-test$, Fisher's exact test, and Pearson's correlation coefficient. Results: The subjects' fatigue, sleep quality, and interpersonal relationship stress were not significantly different between the comprehensive nursing care services wards and the general wards. The interpersonal stress with a superior was statistically higher among general wards than comprehensive nursing care services wards. The relationship stress between patient and caregiver was higher in comprehensive nursing care services wards than in general wards. Conclusion: A follow-up study is needed to develop a strategic method to reduce interpersonal stress in nurses.

노인요양시설의 서비스 질에 영향을 미치는 요인에 관한 연구: 요양보호사의 공공성 인식을 중심으로 (A Study on the Influens of Care Worker's Perception of Publicness on Service Quality in Nursing Facility for the Elderly)

  • 본 연구의 목적은 노인요양서비스의 공공성을 강화하고 서비스 질을 높이기 위한 것이다. 이를 위해 노인요양시설의 요양보호사들이 공공성 인식은 어느 정도이고, 공공성 인식정도가 서비스 질에 영향을 미치는가를 실증 분석하였다. 공공성 인식정도는 요양보호사가 인식하는 규범적 공공성, 조직미션의 동의정도로 측정하였으며, 통제변수로는 조직의 법인여부, 규모 그리고 요양보호사의 성별, 연령, 경력, 자격으로 하였다. 전국의 노인요양시설에서 근무하는 요양보호사를 대상으로 설문조사하여 수집된 자료(318명)를 다중회귀방법으로 분석하였다. 연구결과 요양보호사들이 인지하는 공공성의 정도와 서비스 질은 보통이상 이었다. 요양보호사의 규범적 공공성 인식정도는 서비스 질 구성요소 중 대응성을 제외한 모두에서 긍정적 영향을 보였으며, 조직미션의 동의정도는 모든 요소에 정적인 영향을 나타냈다. 또한 조직요인 중에서 조직의 규모를 보여주는 현원과 법인여부 역시 부분적으로 서비스 질에 긍정적 영향을 나타냈고, 요양보호사의 나이는 신뢰성과 공감성에 유의미한 영향을 보였다.

보육시설 내 여성노인 자원봉사활동 도입을 위한 탐색적 연구 -보육시설 종사자를 대상으로- (A Study on the Elderly Women's Voluntary Activities for Child Nursing Facilities -Centering around workers of child nursing facilities-)

  • The purpose of this study is to develop a program that allows older women to provide voluntary services for child nursing facilities, making those women have more social participation, effectively their spare time and ultimately improve the quality of their life. Results of the study can be described as follows. First, social requests about the use of older women as volunteers for child nursing services were analyzed to show that most people working at child nursing facilities perceived lack in the number of child nursing teachers. Those people recognized the need of using older women as service volunteers for the facilities. They were very positive about such use. Second, it is recommendable that volunteers of child nursing service should be not too much older women, or below 65. Voluntary activities that those women participate include taking care of younger children, reading interesting stories for children, helping works of child homes, teaching manners and living attitudes, instructing dietary etiquettes and cooking. Voluntary service activities should be made 1 to 3 times a week, 1 to 2 hours a time, considering psychological and physical states of older people. Third, such older volunteers should be educated about 6 areas, 'the operation and management of child homes, 'practices', 'child health and safety', 'the quality of child nursing', 'child development and counseling' and 'voluntary service training'. Out of these six areas, 'child health and safety' and 'voluntary service training' are more required to be instructed. such training needs to focus on activities in which older women can actually engage as volunteers. Fourth, it is advisable that the recruitment and management of such volunteers are led by the city and that the development of training programs for voluntary service activities and education of the volunteers are referred to universities.

대상자가 인지한 의료서비스 질과 재이용 의도와의 관계 (The Relationships Between Patient's Perceived Quality of Healthcare Service and Intention to Re-visit)

  • Purpose: This study was performed to identify the relationship between patient's perceived quality of healthcare services and intention to re-visit. Method: The study subjects were 242 patients from three local clinics. The data were analyzed using SPSS PC+ 12.0 program for descriptive, Pearson's correlation coefficient, and multiple regression analysis. Result: Patients' perceived quality of healthcare services was measured 5.73$\pm$0.71 using the 7-point scale. The patients' perceived quality was moderately correlated with patient satisfaction (r=0.647, p<0.001) and intention to re-visit (r=0.604, p<0.001). Also, patient satisfaction was highly correlated with intention to re-visit (r=0.788, p<0.001). 3) the reliability factor of patients' perceived quality of healthcare services and patient satisfaction were found significant predictors of intention to re-visit. Conclusion: In conclusion, service quality evaluated by patients influenced on their intention of re-visit and especially, reliability was the most significantly influential variable.

간호·간병통합서비스 병동 입원환자의 간호만족도에 영향을 미치는 요인 (Factors affecting Nursing Satisfaction of Inpatients in Comprehensive Nursing Care Service Ward)

  • Objectives: The purpose of this study is to evaluate the factors affecting patient satisfaction for the nursing service of comprehensive nursing care service unit in Tertiary hospital Methods: This study employed a descriptive survey to investigate the nursing satisfaction and the study data was collected from 74 patients among 83 patient who were admitted to the nursing care integration service ward of an tertiary hospital for 10 days from May 9, 2018 to May 19, 2018. Data was analyzed by descriptive statistics, t-test, ANOVA, and multiple linear regression using SPSS 21 program. Results: The average with nursing satisfaction was significantly different according to marriage type and KPCS level. It was found that physical factor and educational factor have statistically significant effect on nursing satisfaction. As physical care increase by one unit, patient satisfaction increase by 0.226(95% CI; 0.061-0.390) and also satisfaction increase by 0.443(95% CI; 0.070-0.816) as education care increase by one unit. Conclusions: As a result of study, it was found that it is necessary to provide patients with more careful nursing service in physical and educational perspective, in order to improve the service quality of comprehensive nursing care service unit.

산부인과 환자의 의료기관에 대한 이미지와 서비스 질이 고객만족에 미치는 영향 (Effect of Images about Medical Organizations and the Quality of Medical Service on Customer Satisfaction in Obstetrics and Gynecology Patients)

  • Purpose: The purpose of this study was to identify the effects images about medical organizations and the quality of service on customer satisfaction of obs and gyn patients. Method: The subjects of this study were selected conveniently 220 women among obstetrics and gynecology outpatients( who visited H- doctor's office of the first medical organ, P- hospital of the second medical organization, Chospital of the third medical organization). The data were collected from August, 20th 2004 using structured questionaires which included modified form of SERVPERF and customer's satisfaction scale by oliver & swan(1989), and modified form of image scale by kang(1997). Results: Image of hospital(45.3%), visiting frequencies(9.3%), service provider (3.9%) and convenient use(1.2%) of the quality of medical service were significant predictors to explain customer's satisfaction. Conclusion: To increase customer's satisfaction of obs and gyn patients, it is required to developed strategies that improve image of hospital and the quality of service to service provider and convenient use of hospital.
