  • Nitrous oxide has acquired widespread use as a tool for management of anxious and comprehensive pediatric dental patients. Clinical impressions have suggested that inhalation sedation with $N_2O/O_2$ reinforces the effect of other sedatives. Fifteen uncooperative children, mean age of 37months, who were regarded as fail in the first treatment using chloral hydrate-hydroxyzine orally, midazolam orally or intranasally, were sedated with the same sedatives in conjunction with $N_2O/O_2$. All the children were restrained in a pediwrap and were monitored with pulse oximeter for their heart rate and oxygen saturation. Degree of sleep, crying and movement were evaluated. The clinical results indicated that a satisfactory level of sedation was achieved in approximately two thirds of the cases (about 66 %) under those sedation manner. Complications were rare and those of treatments were successfully completed without difficulty.

Study on the shouting breathing pattern while jogging wearing a mask

  • Because of the COVID-19 epidemic, many countries have made the obligation to wear masks normal. Wearing masks in public places has become a must. At present, wearing a mask to participate in sports makes it very common. People seek to gain health through exercise but ignore the potential respirato-ry health threat. That is, wearing a mask will cause a decrease in oxygen content in the body. This neg-ative impact becomes more prominent as the wear-ing time and oxygen consumption increase. To pro-tect people from viruses and enjoy a healthy life. This paper proposes a breathing pattern that im-proves blood oxygen saturation while wearing a jogging mask and walking. Namely, shouting breathing pattern. Use a pulse oximeter to measure the blood oxygen saturation of running at different speeds and compare the normal breathing pattern and the shouting breathing pattern. The results show that the shouting breathing pattern has a sig-nificant improvement in the blood oxygen satura-tion of low-speed walking and medium-speed jog-ging.

u-EMS : An Emergency Medical Service based on Ubiquitous Sensor Network using Bio-Sensors (u-EMS : 바이오 센서 네트워크 기반의 응급 구조 시스템)

  • The bio-Sensors, which are sensing the vital signs of human bodies, are largely used by the medical equipment. Recently, the sensor network technology, which composes of the sensor interface for small-seize hardware, processor, the wireless communication module and battery in small sized hardware, has been extended to the area of bio-senor network systems due to the advances of the MEMS technology. In this paper we have suggested a design and implementation of a health care information system(called u-EMS) using a bio-sensor network technology that is a combination of the bio-sensor and the sensor network technology. In proposed system, we have used the following vital body sensors such as EKG sensor, the blood pressure sensor, the heart rate sensor, the pulse oximeter sensor and the glucose sensor. We have collected various vital sign data through the sensor network module and processed the data to implement a health care measurement system. Such measured data can be displayed by the wireless terminal(PDA, Cell phone) and the digital-frame display device. Finally, we have conducted a series of tests which considered both patient's vital sign and context-awared information in order to improve the effectiveness of the u-EMS.

Comparison of oxygen saturation, heart rate of cerebral palsy and normal child between the pre-ambulation and post-ambulation (뇌성마비아동과 정상아동의 보행전후 산소포화도 및 심박수 비교에 관한 연구)

  • Since the current tendencies show us the increasing number of cerebral palsy children and the standard longevity, we need to find out more research about the following various problems. Therefore I have tried to figure out the difference of oxygen saturation and heart rate between before ambulation and after. Objects chosen are 17 C.P children on the process of treatment those who were able to walk and 8 normal children in Ah-San hospital, Gang-Nung. They haven't had either any operation or suffered heart disease and I measured their oxygen saturation and heart rate by using pulse-oximeter and are analyzed by SPSS (10.07 version). Results are the followings; 1. There was no difference of oxygen saturation and heart rate between pre-ambulation and post-ambulation of normal and spastic hemiplegia children. 2. There showed the significant statistic difference of oxygen saturation, heart rate between pre-ambulates and post-ambulation of normal and spastic diplegia(p<.05). 3. There was no difference of oxygen saturation, heart rate in C.P between(p>.05), but shows the significant statistic difference in heart rate(p<.05). As I compared the oxygen saturation and heart rate of spastic and normal children on between pre-ambulation and post-ambulation, theres is significant statistic difference on both items (p<.05). However there was no difference of oxygen saturation among hemiplegia, normal and diplegia children while the pulse rate showed the significant difference(p<.05). According to this clinical research, CP children's oxygen saturation and heart rate had no change between pre and post compared to normal children. But there was difference in diplegia. This is why we need to invest time to study these kinds of research about various analysis and comparison of oxygen saturation and heart rate, and furthermore making use of pulse-oxymetry in physical therapy room for the children involved would be beneficial to calculate in accuracy without any discomfort for the patient as well.

DentalVibe versus lignocaine hydrochloride 2% gel in pain reduction during inferior alveolar nerve block in children

  • Background: Inferior alveolar nerve block (IANB) is the most common, painful, and anxiety-provoking procedure involving needle insertion for anesthetic solution deposition. DentalVibeⓇ (DV) delivers vibration at a sustained frequency as a counter-stimulation to the site of injection, thereby alleviating pain. The aim of this study was to evaluate and compare the effectiveness of DV and lignocaine hydrochloride 2% gel (Lox 2% jelly) in pain reduction during IANB in children. Methods: A split-mouth randomized clinical trial was designed with a sample of 60 children (age, 6 to 12 years) requiring bilateral IANB for various dental procedures; DV was used while administering IANB and Lox 2% jelly was used as the topical anesthetic before administering IANB at subsequent appointments. During both appointments, pain perception was measured using the sound, eye, motor (SEM) scale and Wong-Baker faces pain rating scale (WBFPRS); oxygen saturation (SpO2) and pulse rate were measured using a pulse oximeter before, during, and after the IANB procedure. The obtained values were tabulated and subjected to statistical analysis. Wilcoxon test was used for intergroup comparison, and Friedman test, for intragroup comparison of measured variables at different treatment phases. Results: The medians and interquartile ranges of the WBFPRS scores recorded during the IANB procedure for DV and Lox 2% jelly were 2 (2-4) and 2 (0-2), respectively (P < 0.05). The SEM scale scores, mean SpO2, and pulse rate did not show any significant differences during the IANB procedure between both treatments. Conclusion: Both DV and Lox 2% jelly were found to be effective in pain reduction during IANB in children.

Development of Sleep-disordered Breathing Detection System using Air-mattress and Pulse Oximeter (에어 매트리스와 산소 포화도 측정기를 이용한 수면호흡장애 자동 검출 시스템 개발)

  • The present study proposes a system that can detect sleep-disordered breathing automatically using an air mattress and oxygen saturation. A thin air mattress was fabricated to reduce discomfort during sleep, and respiration signals were acquired. The system was configured to be synchronized with a polysomnography to receive signals simultaneously with other bio-signals. The present study has been conducted with nine adult male and female patients with sleep-disordered breathing, and sleep-disordered breathing events have been detected by applying the signals acquired from the subjects to the rule-based detection algorithm. The sensitivity and positive predictive values were found to evaluate the performance of the system, which are 91.4% and 89.7% for all events, respectively. The comparison of apnea hypopnea index(AHI) between the polysomnography and the proposed method showed squared R-value of 0.9. This study presents the possibility of detecting sleep-disordered breathing at hospitals or homes using the proposed system.

Analysis of Research Related to the Neonatal Pain Relief Intervention in Korea (신생아 통증완화 중재 관련 국내 연구분석)

  • Purpose: The purpose of this study was to identify patterns and trends of studies, analyze the research, and improve direction of nursing research related to the neonatal pain relief intervention in Korea. Method: The studies were selected from dissertation, nursing journals and others in Korea. Result: Until 1995, there were no studies related to the neonatal pain relief intervention. The most studies have been increased rapidly after 2005. Pure of true experimental design of research design was used 5, Quasi experimental design was 7, preexperimental design was 7. Participants were preterm baby was 8, full-term baby was 9, and preterm and full-term baby was 2. Utilization of instruments as follows: NIPS, PIPP, ABSS, NFCS, pulse oximeter, EKG monitoring, and stethoscope. Contents of the research studies were classified 4 different types, such as studies of about the effect of auditory stimulation, taste stimulation, tactile stimulation, and topical anesthetic cream. The results of 25 studies were effective for the neonatal pain relief, but the results of 5 studies weren't. Conclusion: In the future studies need to develop the various instrument which is assessment of neonatal pain. It is important to the integrated by meta analysis. Additionally, we should develop protocol nursing intervention for the effective pain release.

  • 본 논문에서는 홈 헬스케어에서 건강측정을 위하여 가장 많이 사용되는 개인 건강 기기(personal health device)인 혈당계, 혈압계, 심박측정계(heart rate monitor), 산소포화도 측정계(pulse-oximeter), 심전도 측정계(ECG monitor)의 5가지 홈 헬스케어 의료기기를 구성하고 표준 핵심 기술 요소인 ISO/IEEE 11073, HL7을 준용하는 게이트웨이 플랫폼을 스마트폰에 적용하여 기존의 홈 헬스케어 시스템을 구성하는 단말기 형태의 게이트웨이나 홈 서버 하드웨어 구성을 필요로 하지 않으면서 집안의 고정된 게이트웨이로 인해서 생기는 위치종속성 문제를 해결할 수 있는 PMG(personal mobile gateway) 기반의 홈&모바일 헬스케어 시스템을 개발하고자 한다.

The Effects of Chest Vibration Prior to Endotracheal Suctioning on Oxygen Saturation, Heart Rate and Lung Secretions in Premature Infants (미숙아의 기관지흡인 전 흉곽진동 간호중재가 산소포화도, 심박동수와 기도분비물의 양에 미치는 영향)

  • Prematurity is the main cause for respiratory disorders in neonates. The goal in respiratory management is to maintain respiration with adequate oxygenation. Chest vibration(CV) prior to on dotracheal suctioning(ETS) has been arbitrarily applied to ventilated premature infants without the scientific evaluation of its safety and efficacy. A repeated measure within subjects experimental study was conducted to investigate the effects of CV prior to 875 on oxygenation and lung secretions in twenty-one ventilated premature infants. The independent variable was the type of research protocol, the control type (275 without C.V) and t he intervention(ETS with CV). The dependent variables were oxygen saturation(SpO₂), heart rate (HR), measured by pulse oximeter, and the amount of lung secretions measured in gram. The results showed there was no difference in SpO₂ responses regardless CV employed before ETS. But there was a significant difference in HR responses between the control and the intervention, even without clinical significance. There was a significant difference in the amount of lung secretions retrieved during ETS with CV, compared to ETS without CV. This study suggested the safety of CV by demonstrating no clinically significant changes in SpO₂ and HR in premature infants. The efficacy of CV could be supported by the increases in the amount of sputum during ETS with CV compared to ETS without CV in premature infants.

A Study on the Temperature Dependence of PPG Measurement Devices (PPG 측정 장치에서의 온도 영향에 대한 연구)

  • This paper presents the temperature dependence of transmission-type and reflection-type PPG measurement devices that have been developed in the previous research. PPG signal can be distorted by external temperature such as skin temperature so that many of research was focused on the skin temperature effect. However, this paper focuses on the temperature of the device itself and we studied on the effect of device internal temperature. Experimental results showed that the temperature was increased like an irrational function graph and the transmission-type was not affected by the internal temperature but the reflection-type was affected by the internal temperature.
