Radiotherapy in aggressive cutaneous pseudolymphoma: a case report and review of literature

  • Pseudolymphoma is a nonspecific disease characterized by lesions with lymphomatous-appearing but benign accumulation of inflammatory cells. They generally present as small ulcero-nodular lesions confined to skin which often respond to local therapies. We describe an unusual presentation of an extensive and locally aggressive cutaneous pseudolymphoma in a 21-year-old male patient who presented with extensive cutaneous eruptions gradually progressing over 6 years to involve the entire circumference of his left arm. Magnetic resonance imaging scans of his left arm showed a lesion deeply infiltrating into the soft tissue reaching up to the humerus with intense periosteal reaction. He was successfully treated with radiotherapy after many failed attempts with surgery and chemotherapy.

과혈관성 종양으로 오인된 간의 가성림프종: 증례보고 (Hepatic Pseudolymphoma Mimicking a Hypervascular Tumor: A Case Report)

  • 간의 가성 림프종은 비종양성 림프구의 절 외 과다증식에 의해 형성되는 드문 양성 종양이다. 검색한 바에 따르면 현재까지 영문으로 46예가 보고되었다. 우리는 75세 여자환자에서 과혈관성 종양으로 오인된 간의 가성 림프종 증례를 보고하고자 한다. 조직검사상 직장의 신경내분비 암종이 진단된 후 시행한 조영증강 전산화단층촬영에서 1.0 cm 크기의 경계가 불분명하고 낮은 감쇄를 보이는 결절이 간에 관찰되었다. 자기공명영상에서는 동맥 조영기에 조영 증강을 보이고, 20분 지연기 조영 영상에서 저신호 강도를 보이며, 확산강조영상에서 고신호 강도를 보였다. Fluorodeoxyglucose (이하 FDG) positron emission tomography (PET)/CT에서는 FDG의 섭취 증가를 보였다. 초음파에서는 저에코성 병변으로 관찰되었고, 초음파 유도 하 조직검사를 통하여 간의 가성 림프종으로 확진 되었다.

  • Immunoglobulin G4 (IgG4)-related disease is an inflammatory condition characterized by IgG4 positive plasma cell infiltration. It can affect any organ in the body and mainly involves the pancreas, liver, biliary tracts, orbits, salivary glands and lymph nodes. It can manifest as an inflammatory pseudotumor. Pseudolymphoma as an inflammatory pseudotumor is a group of benign tumors that exhibit histological and clinical features suggestive of malignant lymphoma. Studies on IgG4-related disease are rarely reported, and no case of the disease that involved the maxillary bone and adjacent soft tissue, except for the skin, has been reported. Therefore, we report herein a case of pseudolymphomatous IgG4-related disease that involved the maxilla, with a literature review.

  • Cutaneous lymphoid hyperplasia (CLH) is a cutaneous pseudolymphoma with a worldwide distribution, equally affecting all races and ethnic groups. Due to its vast array of characteristics, it is most often missed in the differential diagnosis of firm to soft lumps on the head and neck. A systematic approach to the workup and diagnosis along with treatment of such lesions is discussed in this article. A 20-year-old Asian Indian female presented to our Oral and Maxillofacial unit with a lump on the left side of her forehead for 1 month. Local examination revealed a $2.5{\times}3.0cm^2$, well circumscribed swelling over the left para median region that was firm to doughy and non-tender. There was no other significant finding on general examination. Excisional biopsy of the lesion was performed, followed by histopathologic processing. The general etiology, pathogenesis, clinical presentation, differential diagnosis, clinical course, prognosis, treatment, and prevention have been discussed in line with the recent modalities of diagnosis and treatment of CLH. Due to the overlapping clinical and histological characteristics of CLH with many other lesions, it is important to consider this lesion in the differential diagnosis of cutaneous lesions.

Multifocal nodular lymphoid hyperplasia of the lung

  • Nodular lymphoid hyperplasia (NLH) is a benign lymphoproliferative disease that can affect the lung. Because of its rarity, little is known about the etiology and natural history of NLH. Most cases are usually asymptomatic and found incidentally on imaging studies. Imaging finding of NLH has shown most commonly as a solitary lesion, although multifocal pulmonary nodules may be seen. Surgical resection has proved curative in the cases previously described. We report a rare case of NLH in a 55 year-old man who presented with bilateral multiple pulmonary nodules on chest radiography. Open biopsy was performed from the upper and lower lobe of the left lung. The lesions were pathologically diagnosed as pulmonary NLH. Multifocal residual nodules in both lungs remain stable without spontaneous regression during the 3 years of follow-up.

  • Nodular lymphoid hyperplasia is a very rare benign disease and usually shows consolidation on chest X-ray with symptoms of pneumonia due to the proliferation of lymphoid cells in the lung parenchyma. It is common for patients to be diagnosed with pneumonia and treated with antibiotics, but patients often enter a cycle of repeated improvement, followed by aggravation of symptoms. At this point, surgical diagnostic tools are considered in order to differentiate between malignant disease and interstitial lung disease. Here, we report 2 cases of patients with nodular lymphoid hyperplasia and review the associated references.

  • 기관지 연관 림프조직(BALT) 림프종은 폐에서 드물게 발생하는 low grade B-cell 림프종으로서, 대부분 증상이 없고 예후가 좋으며 centrocyte-like cell과 lymphoepithelial lesion이 특징적으로 관찰되어 타 장기의 림프종과는 구별되는 독특한 질병단위를 이루고 있다. 최근 면역조직화학 기법과 분자유전학 기법을 이용하여 단일클론성을 증명함으로써 BALT 림프종의 진단에 많은 도움을 받게 되었다. 저자 등은 폐생검 후 면역조직화학염색과 PCR 등의 방법으로 확진된 BALT 림프종 3예를 경험하였기에 문헌고찰과 함께 보고하는 바이다.
