Effects of Self-evaluation using Smartphone Recording on Nursing Students' Competency in Nursing Skills, Satisfaction, and Learning Motivations: Focusing on Foley Catheterization (자율실습교육에서 스마트폰 동영상을 활용한 자가평가 학습법이 간호술기 수행능력, 자율실습만족도, 학습동기에 미치는 효과: 유치도뇨를 중심으로)

  • Purpose: The purpose of this study was to identify effects of self-evaluation using smartphone recording on competency in nursing skills, satisfaction and learning motivations in nursing students'. The setting was the open laboratory hours. Methods: A quasi-experimental pre/post-test design was conducted in September 2015. The participants were 82 sophomore nursing students from one University. The experimental group did self-evaluation by watching recorded video clips of their performance. The control group did not have any self-evaluation. The primary outcomes were scores from the foley catheterization checklist, scores from a student satisfaction tool, and scores from a learning motivation tool. Data were analyzed using ${\chi}^2$ test, Fisher's exact test, independent t-test, and paired t-test. Results: There was a significant higher mean score in levels of satisfaction by students in the experimental group compared to the control group (t=2.26, p=.027). In addition, improvement in communication skills by the experimental group was significantly higher than improvement in the control group (t=3.96, p<.001). Conclusion: Findings show that self-evaluation using smartphone recordings has positive effects on increasing both communication skills and satisfaction with practice during open laboratory hours. These results indicate that, self-evaluation using smartphone recording is useful as a supplement to traditional open laboratory education.

Conservative infection control on acute pericoronitis in mandibular third molar patients referred from the prison (교도소에서 의뢰된 급성 하악 지치 주위염의 보존적 감염관리: 증례보고)

  • In the presence of acute pericoronitis of mandilbular third molar, antibiotic therapy and early incision and drainage are the method of choice, followed by definitive surgical extraction of the tooth as soon as it becomes subacute. If excision of the overlying tissues is decided on, it should be done adequately. All overlying tissues must be throughly excised, and the crown portion of the unerupted tooth should be completely exposed. After excision has been completed, the wound should be managed with a surgical dressing. This should be allowed to remain approximately 7 days. And then, surgical extraction of the impacted mandibular third molar can be done usually. In this operation, there are many complications, such as, postoperative bleeding, infection, trismus, dysphasia and paresthesia. The surgeon are discredited and medicolegal problem may be occurred in the presence of many distressed complications. Therefore, the relatively nonsurgical treatment is the method of choice. So, authors selected the conservative treatment methods of incision and drainage, primary endodontic drainage, operculectomy without surgical extraction of the mandibular third molars. The results were more favorable without the postoperative complication in Wonju old offender prison.

Advanced Gastric Cancer Associated with Disseminated Intravascular Coagulation Successfully Treated with 5-fluorouracil and Oxaliplatin

  • Gastric cancer patients with acute disseminated intravascular coagulation experiences a rare but severe complication resulting in a dismal prognosis. We report a case of advanced gastric cancer complicated with disseminated intravascular coagulation with intractable tumor bleeding which was successfully treated with chemotherapy consisting of 5-fluorouracil and oxaliplatin. The patient was a 63-yearold man who complained of abdominal pain, melena, and dyspnea on 24 November 2010. We diagnosed stage IV gastric cancer complicated by disseminated intravascular coagulation. Gastric tumor bleeding was not controlled after procedures were repeated three times using gastrofiberscopy. With the patient's consent, we selected the 5-fluorouracil and oxaliplatin combination chemotherapy for treatment. After one cycle of 5-fluorouracil and oxaliplatin therapy, symptoms of bleeding improved and the disseminated intravascular coagulation process was successfully controlled. The primary tumor and multiple metastatic bone lesions were remarkably shrunken and metabolically remitted after eight cycles of chemotherapy. In spite of progression, systemic chemotherapy is effective in disease control; further, the patient gained the longest survival time among cases of gastric cancer with disseminated intravascular coagulation.

Expression of EGFR in Paired New and Recurrent Glioblastomas

  • Background: The aim of this study was to analyse the expression of EGFR in newly diagnosed and recurrent glioblastoma multiforme (GBM). Materials and Methods: Our study included a total of 48 paired samples collected from 24 patients diagnosed with GBM. The intensity of EGFR cytoplasmatic staining was scored on a scale of 1-3+ (weak, intermediate or strong). Results: We found EGFR overexpression in 23 patients (96%) with newly diagnosed GBM, while all recurrent tumours overexpressed EGFR. Ten recurrent tumours (42%) had a lower expression than their new counterpart 13 tumours (54%) had a similar expression, and only one case (2%) had increased expression on recurrence. The expression of EGFR in newly diagnosed GBM was significantly correlated with EGFR expression in recurrent tumour (p = 0.036). In addition, new GBMs with strong EGFR expression had a mean relapse-free interval of 11.5 months (p=0.017). A benefit of combined therapy was observed in the radiotherapy-plus-chemotherapy group where the average time was 11 months (p=0.011), as compared with surgery/radiotherapy alone (average time 6.8 months). Conclusions: The present data show that EGFR is overexpressed in paired GBMs. The discrepancies of EGFR expression between the primary tumour and the recurrence suggest heterogeneity of GBMs but also unity at relapse.

A clinical study for the effect of ointment contained Takrisodok-Eum on recovering the damaged skin barrier by Atopic dermatitis (탁리소독음(托裏消毒飮) 함유 외용제의 아토피 피부염에 대한 임상적 연구)

  • Purposes : The purpose of present study is to detect the effects of ointment contained Takrisodok-Eum on the skin barrier's recovery of the Atopic dermatitis. Methods : A total of 44 patients who visited Semyung Oriental Medical Center from April 27th, 2011 to September 20th, 2011 were included in the study. In this study, the patients were treated with Takrisodok-Eum (experimental group) and positive control group. For 8 weeks of gross examination, hematological examination and instrumental asessment were made before and after the study to examine how well the products for experimental group with those for control group in recovering the damaged skin barriers by Atopic dermatitis. Results : 1. In the primary endpoint, SCORAD Index showed a statistically significant decline in experimental group. 2. In the secondary endpoint index of skin hydration, both the experimental group and the control group showed statistical significance. 3. In the secondary endpoint index of TEWL, the experimental group showed greater statistical significance than the control group. 4. In the secondary endpoint index of skin pH, skin pH showed a statistically significant decline in both the control group and the experimental group within the normal range. 5. In the secondary endpoint index of Total IgE, Eosinophil count, IL-10, IL-13, both the experimental group and the control group showed no statistical significance. 6. To evaluate the safety of the products for the human body, hematological examination and hematological biochemical examination, vital sign check were conducted; both control group and experimental group showed no abnormal level. And There were no severe adverse events during this study. Therefore, it is suggested that the safety of the products, if used for certain period, should be safe for the human body. Conclusions : According to the above experiments, it is suggested that Takrisodok-Eum should be effective for the Atopic dermatitis.

  • Purposes : The object of present study is to detect the Effects of Snail Secretion Filtrate and Hyaluronic acid on the skin barrier's recovery of the Atopic dermatitis. Methods : A total of 20 patients who visited Semyung Hanny Oriental Medical Center from september 1st, 2009 to August 31th, 2010 were included in this study. In this study, they were treated with Snail Secretion Filtrate(experimental group) and Hyaluronic acid(control group). For 4 weeks gross examination, hematological examination and instrumentation through skin-ANBT equipment were made before and after the experiments to see how well the products for experimental group act against those for control group in recovering the damaged skin barriers by Atopic dermatitis. Results : 1. In the primary endpoint, SCORAD Index showed a statistically significant decline in both the control group and the experimental group. However, the experimental group showed greater statistical significance than the control group. 2. In the secondary endpoint index of skin hydration, both the control group and the experimental group did not show a statistically significant increase. However, the degree of skin hydration in the experimental group is greater than in the control group. 3. In global assessment of efficacy, it was higher in the experimental group than in the control group for both the subjects and the researchers. 4. To evaluate the safety of the products for the human body, hematological examination and hematological biochemical examination were conducted; both the control group and the experimental group showed no abnormal level. Therefore, the safety of the products, if used for so long a time, proved to be safe for the human body. 5. Product satisfaction was higher in the experimental group than in the control group. Conclusions : According to above experiments, Snail Secretion Filtrate was effective on the Atopic dermatitis.

An Epidemiological Study for on Outbreak of Typhoid Fever in Busan Area (부산 일부지역에 유행한 장티푸스 환자에 대한 역학적인 조사)

  • An epidemiological study was conducted, from April 28 to June 3, 1975, to investigate source and course of infection of typhoid fever occurred in Munhyeon Dong, Nam Gu, Busan. The obtained results were as follows: 1. The entity of unknown febrile disease was identified as Salmonella typhi, D group and it's symptoms were abdominal pain, diarrhea, hepatomegaly, nausea and vomiting, aplenomegaly, rose spots, tenderness of ileocecal region etc., in that order. 2. The average duration from onset to recovery was 25.2 days and incidence rate was 39.5 per 1,000 population. 3. The source of infection was presumed a charity patient. 4. By the duration of report on the cases from onset, 10-19 days group was the highest. 5. The positive rate of initial stool culture was 38.9% and secondary was 5.6%. 6. Distribution of the cases by age and sex showed that 5-9 years old group was the highest as 30.5%, and male was higher than female. 7. Distribution of the cases by education level, under primary school as 66.6% was the highest. By the living standard, the highest was low income earner group as 77.8%. 8. Utilization rate of medical facilites was drug store (41.7%), herbal drug (8.3%), clinic (5.6%) in that order.

Relation between Location of Pelvic bone Fractures and the Injury to the Urinary bladder, Urethra or Lower gastrointestinal tract (골반 골절 위치와 방광, 요도, 하부 위장관계 손상의 관련성)

  • Purpose: This research was conducted to study whether the specific location of pelvic-bone fractures could increase the risk for injury to the urinary bladder, urethra, or lower gastrointestinal tract. Methods: We retrospectively reviewed the data of 234 patients with pelvic-bone fractures who visited the emergency department of Kyungpook National University Hospital from January 2004 to December 2006. The location of the pelvic-bone fracture was divided into 8 parts. The association of fracture location with injury to the urinary bladder, urethra, or lower gastrointestinal tract was analyzed with Fisher' s-exact test and multiple logistic regression. Results: Nineteen(19) patients had urinary bladder injury, 8 had urethral injury, and 9 had lower gastrointestinal tract injury. The following fracture locations were found to be significant; urinary bladder: sacroiliac (SI) joint (p<0.001), symphysis pubis (p=0.011), and sacrum (p=0.005); urethra: SI joint (p=0.020); lower gastrointestinal tract: symphysis pubis (p=0.028). After the multiple logistic regression analysis, the primary and the independent predictors for each of the injuries were as follows; urinary bladder: sacroiliac joint (p=0.000, odds ratio [OR]=10.469); lower gastrointestinal tract: symphysis pubis (p=0.037, OR=7.009). Conclusion: Consideration of further workup for injuries to the lower gastrointestinal and urinary tract is needed for some locations of pelvic-bone fractures because certain pelvic-bone fracture locations, especially the sacroiliac joint and the symphysis pubis, are associated with increased risk for injury to the lower gastrointestinal and urinary tracts.

Retroperitoneal liposarcoma: the role of adjuvant radiation therapy and the prognostic factors

  • Purpose: To evaluate the benefit of adjuvant radiation therapy (RT) for retroperitoneal liposarcoma (RPLS) following gross tumor removal. Materials and Methods: We reviewed 77 patients with primary RPLS surgically treated between January 2000 and December 2013. Cases with gross residual disease were excluded. Tumor grade was evaluated according to the French Federation of Cancer Centers Sarcoma Group (FNCLCC) system. Adjuvant RT was delivered to 32 patients (42%) using external beam RT alone. Median follow-up time was 36 months (range, 5 to 169). Results: Among 77 patients, 33 (43%) presented with well-differentiated, 31 (40%) with de-differentiated, 8 (10%) with myxoid/round and 4 (5%) with pleomorphic morphology. The RT group included less well-differentiated subtype than surgery group (28% vs. 53%). During follow up, 34 patients (44%) showed local recurrence. Local recurrence rate was lower in the RT group (38%) compared to the surgery group (49%). The 3-year local control rate (LC) was 55.6%, and the 3-year overall survival (OS) was 82.1%. Tumor histology and FNCLCC grade were significantly associated with local recurrence. There was no statistical significance of adding adjuvant RT in LC (p = 0.312). However, patients with tumor histology other than well-differentiated subtype showed marginally decreased local recurrence rate after adjuvant RT (3-year LC, RT 43.9% vs. no RT 35.3%; p = 0.087). Conclusion: RPLS patients receiving RT experienced less local recurrence. We suggest that the addition of adjuvant RT may be related to improvement of LCs, especially in patients with non-favorable histologic subtypes.

Readability of Patient Information Leaflets in Clinical Trials (임상시험 시험대상자설명서의 가독성 평가)

  • Background: Elements of informed consent including capacity, disclosure, understanding, voluntariness, and permission of the participant, are all crucial for clinical trials to be legally and ethically valid. During the informed consent process, the patient information leaflet is an important information source which prospective research subjects can utilize in their decision-making. In the adequate provision of information, KGCP guideline necessitate 20 specific items, as well as the use language that individuals can understand. This study measures the vocabulary level of patient information leaflets in an effort to provide an objective evaluation on the readability of such material. Methods: The word difficulty of 13 leaflets was quantitatively evaluated using Kim kwang Hae's vocabulary grading framework, which was compared to the difficulty level of words found in the $6^{th}$ grade Korean textbook. The quantitative outcomes were statistically analyzed using chi-squared tests and linear by linear association for ordinal data. Results: There was a statistically significant difference between the vocabulary level and frequency of words in leaflets and the 6th Korean textbook. The leaflets were on average 260 sentences and about roughly 15 pages long, including lay language (easier or equal to language used in primary school) of around 12% less; technical language of around 4.5% more. As the vocabulary grades increase, there was a distinct difference in vocabulary level between Korean textbook and each information leaflet (p < 0.001). Conclusion: Patient information leaflets may fail to provide appropriate information for self-determination by clinical trial subject through the difficulty level of its wording. Improvements in the degree of patients' understanding and appropriate use of information leaflets are collaboratively equipped to strengthen patient's autonomy and therefore guaranteeing participant's rights.
