A Case of Intramuscular Abscess in the Sternocleidomastoid Muscle after Intramuscular Stimulation (IMS) (근육내 자극요법 후 목빗근내 농양의 치험례)

  • Purpose: Intramuscular stimulation (IMS) shows good results in the treatment of chronic pain patients who did not respond to other treatments such as oral analgesics, trigger point injection, nerve block and epidural steroid injection. But, IMS procedure especially, patients with diabetes mellitus (DM) has sometimes serious problem. So, we present a very rare case of intramuscular abscess in the sternocleiomastoid muscle after IMS with literature review. Methods: A 66 year old male visited our department 7 days after IMS in the neck. His premorbid conditions and risk factors of deep neck infection was DM and old age. Computed tomographic scans of the head and neck region were performed in this patient: signs of deep neck infection, were seen enhanced abscess in the sternocleidomastoid muscle, cellulitis overlying tissue of the neck, and air bubbles involved muscle. Necrotic wound was excised serially and we treated this with the Vacuum-assisted closure (VAC, Kinetics Concepts International, San Antonio, Texas) system device. After appropriately shaping the sponge in the pockets, continuous negative pressure of 125 mmHg was applied. The VAC therapy was utilized for a period of 12 days. Results: We obtained satisfactory results from wide excision, drainage of the abscess with the VAC system, and then primary closure. The postoperative course was uneventful. Results: We suggest that many of the infectious complications may be preventable by strict adherence to aseptic techniques and that some of the other complications may be minimized by refining the techniques with a clear understanding of the medical disorders of patients. And, the refined technique using the VAC system can provide a means of simple and effective management for the cervical intramuscular abscess, with better cosmetic and functional results.

The Head Scatter Factor For Quasi - small Field Sizes (준소조사면에서의 산란인자)

  • The behavior of the correction factor associated with the collimator opening(head-scatter factor) were investigated for the 6MV x-ray beams of medical linear accelerator. The primary photon fluence was measured in air quasi-small fied size. Consideration in this study was given to the effect of head scatter factor with quasi-small fied size, the upper and lower collimator jaw scatter collection factors of quasi-small field (4-10cm) were measured with ion chamber. In general, the wedge factors which are used clinical practics are ignored of dependency on field sizes and depth. In wedge factors for each wedge filter were measured at various depth by using 6MV X-ray. In this present we inverstigated systematically the depth and field sizes dependency to determine the absorbed dose more accurately. Head scatter(upper-lower collimator jaw)appears to be (1) a small effect, less than 5% over the range of clinical field sizes (2) generated primarily at the flattening filter and therefored influenced most by the upper collimator setting.

  • The basic Trainer Aircraft of The Korean Air force KT-1/KO-1, which was designed and developed by Korean Technology has been a primary candidate as a replacement for the aging basic trainer or armed version for surface support operation for the nations such as Mexico, Philippine, Turkey, Venezuela and etc. In this study, competitors for the same class aircraft and the present operational status of the basic trainer of the world were identified and the fade out periods were also predicted. The result of this market analysis showed the required number of aircraft by the year 2025 for 33 countries as 4500. The exportable number of KT-1 aircraft was predicted considering the political/diplomatical environments, economic environment and also by the ability of producing same class aircraft of the country.

  • The mototic and meiotic chromosomal characteristics of ICR mice were investigated with G-and C-banding techniques. For the puposes, the chromosomal preparations were made with the modified air-drying method of Imal et al. Chromosomal analysis using testis could be observed mitotic as well as meitotic chromosomal behaviors, and the centromeric regions of all chromosomes including X chromosome were strongly stained in C-banded preparations. Nineteen autosomal bivalents and a single uniequal terminally associated X-Y bivalent in normal cells were observed during the late prophase and the metaphase of the meiosis I. The mean frequencies of previously dissociated X-Y chromosomes in the primary apermatocytes of the control group were 7.45%, but the frequencies of X-Y dissociation in the alkylating agents-treated group were about 3-4 times higher than that in the control group. Application of C-banding in meiotic stages could be certainly distinguish between vibalent type and univalents type of sex chromosomes.

Investigation of the Optical and Cloud Forming Properties of Pollution, Biomass Burning, and Mineral Dust Aerosol

  • This thesis describes the use of measured aerosol size distributions and size-resolved hygroscopic growth to examine the physical and chemical properties of several particle classes. The primary objective of this work was to investigate the optical and cloud forming properties of a range of ambient aerosol types measured in a number of different locations. The tool used for most of these analyses is a differential mobility analyzer / tandem differential mobility analyzer (DMA / TDMA) system developed in our research group. To collect the data described in two of the chapters of this thesis, an aircraft-based version of the DMA / TDMA was deployed to Japan and California. The data described in two other chapters were conveniently collected during a period when the aerosol of interest came to us. The unique aspect of this analysis is the use of these data to isolate the size distributions of distinct aerosol types in order to quantify their optical and cloud forming properties. I used collected data during the Asian Aerosol Characterization Experiment (ACE-Asia) to examine the composition and homogeneity of a complex aerosol generated in the deserts and urban regions of China and other Asian countries. An aircraft-based tandem differential mobility analyzer was used for the first time during this campaign to examine the size-resolved hygroscopic properties of particles having diameters between 40 and 586 nm. Asian Dust Above Monterey (ADAM-2003) study was designed both to evaluate the degree to which models can predict the long-range transport of Asian dust, and to examine the physical and optical properties of that aged dust upon reaching the California coast. Aerosol size distributions and hygroscopic growth are measured in College Station, TX to investigate the cloud nucleating and optical properties of a biomass burning aerosol generated from fires on the Yucatan Peninsula. Measured aerosol size distributions and size-resolved hygroscopicity and volatility were used to infer critical supersaturation distributions of the distinct particle types that were observed during this period. The predicted CCN concentrations were used in a cloud model to determine the impact of the different aerosol types on the expected cloud droplet concentration. RH-dependent aerosol extinction coefficients are calculated at a wavelength of 550 nm.

Effects of Expanding Methods on Residual Stress of Expansion Transition Area in Steam Generator Tube of Nuclear Power Plants (원전 증기발생기 전열관 확관법이 확관부위 잔류응력에 미치는 영향)

  • The steam generator tubes of nuclear power plants are pressure boundaries, and if tubes are leaked, the coolant with the radioactive materials was flowed out from the primary system to the secondary system and polluted the plant and the air. Recently most crack defects of tubes are stress corrosion cracks and these defects are located in expansion transition area, sludge pile-up region, and U-bend area. The most effective one of crack initiation factors in expansion transition area and U-bend area is the residual stress. According to the experiences of Korea standard nuclear plants(Optimized Power Reactor-1000), they had the stress corrosion cracks at the tube expansion transition area in early operating stage and especially lots of circumferential cracks were occurred. Therefore in this study, the distributions and conditions of residual stresses by tube expansion methods were compared and the dominant reason of a specific direction was examined.

  • The automobile equipped with a gasoline engine uses the ignition coil, namely, a high voltage generator, to make the mixed fuel ignited and burned in the combustion chamber, which results in the power to drive the engine. The ignition coil functions to convert a low voltage of the primary into a hiか voltage of the secondary by switching method, which will be transmitted to the electrode. Here, if the ignition coil has a defect even a little, it cannot function well. In this study, it was chosen epoxy molding ignition coil in recently and epoxy resin which is insulation material as specimens, and it was measured the characteristics of the partial discharge occurring to the specimens when those were applied to a voltage, and thereby, it was researched and analyzed the distribution of phase angle, amount and count of discharge due to the changing voltage, And as the result is applying to the actual automobile ignition system, it can be expected the enhancement of the performance of the ignition coil and the reliability of the electrical equipment.

  • A magnetic shutter gear is a device that transfers mechanical power by synchronizing the magnetic field between permanent magnet layers facing circumferentially through a harmonic modulator. However, magnetic gears uses many rare-earth permanent magnets to guarantee comparable torque density to that of mechanical reducer. Hence, we propose a novel axial magnetic gear with a dramatically reduced number of permanent magnets and a closed magnetic path. The torque of the system was compared to that of an existing shutter gear through a harmonic analysis of the air-gap magnetic field. The modulator thickness and open ratio were considered as the primary design parameters, and the cogging effect was analyzed for variation of the reduction ratio. A dynamic model between the high-speed side and low-speed side was derived, and position control was performed for a constructed hardware implementation.

  • In this research, railway transportation by means of the characteristics of tourism in the selection of railroad tourism and to clarify the difference in IPA is a primary purpose of this research. In addition, the train travel experience would like to clarify the difference in the importance and performance. This research fellows the indication points. First, by means of transportation to select train tourism product is the difference in the importance and performance in the 25 items. Second, the urgent needs to improve part 'Concentrate Here' side of the train, the temperature of the reasonable air-conditioning, and the appropriate means of transport links to the information service has been identified.'Low Priority' side of the entry tourist train operation of the frequency and interval, a total of 26 items, including the efforts of more than the current level of commitment was expected to be unnecessary. Third, according to the experience of sightseeing train railway transportation as a means to help train tourism product selection was the difference in the importance and performance.

Characteristic of Water-soluble Components of PM10 at Taean and Gangneung Sites in Summer Season (태안과 강릉지역 여름철 PM10의 수용성 성분 특성)

  • [ $PM_{10}$ ]measurements were made at two coastal sites, i.e., Taean and Gangneung, for summer to examine the characterization of water-soluble organic carbon (WSOC) and inorganic ionic species, and to investigate their difference between the sites. The fractions of three major inorganic water-soluble components ($NO_3^-$, $SO_4^{2-}$, and $NH_4^+$) at Taean and Gangneung sites were 30.6% (16.2~62.0%) and 25.6% (13.0~52.5%) of the $PM_{10}$, respectively. $SO_4^{2-}$ is the most dominant species of water-soluble ions at both sites, accounting for up to 20.5% (9.1~44.9%) and 16.3% (5.5~34.2%) of their respective PM10 mass concentrations. Using the paired T-test, $PM_{10}$ (p<0.01), $NO_3^-$ (p<0.05), $SO_4^{2-}$ (p<0.01), $NH_4^+$ (p<0.001), and WSOC (p<0.05) concentrations exhibited strong fluctuations on a daily basis between Taean and Gangneung sites. Relationship between the concentrations of $SO_x$ ($SO_4^{2-}+SO_2$) and CO indicates that the slopes of $SO_x$ /CO were 0.007 and 0.019 in the Taean and Gangneung sites, respectively. The smaller $SO_x$/CO slope in the Taean site could be related to the aged air with wet scavenging of $SO_x$ during transport. The correlation between the concentrations of CO and WSOC suggests that WSOC observed in the Gangneung ($R^2$=0.82) be transported from combustion-related sources, while the WSOC at the Taean site could be formed through atmospheric processing of primary volatile organic species during transport.
