A Case of Lymphangioleiomyomatosis with Pregnancy (임신으로 악화된 폐의 임파관평활근종증 l예)

  • The lymphangioleiomyomatosis(LAM) is a rare disorder, which afflicts mainly young woman of childbearing age, characterized by proliferation of immature smooth muscle cell in the lymphatics. We experienced a case of LAM in 26-years-old pregnant woman, confirmed pathologically by inguinal lymph node biopsy. She has suffered from exertonal dyspnea and dry coughing. The symptoms and chest X-ray were aggravated with pregnancy, but improved after delivery with two times of pregnancy. The chest X-ray showed diffuse reticulonodular infiltration and chest HRCT showed diffuse scattered tiny thin-walled cyst of lung parenchyma. We noted chylous ascites of which triglyceride level is 396 mg/dl. After delivery, the symptoms were getting better. We treated with medroxyprogesterone and planned close observation and follow-up.

Health Risk Behaviors Affecting the Process of Pregnancy (임신경과에 영향을 미치는 건강위험행위)

  • Comprehensive review of the literature was conducted to determine 1) selected risk factors and its impact that affect pregnancy outcome such as smoking, alcohol consumption, and substance abuse 2) these factors can facilitate future strategies for health promotion and prevention for both pregnant women and fetus. Review of literature were extracted from searching MEDLINE(1966 - Oct. 2000). CINAHL (1982 - Oct. 2000) and the domestic literature. The following factors were identified: 1. The effects of risk behaviors on pregnancy. ${\cdot}$Maternal smoking was associated with the occurrence of premature or LBW delivery, fetal growth retardation, extremities defects, heart defects and sudden infant death syndrome. ${\cdot}$Maternal alcohol consumption was associated with spontaneous abortion, premature or LBW delivery, morphologic/neurologic problems, especially fetal alcohol syndrome. ${\cdot}$Heroin was associated with withdrawal after birth in which were born to heroine addicts for gestational age and lung maturation in animal studies. ${\cdot}$Cocaine was associated with spontaneous abortion, abruptio placenta and a poor response to environmental stimuli. ${\cdot}$So far, the effects of caffeine on pregnancy was controversial, but severe caffeine consumption was associated with premature or LBW delivery, spontaneous abortion, still birth and dystocia. 2. Intervention methods and its effects identified were as follows ${\cdot}$Conducted intervention for smoking, alcohol and drug consumption were single or combined. ${\cdot}$Intervention methods were counseling, phone contact, mailing, use of educational videotape, booklet, support person and alternatives such as nicotine patch. ${\cdot}$The interventions increased the rates of smoking cessation during pregnancy and awareness of the risk of drug consumption, and decreased amount of alcohol consumption. ${\cdot}$The intervention outcome found positive effect on birth weight and length. 3. Our recommendations were as follows ${\cdot}$The personal and social cognition should be enhanced through education and the mass media. ${\cdot}$It's necessary to educate and give information of preconceptional care, planned pregnancy and early prenatal care for optimal pregnancy outcome. ${\cdot}$It's necessary to develop comprehensive assessment tool which is reliable and valid on smoking, alcohol consumption and substance abuse to identify supportive or interventional program.

Artificial Insemination and Delivery Rate of Crossbred Goat using Frozen-Thawed Semen (동결정액을 활용한 교잡종 염소의 인공수정 효율 및 분만율 조사)

  • This study examined the artificial fertilization efficiency of crossbred goats from a farmhouse using frozen semen. Electrostimulation was used to ejaculate and collect semen to assess the artificial fertilization efficiency of crossbred goats. The sperm concentration, vitality, and vitality after melting were investigated. The sperm volume was within 2.5~3 ml, and the concentration was 21~25 × 108/ml for each male crossbred goat. The melted semen had high vitality (≥90%). An IDEXX Rapid Visual Pregnancy Test kit was used for an earlier diagnosis of the pregnancy and to determine the pregnancy rate of fertilization using frozen-thawed semen. The reproductive performance of the artificially fertilized crossbred goats had the highest delivery rate (68%) from Farm C and the lowest delivery rate (45%) from farm A. The delivery rate through artificial fertilization was equal to the fertilization rate according to early pregnancy diagnostic kits. The artificial insemination efficiency was 45~68%. These findings can be used as the basis for improvement and breeding goats in goat farms and livestock research institutes.

Comparison of Transabdominal and Transvaginal Selective Fetal Reduction in Multifetal Pregnancy (다태임신에서의 선택적 유산술시 복식 천자와 질식 천자의 비교 연구)

  • The number of multifetal pregnancies has increased dramatically as a result of the widespread clinical use of ovulation induction and assisted reproductive technology(ART) in infertile patients. In multifetal pregnancies, the adverse outcome is directly proportional to the number of fetuses within the uterus, primarily because of an increased predisposition to premature delivery. It is extremely difficult to counsel patients about the expected outcome of pregnancies involving three or more fetuses. To increase the chances of delivering infants mature enough to survive without being irreversibly damaged by the sequelae of marked prematurity, selective fetal reduction(SFR) to the smaller number of fetuses should be considered in multifetal pregnancies. From January, 1991 to December, 1992, transabdominal SFR in multifetal pregnancies was performed in 22 patients including 13 triplet, 7 quadruplet, 1 quintuplet and 1 heptuplet pregnancies. Transabdominal SFR using intracardiac KCI injection and aspiration of amniotic fluid was carried out in 8-13 weeks of gestation. After procedure, 20 patients were remained as twin pregnancies, and 2 patients as triplet pregnancies. There have been 11 sets of twin delivery including 2 stillbirths, 2 sets of triplet delivery including 1 stillbirth, and 1 singleton delivery. Six cases were delivered after 37 weeks of gestation, 4 cases in 33 - 37 weeks, and 1 case in 30 weeks. Unfortunately, 3 stillbirths occurred in 20-24 weeks of gestation, and 4 cases were aborted. As 7 losses of pregnanancy including 1 case of septic abortion occurred, the delayed fetal loss rate was 38.9%(7/18) in transabdominal SFR. All babies born after 30 weeks of gestation were healthy, and no fetal anomaly directly related to the procedure was encountered. From July, 1993 to February, 1995, transvaginal SFR was performed in 20 patients including 15 triplet, 4 quadruplet and 1 quintuplet pregnancies. Transvaginal SFR using the same method as transabdominal SFR was carried out in 8-11 weeks of gestation. After procedure, 19 patients were remained as twin pregnancies, and 1 patient as singleton pregnancy. There have been 13 sets of twin delivery including 2 stillbirths, and 1 singleton delivery. Six cases were delivered after 37 weeks of gestation, 5 cases in 36-37 weeks, and 1 case in 30 weeks. Unfortunately, 2 still-births occurred in 20 weeks and 21 weeks of gestation, respectively, and 2 cases were aborted. As 4 losses of pregnancy including 1 case of septic abortion occurred, the delayed fetal loss rate was 25.0%(4/16) in transvaginal SFR. No fetal anomaly directly related to the procedure was encountered. It is suggested that transvaginal SFR could be performed more easily and earlier with the lower fetal loss rate as compared with transabdominal SFR. In conclusion, SFR is a rather safe and ethically justified procedure that may improve the outcome of multifetal pregnancies.

Studies on Blood Pictures and Serum Composition of Pregnant Rabbit (임신가토(姙娠家兎)의 혈액상(血液像)과 혈청성분(血淸成分)에 관한 연구(硏究))

  • Hemogram and serum composition of pregnant rabbits were studied and the results obtained were as follows: 1. Erythrocyte, hemoglobin concentration and hemotocrit value of pregnant rabbits decreased at the late stage of gestation, and 1 week after delivery the values were recovered to the normal levels. Number of erythrocyte and hematocrit value were not found to be significant, but hemoglobin concentration showed significant differences at the third and fourth week of pregnancy. 2. Total leucocytes of pregnant rabbits increased continuously as gestation progresses and 1 week postpartum the values were returned to normal. Highly significant changes were observed in the third and the fourth week of pregnancy. Neutrophil was revealed highly significant rise at the third and the fourth week of gestation and 1 week postpartum, but lympocyte was decreased highly significantlly. 3. Serum calcium of pregnant rabbits revealed the tendency of decrease as gestation progresses and 1 week after delivery the value was returned to normal, and highly significant decrease was abserved at the fourth week. Serum inorganic phosphorus of pregnant rabbits showed highly significant decrease at the fourth week of gestation, but no differences were observed at the other period of gestation and 1 week postpartum. 4. Serum protein of pregnants rabbits showed the tendency of decrease continuously pregnancy progresses, and 1 week after delivery showed the normal values. Highly significant decrease was found at the fourth week of gestation. the levels of serum lipids in pregnant rabbits was unchanged untill the third week of pregnancy, but at the fourth week and 1 week postpartum, the rise was highly significant. The level of total serum cholesterol in pregnants rabbis was shown the tendency of decrease untill the end of the second week of gestation and significantly dropped at the third week, thereafter, the value was rose significantly at the fourth week of gestation and 1 week postpartum.

Effect of Antenatal Education on Pregnant Women (임부들을 대상으로 한 산전교육의 효과)

  • This study was conducted to assess the knowledge and educational need of pregnant women relating to antepartum, delivery, puerperium period and to identify the variables which influenced their knowledge and educational need. The study subjects were 77 antepartum women who had antenatal educations and 106 antepartum women who did not have antenatal education. Number of total antepartum women were 183. Data were collected from women who visited for antenatal care at general hospitals and public health centers in Pusan and Kyungnam area using qestionnaires from Feb. 1. 1998 to Apr. 20. 1998. SAS/PC program was used for data analysis. ; Chi-square test, t-test, Pearson's Correlation Coefficient, ANOVA(Duncan procedure) and MANOVA. The results of this study were as follows: 1. Relating to the antepartum period between two groups, the knowledge about the number of antenatal care was low, relating to labor the knowledge about the time fix the fetal position and relating to postpartum, the knowledge about the change of blood pressure and body temperature was low. 2. The comparision means of knowledge were adjusted by social demographic characteristics related to knowledge. There were statistically significant differences in total mean score of knowledge about pregnancy, delivery and postpartum between two groups. In full marks 19, the mean scores of the educational group and non-educational group were 13.08 and 11.74 respectively (P=.000). In resultly, there are statistically significant differences between two groups. The effectiveness of antenatal education group was very high and showed its importance. 3. The comparision means of educational need were adjusted by social demographic characteristics related to educational need. The mean scores of educational need were statistically significantly different in pregnancy, delivery and puerperium period. 4. There were positive correlations between the knowledge and educational level, duration of marriage and total numbers of pregnancy. There were negative correlation between the educational need and age, educational level, duration of marriage and total number of pregnancy. The above results showed the effects of antenatal education.

Development of a Scale to Assess Immigrant Women's Needs for Pregnancy and Postpartum Adaptation (결혼이주여성의 임신과 산후 적응 요구 측정도구 개발)

  • Purpose: The purpose of this study was to develop a scale to assess immigrant women's needs for pregnancy and postpartum adaptation and to test the reliability and validity of the scale. Methods: To construct scale items, critical issues and difficulties associated with pregnancy and postpartum adaptation of immigrant women were identified and categorized through a literature review. Fifty-two scale items were constructed, and data for validity and reliability testing was collected with a questionnaire survey from 367 immigrant women. Data were analyzed with descriptive statistics, factor analysis, and reliability coefficients. Results: The final measurement scale to assess immigrant women's pregnancy and postpartum adaptation consisted of 48 items and 7 factors (adaptation to daily activity during pregnancy, cross-cultural understanding and personal respect, understanding of the process of pregnancy and delivery, baby rearing and family support, physical and emotional adaptation after childbirth, nutrition during pregnancy, and sexual life adaptation). The seven factors accounted for 64.26% of the variance, and Cronbach's ${\alpha}$ was .96. Conclusion: The scale developed by this study is a reliable and valid instrument and can be used to assess needs of pregnancy and postpartum adaptation and can be utilized in providing nursing interventions for immigrant women.

What is the best treatment of heterotopic cervical pregnancies for a successful pregnancy outcome?

  • Heterotopic pregnancy is rare event and the risk is increased with assisted reproductive technology procedures. Heterotopic cervical pregnancy is even more unusual. We report a rare case of heterotopic cervical pregnancy that was managed successfully. A 36-year-old women who conceived by IVF-ICSI was diagnosed with heterotopic cervical pregnancy. She visited the emergency room with vaginal bleeding at 5 weeks of gestation and underwent careful intracervical gestational sac reduction with forceps under abdominal guidance the next day. The postoperative course was uneventful and with regular check-ups, the intrauterine pregnancy (IUP) progressed unremarkably through 41 weeks with delivery of a healthy newborn. We reviewed a total of 37 cases of heterotopic pregnancy that have been reported in the English language literature. There have been many attempts to eliminate the cervical embryo while preserving the IUP, and complete cervical evacuation is important in order to avoid infection, bleeding, and premature birth.

A Survey on Health Management of during Pregnancy, Childbirth, and the Postpartum of Immigrant Women in a Multi-cultural Family (다문화가정 이주여성의 임신과 출산 건강관리 실태)

  • Purpose: The purpose of this study was to investigate health management state during pregnancy, childbirth, and postpartum of immigrant women. Methods: A descriptive research design was employed. Data were collected from 182 immigrant women who lived in four provinces and D city. Subjects were completed the following questionnaires: demographic, medical service use, pregnancy and childbirth, and health management during the ante-postpartum. Results: The average age of subjects was 28.96 and 83 subjects (45.6%) were coming from Vietnam. 55 Subjects (30.2%) were within the low-income group with less than 4 million won per month. So, 62 subjects were uninsured women because of the expensive costs. Subjects mainly visited a health care center with their husband. On the using a health care center, subjects complained about communication difficulties and transportation problems. 42 Subjects were pregnant but 21.4% of pregnant subjects did not receive ante-natal care. Subjects got information about pregnancy and childbirth through their husbands and husband's family. Conclusion: Nursing intervention to manage the pregnancy, childbirth, and the postpartum of immigrant women need to be developed and should include strategies to take care of themselves after delivery and provide knowledge and information about ante-postpartum.

Changes of Maternal-fetal Attachment and Self Efficacy for Delivery after the Taekyo-perspective Prenatal Class (태교관점 임부교실 참여 전후 임부 태아애착과 분만자신감의 변화)

  • The purpose of this study was to evaluate the effects on maternal-fetal attachment and self efficacy for delivery using the Taekyo-oriented prenatal class. This class is for 2 hours/week for 4 weeks. The program covers the contents of fetal growth and development including their responding ability, the importance of the uterine environment, sharing the motive and purpose of pregnancy, sharing experiences about pregnancy, sharing of prejudices against delivery, training of maternal-fetal interaction, understanding delivery, relaxation breathing techniques, maternity exercises, writing letters or prayers to the baby, and declaration of loving the baby. This study took place from March 4th to June 15th, 2000, in a university hospital and community care center, and was done by with a pretest-posttest design, with 55 pregnant women who were within 32-36 weeks pregnant and who agreed to participate in this study. Data was measured twice by self-report by the Cranley's Maternal-fetal Attachment Scale(MFAS, 1981), and the Shin's(1997) Self Efficacy for Delivery Scale at the beginning and at the completion of the class. Data was analyzed by SAS. The study results were: 1. The score of maternal-fetal attachment was significantly increased after the Taekyo perspective prenatal class than before the class. (t=7.389, p=0.000) 2. The score of self efficacy for delivery was significantly increased after the Taekyo perspective prenatal class than before the class. (t=8.885, p=0.000) The above results proved that the present Taekyo perspective prenatal education program was effective in increasing maternal-infant attachment and self efficacy for delivery. Therefore, it is concluded that the existing prenatal class should include Taekyo perspective elements. However, further study is needed to compare the effects with preexisted prenatal class.
