Effects of Sex Education Program related to the Unit of Reproduction and Development on Middle School Boys' Sex-related Knowledge and Attitude towards Sex (생식과 발생 단원과 연계한 성교육 프로그램이 중학교 남학생들의 성지식 및 성태도에 미치는 영향)

  • This study developed the sex education program based on the 'reproduction and development' unit of the middle school science, and examined its effectiveness on middle school boys' sex-related knowledge and attitude towards sex. The sex education program consisted of six topics of the structures and functions of generative organs, the importance of life, abortion, the human generation process, venereal diseases and AIDS, and responsibility of self-determination on sexual behavior. The results indicated that the experimental group demonstrated the statistically higher scores of the knowledge on sex than the control groups' scores, especially in the subcategories of 'pregnancy and delivery' and 'sex-related health'. However, both groups did not display any statistical difference on the attitude toward sex. In the subcategory of the attitude towards sex, the experimental group showed the statistically higher scores on 'self-determination and selection' and 'a health relationship'. In addition, the Pearson correlation results indicated the strongest relationship between the attitude toward sex and 'the self-determination and selection'. These results implicated that the sex education program related to the unit of reproduction and development in the middle school science helps the middle school boys develop the better knowledge and attitude towards sex.

Consideration on Domestic Production of Materials and Consumables for Human IVF-ET Program (체외수정 및 배아이식술 관련 재료 및 소모품의 국산화 필요성에 대한 고찰)

  • Human in vitro fertilization and embryo transfer (IVF-ET) program is a general procedure for infertile couples since first successful delivery on 1978 in UK and Korean first on 1985. Recently in Korea, more than 42,000 cases per year of IVF-ET were performed and showed good pregnancy rates compared worldwide data. The human IVF-ET procedure use many consumables, such as ovum pick-up (OPU) needles, centrifuge tubes, culture dish, ICSI pipette, culture media and ET catheters. Major of these materials are supported by the global companies. Thus, Korean IVF-ET program might be placed unstable situation by global economical risks. These uncertain problems could be overcome by the domestic production of IVF-ET materials and consumables. Two times questionnaires for Korean clinicians and researchers about the domestic production were performed and analyzed. Many of them requested domestic OPU needles, ET catheters, culture media and ICSI pipettes under good quality control and quality assurance system. This trial may be contributed to industrialization and to global competence of Korean IVF-ET program. The results of this survey can be provide a fundamental base for development and production of domestic materials and consumables for human IVF-ET program.

Myometrial relaxation of mice via expression of two pore domain acid sensitive K+ (TASK-2) channels

  • Myometrial relaxation of mouse via expression of two-pore domain acid sensitive (TASK) channels was studied. In our previous report, we suggested that two-pore domain acid-sensing $K^+$ channels (TASK-2) might be one of the candidates for the regulation of uterine circular smooth muscles in mice. In this study, we tried to show the mechanisms of relaxation via TASK-2 channels in marine myometrium. Isometric contraction measurements and patch clamp technique were used to verify TASK conductance in murine myometrium. Western blot and immunehistochemical study under confocal microscopy were used to investigate molecular identity of TASK channel. In this study, we showed that TEA and 4-AP insensitive non-inactivating outward $K^+$ current (NIOK) may be responsible for the quiescence of murine pregnant longitudinal myometrium. The characteristics of NIOK coincided with two-pore domain acid-sensing $K^+$ channels (TASK-2). NIOK in the presence of $K^+$ channel blockers was inhibited further by TASK inhibitors such as quinidine, bupivacaine, lidocaine, and extracellular acidosis. Furthermore, oxytocin and estrogen inhibited NIOK in pregnant myometrium. When compared to non-pregnant myometrium, pregnant myometrium showed stronger inhibition of NIOK by quinidine and increased immunohistochemical expression of TASK-2. Finally, TASK-2 inhibitors induced strong myometrial contraction even in the presence of L-methionine, a known inhibitor of stretch-activated channels in the longitudinal myometrium of mouse. Activation of TASK-2 channels seems to play an essential role for relaxing uterus during pregnancy and it might be one of the alternatives for preventing preterm delivery.

The Effect of Anti-Sperm Antibodies on Conventional IVF and Intracytoplasmic Sperm Injection (ICSI) (항정자항체가 일반적 체외수정 방법 및 정자직접 주입법(ICSI)에 미치는 영향에 관한 연구)

  • The purpose of this study was to examine the effects of anti-sperm antibody (ASA) on the fertilization processes using conventional IVF and ICSI procedure in human and hamster oocytes. In human IVF, we have observed restricted fertilization with sperm testing positive for ASA. ($23{\sim}90%$ IgA, 60-97 % IgG). However, if ICSI was perform in the next IVF cycle with the same patients, we could successfully fertilize the oocytes (37%; p<0.001), thus achieving pregnancy and delivery. When the sperm were cocultured in medium containing ASA, there were binding of ASA to sperm surface. In addition, the mean rate of the acrosomal reaction in an in vitro acrosome reaction test was lower for Ab-bound sperm (43.5%) than for Ab-free sperm group (51.3%, p<0.05). We used human sperm and hamster oocytes to confirm the negative effects of the ASA on fertilization. The sperm and/or oocytes have been expose to medium containing ASA before IVF and ICSI. In this experiment, the ASA was bound to the oocyte and sperm surface. The following results were obtain by using various combinations of ASA free or ASA bound sperm with ASA free or ASA bound oocytes for IVF. When ASA free sperm were inseminate with ASA free and ASA bound hamster oocytes, the fertilization rates are 89.6% and 74.3% respectively. However, when ASA bound human sperm were use the results were 62.5% and 55.6% respectively. These shows the fertilization rate was significantly decreased in both ASA bound and ASA free oocytes when using ASA bound sperm. No difference found when ASA are present on the oocyte surface. When the hamster oocytes was treated by ICSI with ASA free or ASA bound human spermatozoa, no significant difference was found. These results showed that ICSI is the most promising method for couples who fertilization was not possible by conventional IVF because of ASA.

A Study on the Physical and Emotional Status, and Nursing Needs of the Pregnant Women Hospitalized by Premature Labor. (입원중인 조기 진통 임부의 신체적, 정서적 상태와 간호요구에 대한 연구)

  • The purpose of this study was to find out physical and emotional status, and nursing needs of the pregnant women who were hospitalized by premature labor. The research respondents were 96 from four university hospitals located in Seoul, from June 30, 1996 to September 15, 1995. The research instrument was consisted of 14 items of physical status(discomforts) (Cronbach's=0.86), 17 items of emotional status (Cronbach's=0.89), 33 items of nursing needs (Cronbach's=0.94), and they were measured by 5 level of Likert Scale. The data were analyzed by frequency, percentage, mean standard deviation, ANOVA, Pearson correlation coefficient as the statistical techniques in the program of SPSS/$PC^+$. The findings were as follows : 1. The perception of physical status was mainly about physicl discomforts during the hospital stay. It included four categories about 'absolute bed rest' 3.48, 'hospital foods' 3.38, 'health care teams' 2.93, 'hospital environment' 2.83 in order of mean of discomforts. The most discomfortable one was "malodor by not doing personal hygiene." The next one was "urination and defecation on the bed using bedpan." 2. The perception of the emotional status was about negative mood related to 'fetus', 'hospitalization' perse, 'personal situation.' The highest score of negative mood was "I am afraid that the baby's condition will be bad if I deliver it before full term." The next one was "I am anxious about whether my baby will be in incubator if I deliver it before full term." 3. The highest mean score among items of nursing needs was "Nurses observe whether the labor come or not with concerns." The next one was "Nurses observe the fetal movement and check up the fetal heart sound." The lowest one was "Nurses help me when I need bedpan." 4. Nursing needs were consisted of four categories : professional, educational, emotional, and physical. The mean score of them was high in professional, educational, emotional, and physical need in order. 5. The physical status was related to "Experience of treatment for maintenance of pregnancy" and "Experience of hospitalization by premature labor". The emotional status was related to "Type of delivery" and "Type of habitation." 6. In the correlation of physical and emotional status, it showed positive correlation between them. The higher score of physical discomfort, the higher score of negative mood(r=0.5113, p=0.0001).

Production and Breeding of Transgenic Cloned Pigs Expressing Human CD73

  • One of the reasons to causing blood coagulation in the tissue of xenografted organs was known to incompatibility of the blood coagulation and anti-coagulation regulatory system between TG pigs and primates. Thus, overexpression of human CD73 (hCD73) in the pig endothelial cells is considered as a method to reduce coagulopathy after pig-to-non-human-primate xenotransplantation. This study was performed to produce and breed transgenic pigs expressing hCD73 for the studies immune rejection responses and could provide a successful application of xenotransplantation. The transgenic cells were constructed an hCD73 expression vector under control porcine Icam2 promoter (pIcam2-hCD73) and established donor cell lines expressing hCD73. The numbers of transferred reconstructed embryos were $127{\pm}18.9$. The pregnancy and delivery rate of surrogates were 8/18 (44%) and 3/18 (16%). The total number of delivered cloned pigs were 10 (2 alive, 7 mummy, and 1 died after birth). Among them, three live hCD73-pigs were successfully delivered by Caesarean section, but one was dead after birth. The two hCD73 TG cloned pigs had normal reproductive ability. They mated with wild type (WT) MGH (Massachusetts General Hospital) female sows and produced totally 16 piglets. Among them, 5 piglets were identified as hCD73 TG pigs. In conclusion, we successfully generated the hCD73 transgenic cloned pigs and produced their litters by natural mating. It can be possible to use a mate for the production of multiple transgenic pigs such as ${\alpha}-1,3-galactosyltransferase$ knock-out /hCD46 for xenotransplantation.

The Relationship between Conical Pap. Smear Findings and Related Factors for Uterine Cervical Cancer in Ullungdo Females (울릉도 여성들의 자궁경부 세포학적 검사소견과 관련요인과의 관계)

  • To study the incidence and epidemiological factors of uterine cervical cancer in medical underserved area females, the questionnaire survey and Pap. smear for uterine cervical cancer was done on total 330 women who lived in Ullungdo from 5th to 12th August, 1998. The results were summarized as follows: The age distribution of subjects was 50s (24.5%), 60s (24.5%) and their educational level was “no schooling” (14.2%) and “elementary school” (42.7%). The first coital age of subjects was 19∼21yrs (30.0%), 16∼18yrs (13.9%) and the first pregnancy age was 22∼24yrs (36.7%) and 19∼21yrs (30.0%). The frequency of total pregnancy of subjects was over 5 times (52.1%). The frequency of total delivery was “3∼4 times” (35.5%) and “5∼6 times” (15.2%). 68.8% of subjects had experience of abortion and 80.0% of their husband were on the phimosis. 172 (52.1%) subjects had gynecological symptoms, their symptoms were leukorrhea (48.3%), pruritus (21.5%) and leukorrhea with pruritus (20.3%). 63.9% of total subjects have been received Pap. smear and the frequency of their Pap. smear was “only 1 time” (44.1 %), “irregularly” (30.3%) and the reason of respondents who have not been received Pap. smea. was “no specific symptom” (51.3%). Among the 330 women screened there were negative (45.8%), inflammation (47.3%), trichomoniasis and candidiasis (1.8%), atypical cells (4.5%) and dysplasia (0.6%).

Obstetrical & Gynecological Health Status and Body Composition of Married Immigrant Women in Busan (결혼이주여성의 생식건강상태와 체성분 조성)

  • The purpose of this study was to examine obstetrical & gynecological health status and body composition for married female immigrants. The participants were 154 immigrant women living in Busan. Data were collected from July 2008 to July 2009 using a questionnaire and a bioelectrical impedance analysis tool (Inbody 720) for analysis of body composition. Most subjects were in the normal range for body composition, but some of those who answered the questionnaire had experienced some gynecological problems such as dysmenorrhea, or irregular menstruation. There was a significant difference in experiences with in vitro fertilization, pregnancy and abortion according to age and in premensturaion syndrome, as well as differences in regulation of menstruation during the past year, leukorrhea odor, experiences in in vitro fertilization, and delivery and breastfeeding according to nationality. There were significant differences in body mass index and waist-hip ratio according to age, as well as differences in percent body fat, body mass index and waist-hip ratio according to nationality. There were also significant differences in body mass index and waist-hip ratio according to period of stay in Korea. Procreative care protocols concerning married female immigrants' characteristics should be developed.

A Survey Of Infant Feeding Practices In Seoul, 1991. (영아의 수유 및 보충식에 대한 조사연구)

  • A survey of infant practices was conducted to provide information on which to base planned nursing interventions. The subjects were a convenience sample of 168 mothers visiting out - patients departments of five general hospitals and one public health center in Seoul for immunizations or treatment for common colds for their infants from two to 12 months of age. Data collection was carried out from July 8th to September 30th, 1991 using a questionnaire of 84 questions, 31 on the type of feeding, 22 on supplementary feeding and 21 on demographic infer mation. The results are summarized as follows : 1) The type of feeding was primarily artificial feed-ing (63.1%), followed by mixed feeding (22%) and breast feeding (14.9%) of the 59 mothers expecting during pregnancy to breast feed, 54. 2% changed artificial feeding and 30.5% went on to breast feed as they had expected. For mothers expecting to continue breast feeding over seven months. only one infant was being breast fed for over seven months. 2) For the 106 mothers using artificial feeding, 70. 8% hed attempted breast feeding, 64% of them for less than a month. Breast milk had been suppressed by for medication (38.7%) : 34.9% had used no specia] means. 3) The major reasons for replacing breast feeding with artificial feeding were the infants' demand for more milk (47.2% ) and insufficient supply of breast milk (49.3%). 4) Most mixed feeding was started at the age of one to three months (59.5%). Only 34.4% gave an artificial feeding after breast feeding : most (46%) alternated breast feeding with artificial feeding. On the whole, the motive for mixed feeding was the lack of breast milk (70.3%). 5) Many mothers (81.8%) were adding vitamin or mineral supplements to artificial milk and 51.5% were adding something to faciliate digestion. As for the method of sterilizing milk bottles and nipples, 56% had sterilized them together in boiling water from the beginning : 27% were just washing the bottles after boiling only once initially when measuring artificial milk powder, 31. 5% of the mothers over filled the measuring spoon rather than to the level. 6) The mother's occupation was related to her way of feeding. Mothers at home full time did more breast feeding than mothers employed outside the home. (x²=5.72, p=〈0.05). 7) Most mothers began supplementary food, from three to four months (48.8%) : 11.2% began later than seven months. Supplementary food was given between milk feedings by 67.2% of the mothers : 19.2% gave it before a milk feeding. Some mothers(26.4%) made their own supplementary food : 19.2% used ready - made supplementary food products for convenience. Recommendations for nursing interventions included : 1. Prenatal education about the advantages of breast feeding and breast care, and home visits after delivery for counselling related to breast feeding. Correct preparation of artificial feeding methods need to be taught in both pre & postnatal periods. In addition, specific education about supplementary feeding needed. 2. Further research is indicated about the Perceived lack of supply of breast milk and about the effectiveness of nursing interventions to Promote breast feeding.

Effect of Prenatal Antibiotic Exposure on Neonatal Outcomes of Preterm Infants

  • Purpose: Antibiotic exposure during pregnancy may affect the fetus and newborn in many ways. This study investigated the impact of prenatal antibiotic exposure duration on neonatal outcomes in very preterm (VP) or very low birth weight (VLBW) infants. Methods: From September 2015 to December 2020, preterm infants with gestational age less than 32 weeks or with a BW less than 1,500 g who were admitted to the neonatal intensive care unit, and their mothers were enrolled. Prenatal antibiotic exposure was defined as antibiotics received by mothers before delivery, and the patients were categorized into the non-antibiotic group, short-duration (SD; ≤7 days) group, or long-duration (LD; >7 days) groups. Results: A total of 93 of 145 infants were exposed to prenatal antibiotics, among which 35 (37.6%) were in the SD group and 58 (62.4%) were in the LD group. Infants in the LD group had a significantly higher birth weight-for-gestational-age (BW/GA) Z-score than those in the non-antibiotic group, even after the adjustment for confounding factors (beta, 0.258; standard error, 0.149; P<0.001). Multivariate logistic regression analysis showed that prolonged prenatal antibiotic exposure was independently associated with death (adjusted odds ratio [aOR], 8.926; 95% confidence interval [CI], 1.482-53.775) and composite outcomes of death, necrotizing enterocolitis (NEC), and late-onset sepsis (LOS) (aOR, 2.375; 95% CI, 1.027-5.492). Conclusions: Prolonged prenatal antibiotic exposure could increase the BW/GA Z-score and the risk of death and composite outcomes of death, NEC, and LOS in VP or VLBW infants.
