뇌졸중 환자에서 스파이랄 테이핑이 자세 안정성에 미치는 영향 (Effects of Spiral Balance Taping on Postural Balance Ability in Stroke Patient)

  • This study was conducted to investigate the influence that it has on the application pre and post balance index using method with spiral and reaction point balance taping application against 20 stroke patients. The taping application pre and post static eye opening balance index was statistically no significant difference in the Mean X Speed, and the Extent in X Dirction but in the Mean Y Speed, the Velocity Moment, and Extent in Y Direction, there were statistically significant difference(p<0.05). The taping application pre and post static eye closing balance inde was statistically no significant difference in the Mean X Speed, in Y Direction, there were statistically significant difference(p<0.05). From above results, the method of the applied taping was the equilibrium stability performance of the Y-axis than the stability of the balance of the X-axis, therefore it is considered to study the taping application method in the balance of detail stroke patients.

  • ■ Objectives The purpose of this clinical study is to evaluate the effect of Sosiho-tang(小柴胡湯) on a patient with Central fever due to pontine hemorrhage. ■ Methods A patient with central fever diagnosed with pontine hemorrhage was treated with herbal medication, acupuncture, electro-acupuncture(EA). Then we evaluated the improvement by average and peak of each day's body temperature. ■ Results Decrease of average and peak of each day's body temperature were observed after taking Sosiho-tang(小柴胡湯). ■ Conclusion This study proved the effect of moxibustion treatment on left side paresthesia due to central post-stroke pain.

가상현실 체험형 운동과 요부안정화운동에 따른 뇌졸중환자의 낙상관련 신체·심리적 비교 분석 (Comparative Analysis of Fall-Related Physicopsychological according to Virtual Exercise and Lumbar Stabilization Exercise in the Patient with Stroke)

  • 이 연구는 뇌졸중 환자를 대상으로 낙상예방을 위하여 가상현실 체험형 게임 운동(Nintendo-Wii Sport-NWS)과 요부안정화 운동을 실시하고 낙상관련 신체 심리 기능 변화를 측정하였다. 뇌졸중환자 30명을 대상으로 신체적 기능은 정적, 동적균형으로 측정하였고 심리적기능은 낙상효능감을 측정하여 운동 전과 운동 후, 각 운동군간을 비교분석하였다. 정적균형은 BBS, FRT, 동적균형은 TUG, 10m 보행검사, 낙상효능감은 낙상 효능감 지수로 측정되었다. 그 결과 두군 모두 신체 심리적기능에서 유의한 개선을 나타내었다. 그러나 BBS, FRT, 10m보행에서 요부안정화 운동군이 더 통계적으로 개선된 것으로 나타났으며 낙상효능감은 가상현실 체험형 게임 운동 군에서 더욱 개선된 것으로 나타났다. 결론적으로 가상현실 체험형 게임운동은 뇌졸중환자의 신체 심리적 기능 증진에 긍정적인 영향을 미친다.

  • ■ Objectives The purpose of this case report is to show the effect of Traditional Korean Medicine(TKM) on a patient with poststroke urinary retention ■ Methods A stroke patient with urinary retention was treated with herbal medication(Paljungsan), acupuncture, electro-acupuncture and moxibustion. To evaluate the effect of TKM on post-stroke urinary retention, we observed voiding pattern and residual urine after voiding. ■ Results Maintenance of regular self voiding and decrease of residual urine after voiding were observed after the TKM treatment. ■ Conclusion This study showed the effect of TKM treatment on Urinary retention of stroke patient.

아급성기 뇌졸중 환자의 균형 및 보행 기능에 대한 체중지지 자세에서의 단계적 PNF 패턴 치료의 효과 - 단일사례연구 - (The Effect of a Stepwise PNF Pattern Therapy in Weight-Bearing Positions on the Balance and Walking Functions of a Patient with Subacute Stroke -A Single Case Study-)

  • Purpose: The purpose of this study was to demonstrate the effects of a stepwise proprioceptive neuromuscular facilitation (PNF) pattern therapy in weightbearing positions on the balance and walking functions of a patient with subacute stroke. Methods: The patient was a 78-year-old man with right post-stroke hemiparesis who had decreased balance and gait function. During the baseline and withdrawal phases, no intervention was applied; however, in the intervention phase, the patient received a stepwise PNF pattern therapy in weight-bearing positions. Results: During the intervention phase, the LOS improved by 296.51% (from $2482.13mm^2$ to $626mm^2$), and walking speed improved by 18.70% (from 0.75 m/s to 0.64 m/s). The LOS and 10MWT values appeared to be clinically significantly improved after the intervention. In addition, the scores of the BBS and ABC scales improved by 100% (from 36 points to 18 points) and 56.52% (from 720 points to 460 points), respectively. Conclusion: These findings suggest that a stepwise PNF pattern therapy may be helpful in enhancing the balance and walking function of a patient with subacute stroke. Further studies are required to validate the results of this study.

가미반하백출천마탕과 침치료를 통한 한의치료로 호전된 뇌경색 후 시야장애 환자 치험 1례 (A Case Report on Treating Post-stroke Visual Impairment with Gami-banhabakchulchunma-tang and Acupuncture)

  • Objectives: This study investigated the effectiveness of acupuncture and herbal medicine (Gami-banhabakchulchunma-tang) for post-stroke visual impairment. Methods: A 79-year-old male with headache, dizziness, visual field defects, and blurred vision caused by a posterior cerebral infarction was treated with acupuncture and Gami-banhabakchulchunma-tang for three months. We used the Numerical Rating Scale (NRS) for each symptom and measured the patient's eye-movement range. Results: Following treatment, the NRS score for headache decreased from 5 to 0, and for dizziness and blurred vision, it decreased from 5 to 1. The NRS score for visual field defects decreased from 5 to 2. The range of the patient's left and right eye movements both improved, and no adverse side effects were observed during the admission period. Conclusions: Korean medical treatment with acupuncture and herbal medicine (Gami-banhabakchulchunma-tang) could effectively treat post-stoke visual impairment.

뇌졸중 환자의 보행속도 향상과 젖힌무릎 개선을 위한 고유수용성신경근촉진법의 적용: 증례보고 (The Effects of Proprioceptive Neuromuscular Facilitation on Gait Speed and Genu Recurvatume in Stroke Patients -A Case Report-)

  • Purpose: The purpose of this study is to describe the effects of proprioceptive neuromuscular facilitation (PNF) on gait speed and genu recurvatum episodes in stroke patients. Methods: The patient is a 58-year-old woman with left hemiplegia who presented with genu recurvatum while walking. Each week the patient received two sessions of PNF that each lasted 45 minutes. During a six-week interval the patient underwent therapeutic exercises and occupational therapy five times a week between the two PNF treatments. During each treatment a preand post-test (a 4 m walk test) was conducted, the patient described their fear of falling down (10-point visual analogue scale), and the number of genu recurvatum episodes was recorded. Results: After the first PNF treatment, the fear of falling down was decreased from 8 points to 7 points, and the number of genu recurvatum episodes decreased from 11 to 6. After the second PNF treatment, the fear of falling down decreased from 6 points to 5 points, and the number of genu recurvatum episodes decreased from 5 to 1. The 4 m walking test time also decreased from 30 seconds to 24 seconds. Conclusion: The PNF treatment is beneficial for reducing the fear of falling down, reducing the number of genu recurvatum episodes, and improving the gait speed of stroke patients.

Urgent Recanalization with Stenting for Severe Intracranial Atherosclerosis after Transient Ischemic Attack or Minor Stroke

  • Objective : Stenting of symptomatic intracranial stenosis has recently become an alternative treatment modality. However, urgent intracranial stenting in patients with intracranial stenosis following a transient ischemic attack (TIA) or minor stroke is open to dispute. We sought to assess the feasibility, safety, and effectiveness of urgent intracranial stenting for severe stenosis (>70%) in TIA or minor stroke patients. Methods : Between June 2009 and October 2010, stent-assisted angioplasty by using a balloon-expandable coronary stent for intracranial severe stenosis (>70%) was performed in 7 patients after TIA and 5 patients after minor stroke (14 stenotic lesions). Technical success rates, complications, angiographic findings, and clinical outcomes were retrospectively analyzed. Results : Stenting was successful in all 12 patients. The mean time from symptom onset to stenting was 2.1 days (1-8 days). Post-procedural angiography showed restoration to a normal luminal diameter in all patients. In-stent thrombosis occurred in one patient (n=1, 8.3%), and was lysed with abciximab. No device-related complications, such as perforations or dissections at the target arteries or intracranial hemorrhaging, occurred in any patient. The mortality rate was 0%. No patient had an ischemic event over the mean follow-up period of 12.5 months (range, 7-21 months), and follow-up angiography (n=7) revealed no significant in-stent restenosis (>50%). Conclusion : Urgent recanalization with stenting is feasible, safe, and effective in patients with TIA or acute minor stroke with intracranial stenosis of ${\geq}$ 70%.

ICF를 적용한 뇌졸중 환자의 문서 기록과 중재 전략의 실례 (A case report of the intervention strategy & documentation in a patient with post stroke applied a International Classification of Functioning, Disability and Health)

  • Background and Purpose : Many Physical therapist are inclined to communicate less effectively each other because they hardly use the standard terminology. The purposes of this case report are (1) to apply ICF-based documentation in evaluation (2) to submit the strategy of intervention process to improve the ability of walking short distance of the client who has post-stroke. Description : The client was 44-years-old man with hemiplegia who was in 1 month post-stroke problems were diagnosed while applying the ICF core set. The goals agreed with client were independently walking short distance, stairs and obstacles. To come up with the intervention strategy, hypothesis was set and 4 weeks of intervention was carried out after proposing the short goal and detailed purpose. Outcome : The client's performance in walking short distance and confidence were increased after impairment focused intervention, that are improved in walking velocity, endurance, supporting ability in lower limbs, rhythmical movement in upper limbs and the coordination of both limbs. Activities focused intervention also enhanced the ability in climbing steps and walking around obstacles. Conclusion : The decided hypothesis and goal that are to solve the problems the client faced were remarkably meaningful.

Vancomycin 내성 장구균 집락 해제에 대한 보중익기탕의 효과 (The Effects of Bojungiki-tang by Withdraw of Isolation of Vancomycin-resistant enterococci Colonization - Case Series)

  • Bojungiki-tang is a traditional oriental medicine to boost the immune system. In this study, we investigated the effects of Bojungiki-tang by withdrawal of isolation of VRE colonization. Four cases of post-stroke patients with VRE colonization took Bojungiki-tang and continuously were followed up with stool cultures. After three times negative stool VRE, we withdrew isolation of VRE colonization. One case patient was diagnosed with VRE colonization in another hospital and had no withdrawal during that period. He was admitted to Kyung-hee University Oriental Medicine where he took Bojungiki-tang. After three times negative stool VRE, we withdrew isolation of the patient. These results demonstrate that Bojungiki-tang is effective on boosting immunity level. Further studies are needed to better characterize this protective effect of Bojungiki-tang.
