Reconstruction of Pharyngolaryngeal Defects with the Ileocolon Free Flap: A Comprehensive Review and How to Optimize Outcomes

  • Several reconstructive methods have been reported to restore the continuity of the aerodigestive tract following resection of pharyngeal and hypopharyngeal cancers. However, high complication rates have been reported after voice prosthesis insertion. In this setting, the ileocolon free flap (ICFF) offers a tubularized flap for reconstruction of the hypopharynx while providing a natural phonation tube. Herein, we systematically reviewed the current evidence on the use of the ICFF for reconstruction of the aerodigestive tract. A systematic literature search was conducted across PubMed MEDLINE, Web of Science, ScienceDirect, Scopus, and Ovid MEDLINE(R). Data on the technical considerations and surgical and functional outcomes were extracted. Twenty-one studies were included. The mean age and follow-up were 54.65 years and 24.72 months, respectively. An isoperistaltic or antiperistaltic standard ICFF, patch flap, or chimeric seromuscular-ICFF can be used depending on the patients' needs. The seromuscular chimeric flap is useful to augment the closure of the distal anastomotic site. The maximum phonation time, frequency, and sound pressure level (dB) were higher with ileal segments of 7 to 15 cm. The incidence of postoperative leakage ranged from 0 to 13.3%, and the majority was occurring at the coloesophageal junction. The revision rate of the microanastomosis ranged from 0 to 16.6%. The ICFF provides a reliable and versatile alternative for reconstruction of middle-size defects of the aerodigestive tract. Its three-dimensional configuration and functional anatomy encourage early speech and deglutition without a prosthetic valve and minimal donor-site morbidity.

Scintillation Properties of Acrylate Based Plastic Scintillator by Photoploymerization Method (아크릴레이트 기반 광중합 플라스틱 섬광체의 섬광 특성)

  • In this study, we prepared and characterized a acrylate based UV-curable plastic scintillator. It was used co-polymers TMPTA, DHPA and Ultima $Gold^{TM}$ LLT organic scintillator. The emission spectrum of the plastic scintillator was located in the range of 380~520 nm, peaking at 423 nm. And the scintillator is more than 50% transparent in the range of 400~800 nm. The emission spectrum is well match to the quantum efficiency of photo-multiplier tube and the fast decay time of the scintillation is 12 ns, approximately. This scintillation material provides the possibility of combining 3D printing technology, and then the applications of the plastic scintillator may be expected in human dosimetry etc.

Micropipette tip intubation in rats as a replacement for conventional endotracheal tube intubation

  • Endotracheal intubation is often necessary in the course of animal experiments, especially in craniofacial surgery. However, endotracheal intubation can be a major burden in this context. The authors performed simple and cost-saving method using a 200 µL yellow micropipette tip, and the success of this method was demonstrated by X-ray and autopsy. We used a total of 30 rats. After the rats were fixed with a plaster, the trachea and vocal cords were visualized with the tongue pulled back. Under direct visualization of the vocal cords, a curving micropipette tip was advanced into the trachea. This method can be learned quickly and applied successfully by general experimenters. We successfully intubated all 30 rats without any complications. The success rate of micropipette tip intubation was 100%. This procedure was performed by one experimenter within 2 to 3 minutes after induction of anesthesia. We demonstrated its superiority by X-ray and autopsy. Herein, we describe endotracheal intubation of rats using micropipette tips. To the best of our knowledge, this method is novel and represents the simplest and most efficient means of intubation in rats, providing an alternative to conventional endotracheal intubation.

  • The wear behaviour of steam generator (SG) tubes (Inconel 600 and 690) against support materials (405 and 409 ferritic stainless steels) has been experimentally studied in room temperature water using reciprocating wear apparatus with tube-an-plate configuration. The results showed that the wear rate of Inconel 690 was lower than that of lnconel 600 with increasing normal loads and sliding amplitudes. Also, plastic deformation layers appear below the surface of both SG tubes, which have a specific thickness and are small compared with their grain size. This means that wear rate of SG tubes in water condition is closely related to the formation and fracture of plastic deformation layers.

Analysis of Burst Pressure for Wear-Damaged Steam Generator Tubes (마멸에 의해 손상된 증기발생기 전열관의 파열압력 해석)

  • Generally the rupture of steam generator tubes proceeds from significant plastic deformation before failure. In this study, the burst pressures of damaged steam generator tubes were calculated from the plastic instability analysis with the finite element method. Two wear types, flat and circumferential types were considered. An equation for the burst pressure was proposed by using the strength reduction factor and the Svensson equation. The analysis results were compared with the experiment data from published references and they showed a good agreement with the experiment data.

  • In this study, a study was carried out that could effectively produce a heat dissipation effect on plastic materials. Using carbon nanotube (CNT), aluminum powder and plastic, the material properties were tested in 2 cases of compounding ratio. The test sample mold was designed and constructed prior to the experiment. The experiments include tensile strength, elongation rate, flexural strength, flexural elasticity rate, eye-jaw impact strength, gravity and thermal conductivity. Results from 60% and 70% mixture of aluminium to plastic were tested, and a 10% less combined result was a relatively good property. For research purposes, the heat dissipation effect and light weighting obtained a good measure when the combined amount of Al was 60%.

Experimental and Analytical Study on the Burst Pressure of Steam Generator Tubes with T-type Combination Cracks (증기발생기 전열관에 존재하는 T-형 복합 균열의 파열압력 시험 및 해석)

  • Several nuclear power plants reported that they often found the combination cracks, which consist of longitudinal and circumferential cracks in the tubes. For the burst pressure of a tube with a single longitudinal or circumferential crack several experimental equations have been proposed in published literatures. But for the combination crack appropriate fracture criterion has not been proposed yet. In this study the burst pressures of a tube with a longitudinal crack or a T-type combination crack consisting of longitudinal and circumferential cracks were obtained experimentally and analytically. Fracture parameters such as crack opening angle (COA) were investigated by using elastic plastic analysis. Also the burst pressure far a T-type combination crack located near a tubesheet was considered to develop a length-based criterion. Because most of the axial, circumferential or combination cracks initiate in roll transition zone near the tubesheet.

  • One of the methods to strengthen the structures against the seismic lateral loading is the employment of the composite columns. A concrete-filled tube (CFT) has the cumulative advantages of steel and concrete. Concrete-filled steel tube columns have been widely used in the moment-resisting frame (MRF) structures, located in both non-seismic zones and high-risk seismic zones. In this paper, the results of studies on two important seismic parameters of ductility and the response modification factor (RMF) of the MRFs with CFT columns are submitted. While the studies are carried out, the effects of span length-story height ratio, the strength of materials and seismic behavior of MRFs are considered. In this regard, the ductility, RMF and the strength of 36 models of the steel MRFs with CFTs are analyzed. The fiber plastic hinges numerical simulation and pushover analysis method are used in the calculations. Based on the obtained results, the RMFs suitable for the 5-, 10- and 15- story frames are proposed.
