Human coronavirus diseases, particularly severe acute respiratory syndrome coronavirus 2, still remain a persistent public health issue, and many recent studies are focusing on the quest for new leads against coronaviruses. To contribute to this growing pool of knowledge and explore the available marine natural products against coronaviruses, this study investigated the antiviral effects of fucoxanthin isolated from Sargassum siliquastrum-a brown alga found on Jeju Island, South Korea. The antiviral effects of fucoxanthin were confirmed in severe acute respiratory syndrome coronavirus 2-infected Vero cells, and its structural characteristics were verified in silico using molecular docking and molecular dynamic simulations and in vitro colorimetric method. Fucoxanthin inhibited the infection in a concentration-dependent manner, without showing cytotoxicity. Molecular docking simulations revealed that fucoxanthin binds to the angiotensinconverting enzyme 2-spike protein (binding energy -318.306 kcal mol-1) and main protease (binding energy -205.118 kcal mol-1). Moreover, molecular dynamic simulations showed that fucoxanthin remains docked to angiotensin-converting enzyme 2-spike protein for 20 ns, whereas it breaks away from main protease after 3 ns. Also, the in silico prediction of the fucoxanthin was verified through the in vitro colorimetric method by inhibiting the binding between angiotensinconverting enzyme 2 and spike protein in a concentration-dependent manner. These results indicate that fucoxanthin exhibits antiviral effects against severe acute respiratory syndrome coronavirus 2 by blocking the entry of the virus. Therefore, fucoxanthin from S. siliquastrum can be a potential candidate for treating coronavirus infection.
Post-tuberculosis lung disease (PTLD) is emerging as a significant area of global interest. As the number of patients surviving tuberculosis (TB) increases, the subsequent long-term repercussions have drawn increased attention due to their profound clinical and socioeconomic impacts. A primary obstacle to its comprehensive study has been its marked heterogeneity. The disease presents a spectrum of clinical manifestations which encompass tracheobronchial stenosis, bronchiectasis, granulomas with fibrosis, cavitation with associated aspergillosis, chronic pleural diseases, and small airway diseases-all persistent consequences of PTLD. The spectrum of symptoms a patient may experience varies based on the severity of the initial infection and the efficacy of the treatment received. As a result, the long-term management of PTLD necessitates a detailed and specific approach, addressing each manifestation individually-a tailored strategy. In the immediate aftermath (0-12 months after anti-TB chemotherapy), there should be an emphasis on monitoring for relapse, tracheobronchial stenosis, and smoking cessation. Subsequent management should focus on addressing hemoptysis, managing infection including aspergillosis, and TB-associated chronic obstructive pulmonary disease or restrictive lung function. There remains a vast expanse of knowledge to be discovered in PTLD. This review emphasizes the pressing need for comprehensive, consolidated guidelines for management of patients with PTLD.
This study explored epidemiological trends in trichomoniasis in Daegu, South Korea. Wet mount microscopy, PCR, and multiplex PCR were used to test for Trichomonas vaginalis in vaginal swab samples obtained from 621 women visiting 2 clinics in Daegu. Of the 621 women tested, microscopy detected T. vaginalis in 4 (0.6%) patients, PCR detected T. vaginalis in 19 (3.0%) patients, and multiplex PCR detected T. vaginalis in 12 (1.9%) patients. Testing via PCR demonstrated high sensitivity and high negative predictive value for T. vaginalis. Among the 19 women who tested positive for T. vaginalis according to PCR, 94.7% (18/19) reported vaginal signs and symptoms. Notably, more than 50% of T. vaginalis infections occurred in females younger than 30 years old, and 58% were unmarried. Multiplex PCR, which simultaneously detects pathogens from various sexually transmitted infections, revealed that 91.7% (11/12) of patients were infected with 2 or more pathogens. Mycoplasma hominis was the most prevalent co-infection pathogen with T. vaginalis, followed by Ureaplasma urealyticum and Chlamydia trachomatis. Our results indicate that PCR and multiplex PCR are the most sensitive tools for T. vaginalis diagnosis, rather than microscopy which has been routinely used to detect T. vaginalis infections in South Korea. Therefore, clinicians should take note of the high prevalence of T. vaginalis infections among adolescent and young women in order to prevent persistent infection and transmission of this disease.
Follow-up studies have been conducted every three years on the endemicity of Gymnophalloides seoi infection in a small coastal village of Chollanam-do (Province), Korea, since it was first known as an endemic area in 1994. Special attention was given to its egg laying capacity in the human host. In fecal examinations, the overall helminth egg and/or cyst positive rate was 78.7% (74/94) in 1997 and 76.6% (82/107) in 2000. Among them G. seoi eggs showed the highest rate; 71.3% (67/94) in 1997 and 72.0% (77/107) in 2000. The average number of eggs per gram of feces (EPG) was 1,015 in 1997, while a reduced rate of 353 was observed in 2000. In 1997, total of 320,677 adult flukes of G. seoi (av. 10,344/person, 94-69,125 in range) were collected from the diarrheic stools of 31 treated patients. The EPG/worm obtained from 21 cases ranged from 0.04 to 0.77 (av. 0.23), suggesting density-dependent constraints on the worm fecundity. The relationship between the worm burden (X) and EPG/worm (Y) can be expressed as $Y=0.42{\cdot}e{\cdot}^{-l.2x}(r=0.49)$. The results showed that G. seoi infection is persistently endemic in this village.
유방염은 젖소에 있어서 유량의 감소, 유질 저하로 인한 우유 폐기문제, 젖소의 도태, 치료비 및 노동력의 부담등을 초래하는 가장 경제적 손실이 큰 질환이다. 현재까지 비유기 최종 착유 후에 항생제를 주입하는 건유기 치료법이 유방염 관리에 가장 효과적이며 폭넓게 쓰이고 있는 방법으로 알려져 있다. 하지만, 건유기 치료는 건유기 주입 항생제 제품들이 건유기 초기에만 지속적인 활성을 지니기 때문에 건유기 말기와 분만 전기에 있어서의 신규 감염은 방어할 수 없다. 따라서 본 연구는 건유기 유방염 관리를 위해 PLGA를 이용한 서방형 생분해 cephalexin microsphere를 제조하여 이의 임상적 효능을 평가하고자 하였다. PLGA는 무독성의 조직 반응을 일으키지 않는 특성으로 인해 약물 방출 조절 체계의 일환으로 인정을 받아왔다. 본 연구에서 cephalexin microsphere는 표면에 특징적인 구멍을 가지고 있는 구형 모양으로 확인이 되었으며 약물 방출 시험에서 초기 과다 방출 이후로 21일간 약물의 방출이 점차 감소한 뒤 3주와 4주 사이에 2차 방출을 보이는 맥동성 방출 양상이 관찰되었다. 현장적용 시험에서는 cephalexin microsphere를 주입한 실험군에서 대조군에 비해 분만 후 신규 감염율 및 치료율, 평균 체세포 수의 변화의 측면에서 볼 때 유의성 있는 차이가 있음을 확인하였다. 따라서 생분해 microsphere를 이용한 건유기 치료법은 건유기 동안 신규 감염을 예방하고 기감염을 감소시킴으로써 종전의 건유기 치료를 좀 더 효과적으로 증진시킬 수 있을 것으로 사료된다.
일본뇌염바이러스(Japanese encephalitis virus, JEV) Nakayama strain을 초기 multiplicity of infection 5.0으로 Vero세포에 감염하여 1년 이상 안정적으로 바이러스를 방출하는 지속감염(persistently-infected)세포주를 확립하였다. 지속감염 세포에서 지속적으로 방출되는 총 11 개의 Plaque 형태 변이바이러스(morphology mutants)클론을 확보하였다. 분리된 변이바이러스의 복제효율을 분석한 결과 생물학적 표현형과 복제효율은 유의하게 상관하였다. 변이바이러스 RNA 게놈 양 말단의 non-coding region 및 envelop 단백질의 ORF에서는 유의한 염기서열 변화가 관찰되지 않아 JEV 약독화에 새로운 인자가 추가로 관여할 가능성을 제시하였다. 변이바이러스에 감염된 신선한 Vero세포는 wild-type JEV의 일반적 감염성상과 다르게 대다수의 세포가 유의할 만한 세포병변현상을 나타내지 않았다. 감염된 Vero세포에서 wild-type JEV 및 large plaque을 형성하는 변이바이러스의 경우 mRNA와 함께 Bcl-2의 발현은 모두 유의하게 감소하였으며 p53은 뚜렷하게 증가하였다. 반면 small plaque을 형성하는 변이바이러스의 감염세포에서는 Bcl-2와 p53 모두 유의한 변화를 볼 수 없었다. 이상의 결과들과 함께, 감염된 Vero세포의 internucleosomal DNA fragmentation과DNA profile의 유형분석에 따르면 궁극적 인 세포병변효과의 변화는 변이바이러스의 복제효율과 더불어 p53에 비의존적인 apoptosis 수위의 전반적인 감소에 기인하는 것으로 판단된다.
목 적 : 호흡기계 섬모 이상은 점막 섬모의 청소 기능에 장애를 초래하므로 반복적 혹은 지속적인 호흡기 계통의 질환 즉, 부비동염, 기관지염, 기관지확장증, 비염 또는 중이염 등을 동반한다. 이 질환은 주로 선천적으로 발생하지만, 드물게는 후천성 혹은 일과성으로 발생되기도 한다. 본 연구에서는 자주 재발하거나 지속되는 호흡기 질환의 증상을 가진 환아들을 대상으로 호흡기계 섬모 이상이 선행하는지를 알아보고자 하였다. 방 법 : 2000년 8월부터 2003년 7월까지 한양대학교병원 소아과에 입원하였거나 외래에 내원한 3개월 이상 지속되는 만성 또는 재발하는 호흡기 질환 환아들 중 17례에서 비강 내 점막 조직생검을 시행한 후 전자현미경을 이용하여 섬모의 상태를 검사하였다. 결 과 : 대상 환아 중 남아가 7례였고 여아가 10례였으며, 연령분포는 2세에서 10세 사이였다. 대상 환아 17례 중 만성 부비동염이 12례, 만성 기침이 9례, 잦은 상기도 감염이 9례에서 관찰되었으며 그 다음으로는 재발성 중이염(7례), 재발성 폐렴(4례), 기관지 천식(4례) 순으로 관찰되었다. 조직검사를 시행한 17례 중 4례가 병적인 소견을 나타냈는데 1례에서는 inner dynein arm 결손이 있었고 1례는 microtubular transposition, 1례는 supernumerous tubules, 1례는 singlet을 가지고 있었다. 결 론 : 자주 재발하거나 지속되는 호흡기 질환의 증상을 가진 환아에서 면역 결핍증 등이 배제된 경우 기도 섬모의 상태를 반영하는 것으로 사료되는 비강 내 점막 생검 실시를 통한 섬모의 미세구조 이상을 확인함으로써 선천적 또는 후천적 원인의 감별 및 효과적인 치료 방법을 결정하는데 도움이 될 것으로 사료된다.
드문 예인 좌심방이-농양을 수술, 항생제 및 종격동 세척술로 치료한 성공적인 치험 예를 보고하고자한다. 요로감염의 과거력을 가진 9세 여아가 지속적 발열을 주소로 내원하였다. 심초음파와 자기공명 영상에서 다량의 심낭삼출과 좌상심연의 종괴가 관찰되었다. 심낭천자액 균배양에서 포도상구균이 검출되었다. 환아는 체외순환하에 종괴 절제술을 받았는데 종괴는 좌심방이에 위치하였으며 내부는 섬유화농성 농양이 형성되어 있었다. 수술 후 종격동 세척술을 시행하였다. 병리검사소견은 기질화된 혈전을 동반한 섬유화성 벽심내막염이었다. 환아는 특별한 합병증 없이 술 후 21일에 퇴원하였다.
Cervical cancer is one of the most frequent cancers in women worldwide. Persistent infection with a human papillomavirus (HPV) is the main cause for cervical cancer. Vaccination and Pap smear screening are the best methods for prevention of the disease. The objective of this cross-sectional study was to assess the knowledge and views of upper secondary school female students in Kuala Lumpur, Malaysia, toward prevention of cervical cancer. This study was conducted from April 2009 to September 2009 in 8 schools in Kuala Lumpur area using pre-tested and validated questionnaires. Results indicated that the respondents had low knowledge of cervical cancer and its prevention although the majority of students (80.4%) had heard about the disease. The level of knowledge of cervical cancr and its prevention was significantly higher among students from the science stream (p<0.001) compared to students from the art stream. Most students (69.3%) agreed to take the vaccination if the service was available in schools. A high percentage of students (82.2%) agreed that the vaccination should be compulsory to the students. In conclusion, most students had low knowledge of cervical cancer and its prevention but they had positive attitude toward vaccination and agreed that vaccination should be compulsory. Therefore, suitable educational programmes should be developed to improve the knowledge of secondary school students on the prevention of cervical cancer.
Purpose: Periodontal pathogens can invade the host tissue. Morphologic studies have revealed bacteria within the pocket epithelium, gingival connective tissues, alveolar bone, and oral epithelium. The objective of this study was to visualize and evaluate presence of Porphyromonas gingivalis and Tannerella forsythia in crevicular epithelial cells of periodontally healthy subjects and chronic periodontitis patients. Materials and Methods: A total of 666 crevicular epithelial cells in the samples obtained from 27 chronic periodontitis patients and 9 healthy volunteers were examined. Specific probes for P. gingivalis and T. forsythia and a universal probe for detection of all eubacteria targeting 168 rRNA for fluorescence in situ hybridization was used in conjunction with confocal laser scanning microscopy. Results: 98.99% of sulcular epithelial cells from healthy volunteers and 84.40% of pocket epithelial cells from periodontitis patients were found to harbor bacteria. P. gingivalis and T. forsythia were discovered more often in crevicular epithelial cells from periodontitis patients. Conclusion: P. gingivalis and T. forsythia can invade crevicular epithelial cells and intracellular bacteria may act as a source of bacteria for persistent infection.
