Femoral Periprosthetic Fractures after Total Knee Arthroplasty: New Surgically Oriented Classification with a Review of Current Treatments

  • Purpose: As the number of total knee arthroplasties (TKAs) increases, the incidence of femoral periprosthetic fractures after TKA is also increasing. This review aimed to suggest a new surgically oriented classification system for femoral periprosthetic fractures. Methods: We investigated the classifications, and current treatment trends for femoral periprosthetic fractures after TKA by means of a thorough review of the relevant literature. Results: Numerous studies reported good results of surgical treatment with modern fixatives including locking compression plates and retrograde intramedullary nails. However, few classifications of femoral periprosthetic fractures reflect the recent developments in surgical treatment. Conclusions: We recommend that surgical management be considered the first-line treatment for femoral periprosthetic fractures after TKA. Our new classification will help in deciding the surgical treatment option for femoral periprosthetic fractures after TKA.

Periprosthetic Occult Femoral Fracture: An Unknown Side Effect of Press-Fit Fixation in Primary Cementless Total Hip Arthroplasty

  • Purpose: The objectives of this study were to examine the prevalence and risk factors for development of periprosthetic occult femoral fractures during primary cementless total hip arthroplasty (THA) and to assess the clinical consequences of these fractures. Materials and Methods: A total of 199 hips were examined. Periprosthetic occult femoral fractures were defined as fractures not detected intraoperatively and on postoperative radiographs, but only observed on postoperative computed tomography (CT). Clinical, surgical, and radiographic analysis of variables was performed for identification of risk factors for periprosthetic occult femoral fractures. A comparison of stem subsidence, stem alignment, and thigh pain between the occult fracture group and the non-fracture group was also performed. Results: Periprosthetic occult femoral fractures were detected during the operation in 21 (10.6%) of 199 hips. Of eight hips with periprosthetic occult femoral fractures that were detected around the lesser trochanter, concurrent periprosthetic occult femoral fractures located at different levels were detected in six hips (75.0%). Only the female sex showed significant association with an increased risk of periprosthetic occult femoral fractures (odds ratio for males, 0.38; 95% confidence interval, 0.15-1.01; P=0.04). A significant difference in the incidence of thigh pain was observed between the occult fracture group and the non-fracture group (P<0.05). Conclusion: Occurrence of periprosthetic occult femoral fractures is relatively common during primary THA using tapered wedge stems. We recommend CT referral for female patients who report unexplained early postoperative thigh pain or developed periprosthetic intraoperative femoral fractures around the lesser trochanter during primary THA using tapered wedge stems.

  • 대퇴 삽입물 주위 골절은 고관절 치환술 후 발생하는 가장 치료하기 어려운 합병증 중 하나이며, 만족스러운 치료 결과를 얻으려면 면밀한 임상 및 방사선 사진 평가, 정확한 분류 및 치료 원칙에 대한 이해가 필수적이다. Vancouver 분류 시스템은 이러한 삽입물 주위 골절의 치료를 계획하기 위한 간단하고 효과적인 방법이다. 대퇴 스템이 안정된 골절은 견고한 내고정으로 효과적으로 치료할 수 있지만 대퇴 스템 해리가 동반된 경우 삽입물 주위 대퇴 골절은 재치환술이 필요하다. 고관절 치환술 후 발생할 수 있는 삽입물 주위 대퇴 골절 환자의 치료 원리와 대퇴 스템의 안정성을 평가하는 방법에 대해 설명하고자 한다.

Atypical Vancouver B1 periprosthetic fracture of the proximal femur in the United Kingdom: a case report challenged by myeloma, osteoporosis, infection, and recurrent implant failures

  • The indications for total hip replacement are increasing and not limited to osteoarthritis. Total hip replacement may also be done for trauma and pathological fractures in patients otherwise physiologically fit and active. This trend has led to an inevitable rise in complications such as periprosthetic femoral fracture. Periprosthetic femoral fracture can be challenging due to poor bone quality, osteoporosis, and stress fractures. We present a case of periprosthetic femoral fracture in a 71-year-old woman with some components of an atypical femoral fracture. The fracture was internally fixed but was subsequently complicated by infection, implant failure needing revision, and later stress fracture. She was on a bisphosphonate after her index total hip replacement surgery for an impending pathological left proximal femur fracture, and this may have caused the later stress fracture. Unfortunately, she then experienced implant breakage (nonunion), which was treated with a biplanar locking plate and bone grafting. The patient finally regained her premorbid mobility 13 months after the last surgery and progressed satisfactorily towards bony union.

Open Reduction and Internal Fixation for Vancouver B1 and B2 Periprosthetic Femoral Fractures: A Proportional Meta-Analysis

  • Purpose: Periprosthetic femoral fracture (PFF) is a common complication after total hip arthroplasty, and open reduction and internal fixation (ORIF) is a common surgical treatment. We conducted a meta-analysis to compare the outcomes of ORIF in patients with different fracture patterns (Vancouver B1 and B2). Materials and Methods: We conducted a systematic search of PubMed, Embase, Cochrane Library and KoreaMed from inception to August 2022. We conducted a pair-wise meta-analysis (with a fixed-effects model) on the 10 comparative studies and a proportional meta-analysis on the data from the 39 articles to determine a consensus. The outcomes were the incidence of reoperations that included osteosynthesis, irrigation/debridement and revision arthroplasty. Results: The pair-wise meta-analysis showed similar outcomes between two groups; the risk of reoperation (odds ratio [OR]=0.82, confidence interval [CI] 0.43-1.55, P=0.542), nonunion (OR=0.49; CI 0.22-1.10, P=0.085) and deep infection (OR=1.89, CI 0.48-7.46, P=0.361). In proportion meta-analysis, pooled prevalence of reoperation was 9% (95% CI, 6-12) in B1 and 8% (95% CI, 2-15) in B2 (heterogeneity between two groups (Q), P=0.772). The pooled prevalence of nonunion was same as of 4% in B1 and B2 (Q, P=0.678), and deep infection was 2% (95% CI, 1-3) in B1 and 4% (95% CI, 2-7) in B2 (Q, P=0.130). Conclusion: ORIF is a feasible treatment for B1 and B2 periprosthetic femoral fractures, with acceptable outcomes in terms of, nonunion and infection. The results of this study would help clinicians and provide baseline data for further studies validating PFF.

종양인공관절 주위 골절의 피질골 지주 중첩 동종골 이식술 유무에 따른 결과 비교 (Periprosthetic Fracture around Tumor Prosthesis, Comparison of Results with or without Cortical Strut Onlay Allograft)

  • 목적: 종양인공관절 주위 골절은 안정된 고정의 어려움과 골유합 후에도 골질 약화로 해리 및 재골절 위험이 많다. 피질골 지주 중첩 동종골 이식술을 종양인공관절 주위 골절에 적용하면 내고정도 쉽고, 골유합 기간도 단축되며, 합병증도 줄일 수 있을 것이라고 생각하였다. 본 연구는 종양인공관절 주위 골절 27예(30 골절)의 골절의 양상과 치료 후 재건 술식에 따른 생존율 및 합병증, 중첩 동종골 이식술 추가 여부에 따라 골유합 기간 및 기능적 결과가 차이가 있는지 분석하였다. 대상 및 방법: 골절 치료 시 판형 동종골 이식을 한 군 16예와 시행하지 않은 군 14예를 비교하였다. 분석항목은 종양인공관절 치환술부터 골절까지 기간, 골절의 양상, 골유합 기간 차이, 합병증, 및 기능적 결과를 기술하였다. 결과: 골절 양상은 unified classification system (UCS) B형이 21예(70.0%, 21/30)로 가장 많았으며 그 중 B1이 14예(46.7%, 14/30), B2가 1예(3.4%, 1/30), B3가 6예(20.0%, 6/30)였고 C형이 9예(30.0%, 9/30)였다. Kaplan-Meier 법에 의한 30 재건 부위의 5년, 10년 생존율은 각각 84.5%±4.18%, 42.2%±7.83%였다. 전체 30예의 골유합 기간은 평균 5.1개월(범위, 2.0-11.2개월)이었다. 동종골 이식군은 평균 3.5개월(범위, 2.0-6.26개월)로 고식적 고정군의 평균 7.2개월(범위, 4.0-11.2개월)보다 짧았다(p<0.0001). 최종 기능평가상 동종골 이식군은 평균 26.1점으로 고식적 고정군의 평균 20.9점보다 높았다(p<0.0001). 합병증은 4예로 모두 동종골을 사용하지 않은 군에서 있었다. 결론: 종양인공관절 주위 골절은 드물고 골유합도 비교적 잘 이루어지나 유합 후 합병증 최소화가 중요하다. 판형 중첩 동종골 이식술은 골유합 기간을 단축, 골질 증가, 해리위험성 감소 효과가 있으며 종양인공관절 주위 골절에 유용한 방법이다.

슬관절 전치환술 후 발생한 대퇴골 삽입물 주위 골절의 이중 금속판 고정술 (Dual Plate Fixation for Periprosthetic Femur Fracture after Total Knee Arthroplasty)

  • 목적: 슬관절 전치환술 후 발생한 대퇴 삽입물 주위 골절에 이중 금속판을 이용한 고정술 후 결과를 평가하고자 한다. 대상 및 방법: 2007년 10월부터 2013년 2월까지 슬관절 전치환술 후 발생한 대퇴골 삽입물 주위 골절 23예 중 Lewis와 Rorabeck의 분류상 II형으로 삽입물 이완은 없으나 과상부 분쇄골절이거나 골절선이 외상과까지 연장됐거나 주대(stem)가 있어 편측 고정만으로 골절부의 안정성을 얻을 수 없다고 판단한 경우에 내, 외측 이중 금속판을 이용하여 내고정술을 시행하고 최소 1년 이상 추시가 가능했던 13예를 대상으로 하였다. 환자의 평균 연령은 72세(65-82세)였고 여자가 11예, 남자가 2예였다. 수상 기전은 낙상이 9예, 교통사고가 4예였다. 류마티스관절염 1예를 제외하고 12예에서 골관절염으로 슬관절 전치환술을 받았고 3예에서 재치환술 등의 이유로 주대가 있었다. 슬관절 전치환술 후 골절 발생까지의 평균 기간은 28개월(1-108개월)이었고, 술 전 환자의 평균 골밀도는 T score -3.2 (-1.7에서 -4.4)였으며, 평균 추시기간은 23개월(12-65개월)이었다. 수술법은 골절부의 내측에 먼저 광근 하방으로 접근하여 해부학적 정복을 시행하고 금속판으로 내고정을 시행한 후 추가로 외측에 최소 절개법을 통해 잠김 압박 금속판을 사용하여 내고정하였다. 골유합 기간 및 합병증, 최종 추시 시 Hospital for Special Surgery Knee Score (HSS)를 평가하였다. 결과: 평균 골유합 기간은 17.4주(7-40주)였으며 2예에서 지연 유합이 발생하였으나 추가적 수술 없이 36주와 40주에 골유합을 얻었다. 1예에서 불유합이 발생하여 재고정술 및 자가골 이식술을 시행 후 3개월에 완전한 골유합을 얻었다. 최종 추시 시 전후방각형성은 평균 2.86도(0-4.9도), 내외반 각형성은 평균 1.67도(-1.2-4.9도)로 전체 예에서 부정 유합은 없었다. 최종 추시 시 슬관절 운동 범위는 평균 90도였고, HSS 점수는 평균 85점(70-95점)이었다. 결론: 슬관절 전치환술 후 발생한 대퇴골 삽입물 주위 골절 시 편측 고정만으로 골절부의 안정성을 얻을 수 없는 경우 이중 금속판을 이용한 내고정술은 안정적인 고정으로 빠른 재활이 가능하고 임상적으로 양호한 결과를 얻을 수 있는 방법으로 생각된다.

Intraoperative periprosthetic humeral fracture during reverse shoulder arthroplasty: a sequelae of prior biceps tenodesis

  • Intraoperative periprosthetic humeral fractures are a rare but debilitating complication of reverse shoulder arthroplasty and can occur during multiple stages of the procedure. Prior biceps tenodesis has been found to reduce cortical humeral strength and predispose the patient to humeral fracture. We present a case of a 68-year-old female with a previous history of biceps tenodesis due to an irreparable rotator cuff tear. Months later, and after symptoms persisted, a reverse shoulder arthroplasty was performed. During the surgery and while performing final reduction, a fracture line was observed involving the hole used for the previous tenodesis procedure. The fracture was repaired, and the patient reported favorable outcomes. We report several factors that might have contributed to sustenance of this intraoperative fracture including prior biceps tenodesis, use of a press fit humeral stem, and the sex of the patient. Internal Fixation Using Clavicle Hook Plates for Distal Clavicle Fractures

  • Background: To report the radiological and clinical outcomes of internal fixation using distal clavicle hook plates for distal clavicle fractures. Methods: From April 2008 to December 2012, 32 patients with distal clavicle fractures underwent surgery using an AO hook plate. The reduction was qualified and evaluated according to the radiological findings. The evaluation of the clinical outcomes was performed with the University of California at Los Angeles (UCLA) score, the Korean Shoulder score, and the visual analogue scale (VAS) pain score. Results: By radiological evaluation, we found that 31 of 32 patients showed anatomical reduction and solid bone union. Although we obtained satisfactory UCLA scores, Korean Shoulder Scale scores, and VAS pain scores, 12 cases of complications were present. We found 4 cases of osteolysis of the acromion, 1 case of nonunion, 3 cases of periprosthetic fractures, 3 cases of subacromial pain, and 1 case of skin irritation. We performed re-operations in 2 patients. Conclusions: To avoid complications associated with clavicle hook plates, choosing the appropriate hook size and bending of the hook according to the slope of the acromion undersurface is critical. Also, we believe that early removal of clavicle plates may help reduce complications.

Long-term cumulative survival and mechanical complications of single-tooth Ankylos Implants: focus on the abutment neck fractures

  • PURPOSE. To evaluate the cumulative survival rate (CSR) and mechanical complications of single-tooth $Ankylos^{(R)}$ implants. MATERIALS AND METHODS. This was a retrospective clinical study that analyzed 450 single $Ankylos^{(R)}$ implants installed in 275 patients between December 2005 and December 2012. The main outcomes were survival results CSR and implant failure) and mechanical complications (screw loosening, fracture, and cumulative fracture rate [CFR]). The main outcomes were analyzed according to age, sex, implant length or diameter, bone graft, arch, and position. RESULTS. The 8-year CSR was 96.9%. Thirteen (2.9%) implants failed because of early osseointegration failure in 3, marginal bone loss in 6, and abutment fracture in 4. Screw loosening occurred in 10 implants (2.2%), and 10 abutment fractures occurred. All abutment fractures were located in the neck, and concurrent screw fractures were observed. The CSR and rate of screw loosening did not differ significantly according to factors. The CFR was higher in middle-aged patients (5.3% vs 0.0% in younger and older patients); for teeth in a molar position (5.8% vs 0.0% for premolar or 1.1% for anterior position); and for larger-diameter implants (4.5% for 4.5 mm and 6.7% for 5.5 mm diameter vs 0.5% for 3.5 mm diameter) (all P<.05). CONCLUSION. The $Ankylos^{(R)}$ implant is suitable for single-tooth restoration in Koreans. However, relatively frequent abutment fractures (2.2%) were observed and some fractures resulted in implant failures. Middle-aged patients, the molar position, and a large implant diameter were associated with a high incidence of abutment fracture.
