Case series of maxillary sinus augmentation with biphasic calcium phosphate: a clinical and radiographic study

  • Purpose: The aim of this study was to evaluate 3.5 years-cumulative survival rate of implants placed on augmentedsinus using Osteon, a bone graft material, and to assess the height of the grafted material through radiographic evaluation. Methods: Twenty patients were treated with maxillary sinus augmentation and 45 implant fixtures were installed simultaneously or after 6 months healing period. The height of the augmented sinus and the loss of marginal bone were measured by panoramic and intraoral radiographs immediately after augmentation and up to 42 months (mean, 19.4 months) subsequently. Changes in the height of the sinus graft material were calculated radiographically. Results: The cumulative survival rate was 95.56% in all 45 implants. Additionall, normal healing process without any complication was observed in all patients. The original sinus height was mean 4.3 mm and the augmented sinus height was mean 13.4 mm after the surgery. The mean marginal bone loss till 42 months was $0.52{\pm}0.56\;mm$. The reduced height of Osteon was $0.83{\pm}0.38\;mm$ and it did not show significant correlation with the follow up periods (P=0.102). There were no statistically significant differences in reduced height of Osteon according to the simultaneous/delayed implantation (P=0.299) and particle size of Osteon (P=0.644). Conclusions: It can be suggested that Osteon may have predictable result when it was used as a grafting material for sinus floor augmentation.

A comparative clinical study on oxidized titanium implants and sandblasted large-grit acid etched implants in soft bone

  • Purpose: The aim of this retrospective study was to compare the survival rate of oxidized titanium implants and sandblasted large-grit acid etched implants in soft bone. Methods: 201 oxidized titanium implants were inserted in 84 patients between May 1999 and May 2004. 120 sandblasted large-grit acid etched implants were inserted in 74 patients between December 2000 and May 2004. The patients were followed-up 0${\sim}$5 years in ITI group or 0${\sim}$6 years in BRA group, respectively. The following information was collected from the patient records: age, gender, systemic disease, implant type, number, length and diameter of the implants, their location in the jaws, bone quantity, the number of failed implants, the causes of failure, and advanced surgery for bone augmentation. Results: In the oxidized titanium implants, 8 implants showed early failure, and 1 implant showed late failure, respectively. The cumulative survival rate was 95.48%. In the sandblasted large-grit acid etched implants, 1 implant showed late failure and cumulative survival rate was 99.10%. The cumulative survival rate and the survival rates in the case of the advanced procedure during the implant placement were not significantly different in both groups. Conclusions: Oxidized titanium implants and sandblasted large-grit acid etched implants can be used successfully in soft bone regardless of the surgical methods used during the implant placement.   • The recent trend of research and development on guided tissue regeneration focuses on the biodegradable membranes, which eliminate the need for subsequent surgical removal. They have demonstrated significant and equivalent clinical improvements to the ePTFE membranes. This study evaluate guided tissue regeneration wound healing in surgically induced intrabony periodontal defects following surgical treatment with a synthetic biodegradable membranes, made from a copolymer of glycolide and lactide, in 8 beagle dogs. After full thickeness flap reflection, exposed buccal bone of maxillary and mandibular canine and premolar was removed surgically mesiodistally and occlusoapically at $6mm{\times}6mm$ in size for preparation of periodontal defects. In experimental sites a customized barrier was formed and fitted to cover the defect. Flap was replaced slightly coronal to CEJ and sutured. Plaque control program was initiated and maintained until completion of the study. In 4, 8, 16 and 24 weeks after surgery, the animals were sacrificed and then undecalcified specimens were prepared for histologic evaluation. Histologic examination indicated significant periodontal regeneration characterized by new connective tissue attachment, cementum formation and bone formation. These membranes showed good biocompatibility throughout experiodontal period. The barriers had been completely resorbed with no apparent adverse effect on periodontal wound healing at 24 weeks. These results implicated that present synthetic biodegradable membrane facilitated guided tissue regeneration in periodontal defect.

이중미세나사산 임플란트의 임상적 및 방사선학적 1년 평가 (Clinical and radiographic evaluation of implants with dual-microthread: 1-year study)

  • Purpose: The stability of periodontal condition and marginal bone level were important to achieve long-term success of dental implant treatment. The aim of this study was to evaluate periodontal conditions and marginal bone loss around 67 GSII(OSSTEM, Seoul, Korea) dental implants with dual-microthread at the neck portion, 1 year after prosthetic loading. Materials and methods: Sixty-seven GS II dental implants in 27 patients(mean age; $47.4{\pm}14.0$ years) who received implant treatments at Pusan National University Hospital, were included in this study. Thirteen US II(OSSTEM, Seoul, Korea) implants with smooth neck design were selected for the control group. Periodontal and radiographic evaluations were carried out at baseline, 6 months and 12 months after prosthetic loading. Results: In the GS II group, plaque index(PI), gingival index(GI) and probing depth(PD) increased as time passed. In the US II group, GI and PD increased. Although marginal bone level was lower in the US II group in all evaluation periods, the changes between the periods were not statistically significant(p>0.05). In each period, periodontal parameters were not statistically significant between groups. Conclusion: One year after prosthetic loading, GS II and US II dental implants showed similar periodontal conditions and marginal bone response, and were within the criteria of success.

Mimicking Odontogenic Pain Caused by Burkitt's Lymphoma: A Case Report

  • Burkitt's lymphoma is a malignant monoclonal proliferation of early B-lymphocyte. Since Burkitt's lymphoma is a highly aggressive disease, early detection is a crucial. This disease often involves jaw and mandibular mass or swelling may also be seen, but in the early phase of Burkitt's lymphoma these symptoms cannot be observed. A rare case of Burkitt's lymphoma without any mandibular mass and the general symptoms was present. The excruciating toothache led the patient to visit the dental clinic and misdiagnosis of chronic periodontal abscess was made initially. Dentists should consider the oral manifestations of systemic disease when the multiple periodontal ligament space widening is observed and the dental treatment for mimicking odontogenic pain has no effect.

  • Sugar consumption has been increased in recent days according to an improvement in living conditions. It is generally accepted that sugar is one of the causes of caries and that diabetes mellitus is closely related to the periodontal diseases. This investigation was designed to gain informations on the influence of overdosage of sugar and/or high blood sugar level on the periodontal tissue. 12 rats weighing about 1.5Kg were divided into 4 groups, control one and 3-day, 7-day, 14-day ones after daily administration of 30-50 gm of sucrose. The results were as follows: 1. Daily adminisration of 30-50 gm of sucrose elevated blood glucose level as much as 10-20 mg% than before. 2. Epithelial keratinization was gradually conspicuous to the dosage of sucrose. 3. The severity of inflammatory infiltration was also increased to the dosage of sucrose. 4. Inflammatory infiltration was encountered in marginal gingiva more than other periodontal tissue.

Bacterial adhesion and colonization differences between zirconia and titanium implant abutments: an in vivo human study

  • Purpose: Several parameters have been described for determining the success or failure of dental implants. The surface properties of transgingival implant components have had a great impact on the long-term success of dental implants. The purpose of this study was to compare the tendency of two periodontal pathogens to adhere to and colonize zirconia abutments and titanium alloys both in hard surfaces and soft tissues. Methods: Twelve patients participated in this study. Three months after implant placement, the abutments were connected. Five weeks following the abutment connections, the abutments were removed, probing depth measurements were recorded, and gingival biopsies were performed. The abutments and gingival biopsies taken from the buccal gingiva were analyzed using real-time polymerase chain reaction to compare the DNA copy numbers of Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, and total bacteria. The surface free energy of the abutments was calculated using the sessile water drop method before replacement. Data analyses used the Mann Whitney U-test, and P-values below 0.05 find statistical significance. Results: The present study showed no statistically significant differences between the DNA copy numbers of A. actinomycetemcomitans, P. gingivalis, and total bacteria for both the titanium and zirconia abutments and the biopsies taken from their buccal gingiva. The differences between the free surface energy of the abutments had no influence on the microbiological findings. Conclusions: Zirconia surfaces have comparable properties to titanium alloy surfaces and may be suitable and safe materials for the long-term success of dental implants.

성견 2급 치근이개부 병변 치료시 이종골 이식 및 혈소판 농축 혈장의 골재생에 관한 효과 (The Effect of Platelet Rich Plasma Combined with Bovine Bone on the Treatment of Grade II Furcation Defects in Beagle Dogs)

  • New techniques for regenerating the destructed periodontal tissue have been studied for many years. Current acceptable methods of promoting periodontal regeneration are basis of removal of diseased soft tissue, root treatment, guided tissue regeneration, graft materials, and biological mediators. Platelet Rich Plasma has been reported as a biological mediator which regulates activities of wound healing progress including cell proliferation, migration, and metabolism. The purpose of this study is to evaluate the effects of using the Platelet Rich Plasma as a regeneration promoting agent for furcation involvement defect. Five adult beagle dogs were used in this experiment. The dogs were anesthetized with Ketamin HCl(0.1 ml/kg, IV)and Xylazine hydrochloride($Rompun^{(R)}$, Bayer, 0.1 ml/kg, IM) and conventional periodontal prophylaxis were performed with ultrasonic scaler and hand instruments. With intrasulcular and crestal incision, mucoperiosteal flap was elevated. Following decortication with 1/2 high speed round bur, degree II furcation defect was made on mandibular third(P3), forth(P4) and fifth(P5) premolar, and stopping was inserted. After 4 weeks, stopping was removed, and bone graft was performed. Ca-P was grafted in P3(experimental group I), Combination of Ca-P and plasma rich platelet were grafted in P4(experimental group II), and P5 was remained at control group.Systemic antibiotics(gentamicin sulfate)and anlgesics(phenyl butazone) were administrated intramuscular for 2 weeks after surgery. Irrigation with 0.1% Chlorhexidine Gluconate around operate sites was performed during the whole experimental period except one day immediate after surgery. Soft diets were fed through the whole experiment period. After 4, 8 weeks, the animals were sacrificed by perfusion technique. Tissue block was excised including the tooth and prepared for light microscope with Gomori's trichrome staining. At 4 weeks after surgery, there were rapid osteogenesis phenomenon on the defected area of the Platelet Rich Plasma plus Ca-P BBP group and early trabeculation pattern was made with new osteoid tissue produced by activated osteoblast. Bone formation was almost completed to the fornix of furcation by 8 weeks after surgery. In conclusion, Platelet Rich Plasma can promote rapid osteogenesis during healing of periodontalregeneration.

유성견에서 periodontal distraction에 의한 급속 치아견인 시 치수 및 치주조직의 변화에 관한 연구 (Pulp and periodontal tissue changes following rapid tooth retraction by periodontal distraction in young adult dogs)

  • 유성견에서 periodontal distraction 과정을 통해 급속 견인 후에 치수와 치주조직의 변화를 평가하였다. 유성견의 상악 제2소구치를 발거하고 제3소구치 근심의 치간골을 부분 제거하였다. 견인장치는 6일 동안 하루에 2번씩 견인하고, 강화기 동안 0주, 1주, 3주, 5주, 7주, 9주에 치수와 치주조직의 변화를 임상적, 방사선학적, 조직학적, 면역조직화화학적으로 관찰하였다. 신장된 치주인대 내에서 견인 직후부터 4주까지 활발한 골밀도 증가를 보였으며, 특히 2-3주 사이에서 가장 활발하였다. 강화기 0주에 압박측 치조골의 흡수 및 파골세포 출현, 염증세포 침윤이 가장 많이 관찰되었고 1주의 신장측 치주인대에서 특징적인 골형성을 보이기 시작하였다. 신생골 형성은 1주와 3주에 가장 많이 관찰되었으며 9주에는 성숙골로의 대치 및 치주인대의 재생으로 대조군과 차이가 없었다. 실험군의 calcitonin gene-related peptide의 발현은 치수, 치주인대 내의 염증 부위에서 나타났으며 0주, 1주에 압박측 치주인대에서 증가하였다가 5주 이후에는 감소하여 대조군과 차이가 없었다. 이상과 같은 소견은 periodontal distraction에 의한 급속 치아 이동이 정상적인 골 재형성 과정을 도모하는 새로운 치아이동 방법이 될 수 있음을 시사한다.

구개 저작 점막의 두께에 대한 연구 (The thickness of palatal masticatory mucosa)

  • The Palatal masticatory mucosa was widely used as a donor site in periodontal and implant surgery. but there were relatively few studies investigating the thickness of the palatal mucosa in dentate subjects. The purpose of this study was to study the thickness of palatal masticatory mucosa in korean subjects by direct clinical technique. Forty systemically and periodontally healthy subjects(20 males:20 females) participated in this study. A bone sounding method using a periodontal probe with minimal anesthesia and a prepared clear acrylic stent was utilized to assess the thickness of palatal mucosa at 24 measurement points defined according to the gingival margin and mid palatal suture. The results are as follows; 1. Mean thickness of palatal masticatory mucosa was $3.5{\pm}0.4mm$. and no gender differences were identified in the thickness of palatal masticatory mucosa. 2. The thickness of palatal masticatory mucosa increased from canine to second molar area(with the exception of the first molar area). canine and first molar areas were significantly thinner than other areas(P<0.05). 3. The thickness of palatal masticatory mucosa significantly increased in the sites farther from the gingival margin towarding the mid-palate(P<0.05). The results suggest that within the limits of the present study, premolar area appears to be the most appropriate donor site for soft tissue grafting procedures.
