Sex-Specific Trends in the Prevalence of Hypertension and the Number of People With Hypertension: Analysis of the Korea National Health and Nutrition Examination Survey (KNHANES) 1998-2018

  • Background and Objectives: As the Korean population ages fast, it is estimated that the people with hypertension, especially female patients, will increase rapidly. However, there are few data comparing the size of female and male hypertensive patients in the Korean population. Thus we assessed sex-specific trends in the prevalence and the number of people with hypertension. Methods: We analyzed data for 128,949 adults aged ≥20 years with valid blood pressure measurements from the 1998 to 2018 Korea National Health and Nutrition Examination Survey (KNHANES). The prevalence and the absolute number of hypertension were estimated with taking into the sampling weights separately for women and men. Results: Overall prevalence of hypertension is higher in men than in women. But, in older adults, women show higher prevalence and the number of people with hypertension. Between 1998 and 2018, prevalence of hypertension increased from 61.8% to 65.9% in elderly (age 65+) women, and from 49.0% to 59.4% in elderly men. During the same period, the number of elderly women with hypertension increased from 1.18 to 2.70 million, while the number of elderly men with hypertension increased from 0.57 to 1.78 million. Among hypertensive patients, undiagnosed hypertension and diagnosed-but-untreated hypertension were more common in men, while treated-but-uncontrolled hypertension were more common in women. Conclusion: The fast-growing number of elderly women with hypertension will be an important public health challenge for the Korean society to solve in order to reduce the burden of cardiovascular disease.

Occupational Exposure to Potentially Infectious Biological Material Among Physicians, Dentists, and Nurses at a University

  • Objective: The objective of this study was to evaluate the prevalence and incidence of accidents with biological material, the level of knowledge, and compliance to standard precautions (SPs) among dentists, physicians, nurses, and dental and medical students. Methods: A closed cohort study with a prospective and retrospective component was conducted between August 2014 and September 2015. The participants were contacted in two moments during the follow-up period, during which a structured questionnaire divided into six sections was used; the interviews were conducted during the follow-up period (Month 6) and at the end of the observation period (Month 12). Results: The global prevalence of accidents in the previous 12 months was 10.2%, with a difference between professionals and students (13.0% vs. 5.1%, respectively; p < 0.003). The incidence rate was 6.49 per 100 person/year, with difference between the groups (6.09 per 100 person/year in professionals and 7.26 per 100 person/year in students), type of specialization (hazard ratio, 3.27), and hours worked per week (hazard ratio, 2.27). The mean of compliance to SP was 31.99 (±3.85) points, with a median of 33 (30, 35) points against the expected 27.75 points. Adherence to SP was associated with the accident report (p < 0.020). Conclusion: We conclude that the proportion/incidence rate of accidents with biological material was high in relation to that in the literature, being higher in professionals and especially among physicians. The levels of knowledge and adherence to SP were good, with the best found in dentists and dental students.

경기남부지역의 닭에 있어서 Cryptosporidium baileyi의 감염상황 (Prevalence of Cryptosporidium baileyi Infection of Chickens In South Kyeonggi Area)

  • A total of 1050 layer and broiler chickens from 63 flocks of 21 poultry farms in South-Kyeonggi area, aged from 1 to 18 weeks, were investigated for the prevalence of Cryptosporidium baileyi infection from May 1992 to March 1993. The results have shown that 44 chickens(4.19%) were infected with C.(baileyi during the period of investigation) Fecal samples were treated by using Sheather's flotating technique and were examined under the light and phase contrast microscopy and then stained by Kinyoun method. Cryptosporidium oocysts were found in 35 out of 650 broilers(5.4%) and 9 out of 400 layer chickens(2.3%) aged mainly from 2 to 12 weeks. The regional infection rate were 4.7% in Pyeong-taek, 5.1% in An-sung and 2.3% in Yong-in, respectively. The average size of isolated oocyst was about $5.23{\times}4.92{\mu}m$ and the oocysts were orally inoculated into 2-day-old SPF chickens for the histological examination of oocyst in bursa of Fabricius. The study has concluded that C. baileyi infects chickens and oocysts were isolated in South-Kyeonggi area.

Green Tobacco Sickness Among Tobacco Harvesters in a Korean Village

  • Background: Green tobacco sickness (GTS), an occupational disease in tobacco harvesters, is a form of acute nicotine intoxication by nicotine absorption through the skin from the wet green tobacco plant. We carried out a questionnaire survey and measured cotinine concentration, the metabolic product of nicotine, to determine the prevalence, incidence, and risk factors of GTS in Korean tobacco harvesters. Methods: We measured cotinine concentrations, and administered a questionnaire survey to tobacco harvesters in Cheongsong-gun, Gyeongsangbuk-do, Korea. We repeatedly measured urine cotinine concentration five times with a questionnaire survey. Results: Cotinine concentration at dawn was significantly higher than that at other times; it was significantly lower during the nonharvesting period than during the harvesting period. However, little change in cotinine concentration was detected in the daytime during the harvesting period. Study participants included 20 men and 20 women. The prevalence of GTS was 37.5% and was significantly higher in women than in men (55.0% vs. 20.0%, p < 0.01). GTS incidence according to number of workdays was 3.4 occurrences/100 person days. Conclusion: In this study, nicotine exposure and metabolism were experimentally determined from the time of cotinine exposure, and biological monitoring was performed in each season. In the future, this information may be valuable for medical decision-making in GTS prevention.

국내 털진드기(Acarina: Trombiculidae)의 계절적 소장 및 지리적 분포 조사 (Seasonal Prevalence and Geographical Distribution of Trombiculid Mites(Acarina: Trombiculidae) in Korea)

  • 털진드기 개체군에 대한 지리적 분포상과 계절적 소장에 관한 조사를 1990년 8월부터 1991년 12월까지 실시하여 다음과 같은 결과를 얻었다. Sherman trap을 사용하여 포획한 325마리의 들쥐들로부터 총 15,679개체의 털진드기를 채집하여 4속 12종을 동정하였는데, 쭈쭈가무시병의 매개종인 Leptotrombidium pallidum이 4,751개체로 전체의 30.3%를 차지하여 우점종이었고, 제2의 매개종으로 알려진 L. scutellare는 1,132개체로 전체의 7.2%을 차지하였다. 털진드기 개체군의 지리적 분포상이 종에 따라 심한 차이를 보이며 각 조사 지역마다 우점종이 다르게 나타났는데, 경기도 고양군 도래 5리에서는 L. pallidum, 포천군 산정리에서는 Neotrombicula kwangneungensis, 파주군 장곡리에서는 N. tamiyai, 전북 익산군 장신리에서는 L. orientale, 이리시 신용동에서는 L. scutellare 등이었다. 털진드기 개체군의 계절적 소장을 보면, L. pallidum은 2개의 정점을 보였는데 10-11월에 큰 정점을, 4-5월에 작은 정점을 보인 후 여름에는 거의 자취를 감추었고 L. scutellare는 10월에만 집중적으로 나타났다.

한국에서 사육중인 모돈 및 30, 60, 90일령 돼지의 돼지파보바이러스에 대한 혈청학적 역학조사 (Sero-prevalence against porcine parvovirus in sows and 30-, 60-, 90-day-old pigs in Korea)

  • A total of 701 swine sera from 55 swine farms (Mar, 1998 through Feb, 2001) were nation-widely collected for the presence of antibody to porcine parvovirus (PPV) in sows and 30-, 60-, 90-day-old pigs. Sero-prevalence by haemagglutination inhibition assay with guinea pig red blood cells was investigated on the basis of year, region and season, respectively. In general, there was no significant difference with gradual decrease of passive immunity for the sero-prevalence to PPV in sows and 30-, 60-, 90-day-old pigs for the period of 1999 and 2000. However, regional variation was observed in the provinces of Kyonggi, Choongnam and Kyungnam, Natural infection of the virus in 90-day-old pigs was increased during the fall and the winter. Thus, it seems that the natural infection of PPV in growing pigs may be attributed to the increased outbreak of postweaning multisystemic wasting syndorme in co-infection with porcine circoviruses.

Prevalence and risk factors of peri-implant mucositis and peri-implantitis after at least 7 years of loading

  • Purpose: This study examined the prevalence and risk factors of peri-implant disease after at least 7 years of dental implant loading. Methods: A total of 111 patients with 218 dental implants were treated. The follow-up period for all implants was at least 7 years. The patients' dental records were collected and risk factors of peri-implant disease were investigated through logistic regression analysis. Results: The overall implant survival rate was 95.87%, because 9 of the 218 implants failed. The prevalence of peri-implant mucositis and peri-implantitis was 39.7% and 16.7%, respectively. As risk factors, smoking and prosthetic splinting showed significant associations with peri-implantitis (P<0.05). Conclusions: Within the limits of this study, no significant correlations were found between any risk factors and peri-implant mucositis, but a significantly elevated risk of peri-implantitis was observed in patients who smoked or had splinted prostheses in 2 or more implants.

Prevalence of Abnormal Anal Cytology in HIV-Infected Women: a Hospital-Based Study

  • Background: To study the prevalence of abnormal anal cytology by Papanicolaou (Pap) technique in HIV-infected women who attended a HIV clinic at Prapokklao Hospital, Chanthaburi, Thailand. Materials and Methods: HIV-infected women who attended a HIV clinic at Prapokklao Hospital from March 2013 to February 2014 were recruited for anal Pap smears. Participants who had abnormal results of equally or over "abnormal squamous/glandular cells of undetermined significance" (ASC-US) were classified as abnormal anal cytology. Results: A total of 590 anal Pap smears were performed at HIV clinic of Prapokklao Hospital during the study period. There were only 13 patients who had abnormal Pap tests, which were: 11 ASC-US and 2 HSIL (high grade squamous intraepithelial lesion). The prevalence of abnormal anal Pap smears in HIV-infected women who attended HIV clinic at Prapokklao Hospital was 2.2 percent. Percentage of high risk HPV in patients who had abnormal Pap test was 88.9 (8/9). Conclusions: The prevalence of abnormal anal Papanicolaou smears in HIV-infected women who attended the HIV clinic at Prapokklao hospital was quite low in comparison to the earlier literature.

Infection of marine diatom Coscinodiscus wailesii(Bacillariophyceae) by the parasitic nanoflagellate Pirsonia diadema(Stramenopiles) from Yongho Bay in Korea

  • The infection of marine diatom Coscinodiscus wailesii by a parasitic protist from the Yongho Bay of Busan, Korea was observed during the diatom bloom events in 2017 through 2018. The morphological and molecular features suggested that the parasitic nanoflagellate Pirsonia diadema was responsible for the infection. During the study period, the parasite prevalence ranged from 0.3% to 3.3%, and infected C. wailesii cells were observed only at surface seawater temperatures ranging between 10.9 and 19.9℃, although the host population appeared at temperatures above 25℃. The parasite and host system was successfully established as cultures. Using the cultures, we determined the morphological features over the infection cycle, parasite generation time, parasite prevalence as a function of inoculum size, and zoospore infectivity and survival time. The diatom C. wailesii was readily infected by the parasite P. diadema, with a parasite prevalence reaching up to 100% and a zoospore to host inoculum ratio above 20:1. The survival and infectivity of the parasite zoospores decreased with age. While the zoospores could survive up to 88 hours, they quickly lost their ability to infect after 48 hours. These results could lead to a better understanding of the biology and ecology of the parasitoid infecting the giant-sized diatoms in coastal waters.

Epidemiology of Urolithiasis with Sex and Working Status Stratification Based on the National Representative Cohort in Republic of Korea

  • Background: This study aimed to estimate the annual prevalence and incidence of urolithiasis stratified by work status based on a large nationwide sample. Methods: This study used data from the National Health Insurance Service-National Sample Cohort from 2002 to 2015. The prevalence and incidence of urolithiasis were estimated based on work status and gender stratification. The risk of urolithiasis among workers was calculated using age-standardized incidence ratio with stratification of work type. Results: The prevalence of urolithiasis was significantly higher in workers than in non-workers, especially men, during the follow-up period. The total estimated number of urolithiasis cases was 41,086 and the overall incidence of urolithiasis was 0.3%. The age-standardized incidence ratio of urolithiasis was significantly higher among the total workers (1.14; 95% confidence interval, 1.13-1.16), self-employed workers (1.08; 95% confidence interval, 1.06-1.11), and paid workers (1.19; 95% confidence interval, 1.17-1.21) than among the non-working population. Conclusions: Workers, especially paid workers and men, were vulnerable to urolithiasis. Further studies are required to investigate the effects of working conditions on urolithiasis.
