흉부 관통상에 의해 심실중격결손이 동반된 대부분의 환자들은 내원 시 활력징후가 불안정하여 심폐소생술 및 응급 개흉술을 시행받게 되는데, 이러한 경우 심장과 흉부 손상의 위치 및 정도에 대한 자세한 검사가 이루어지지 않은 상태이므로 발견되지 않았거나 잔존하는 이상 소견들이 수술 후 검사에서 발견될 수 있고 이에 대한 재수술을 필요로 하는 경우도 있다. 심실중격결손의 심도자술을 통한 폐쇄는 수술을 대처할 수 있는 방법으로서, 특히 수술 후 잔존하는 단락의 경우 이차 수술의 위험을 피할 수 있다. 저자들은 흉부 자상에 의한 심실벽의 열창 및 심실중격결손을 수술적 방법으로 봉합한 후 잔존하는 심실중격결손을 기구($Amplatzer^{(R)}$ VSD occluder)를 이용한 중재적 심도자술로 치료한 증례를 문헌 고찰과 함께 보고한다. 심도자술 6개월 후 시행한 심초음파검사에서 잔존하는 심실 좌우 단락은 없었고, 심실 크기와 기능도 정상이었고, 현재 상태는 양호하여 정상적으로 학교생활을 하고 있다.
외상에 의한 기관-기관지 손상은 비교적 드물고 경부 부위를 제외하곤 국내에서는 대부분 둔상에 기인한다. 기관-기관지는 흉부 내에 깊이 있어 비교적 자상에 의한 손상은 적다. 저자들은 제5흉추 우측 옆부위의 자상으로 인한 기정맥과 좌측 주 기관지의 파열 1례를 치험하였다. 환자는 24세 남자로 응급실에서 촬영한 단순 흉부엑스선 사진상 흉부내에 칼이 보였다. 칼은 응급실에서 제거하지 않았다. 환자는 기관지 내시경 검사나 전산화 단층촬영 없이 내원 30분내 수술실로 옮겨 개흉하여 과도을 제거하였다. 사선으로 파열된 기정맥과 좌측 주 기관지를 봉합하였다. 환자는 술 후 14일째 특별한 문제없이 퇴원하였다.
Purpose: A eutectic mixture of local anesthetics (EMLA$^{(R)}$) cream has been used as a topical anesthetic to reduce the pain of procedures penetrating the skin. It is generally applied for 40 to 60 minutes before the painful procedure. Because of the long application period, EMLA$^{(R)}$ is not useful in the emergency department (ED). The purpose of this study was to determine whether a 20-minute application of 9.6% lidocaine would be useful in reducing the pain of routine peripheral intravenous cannulation in the ED. Methods: We examined 27 male and 19 female patients ages over 18 years of age who required intravenous cannula insertion. Intravenous insertion was performed on 46 patients: 24 patients in the placebo group (mean age: 40.0 years) and 22 in the 9.6% lidocaine group (mean age: 37.6 years). The 9.6% lidocaine or placebo gel was applied and covered with an occlusive dressing for 20 minutes. Pain was scored by the patients using a 0- to 10-cm visual analogue scale. Results: The patients in the 9.6% lidocaine group (mean pain score: 3.4) experienced less pain than those in the placebo group (mean: 5.3), and the difference was statistically significant (p=0.029). Conclusion: We concluded that a 20-minute application of 9.6% lidocaine is safe and effective for reducing pain associated with venipuncture.
배경: 외상에 의한 횡격막 파열은 사회환경의 변화로 점차 증가하는 추세이며 이 질환의 조기발견 및 치료에 대한 많은 연구가 행하여지고 있다. 이에 저자들은 횡경막파열로 수술을 받은 환자를 대상으로 후향적 조사를 하여 조기발견 및 치료에 도움을 얻고자 본 연구를 시작하였다. 대상 및 방법: 1994년 1월부터 1998년 4월까지 본원 흉부외과교실에서 술전 및 술후 외상성 횡경막파열로 진단 받은 20례의 환자를 대상으로 하여 술전 임상양상과 진단율, 동반질환 및 술후 합병증 등을 조사하였다. 결과: 사회활동력이 왕성한 연령의 남성에서 많이 발생하였으며 둔상 75%, 관통상 25%이었다. 좌측횡경막파열 16례(80%), 우측 3례(15%), 양측 1례(5%)였으며 증상으로는 흉통 및 호흡곤란이 가장 많았다. 술전 진단은 10례(50%)에서 가능하였으며 술중 진단 6례(30%), 만기 진단 4례(20%)였다. 술후 창상감염이 가장 많은 합병증이었으며, 사망은 2례가 있었다. 결론: 외상 후 조속한 진단을 위해 임상증상 및 흉부 단순촬영상 이상소견이 의심되면 지속적인 관찰과 검사가 필요하며 동반질환의 치료를 위해 관련된 과간의 협조가 필수적이라고 사료된다.
Objective : In the pediatric population the skull has not yet undergone ossification and it is assumed that the diagnostic rate of skull fractures by simple X-rays are lower than that of adults. It has been recently proposed that the diagnostic rates of skull fractures by 3-dimensional computer tomography (3D-CT) are higher than simple X-rays. The authors therefore attempted to compare the diagnostic rates of pediatric skull fractures by simple X-rays and 3D-CTs with respect to the type of fracture. Methods : One-hundred patients aged less than 12 years who visited the Emergency Center for cranial injury were subject to simple X-rays and 3D-CTs. The type and location of the fractures were compared and Kappa statistical analysis and the t-test were conducted. Results : Among the 100 pediatric patients, 65 were male and 35 were female. The mean age was $50{\pm}45$ months. 63 patients had simple skull fractures and 22 had complex fractures, and the types of fractures were linear fractures in 74, diastatic fractures 15, depressed fractures in 10, penetrating fracture in 1, and greenstick fractures in 3 patients. Statistical difference was observed for the predictive value of simple skull fractures' diagnostic rate depending on the method for diagnosis. A significant difference of the Kappa value was noted in the diagnosis of depressed skull fractures and diastatic skull fractures. Conclusion : In the majority of pediatric skull fractures, 3D-CT showed superior diagnosis rates compared to simple skull X-rays and therefore 3D-CT is recommended whenever skull fractures are suspected. This is especially true for depressed skull fractures and diastatic skull fractures.
관통성 손상 후 외부 이물질이 신체 조직 내 남아 있는 경우는 응급실을 방문하는 주요 원인 중의 하나이다. 수부에서 발견되는 이물질은 대부분 외상에 의해 발생하며 식물성 조각들이 가장 흔하고, 다음으로 유리조각, 금속조각의 순서로 발생한다. 특히 나무조각이나, 플라스틱 조각 등은 방사선투과성(radiolucency)을 가지고 있어 단순 방사선 사진으로 쉽게 발견하기 어려운 경우가 많아 외부 이물질을 찾는 것이 어려울 뿐만 아니라 일부 제거하더라도 이물질의 잔존 여부를 감별하기가 어렵다. 이에 저자들은 나무가시에 수상 당한 후 1차 의료기간에서 일부 나무가시를 제거 후 잔존 나무가시를 발견하지 못하고 수상 4주 이후 초음파을 이용하여 잔존 이물질을 진단하여 수술적으로 치료 후 동통의 감소를 보인 1례를 경험하였기에 문헌 고찰과 함께 보고하는 바이다.
경북대학교병원 흉븐외과엔서는 1995년 8월까지 약 10년간 본과에 입원하여 치료한 60명의 15세이하 소아연령의 흉부외상 환아 60울을 대상으로, 외상의 원인과 양상 및 치료결과 등을 분석하여, 국내에서 발표됐던 전체 연령군에 대한 결과와 비교하여 소아 연령층의 특수성을 파악하는데 중점을 두어 분석해 보았다. 남아가 46명으로 77%를 차지하였고, 평균연령은 9세였다. 50명의 환아가 들상에 기인하였으며, 둔상환아의 60%이상이 교통사고와 관련되어 있었다. 들상의 경우 늑골골절이 52%로 가장 흔하였고, 그 다음이 기흉, 혈흉 등의 순이었다. 특히 둔상환아의 32%가 늑골골절없이 기흉 등 흉곽내 장기 손상이 있었다. 관통상의 경우 10명 모두 남아였으며, 기흉, 혈흉이 가장 흔하였다. 42%의 환아에서 흉 부이외의 동반손상이 있었다. 치료를 위해 가장 흔히 시행한 외과적 처치는흉강삽관술이었으며(45%), 10례에서 개흉술이 필요하였다. 동반손상아 있었던 경우 MISS score와 중환자실 입실기간 사이에는 유의한 상관관계가 있었다(p<0.05). 9명의 환아에서 합병증이 있었으며 폐혈증으로 1명(1.7%)이 사망하였다.

Introduction: Maxillomandibular fixation (MMF) is essential before surgery under general anesthesia in maxillofacial trauma patients. MMF is used basically to reconstruct the occlusion and occlusal stability to recover the facial shape and oral functions. The arch bar and wire is a traditional method for MMF, but it can not only bring pressure to the periodontal ligaments and teeth but also cause a penetrating injury to the surgeons. Materials and Methods: In this study, 198 patients with an open reduction using a manual reduction without MMF from September 2005 to May 2010 in Dankook University Dental Hospital were subjected to a follow-up evaluation during the postoperative 4 months periods. This study evaluated the incidence of complications according to the condition of the patient (gender, age), the state of bony union of the fracture sites and a numeric rating scale evaluation for postoperative pain scoring. Results: 1. The complications were classified into major and minor according to the seriousness, and the major complication rate was as low as 2.02%. Only 2 cases of re-operations (1.01%) were encountered. In the classification according to the fracture line, plate fracture was observed in both cases of mandibular symphysis fracture, and angle fractures and loosening of two screws were noted in the case of mandibular angle fracture. 2. The complication rate was similar regardless of gender and age. 3. The degree of bony union was satisfactory, and the complication rate was reduced as the bony union improved. 4. More patients complained of pain as the operation time was increased. Conclusion: The use of MMF is not always necessary if a skilled assistant is provided to help manually reduce the fracture site. Compared to other studies of mandibular fracture surgery using MMF, the complication rate was similar using only manual reduction and the patients' discomfort was reduced without MMF.
BACKGROUND: Collagen organization within tissues has a critical role in wound regeneration. Collagen fibril diameter, arrangements and maturity between connective tissue growth factor (CTGF) small interfering RNA (siRNA) and mismatch scrambled siRNA-treated wound were compared to evaluate the efficacy of CTGF siRNA as a future implement for scar preventive medicine. METHODS: Nanocomplexes of CTGF small interfering RNA (CTGF siRNA) with cell penetrating peptides (KALA and $MPG^{{\Delta}NLS}$) were formulated and their effects on CTGF downregulation, collagen fibril diameter and arrangement were investigated. Various ratios of CTGF siRNA and peptide complexes were prepared and down-regulation were evaluated by immunoblot analysis. Control and CTGF siRNA modified cells-populated collagen lattices were prepared and rates of contraction measured. Collagen organization in rabbit ear 8 mm biopsy punch wound at 1 day to 8 wks post injury time were investigated by transmission electron microscopy and histology was investigated with Olympus System and TS-Auto software. CONCLUSION: CTGF expression was down-regulated to 40% of control by CTGF siRNA/KALA (1:24) complexes (p<0.01) and collagen lattice contraction was inhibited. However, down-regulated of CTGF by CTGF $siRNA/MPG^{{\Delta}NLS}$ complexes was not statistically significant. CTGF KALA-treated wound appeared with well formed-basket weave pattern of collagen fibrils with mean diameter of $128{\pm}22nm$ (n = 821). Mismatch siRNA/KALA-treated wound showed a high frequency of parallel small diameter fibrils (mean $90{\pm}20nm$, n = 563). CONCLUSION: Controlling over-expression of CTGF by peptide-mediated siRNA delivery could improve the collagen orientation and tissue remodeling in full thickness rabbit ear wound.

Osteomyelitis typically occurs because of the direct inoculation of bacteria or fungi after penetrating trauma or surgical contamination or, by extension, from soft tissue infection. Osteomyelitis is rarely reported in wildlife animals, though severe chronic osteomyelitis cases do exist in wildlife owing to the scarcity of medical support in the wild environment. This report describes three cases of chronic osteomyelitis in wild raccoon dogs related to trauma. The typical symptoms of three reported cases were ataxia, stiffness, muscle atrophy, and lethargy. All three cases were relevant to traumatic or severe external injury, and skin infestation caused by ectoparasites was apparent on an ocular inspection. In the radiographic examination, diffuse sites of osteolytic lesions and remarkable periosteal responses were demonstrated around the injured limb in all three cases. Apparent neutrophilia with a left shift, lymphocytosis, and monocytosis in hematological examinations generally indicated chronic infection as shown in case 1 and 3. Treatment was attempted with broad-spectrum antibiotics and non-steroidal anti-inflammatory drugs, such as amoxicillin/clavulanic acid, enrofloxacin, clindamycin, and meloxicam. These treatment options helped improve the overall prognosis of chronic osteomyelitis, but the outcomes did not meet the treatment goal entirely. Osteomyelitis can be extremely challenging to treat, particularly in wild animals, because of their distinctive traits, such as masking phenomenon and uncontrolled exposure to ectoparasites. Earlier diagnosis with a radiographic examination, hematological examinations, and careful patient monitoring, followed by prolonged antibiotic therapy and restricted exercise, are the key factors leading to a better prognosis.
