Various Approaches and Treatments for Pelvic Organ Prolapse in Women

  • Pelvic organ prolapse (POP) is bulging of one or more of the pelvic organs into the vagina and triggered by multiple causes. It is a very common disorder, especially among older women. POP is characterized by protrusion of the presentation part visible by the naked eye, and problems with urination or bowel movements. POP can be diagnosed based on the onset of symptoms and a pelvic exam, and management options include medical and surgical treatment. Although medical treatment cannot correct the abnormal herniation of the pelvic structures, this can help alleviate symptoms. One of the disadvantages of surgical interventions is recurrence, and advances in surgical techniques have decreased recurrence rates of POP. Therefore, author will explain the gynecology and urology approach and treatment.

산후 골반장기탈출증에 대한 한의학적 임상 연구 동향 (Review of Clinical Research of Korean Medicine on Postpartum Pelvic Organ Prolapse)

  • Objectives: The purpose of this study is to review the clinical research trends of postpartum pelvic organ prolapse and to recognize the efficacy of Korean medicine intervention. Methods: Based on seven domestic and foreign databases, including Research Information Sharing Service (RISS), Oriental Medicine Advanced Searching Integrated System (OASIS), Journal of Korean Obstetric and Gynecology, Cochrane Library Central, Pubmed, China National Knowledge Infrastructure (CNKI) and WangFang Med Online, we analyzed the clinical trials using Korean medicine intervention, which included acupuncture and herbal medicine. Data retrieval was carried out on May 18 to 20, 2020, and a total of 13 papers were included. Results: All papers were published in China and it contains nine randomized controlled trials, three clinical trials, and one case. The most frequently used intervention was herbal medicine, and Bupleuri Radix, Cimicifugae Rhizoma were used. The treatment group treated by Korean medicine intervention was more effective than the control group. Also, there were no significant side effects of Korean medicine. Conclusion: This study shows that Korean medicine can be effective and safe medical alternatives or options for pelvic organ prolapse patients. However, to laying the foundation of clinical guidelines and applying it to the real-world clinical scene, further follow-up research is needed.

만성 기능성 변비 환자에서 동적 MR Defecography의 최신동향 (The Latest Trend of Dynamic MR Defecography for the Chronic Constipation Patient)

  • 최근 의학 영상 기술의 발전과 함께 동적 골반 MRI(Pelvic magnetic resonance imaging)가 소개되었고 이러한 기술을 이용하여 동적 MR 배변조영술이 소개되어 환자의 진단에 도움을 주기 위해 사용되고 있다. 처음에 도입 당시에는 탈장(Enterocele)과 방광탈출증(Cystocele) 등을 진단하기 위하여 사용되었지만, 이후 장기의 탈출증(Prolapse), 또는 다른 골반 장기의 이상에 점점 그 유용성이 증명되어 현재는 기능성변비 등 다른 질환에서도 많은 보고가 있다. 본 논고에서는 골반 MRI 및 동적 MR 배변조영술에 대해서 소개하고 앞으로의 임상적 응용에 대한 전망을 살펴보도록 한다. 최근까지도 Pelvic MRI의 결과들은 앙와위(Supine position)에서 골반의 움직임을 관찰하는데 Yang 등은 26명의 골반 기관의 탈출증(Pelvic organ prolapse)을 가진 환자들을 관찰하고 Pubococcygeal line을 해부학적인 지표로 이용하여 좋은 결과를 얻었다고 보고했다. 이들은 Fast gradient recalled acquisition(fast GRASS)을 이용하여, Cystocoele, Prolapse, Enterocoele 그리고 Rectocoele 등을 증상이 없는 대조군과 비교하여 보고하였다. Kruyt 등은 Posterior compartment를 주로 관찰하여 MRI가 Fluoroscopy에 비하여 더 도움이 된다고 보고하였다. Healy 등은 Fast GRASS sequence를 이용하여 변비를 가진 환자와 변실금을 가진 환자, 그리고 증상이 없는 대조군을 대상으로 하여 동적 MRI 검사를 실시하였다. 이들은 변비나 변실금 증상과 관련되어 환자들의 Posterior compartment에 여러 곳에서 Prolapse을 관찰할 수 있었다. 이후 MRI 기술은 Lienemann 등에 의해서 더욱 발전했는데 그는 Fast T2 weighted turbo spin echo 기술을 이용하여 영상을 좀더 세밀하게 얻을 수 있게 되었다. 지금까지의 앙와위에서의 검사로 진단에 한계가 있었던 Intussusception 등의 질환을 Open MR 등의 방법으로 극복할 수 있다면 장래에는 방사선학적 배변조영술을 대체할 수 있는 검사법으로 발전할 수 있을 것으로 생각된다.

Interaction and main effects of physical and depressive symptoms on quality of life in Korean women seeking care for rectal prolapse: a cross-sectional observational study

  • Purpose: Rectal prolapse is still a relatively understudied medical condition, especially in women, whereas physical symptoms, depressive symptoms, and quality of life (QOL) in women with pelvic organ prolapse have been steadily studied. This study aimed to examine the interaction and main effects of physical and depressive symptoms on physical and mental QOL of women seeking care for rectal prolapse in Korea. Methods: Ninety-two women with rectal prolapse were recruited from a colorectal surgery clinic of a tertiary teaching hospital in Gwangju, Korea. Physical symptoms related to rectal prolapse (pelvic organ prolapse distress, POPD; colorectal-anal distress, CRAD; and urinary distress, UD), depression, and QOL were measured. The data were analyzed using descriptive statistics, Pearson correlation coefficient, and two-way analysis of variance. Results: The interaction between POPD symptoms and depressive symptoms (F=4.51, p=.037) affected physical QOL. The interaction between POPD (F=9.66, p=.003) and CRAD symptoms (F=7.48, p=.008), respectively, and depressive symptoms affected mental QOL. Depressive symptoms had a significant main effect on the physical QOL in the CRAD (F=6.22, p=.014) and UD (F=6.01, p=.016) groups and on mental QOL in the UD group (F=24.54, p<.001). Conclusion: Physical and depressive symptoms should be considered together to improve the QOL in women with rectal prolapse. Nursing interventions and medical treatments to decrease rectal prolapse-related physical and depressive symptoms are required to improve QOL in women with symptomatic rectal prolapse.

The Beneficial and Adverse Effects of Raloxifene in Menopausal Women: A Mini Review

  • Objectives: The present mini review aimed to summarize the existing knowledge regarding the beneficial and adverse effects of raloxifene in menopausal women. Methods: This study is a review of relevant publications about the effects of raloxifene on sleep disorder, depression, venous thromboembolism, the plasma concentration of lipoprotein, breast cancer, and cognitive function among menopausal women. Results: Raloxifene showed no significant effect on depression and sleep disorder. Verbal memory improved with administration of 60 mg/day of raloxifene while a mild cognitive impairment risk reduction by 33% was observed with administration of 120 mg/day of raloxifene. Raloxifene was associated with a 50% decrease in the need for prolapse surgery. The result of a meta-analysis showed a significant decline in the plasma concentration of lipoprotein in the raloxifene group compared to placebo (standardized mean difference, -0.43; 10 trials). A network meta-analysis showed that raloxifene significantly decreased the risk of breast cancer (relative risk, 0.572; 95% confidence interval, 0.327-0.881; P = 0.01). In terms of adverse effects of raloxifene, the odds ratio (OR) was observed to be 1.54 (P = 0.006), indicating 54% increase in the risk of deep vein thrombosis (DVT) while the OR for pulmonary embolism (PE) was 1.05, suggesting a 91% increase in the risk of PE alone (P = 0.03). Conclusions: Raloxifene had no significant effect on depression and sleep disorder but decreased the concentration of lipoprotein. Raloxifene administration was associated with an increased risk of DVT and PE and a decreased risk of breast cancer and pelvic organ prolapse in postmenopausal women.

Clinical outcomes of hysterectomy for benign diseases in the female genital tract: 6 years' experience in a single institute

  • Background: Hysterectomy is one of the major gynecologic surgeries. Historically, several surgical procedures have been used for hysterectomy. The present study aims to evaluate the surgical trends and clinical outcomes of hysterectomy performed for benign diseases at the Yeungnam University Hospital. Methods: We retrospectively reviewed patients who underwent a hysterectomy for benign diseases from 2013 to 2018. Data included the patients' demographic characteristics, surgical indications, hysterectomy procedures, postoperative pathologies, and perioperative outcomes. Results: A total of 809 patients were included. The three major indications for hysterectomy were uterine leiomyoma, pelvic organ prolapse, and adenomyosis. The most common procedure was total laparoscopic hysterectomy (TLH, 45.2%), followed by open hysterectomy (32.6%). During the study period, the rate of open hysterectomy was nearly constant (29.4%-38.1%). The mean operative time was the shortest in the single-port laparoscopic assisted vaginal hysterectomy (LAVH, 89.5 minutes), followed by vaginal hysterectomy (VH, 96.8 minutes) and TLH (105 minutes). The mean decrease in postoperative hemoglobin level was minimum in single-port LAVH (1.8 g/dL) and VH (1.8 g/dL). Conversion to open surgery or multi-port surgery occurred in five cases (0.6%). Surgical complications including wound dehiscence, organ injuries, and conditions requiring reoperation were observed in 52 cases (6.4%). Conclusion: Minimally invasive approach was used for most hysterectomies for benign diseases, but the rate of open hysterectomy has mostly remained constant. Single-port LAVH and VH showed the most tolerable outcomes in terms of operative time and postoperative drop in hemoglobin level in selected cases.

Predictors of Acute Postoperative Urinary Retention after Transvaginal Uterosacral Suspension Surgery

  • Objectives: To investigate the rate of postoperative urinary retention (POUR) and identify the risk factors for this complication in women who underwent transvaginal uterosacral suspension surgery. Methods: A retrospective chart review was conducted for 75 women who underwent transvaginal uterosacral suspension surgery with vaginal hysterectomy, repair of cystocele, and levator myorrhaphy with/without transobturator anti-incontinence surgery. POUR was defined as a need for continuous intermittent catheterization on the third day subsequent to removal of the urethral indwelling catheter. Results: Acute POUR was reported in 18 women (24.0%). Thirty-six of the 75 patients (48.0%) had undergone anti-incontinence surgery. Crude analysis revealed significant association between the following variables and the risk of POUR: hypertension, the lower average flow rate in the pressure-flow study (PFS), greater post-void residual (PVR) urine volume in PFS, and PVR >30% of the total bladder capacity (TBC) in PFS. In the logistic regression analysis, PVR >30% of the TBC in PFS was identified as the only significant predictor of POUR (odds ratio, 15.4; 95% confidence interval, 2.5-90.9; P = 0.003). Conclusions: The PVR >30% of the TBC in PFS was identified as the only predictive factor of acute POUR in women who underwent transvaginal uterosacral suspension surgery.

Content and quality of YouTube regarding women's health: a scoping review

  • Purpose: This scoping review investigated the content and quality of YouTube videos on women's health. Methods: A literature search of the Cochrane Library, PubMed, Embase, CINAHL, ERIC, and RISS databases was performed using the keywords "('youtube'/exp OR youtube OR 'social media'/exp OR 'social media' OR (('social'/exp OR social) AND ('media'/exp OR media))) AND ('female health care' OR (('female'/exp OR female) AND ('health'/exp OR health) AND ('care'/exp OR care)))" from February 21 to 27, 2023. Peer-reviewed analytic studies in English or Korean that focused on women's health using YouTube were included. Results: The review identified 21 articles that covered various themes related to women's health, such as breast cancer, urinary disease, sexual health, pelvic organ prolapse, the human papillomavirus vaccine, Papanikolaou smears, contraception, women's health information during the coronavirus disease 2019 pandemic, obstetric epidural anesthesia, and placenta accreta. However, the overall quality of the content was low, inaccurate, unreliable, and misleading. Conclusion: This scoping review demonstrated that YouTube videos on women's health covered diverse topics, but the quality of the content needed improvement. More reliable and high-quality videos produced by academic institutes and healthcare professionals specializing in women's health are needed for social media to be usable as a reliable source of women's health information. The high number of views and shares received by the videos underscores the importance of providing accurate and reliable information on women's health.
