The Major Factors Influencing on the Financial Performance of the Profit and Loss-Making Hospitals - With Cases of the Provincial Hospitals - (흑자 및 적자병원의 경영성과요인 -지방공사의료원을 중심으로-)

  • This study was designed to find out the factors which influence on the financial performance of the hospital. Out of 32 provincial hospitals which were established by the government, 10 hospitals were selected as sample hospitals. Ten hospitals were divided into two groups(5 hospitals each), one of which was profit-making and the other loss-making. The criteria in selecting profit or loss-making hospitals was net profit to total revenue. The major finding of the study was as follows; 1. Whether or not a hospital had specialized in certain departments was proved to be the major factor influencing on the financial performance. Three out of five profit-making hospitals could harvest following results by operating specific departments. (1) Man powers needed for the operation of specific departments were 14.6 persons per 100 bed, which was only 1/7 of the general hospital. (2) The number of doctors has not increased in proportion to the increase of the number of beds. (3) Ratio of total revenue to MD.'s payroll expenses of the profit-making hospitals was 75.0% higher than the loss-making hospitals. (4) The average length of stay of specific department was very long(388.1 days). However, the specific departments were found to have contributed much to the financial performance because the occupancy rate of such departments was very high(94.5%). 2. The headcount per 100 bed of the profit-making hospitals was 23.9 persons(24.0%) less than the loss-making hospitals and the ratio of payroll expenses to total revenue 15.1% less. 3. Averagel revenue per specialist of the profit-making hospitals was 100 million(25.1%) more than loss-making hospitals and the ratio of total revenue to MD's payroll expenses of profit-making hospital was 75.0% higher. 4. Profit-making hospitals have introduced new systems or renovation in 36 fields, such as incentive payment system, utilization of contracted man powers, change of the payroll structure of the nurses, specialization in certain departments, etc; however, loss-making hospitals introduced only 25 new systems or renovations. These kind of renovation could not be achieved without the cooperation of the labor union and the strong will of the top management. Therefore, it could be said that the labor union of the profit-making hospitals seems to have been very cooperative compared with that of loss-making hospitals.

  • 워크플로우 관리 시스템은 정부나 기업과 같은 조직의 작업을 처리하기 위한 비즈니스 프로세스를 컴퓨터를 기반으로 자동화함으로서 작업의 효율을 높이고 비용을 절감한다. 현재에 이르러 이런 워크플로우 시스템을 사용하는 조직들이 점차 거대화되어 가고 네트워크의 발달과 인터넷의 출현으로 인하여 워크플로우 시스템이 처리하여야 하는 작업의 수와 고객과 작업자 수 등이 빠른 속도로 증가하는 추세이다. 이런 추세에서 워크플로우 시스템은 거대 조직 환경에 적합한 워크플로우 시스템 아키텍쳐를 필요하게 된다. 이에 본 논문은 거대 조직 환경을 관리할 수 있는 워크플로우 관리 시스템으로 워크케이스 기반의 초대형 워크플로우 시스템의 아키텍쳐를 설계 및 구현 하고자 한다. 그리고 워크플로우 시스템 아키텍쳐를 분류, 분석하여 장단점을 가려내어 이를 기반으로 워크플로우 시스템 아키텍쳐의 성능을 예측하여 워크케이스 기반 워크플로우 시스템 아키텍쳐가 본 논문에서 제안하는 초대형 워크플로우 시스템의 아키텍쳐라는 것을 예측하여 본다. 또한 초대형 워크플로우 시스템을 위하하부 구조로 EJB(Enterprise Java Beans)를 사용하고 사용 이유를 기술한다. 본 논문에서는 이런 워크케이스 기반의 초대형 워크플로우 시스템 아키텍쳐를 위하여 개념적인 단계와 설계 단계, 구현 단계로 나누어 설계 및 구현을 하며 개념적인 단계에서는 워크케이스 기반 워크플로우 시스템 아키텍쳐에 대하여 상세히 기술하고 설계단계에서는 전체적인 기능 정의와 초대형 워크플로우 시스템의 구조를 설계한다. 그리고 구현 단계에서는 워크케이스 기반의 초대형 워크플로우 시스템 아키텍쳐를 실제 구현하기 위한 환경을 선택하고 구현 단계의 문제점들과 해결책을 기술한다. 다 솔레노이드방식 감압건조장치로 건조한 표고버섯으로 품위에 대한 유의성 검증결과, 표고버섯의 경우 온도별로는 색택과 복원률, 건조실 내부 압력별로는 수축률, 복원률에서 유의차가 있는 것으로 나타났다. 라. 본 연구에서 구명된 감압건조특성을 기초로 하여 배치식 감압건조기를 설계 제작에 활용하고자 한다.ational banks.   • For the past few years, unlike wireless widespread United States and Japan, expensive cost and platform of the mobile web carrier underground and self-centered made achievement of domestic mobile web market sluggish. Nowadays, more and more people pay attention to the mobile web service because of the increased quality of wireless internet, development of mobile device and variety of the flat-rate payment system. However, another big problem is that mobile web is too hard to use. Also, the poor content of mobile web service is an extremely serious problem. In this paper, we put forward a new method for easy to access mobile web service which is use WINC expansion numbers and users can use these numbers to shit their own website. Along with the benefits of purchase WINC number general users are able to make website and control their website at less time and energy.

Economic Evaluation of Eastern, Western and Collaborative Treatments for Patients with Frozen Shoulder Pain (견비통의 한${\cdot}$양방 진료 및 협진의 경제성 평가)

  • Objectives : The purpose of this study was to evaluate and compare the cost-effectiveness of Eastern, Western, and collaborative treatments and suggest a cost-effective approach for patients with frozen shoulder pain. Methods : Using the data of fifty-two patients, treatment effectiveness was measured by CSA, SPABI, and ROM scales and changes from the baseline score were evaluated. Data source for cost estimation was based on the national health insurance (NHI) payment system. Because the price in NHI was differentiated by health care institutions, five collaborative types were considered in assessing costs. Cost-effective ratios were computed for economic evaluation. Results : Compared with Eastern treatment, collaborative and Western treatments showed better effects on CSA scale after 4 weeks' treatment. The collaborative approach was also the most effective treatment on SPADI and ROM scales. The direct cost per patient receiving Eastern treatment was less than other treatments. In general, collaborative treatment dominated Eastern and Western treatments in cost-effectiveness an analysis. However, the cost-effectiveness ratio of Eastern treatment resulted in \9,000 compared to \29,000 of collaborative treatment on SPADI. Four different indicators of ROM scales resulted in different approaches as the cost-effective treatment. Conclusions : Considering cost-effectiveness ratios, collaborative treatment was the best treatment on CSh and SPADI scales after 4 weeks' treatment. As for ROM scales, the recommended alternatives were Eastern treatment for patients with abduction and adduction disabilities, Western treatment for those with flexion disability, and collaborative approach fir those with extension disabiliry.

A Study on the Recent Labor-Management Dispute Cases at Medical Institutions (의료기관 노사분규 사례분석연구)

  • Recently, a long strike by hospital labor union emerged as a serious social issue. During the Worldcup Games in June, 2002, labor strikes broke up at 'C', 'K' and other hospitals, and in 2007, 'Y' hospital suffered much from a strike. Such series of extreme labor disputes have awakened people of importance of a more stable labor-management relationship for the medical institutions responsible for people's health than any other business organization. The purpose of this study was to examine the labor-management disputes at 'Y' hospital in 2007 and 'C' and 'K' hospitals in 2002. The results of this study can be summarized as follows; First, requests of the labor union such as pay raise, reemployment of the irregular workers as regular employees and participation of the labor union in personnel affairs are the long-held or core issues suffered by the medical institutions. Such issues are not independent from each other but complicated with each other surrounding the pay raise. Accordingly, it is not easy to determine the genuine bone of issue for labor-management disputes. Second, the model type of disputes between labor and management at medical institutions may be strike. However, it is conceived that the type of disputes would be subject to change as the essential medical service area system began to be operated since 2008. Third, the common characteristic of the labor strike among the 3 sample hospitals was occupation of the hospital lobby for a sit-in strike to maximize the negative effects of strike. Article 42 (Prohibition of Violence) of Labor Union and Labor Relation Coordination Act prohibits occupation of production or other important business facilities. In addition, since Ministry of Labor interprets that the hospital lobby belongs to the important business facilities enumerated by Article 42 of the above act, occupation of the hospital lobby for a sit-in strike may be too controversial to be admitted as a fair act of labor dispute when its legitimacy should be judged. Fourth, the counter-measures taken by the hospitals against the strike were observance of the principle 'no labor no pay,' closure, legal action, accusation, claim for recovery of damage, provisional seizure, disciplinary punishment, etc., but the principle of 'no labor, no pay' was not applied in a fair manner by 'C' and 'K' hospitals. However, 'Y' hospital applied this principle thoroughly to the strike; the hospital conduced to correction of the wrong labor-management relationship by refusing inclusion in the labor collective agreement of a provision about payment of wage during the period of strike or labor union's request to that effect during a strike. In addition, 'Y' hospital took an effective measure to end the strike earlier by notifying the labor union of cancellation of the collective agreement and banning the unionists from entering the hospital.

Public and Experts Perception Analysis about Negative Effects in Nanotechnology Based on Conjoint Analysis (컨조인트 분석을 이용한 나노기술의 부정적 영향에 대한 일반인과 전문가의 인식분석)

  • Nanotechnology has been growing constantly and it is becoming the leading technology in scientific research and development. Although nanotechnology has important applications in broad variety of fields without boundary of any particular industrial area, the study of nanotechnology related to its commercialization has been conducted in a few ways. To put that figure in context, this study investigates public and expert perceptions about negative potentials of nanotechnology. Through a series of surveys with public (N = 541) and experts (N = 62), we analyzed about public willingness to pay for nano-applied products. Survey results showed that public and experts preferred nano-applied products in the order of electronics, cosmetics, and food and medicine. Experts express high payment intention to electronics rather than public intention. In addition, the survey results showed the purchasing intention of both public and expert group was affected by the attributes of nano-applied products in the order of risk fatality, risk chance, certification, and labeling. But experts put more importance in risk fatality than risk chance comparing to public. Through the case analysis of the effects of labeling and certification, we revealed either labeling or certification can induce both public and experts to buy the nano-applied products with high risk chance and low risk fatality. However, for the nano-applied product with high risk fatality and low risk chance, both labeling and certification are simultaneously required to make customers have positive purchasing intention. The result of this study could be utilized for the nanotechnology-based company to get the consumer behavior information about nano-based product and to establish their marketing strategy.

  • In context of e-commerce, negotiation is a procedure to help negotiate between buyer and seller by adjusting their negotiation issues such as price and in terms of payment. We used intelligent agent and mobile device to promote new framework of e-commerce. Moreover, this framework can help buyers and sellers to carry their commercial transactions effectively. In regard to that issue, we need to carry out the research of negotiation agent that can be used in e-commerce fields. In this paper, we modeled the negotiation using CSP for the performance of agent in m-commerce environment. Furthermore we implemented interface for mobile device to extract buyer's requirement and preference easily Besides that we used utility function to make a decision for various evaluation functions and suggestions that are used for evaluation of negotiation issues. A difficulty of generating offer is dependent on the number of negotiation issues and the range of the values. Therefore, if any offer has a number of negotiation issues and the range of values are wide, the search space will be exponentially expanded. There have been many studies fur solving this problem, we applied those techniques to improve the agent's ability of negotiation. For example, a contract can be accomplished by exchanging seller and buyer's offer that is generated by agent to adjust the requisite profit for each party. Finally, we show the improvement of satisfaction as the negotiation is processed.

  • Since people have come to access more and more information about products with the proliferation of the Internet, more and more customer-to-customer e-marketplaces are being introduced with rapidly increasing transaction volumes. As the role of customer-to-customer electronic commerce has become increasingly regarded as vital to the economy, much research has delved into developing intelligent agent systems to efficiently support customer-to-customer electronic commerce. Most of this research, however, has focused only on supporting simple negotiation for the price of common goods along the time span. To expand the negotiation object to the differentiated goods, the customer must be allowed to negotiate over multi-at-tributes of the product besides the price, including the attributes related to the transaction activity itself, such as delivery time and payment method. To satisfy this requirement, we propose an agent marketplace for differentiated goods where the agent of a customer can negotiate not only the price but also the various attributes of the product and the transaction in order to achieve a better utility level for both buyer and seller. The formal protocol and the architecture issues to support the proposed e-marketplace and agents are also addressed.

Expanding User Types for Utilizing Certified e-Document Authorities (공인전자문서보관소의 이용 활성화를 위한 사용자 유형 확대방안)

  • Electronic records are generated not only in public sector but also in private sector. Records will be used across the public-private boundary. The Certified e-Document Authorities(CeDAs) may keep electronic documents in private sector for preservation and evidence, like the official Record Management Systems for Public sector. A CeDA is the Trusted Third Party (TTP) as a business to be entrusted and proof interchanging documents between parties. This CeDA system could be sustainable only if the CeDA earn the enough sales through enough uses. And yet, all the eight CeDA companies have not had enough users. How to utilize CeDAs is one of the hot issues in this area. In this paper, We analyze the threat to trustworthiness of CeDA due to payment of only one party among others, and describe the difficulty in use of CeDA for an individual user. These things make CeDAs cannot have enough users. To do address these, We expand the boundary of relevant parties for a document, present a delegate-establishing option under a joint name, show the needs of identifying and notifying minimum relevant parties, and suggest the proxy parties to help the individual users.

A Study on the Supply of First/Last Mile Transportation Methods Based on ABATA Travel Patterns Analysis for the Provision of MaaS (MaaS 제공을 위한 ABATA 통행 분석 기반의 First/Last Mile 이동 수단 공급 방안 연구)

  • Today, people in cities use differentthe types of transportation that rangepeople use in cities have diversified from existing public transportation, cars, taxis to shared bicycles and shared electric kickboards. In addition, with the development of mobile platform -based search, order, and payment services, and transportation services have also begun to change into platform-based integrated services. In particular, MaaS, which has emerged as an integrated mobile service and, is currently being studied and operated worldwide., However, MaaS but remains at the level of the integrated provision of the existing public transportation. As a result of Specifically, the results of a literature review on this issue reveal that, the First/Last Mile problem raised at the current level of MaaS is likely to be solved by establishing an improved policy incorporating new means of transportation. Therefore, this study aims to establish a First/Last Mile transportation supply plan for successful MaaS provision. This establishment is realized by analyzing the traffic patterns of urban populations usingbased on the ABATA system,, an activity-based traffic analysis model withevaluated as having higher analysis power on people's traffic.
