Purpose: The purpose of this study was to explore adult diabetics' eating styles and factors which influence them. Method: The study adopted an ethnographic method based on a perspective which views the eating style as a cultural phenomenon. Data was collected through a personal interview, participant observation, and documented materials from Oct.2001 to Sept. 2002. In this study, fifteen adult diabetics, with an average age of 57, participated. Data analysis was done by the Spradley's taxonomic analysis technique. Result: The patients' eating styles were rooted in their viewpoint on illness as well as the meaning of food. Eating styles were classified into 4 types: Pathology-centered, symptom-centered, need-centered, and role-centered. Conclusion: A conventional approach to the treatment and management of diabetes did not consider the patient's inner world which may play an important role in the successful management of the disease. We found that it was critical for health care personnel to understand patients' values, beliefs and their way of life in order to facilitate the most successful self-care diet.
Purpose: The purpose of this study was to examine the impact of the working environment on person-centered care for home visiting dementia caregivers. Methods: Participants in this study were 146 caregivers, with the least 6 months of experience working at visiting care centers in Busan, and used a total of 133 questionnaires suitable for analysis. Data were collected from June 9 to July 20, 2023, and were analyzed using descriptive statistics, t-test, ANOVA, pearson's correlation coefficients, and multiple linear regression using IBM/SPSS 27.0. Results: As a result of multiple linear regression analysis, organizational factors (β=-.37, p<.001), job-related factors (β=-.27, p=.001), and religion (β=.18, p<.013) were factors affecting person-centered care of home visiting dementia caregivers, and working environment explained 37% (F=10.63, p<.001) of person-centered care. Conclusion: Based on the results of this study, the exploration of strategies to improve the working environment will be necessary in order to increase person-centered care for home visiting dementia caregivers.

This study tried to answer the question : 'How does the human communication happen at the oriental medicine hospital between nurse and patient?' To answer that, a micro-ethnographic research method was used. Researcher visited T university hospital of oriental medicine and observed nurse-patient communication from September 1997 to December 1997. The data was obtained through participant observation, interview, audio-tape recording, home video camera, field note-taking, and related documents. After reviewing the whole data and deliberate analysis, first, I learned that most oriental medicine nurses communicate with their patients for their routine nursing job like recording, hand-over to the next duty, report to doctor, etc. I named this type of communication as 'ritual communication'. Second, I can define major argument as follow : Human communication of oriental medicine between nurse and patient is performed more frequently and variously when nurse contacts the patient for the routine nursing activities than for the incidental activities. As a result of these understandings, I suggest that oriental nursing need to develop the body of knowledge and expand its role and independent nursing activity. Also the bureaucratic hospital management centered doctors must be changed reasonalbly.
Purpose: There is a need to examine changes in the health care environment and the impact on gerontological nursing care in the era of the Fourth Industrial Revolution. In this article recent healthcare paradigm changes, gerontechnology, high tech and high touch, person-centered approaches are discussed. Methods: A narrative review was used. Results: Cyber physical system, artificial intelligence, advance and convergence of bioengineering, and information communication technology are changing the health care paradigm to "precision", "prediction" and "personalization". Entry into the global aging society and the surge in the elderly population worldwide has led to searches for a new means to prepare for projected demands of this growing population. Thus, efforts such as gerontechnology have been made to apply and utilize recent innovative science and technology in order to promote the health and life of elders. There is a great emphasis on the convergence of high tech and high touch, in which humanistic and artistic approach are critical in order to assure that technology is beneficial to human beings rather than harmful. Conclusion: Positive healthcare experiences among patients and their families are emphasized by utilizing new technology and employing high touch while providing personalized care with a person-centered approach.
Purpose: The purpose of this study was to evaluate the validity and reliability of the Korean version of Person-Centered Practice Inventory-Staff (PCPI-S) for nurses. Methods: The English PCPI-S was translated into Korean with forward and backward translation. Data were collected from 338 nurses at one general hospital in Korea. Construct validity was evaluated with confirmatory factor analysis, convergent validity, and discriminant validity. Known-group validity was also evaluated. Cronbach's α was used to assess the reliability. Results: The PCPI-S Korean version consisted of 51 items in three areas: prerequisites, the care environment, and person-centered process. The comparative fit index (CFI) and values of person-centered care process were improved after engagement and having sympathetic presence items were combined as one component. The construct validity of PCPI-S Korean version was verified using four-factor structures (.05 < RMSEA < .10, AGFI > .70, CFI > .70, and AIC). The convergent validity and discriminant validity of the entire PCPI-S question were verified using a two-factor structures (AVE > .50, construct reliability > .70). There was an acceptable known-group validity with a significant correlation between the PCPI-S level and the degree of person-centered care awareness and education. Internal consistency was reliable with Cronbach's α .95. Conclusion: The Korean version of PCPI-S is valid and reliable. It can be used as a standardized Korean version of person-centered care measurement tool. Purpose: The purpose of this study was to identify effective nurse interaction patterns with patients in the emergency department. Methods: For this study, video technology was used to record complete conversations between the nurse and patient. The participants were 28 nurses and 63 patients in the emergency department at one university hospital located in Seoul. The data were collected from November, 2002 to April, 2003. The video recordings were observed for 4 hr for each case and coded using an adapted version of Roter's Interaction Analysis System (RIAS). The data were analyzed using cluster analysis to identify the patterns of nurse-patient interaction. Results: Cluster analysis revealed 4 distinct nurse interaction patterns; 1) "closed" characterized by orientation and negative talk, 2) "positive" characterized by positive affective talk, 3) "informative and directing" characterized by task-focused behavior including data gathering, and giving information about medical condition and treatment, 4) "facilitative" characterized by balance of psychosocial and biomedical topics. Patient satisfaction was highest in the facilitative interaction pattern. Conclusion: The patient centered interaction pattern, balancing information exchange and psychosocial exchange are the most effective interactions in the emergency department, suggesting that effective interaction skill is a core clinical nursing intervention in acute care.
Purpose : This study aims to identify the relationship between person-centered critical care nursing (PCCN) and family satisfaction for intensive care unit (ICU) nurses. Methods : This research used a cross-sectional survey. The participants were 142 nurses and 121 family members from 8 ICUs in 4 hospitals in B city. The questionnaire survey was conducted between December 2018 and March 2019. The eight ICUs were divided into those that scored high and those that scored low for person-centered care relative to the average score for PCCN, and the difference in family satisfaction between the ICUs with high and low scores was compared using t-test. Results : The mean score of PCCN was $3.68{\pm}0.40$ out of 5. In the subcategories of PCCN, "comfort"was the highest at $3.95{\pm}0.49$, followed by "respect" at $3.73{\pm}0.57$, "compassion" at $3.59{\pm}0.57$, and "individuality" at $3.47{\pm}0.54$. The average score of family satisfaction with the ICUs was $3.45{\pm}0.67$ out of 5. In its subcategories, "emotion" was the highest at $3.69{\pm}0.72$, followed by "information" at $3.61{\pm}0.75$, "participation" at $3.30{\pm}0.79$, and "resources" at $3.20{\pm}0.80$. The family satisfaction ($3.75{\pm}0.54$) with the ICU of the highest-scoring PCCN was significantly higher than the family satisfaction ($3.25{\pm}0.53$) with the ICU with the lowest-scoring PCCN (t=4.97, p<.001). Conclusion: The results suggest that the PCCN scores of ICU nurses should be promoted to improve family satisfaction. It is necessary to pursue a variety of ways to secure the provision of person-centered nursing practice in ICUs.
Purpose: This concept analysis was done to clarify therapeutic intervention in nursing. Methods: For this study the process of Walker & Avant's concept analysis was used. Twenty-nine studies from electronic data bases met criteria for selection. Results: Therapeutic intervention can be defined by the following attributes: 1) goal-oriented, 2) forming a therapeutic relationship, 3) reinforcement of coping ability, 4) patient-centered, 5) a holistic integrated approach. The consequences of therapeutic intervention were improvement of quality of life, coping skills, self-care. Conclusion: Attributes of therapeutic intervention identified in this study can be applied to develop tools for measuring therapeutic nursing behaviors or to develop therapeutic nursing intervention programs for patient or family.
Purpose: This study is to identify the types of role awareness of home health care specialists working at medical institutions, to understand their role awareness correctly by grasping the quality of each type, and to provide useful help in the education of home health care specialists. Method: Q-methodology is used to objectify role awareness of medical institution specialists who may recognize situations differently according to their individual experience and comprehension based on the view of behavior. Q-classification was carried out on 30 home health care specialists working at medical institutions using 30 selected questions. Collected data were examined through factor analysis using QUANL PC program. Results: Three different types of role awareness of home health care specialists working at medical institutions were identified. Type I is 'educational-function-centered', Type II 'patient-centered' and Type III 'professional-service-centered'. Conclusion: Regardless of these types, home health care specialists commonly had a high pride as a specialist and a sense of mission regarding themselves as important persons responsible for patients' health.
A considerable change of the Korean nursing system has been made during the last decade not only in its philosophy but also in its function and structure to meet political and scientific need of the modern society. The main purpose of this study is to develope the new concept of comprehensive nursing care, both its Philosophy and ethics, as the basic of modern clinical nursing theory and practice. Comprehensive nursing care is the concept of human centered nursing care, and it helps a man to meet the basic physical, psychological, environmental, socioeconomic and teaching needs. It also helps him to help himself to meet these needs. This concept starts from the individualized nursing care and its ultimate goal is to improve a man to have a better position in his own community so that he may able to have a meaningful life. To accomplish this goal, an individualized nursing care plan as a nursing diagnosis and problem solving method should be set up for different patients with similar diagnosis to meet their needs, because each patient has a different social background. from this viewpoint, nursing is a science as well as abstruse humanity. The performance of comprehensive nursing care is a goal and issue of modern clinical nursing care. If nursing is a science and a profession for man, it should have ethics which recognize the dignity of man and offers infinite service voluntarily, and should be able to show leadership in carrying out the nursing responsibility. This leadership finds a person's potential and encourages him to utilize it. Such concepts should develop into a nursing ideology and this ideology should become a priority in comprehensive nursing care. The following statements are the conclusion of this study. 1) Modern nursing has been developed from disease centered nursing care to comprehensive nursing care based on humanity. The primary principle of nursing was to assist in the treatment of disease, but it has been changed to the professional nursing system independently. 2) The concept of nursing is one of continuous or endless scope of dispersion. It proves that nursing is grasping the professional responsibility to be able to coordinate scientific principles Patient health problems are according to scientific principles rather than adhering to nursing technical discipline as a daily work. 3) In chapter I and Ⅱ, the philosophy and ideology of nursing have been discussed and the flow of concept of clinical nursing and the rate of progress which emerges from naturalizing performance of the concept of comprehensive nursing in clinical nursing studied. The discussion developed the theory that a nurse should be to embody nursing ideas and objectives by establishing definite conviction of professions and study. 4) In chapter lil, nursing planning based on nursing diagnosis as a method to attain ideal nursing care for humanity with a definite idea of establishing philosophy of nursing was presented. 5) From the result of survey on patient needs about treatment and nursing, it was observed that all patient had emotional stress from unknown factors. Therefore it was concluded that nurses should not only educate the patient but also give them the opportunity to communicate freely their needs and anxieties. Furthermore complaints and doubts of the patient should be carefully noted and must be considered to meet these needs. 6) Patient teaching is the most important part of comprehensive nursing care. In chapter, Ⅲ, the important of patient teaching was emphasized by demonstrating the effect of patient teaching for diabetic patient. 7) In Chapter Ⅳ, from the result of the study on nurses attitudes to comprehensive nursing care, it was pointed that the evolution of nursing education and the establishment of a complete concept and value of comprehensive nursing was necessary.
