A Comparative Study of Floor Area and Average Distance of Nurse Working of Planned Courtyard in Ward (중정이 계획된 병동부 바닥면적과 간호동선 평균거리 비교 연구)

  • Purpose: The courtyard in the ward can light and ventilate in the central space and corridor of the ward. It is possible to improve the disadvantages of the existing double-corridor ward. Also, there is the advantage in that patients are able to contact with nature indoors and cause positive activities. So, the purpose of this study is quantitatively analyze the floor area and the average distance of nurse working of the courtyard in ward. Methods: The subjects are converted through the same criteria setting. And it investigate and analyze the influence of the courtyard in the ward through the analysis of the spatial composition. Result: The factors that affected the floor area increase are the planning courtyard, the corridor type and the dispersion of the medical rooms. The average distance of nurse working is more influenced by the nursing management system of the hospital than by the courtyard. So, It can improve according to the position and number of nurse station. Implications: It can be used as a primary data for courtyard plan in the ward for positive patient environment plan considering the floor area, average distance of nurse working.

  • Lymphorrhea is a rare but potentially severe complication that occurs after various surgical procedures. Untreated lymphorrhea may lead to wound dehiscence, infection, and prolonged hospital stay. Currently, there is no standard effective treatment. Early management usually includes leg elevation, drainage, and pressure dressing. However, these methods are associated with prolonged recovery and high recurrence rates. We report a case of lymphorrhea from a calf wound after endoscopic great saphenous vein (GSV) harvesting for coronary artery bypass grafting (CABG). The patient presented with intractable oozing from the postoperative wound on the right calf. Lymphorrhea persisted for 6 weeks despite negative-pressure wound therapy with a long-acting somatostatin. We performed unilateral pedal lymphangiography that confirmed wound lymphorrhea, followed by glue embolization. No recurrence was observed after 8 months of follow-up. This case report demonstrates the successful use of lymphangiography with glue embolization in the control of lymphorrhea after GSV harvesting for CABG.

  • 환자의 생체신호 측정 및 관찰, 영상 위생 등을 포함하는 직접간호는 간호사들의 총 간호활동 시간 중 내과는 48%, 외과는 40% 로 간호사들의 업무 부담이 되고 있다. 또한 의료기관에서 사용되는 의료기기들은 여러 회사에서 구매하여 사용되기 때문에 각 회사마다 상이한 프로토콜을 가지고 있어 하나의 시스템으로 생체신호를 모으기가 쉽지 않다. 따라서 여러 장비에서 생체신호를 실시간으로 취득하여 통합 관리할 수 있는 시스템 개발을 통해 간호사의 직접간호 업무량을 줄여 간호사의 근무환경 개선뿐만 아니라 중증환자의 경우 환자 생체신호에 대한 실시간 원격감시가 가능하고 환자에게서 발생된 모든 생체신호가 데이터베이스 시스템으로 기록관리 됨으로 인해 환자의 생체 신호에 대한 이력 추적관리가 가능함으로써, 양질의 의료 서비스가 가능한 환자케어시스템을 개발하고자 한다.

Exome and genome sequencing for diagnosing patients with suspected rare genetic disease

  • Rare diseases, even though defined as fewer than 20,000 in South Korea, with over 8,000 rare Mendelian disorders having been identified, they collectively impact 6-8% of the global population. Many of the rare diseases pose significant challenges to patients, patients' families, and the healthcare system. The diagnostic journey for rare disease patients is often lengthy and arduous, hampered by the genetic diversity and phenotypic complexity of these conditions. With the advent of next-generation sequencing technology and clinical implementation of exome sequencing (ES) and genome sequencing (GS), the diagnostic rate for rare diseases is 25-50% depending on the disease category. It is also allowing more rapid new gene-disease association discovery and equipping us to practice precision medicine by offering tailored medical management plans, early intervention, family planning options. However, a substantial number of patients remain undiagnosed, and it could be due to several factors. Some may not have genetic disorders. Some may have disease-causing variants that are not detectable or interpretable by ES and GS. It's also possible that some patient might have a disease-causing variant in a gene that hasn't yet been linked to a disease. For patients who remain undiagnosed, reanalysis of existing data has shown promises in providing new molecular diagnoses achieved by new gene-disease associations, new variant discovery, and variant reclassification, leading to a 5-10% increase in the diagnostic rate. More advanced approach such as long-read sequencing, transcriptome sequencing and integration of multi-omics data may provide potential values in uncovering elusive genetic causes.

A Study on Diabetes Management System Based on Logistic Regression and Random Forest

  • In the quest for advancing diabetes diagnosis, this study introduces a novel two-step machine learning approach that synergizes the probabilistic predictions of Logistic Regression with the classification prowess of Random Forest. Diabetes, a pervasive chronic disease impacting millions globally, necessitates precise and early detection to mitigate long-term complications. Traditional diagnostic methods, while effective, often entail invasive testing and may not fully leverage the patterns hidden in patient data. Addressing this gap, our research harnesses the predictive capability of Logistic Regression to estimate the likelihood of diabetes presence, followed by employing Random Forest to classify individuals into diabetic, pre-diabetic or nondiabetic categories based on the computed probabilities. This methodology not only capitalizes on the strengths of both algorithms-Logistic Regression's proficiency in estimating nuanced probabilities and Random Forest's robustness in classification-but also introduces a refined mechanism to enhance diagnostic accuracy. Through the application of this model to a comprehensive diabetes dataset, we demonstrate a marked improvement in diagnostic precision, as evidenced by superior performance metrics when compared to other machine learning approaches. Our findings underscore the potential of integrating diverse machine learning models to improve clinical decision-making processes, offering a promising avenue for the early and accurate diagnosis of diabetes and potentially other complex diseases.

A Case Study on the Hospital Information Systems Usability (병원정보시스템 사용성에 대한 실증연구)

  • This study is a development of quality scale and a measurement of quality levels for the Hospital Information System(HIS) used in the hospital industry. This study is to provide decision-making guidelines for hospital managers, system analysts, and software designers, and to provide feedback for the users' information needs. The target people of the study were medical doctors, nurses, pharmacists, and clerical staff. The service contexts of usability were diagnosis/treatment, nursing, medicine preparation, and treatment fee receipt/claim. The quality factors were the efficiency, effectiveness, and satisfaction of IS09241-11. This study shows that the current domestic HISs are mostly based on patient diagnosis/treatment and treatment fee accounting, and that other quality scales are necessary according to different job categories, specializations, positions, and service domains. Especially, the low quality of HIS was found in the abnormal service processing, information sharing by outside institutions, and standardization. It may be added that the HIS should be improved and developed in these respects.

Reconstruction of Ankle and Heel Defects with Peroneal Artery Perforator-Based Pedicled Flaps

  • Background The reconstruction of ankle and heel defects remains a significant problem for plastic surgeons. The following options exist for reconstructing such defects: local random flaps, reverse flow island flaps, and free flaps. However, each of these methods has certain drawbacks. Peroneal artery perforators have many advantages; in particular, they are predictable and reliable for ankle and heel reconstructions. In this study, we report our clinical experience with peroneal artery perforator-based pedicled flaps in ankle and heel reconstructions. Methods From July 2005 to October 2012, 12 patients underwent the reconstruction of soft tissue defects in the ankle and heel using a peroneal artery perforator-based pedicled flap. These 12 cases were classified according to the anatomical area involved. The cause of the wound, comorbidities, flap size, operative results, and complications were analyzed through retrospective chart review. Results The mean age of the patients was 52.4 years. The size of the flaps ranged from $5{\times}4$ to $20{\times}8cm^2$. The defects were classified into two groups based on whether they occurred in the Achilles tendon (n=9) or heel pad (n=3). In all 12 patients, complete flap survival was achieved without significant complications; however, two patients experienced minor wound dehiscence. Nevertheless, these wounds healed in response to subsequent debridement and conservative management. No patient had any functional deficits of the lower extremities. Conclusions Peroneal artery perforator-based pedicled flaps were found to be a useful option for the reconstruction of soft tissue defects of the ankle and heel.

A Design of Clinical Information Exchange Framework for Performance Improvement based on Lazy Response Model (지연 응답 모델에 기반한 성능 개선 진료정보 교류 프레임워크의 설계)

  • Recently medical service environment, the clinical information exchange which contribute to medical safety, promotion of service quality and patient's convenience, efficiency of medical procedures and medical management is essential medical service model. But, practical exchange of clinical information which variation of information level, absence of standardization system, build of heterogeneous information systems is difficult in each medical institute. In this paper, We analyzed the related technical standardizations and the models of clinical information exchange. So, we designed the clinical information exchange system based on the ideal lazy response model which is aimed at vitalizations the exchange of clinical information under domestic law environment. In case of exchange the clinical information, we separate CDA document flow from metadata flow. As a experimental result we acquired 24% improved performance compared with existed system based on the lazy response model.

  • In recent years the combination of IT and BT for U-Healthcare Medical Services has attracted popular attention as an alternative for solving health problems due to aging issue. Active oxygen as free radicals causes unstable state of the body resulting to other disease on which approximate 90% of the diseases are associated with active oxygen. The severity is even higher along with increasing age. Therefore, there is a need for systematic management of active oxygen saturation for the elderly and chronic patients. In this paper, the data about active oxygen measurements from the body measurement is sent to the data storage of medical facilities by the service provider. Medical information is measured by devices provided complying with the standard, support and information using Bluetooth communication. Users can check their health status themselves using the proposed system through 3G/4G mobile devices like smart phone to medical institutions in which the smart phone act as a gateway for medical data transferred from patient to medical institutions and vice versa. Users can get and experience a diagnosis and effective U-Environmental Health Services from Medical institutions anywhere using the proposed built system.

A Survey of the Controlling System for the Pulmonary Tuberculosis in Jejudo, 2002 (2002년 제주도 폐결핵 등록환자의 관리실태 조사)

  • Background : The nationwide tuberculosis registry system in Korea has been operated with the dual reporting system - public health center (PHC) and private medical services (PMS) - since 2000. While three-fifths of the registered cases in 2002 was reported by PMS, it was reported that the success rate of patients at PMS was lower than that at PHC. To identify the problems of the patients' management at the PMS as well as the PHC in a community is the aim of this paper. Methods : The subject was selected from the registered cases of the pulmonary tuberculosis in Jejudo, 2002. The needed items for this study were gathered by the reviewing the medical charts directly. Results : Of the 262 patients included in the subject, 92 cases (35%) were treated initially at PMS. The sputum smear as an essentially diagnostic test was carried out with 69.2% at private clinics compared with 98.2% at PHC. The success rate of the treatment at PMS was 59.8% so that it had a statistically significant difference from that at PHC (80.0%; p<0.01). Conclusion : These results showed that there were several problems in diagnosis and treatment for the patient of pulmonary tuberculosis at PMS. To improve the success rate of the treatment at PMS, the nationwide clinical guideline for control of pulmonary tuberculosis should be systematically disseminated and strongly recommended at PMS by the government.
