골반 경사가 동반된 만성 통증 환자에게 족부 보조기를 적용한 증례 4례 (The Application of Foot Orthotic to Chronic Pain Patient with Pelvic Obliquity : 4 Cases Report)

  • Objectives : To evaluate the effect of foot orthotic in patient with chronic pain and pelvic obliquity in standing. Methods : Four cases of functional spinal scoliosis, were investigated for the changes in the calcaneal stance position angle, pelvic height, pelvic angle, Cobb's angle and walking pattern. Standing full spine X-ray for measuring the pelvic height, pelvic angle, Cobb's angle were checked before and after application of foot orthosis. The foot orthosis was composed of polyprophylen and chamude cover. Results : 1. There was no change in resting calcaneal stance position. 2. Difference of pelvic height and pelvic angle was reduced after application of a foot orthosis. 3. Cobb's angle in 2 cases was reduced after application of a foot orthosis. 4. Walking balance was improved. 5. Visual analogue scale was decreased. Conclusions : The study showed that foot orthosis seemed to be effective for chronic pain, spinal curve, pelvic obliquity and walking balance.

  • The first patient of this case was 13 year and 8 month old girl who had the Angle's class I malocclusion with uneruption of upper left central incisor and left canine. The second patient was 12 year old boy who had the Angle's class I malocclusion with uneruption of right central incisor. After surgical exposure of impacted teeth of those patients and bonding of attachment on exposed crown, those teeth were orthodontically pulled with removable appliances. The results were obtained as follow. 1. The functionally proper occlusion was established by guidance of the impacted teeth into the normal position in the arch. 2. Establishment of esthetic and balanced arch form was achieved. 3. The possibility of restorative prosthetic treatment induced by extraction of impacted teeth was eliminated. 4. The psychologic relief of pubertal patient who was very sensitive to facial esthetics was gained.

  • Digital extensor hypoplasia (DEH) is a rare malformation that presents with loss of active finger extension at the metacarpophalangeal (MCP) joints. Descriptions of optimal treatment and outcomes in this population are sparse. We describe successful operative treatment of a child with DEH involving the extensor digitorum communis, extensor digiti minimi, and the extensor indicis proprius tendons. The 5-year-old male patient was referred for severe limitation on bilateral finger extension since birth. He had been previously diagnosed with arthrogryposis and managed conservatively. Due to lack of improvement, magnetic resonance imaging was done evidencing hypoplasia/aplasia of the extensor tendons. The patient underwent successful tendon transfers using extensor carpi radialis longus to the common extensor tendons, and one hand required an additional tenolysis procedure. 2 years postoperatively, his MCP position and finger extension are markedly improved, and he is able to grip objects without limitation or difficulty. The patient returned to full activity without restriction.

Recording natural head position using an accelerometer and reconstruction from computed tomographic images

  • Objectives: The concept of natural head position (NHP) was first introduced by Broca in 1862, and was described as a person's stable physiologic position "when a man is standing and his visual axis is horizontal." NHP has been used routinely for clinical examination; however, a patient's head position is random during cone-beam computed tomography (CBCT) acquisition. To solve this problem, we developed an accelerometer to record patients' NHP and reproduce them for CBCT images. In this study, we also tested the accuracy and reproducibility of our accelerometer. Materials and Methods: A total of 15 subjects participated in this study. We invented an accelerometer that measured acceleration on three axes and that could record roll and pitch calculations. Recorded roll and pitch data for each NHP were applied to a reoriented virtual image using three-dimensional (3D) imaging software. The data between the 3D models and the clinical photos were statistically analyzed side by side. Paired t-tests were used to statistically analyze the measurements. Results: The average difference in the angles between the clinical photograph and the 3D model was $0.04^{\circ}$ for roll and $0.29^{\circ}$ for pitch. The paired t-tests for the roll data (P=0.781) and the pitch data (P=0.169) showed no significant difference between the clinical photographs and the 3D model (P>0.05). Conclusion: By overcoming the limitations of previous NHP-recording techniques, our new method can accurately record patient NHP in a time-efficient manner. Our method can also accurately transfer the NHP to a 3D virtual model.

불안정한 면에서의 견갑골 안정화 운동이 외측상과염에 미치는 영향-사례연구 (The Effects of Scapular Stability Exercise with the Unstable Surface on Pain Relief of Lateral Epicondylitis : Case Study)

  • Purpose: The purpose of this study is to figure out that the scapular stability exercise on unstable position can effect on the pain relief of the patient with lateral epicondylitis. Methods: The subject was 35 year old male, diagnosed as lateral epicondylitis. This patient was controled to do scapular stability exercise with crawling position on unstable surface, changing elbow movement from flexion to extension for 4 weeks, 5 times a week, 20 times in total. We used VAS to find the degree of pain and Cozen's test, Mill's test and Resisted middle finger test were determined for the measurement of epicondylitis changed. Results: The following is the result of this study. 1. Pain on lateral epicondylitis was relived from VAS 7 to VAS 0. 2. There were improvements that the results of tests for epicondylitis, Cozen's test, Mill's test and Resisted middle finger test, changed positive into negative signs. Conclusion: The result of this study indicates that the application scapular stability exercise on the unstable surface to the patient with lateral epicondylitis can relief the pain degree on the lateral epicondylitis and be used as one of lateral epicondylitis treatments.

컴퓨터 응용 인공고관절 모사 시술 방법 연구 (A Study on Computer Aided Surgical Simulation Method for Total Hip Arthroplasty)

  • 현재 인공고관절 전치환 수술 과정의 높은 난이도 때문에 숙련된 의사의 경험에 상당히 의존하고 있는 실정이다. 비구컵과 대퇴 임플란트의 위치 선정은 수술의 성공 여부와 밀접한 관계가 있지만 아직까지는 의사의 눈대중에 의해 이루어지는 경우가 많다. 이러한 이유로 인하여 인공삽입물(비구컵, 대퇴시스템) 삽입의 정확성이 낮아서 재치환술을 해야 하는 경우가 자주 발생한다. 본 연구에서는 환자 골반골(pelvis)과 대퇴골(femur)의 3차원적인 고유 기하학적 정보를 이용하는 모사 시술을 통하여 비구컵과 대퇴 임플란트의 정확한 위치를 선정하는 방법을 결정하였다. 이를 위하여 우선 비구컵의 위치 변화에 따른 대퇴 전염각 및 대퇴 경간각의 변화 양상을 살펴보았고 또한 대퇴 임플란트의 삽입 위치에 따른 대퇴스템 경부 길이 및 대퇴스템각의 변화 양상을 조사하였다. 본 연구에서 다양한 모사 시술을 통한 기하학적 정보의 분석 결과 비구컵과 대퇴 임플란트의 정확한 위치 선정이 가능함을 확인하였다. 본 연구의 모사 시술 방법을 이용하여 의사가 인공고관절 수술을 선행한다면 수술의 정확도와 숙련도를 향상시킬 수 있을 것으로 기대된다.

자동노출제어장치를 적용한 흉부 측면 방사선검사 시 환자 위치 변화가 주변 장기의 선량에 미치는 영향 (Effects of Change in Patient Position on Radiation Dose to Surrounding Organs During Chest Lateral Radiography with Auto Exposure Control Mode)

  • 본 연구는 자동노출제어장치를 이용한 흉부 측방향 검사 시에 환자의 중심위치 변화가 주변 장기의 피폭선량과 화질에 미치는 영향을 탐구하는데 목적이 있다. 실험은 인체모형팬텀을 대상으로 하였다. 바늘침을 팬텀의 관상선 중심 하단부에 부착하였고, 납 자를 검출기 하단부에 부착하여, 50 cm 지점이 AEC 이온챔버의 중앙 하단부에 위치하도록 하였다. 조사조건은 125 kVp, 320 mA, 초점-영상검출기간 거리는 180 cm, 조사야 크기는 14×17 inch를 사용하였다. AEC 이온 챔버는 중앙-하단 1개만 사용했고, Sensitivity 'Middle', Density '0' 으로 설정하여, 중심 X선은 6번째 흉추를 향해 수직입사 하였다. AEC mode를 적용한 상태에서 바늘침과 납 자의 50 cm 지점이 일치되게 위치시킨 후 팬텀을 배 쪽으로 5 cm (F5), 등 쪽으로 5 cm (B5) 씩 이동시킨 후 ESD를 측정하여 선량 인자를 분석하였다. 환자 중심위치 변화에 따른 갑상선의 ESD는 Center의 경우 232.60±2.20 μGy, F5는 231.22±1.53 μGy, B5는 184.37±1.19 μGy로 나타났으며, 유방의 ESD는 Center의 경우 288.54±3.03 μGy, F5는 260.97±1.93 μGy, B5는 229.80±1.62 μGy, 폐 중심부의 ESD는 Center의 경우 337.02±3.25 μGy, F5는 336.09±2.29 μGy, B5는 261.76±1.68 μGy 로 나타났다. 선량 인자의 각 그룹 간 평균값의 차이를 비교한 결과 통계적으로 유의한 차이가 나타났으며(p<0.01), 각각 독립적인 그룹으로 나타났다. 연구의 결과, 환자 중심위치 변화에 따른 갑상선, 유방, 폐 중심부의 선량의 차이는 환자가 전방 5 cm 정도의 움직임에서는 유방(10%)를 제외한 장기에서는 큰 차이가 없었으나, 후방 5 cm 정도의 움직임에서는 각 부위에서 평균 23.7%의 선량 감소를 나타냈다. 또한, 환자 중심위치가 후방으로 이동 시 화질의 저하가 나타났다.

엇갈린 교합 환자의 임플란트 지지 고정성 보철물과 Kennedy class IV 가철성 국소의치를 이용한 수복 증례 (Rehabilitation of a patient with crossed occlusion using mandibular implant-supported fixed and maxillary Kennedy class IV removable dental prostheses: A case report)

  • The term, 'crossed occlusion' implies clinical situation in which the residual teeth in one arch have no contact with those in the antagonistic arch, resulting in the collapse of occlusal vertical dimension. The treatment goal of this pathologic condition is restoration of the collapsed vertical dimension and stabilization of abnormal mandibular position. Previously, konus removable prostheses or tooth supported overdentures were suggested to solve crossed occlusion. Nowadays, dental implants have been used for definitive support to solve this problem. In this case report, a 65 years old female patient had a crossed occlusion, in which the maxillary posterior residual teeth and mandibular anterior residual teeth cross. Interim removable and fixed dental prostheses were used to confirm the proper vertical and horizontal jaw relation. After that, the mandibular posterior edentulous region was restored with implant-supported fixed dental prostheses. Computer tomography guided implant surgery was performed according to the concept of the restoration-driven implant placement. The maxillary anterior edentulous region was restored with Kennedy class IV removable prosthesis, considering the patient's economic status. The patient's jaw position and prostheses have been well maintained at the follow-up after 6 months of definitive restoration. The antero-posterior crossed occlusion problems appeared to be effectively solved with the combination of removable in one arch and implant-supported fixed prostheses in the other.

복횡근의 강화운동과 고유수용성신경근촉진법을 이용한 호흡저항운동이 뇌졸중 환자의 폐기능과 정적인 앉은자세에 미치는 영향 : 증례보고 (Resistive breathing exercise using transverse abdominis strengthening and P.N.F affects pulmonary functions and static sitting posture of adult stroke patient : a case report)

  • Pupose : This study aimed to examine the Influence of stregthening for Transverse abdominis and resistive breathing exercise to use PNF on fulmonary functions and static sitting posture in adult stroke patient. Methods : Left hemi-plegic patient was masured pulmonary function test and analyzed sitting posture of alignment at pre intervention and post-intervention in 4weeks. Results : The pulmonary functions was increased and changed good alignment of trunk on sitting position after treatments. Conclusion : We were found that stregthening for Transverse abdominis and resistive breathing exercise to use PNF could improve pulmonary functions and sitting posture for stroke patient.
