Comparison of Manual Chest Compression and Chest Compression Using AutoPulseTM Device in Pre-Hospital Simulation Cardiac arrest

  • The purpose of this study was to find more effective method through comparison of manual chest compression and chest compression using $AutoPulse^{TM}$ device in pre-hospital simulation cardiac arrest. In order to achieve the purpose of the study, ambulance workers did two different style CPR in pre-hospital simulation cardiac arrest. Data analyzed by T test and ANOVA. Findings of this study are as follows. Firstly, manual chest compression is more effective than chest compression using $AutoPulse^{TM}$ device on scene. Secondly, chest compression using $AutoPulse^{TM}$ device is more effective manual chest compression in ambulance and in elevator. In conclusion, these findings provide strong evidence for the importance of hands off time and stable CPR before hospital arrival in explaining patient's prognosis. Therefore, strategies to conduct precise hands off time and stable CPR are needed to improve patient's prognosis.

  • About twenty years ago the computer was introduced into orthodontic, but not in clinics because it was inconvenient in cost, size, capacity and operation. But recent advances in the technology of electronics have produced a personal computer which is suitable for use in clinics. We have developed useful program for cephalometric Ax, model analysis and patient management. The computer program greatly aided in saving time & effort and performed well in the management & searching for patient data. But further studies about three dimensional analysis, S.T.O. and derivation of automatic treatment planning from analysis are needed.

Predictors of Behavioral and Psychological Symptoms of Dementia: Based on the Model of Multi-Dimensional Behavior (다차원적 행동 모델에 근거한 치매 노인의 정신행동 증상 예측요인)

  • Purpose: The purpose of this study was to identify factors predicting behavioral and psychological symptoms of dementia (BPSD) in persons with dementia. Factors including the patient, caregiver, and environment based on the multi-dimensional behavioral model were tested. Methods: The subjects of the study were 139 pairs of persons with dementia and their caregivers selected from four geriatric long-term care facilities located in S city, G province, Korea. Data analysis included descriptive statistics, inverse normal transformations, Pearson correlation coefficients, Spearman's correlation coefficients and hierarchical multiple regression with the SPSS Statistics 22.0 for Windows program. Results: Mean score for BPSD was 40.16. Depression (${\beta}=.42$, p<.001), exposure to noise in the evening noise (${\beta}=-.20$, p=.014), and gender (${\beta}=.17$, p=.042) were factors predicting BPSD in long-term care facilities, which explained 25.2% of the variance in the model. Conclusion: To decrease BPSD in persons with dementia, integrated nursing interventions should consider factors of the patient, caregiver, and environment.

Capacitive sensor for the detection of residual quantity of intravenous drip solution in a plastic intravenous bag

  • Intravenous(IV) drip therapy is extensively used for all kinds of treatments. It works by delivering medicine directly into the vein. When the medicine has been fully dispensed, a dangerous situation occurs since air in the IV drip bag could enter the patient's vein, which is hazardous to the patient’s safety. In this paper, using capacitive sensors to detect the residual quantity of a plastic IV drip pack is proposed. A simulation model of this technology was shown by a finite elements analysis(FEA) program to find out its feasibility and analyze the proper geometrical dimension of a capacitive sensor. According to the FEA simulation, 3 kinds of capacitive sensors were attached to the bottom surface of the plastic IV drip bag to detect the residual quantity in the experiment. Experimental data showed an agreement with the FEA simulation model estimation and confirmed that the sensor works.

Secondary Adjuvant Operation after Free Flap for Functional and Aesthetic Purposes (유리 피판술 후 기능적, 미용적 목적을 위한 2차 보완수술)

  • Purpose: The free flap has been widely used as one stage reconstructive procedure the skin and soft tissue defect. The secondary adjuvant operations are often needed for better results as functional and aesthetic compartment. Therefore, we focus on the secondary adjuvant surgeries for better outcome after free flaps. Methods: One hundred ninety six consecutive patients underwent free flaps between January, 2002 and February 2009. The cases constituted ninety two male patients and one hundred four female patients. For the patients who got free flap operation, we investigated secondary adjuvant operations what type of procedures was applied according to the reconstructed sites. All clinical data were based on the patient's medical records. Results: Of one hundred ninety six patients in whom free flap was performed, a total of eighty two patients(41.8%) received the secondary adjuvant operation. Because of many patients got multiple secondary adjuvant operations, the number of the secondary adjuvant operation become one hundred fifty five cases. The most common used procedure is a liposuction, and the second one is a lipoinjection. Conclusion: As the secondary adjuvant operation like a liposuction, lipoinjection, etc., satisfaction for operative result was increased in the aspects of function and aesthetics. So the secondary adjuvant operation in free flap surgeries contributed in raising the patient's quality of life.

Factors Affecting the Burden of Medical Costs for Inpatients (입원환자 의료비 부담에 영향을 미치는 요인)

  • This study analyzes Korea health panel data (2008) (beta version 1.2) of Korea Institute for Health and Social Affairs, and National Health Insurance Corporation to figure out determinants of healthcare expenditure. In result of Multiple Logistic Analysis, in-patents felt burden on the medical expenditure were 70.0%. As to the patients' payment of medical expenditure, patients over 65 years old had 4.765 times higher than those under 14 years, disabled patients 2.778 than non-disabled patients, chronic patients 1.632 times than non-chronic patients, patients belonging to 12 million won ~ 46 million won and under 12 million won in family income had 1.680 times and 2.168 times respectively than patients with over 46 million won, patients in professional recuperation facility 1.546 times than patients in hospital, patients in private medical institutions 1.700 times than patients in national and public medical institutions, patients using upper grade rooms 1.701 times than patients in non-upper grade rooms. As a health care safety net mechanism to protect people from medical expenditure burden, there is the patients' payment ceiling in the National Health Insurance System. Thus, in order to facilitate the patient's payment ceiling, it is required that the level of ceiling is to be specified according to the income level, and self-payment items is to be included.

Comparison and Verification of Deep Learning Models for Automatic Recognition of Pills (알약 자동 인식을 위한 딥러닝 모델간 비교 및 검증)

  • When a prescription change occurs in the hospital depending on a patient's improvement status, pharmacists directly classify manually returned pills which are not taken by a patient. There are hundreds of kinds of pills to classify. Because it is manual, mistakes can occur and which can lead to medical accidents. In this study, we have compared YOLO, Faster R-CNN and RetinaNet to classify and detect pills. The data consisted of 10 classes and used 100 images per class. To evaluate the performance of each model, we used cross-validation. As a result, the YOLO Model had sensitivity of 91.05%, FPs/image of 0.0507. The Faster R-CNN's sensitivity was 99.6% and FPs/image was 0.0089. The RetinaNet showed sensitivity of 98.31% and FPs/image of 0.0119. Faster RCNN showed the best performance among these three models tested. Thus, the most appropriate model for classifying pills among the three models is the Faster R-CNN with the most accurate detection and classification results and a low FP/image.

  • Purpose: This qualitative study aimed to explore the experiences of patients with diabetes provided with medication monitoring using an artificial intelligence-based smartwatch. Methods: Giorgi's descriptive phenomenological methodology was applied to collect and analyze data from November 9 to December 23, 2021. The study samples were recruited by convenience sampling, and even patients with diabetes participated in in-depth interviews via video conference and telephone calls or face-to-face visits. Results: Ten sub-themes and four themes were finally revealed. The four themes were as follows: journey with unfamiliar devices, a less-than-acceptable smartwatch, insufficient functions and content for patients with diabetes to use, and efforts for regular medication behaviors and daily monitoring of patient's health conditions. Conclusion: To effectively manage diabetic conditions using digital healthcare technologies, nursing interventions were needed to identify personal needs and consider technological, psychological, aesthetic, and socioeconomic aspects of wearable devices.

Essential Functions Required by Patients and Physical Therapists in the Rehabilitation Process of Stroke Patients: A Survey Study (뇌졸중 환자의 재활 과정에서 환자와 물리치료사가 요구하는 기능에 대한 조사연구)

  • Purpose: In this study, 100 stroke patients and 205 physical therapists were surveyed to determine the essential functions needed in the rehabilitation process of stroke patients. Methods: This study involved 100 stroke patients and 205 physical therapists. Sixteen functions suggested in the previous study as necessary in the rehabilitation process of stroke patients were selected, and a revised questionnaire was prepared and distributed to several institutions. A frequency analysis of the collected data was conducted to aggregate the functions required in rehabilitation, and a scoring process was used to determine their ranking among the 16 functions. Results: The functions required in the rehabilitation process, as selected by stroke patients, were ranked as follows: walking, toileting, eating, using products and technology for communication, and washing oneself. The functions required in the rehabilitation process, as selected by physical therapists, were ranked as follows: muscle power functions, maintaining body position, muscle tone functions, attention functions, and walking. Conclusion: The results of the study confirm the importance of an agreed goal between the stroke patient and the therapist regarding the functions required for the rehabilitation. This understanding plays a significant role in achieving the patient's expectations and the therapist's predicted performance, thereby providing reassurance and confidence in the impact of the research.

Analysis of Arrival Information and Status of the Patients in Emergency Department (응급의료센터 환자의 내원 정보 및 실태 분석)

  • Background: For effective and systematic management of patients in the emergency department(ED), the data on patient arrival and status in ED of Yeungnam University Hospital were evaluated. Materials and Methods: During the seven days from Apr. 1 to Apr. 7, 1998, the general patient information such as onset time and place, factors associated with transportation, causes of admission, cared department and patient disposition were recorded. Results: Total of 464 patients visited the ED during the seven days, and the mean number of patients per day was 66.3. Male to female ratio was 1:0.71. Daily staying patients were 17.3, and 83.6 patients were cared totally each day. The methods of transportation and distribution of patients according to region and event were as follows: visit by walk(57.3%). transportation by car(58.0%), place of event in residence(85.3%), regional distribution in Taegu(81.5%), and direct visit(97.4%). Cause of admission due to diseases was 74.6%. The percentages of departments which cared the patients were internal medicine 26.6%, pediatrics 16.8%, orthopedics 8.6%, neurology 8.2%, neurosurgery 7.8% and other department including emergency medicine 8.2%, respectively. Patient dispositions were admission 38.4%, discharge 61.0% and death on arrival(DOA) 0.6%, but referred patient-to-another-hospital was zero. Conclusion: Improvements in several aspects of ED's caring system such as "fast tracking" system and reinforcement of disease and trauma caring system, would be helpful for effective management of emergency patients.
