Development of Wholistic Hospice Nursing Intervention Program for In-patient of Hospice Palliative Care Unit (병동형 호스피스 대상자를 위한 전인적 호스피스 간호중재 프로그램의 개발)

  • People in the end of life and their families suffer in their physical disease and other aspects as a whole person. They need hospice care to palliate their total suffering in physical, emotional, social and also spiritual aspect through professional hospice team. To care their whole personal needs, hospice team must be a multi-discipline team which consists of medical doctors, nurses, social workers, pastors and volunteers. Recently those who die in hospice palliative care unit have trend to increase more than in home year by year. So it is necessary to develop the nursing intervention program to be performed by multi-discipline team approach for in-patient of hospice palliative care unit. The purposes of this study were to develop of wholistic hospice nursing intervention program for inpatient of hospice palliative care unit. The subjects of study were collected from 30 patients those who were over 18 years old and admitted in hospice palliative care unit of S hospital in P city with agreement in hospice palliative care in their terminal disease. The period of data collection was from December 15, 2003 to March 15, 2004. The result were as follows : 1. The result of Wholistic Hospice Nursing Program's development was as follow : A Wholistic Hospice Nursing Program was developed by me in this study is one of the service program for hospice palliative care unit. It was named as ‘Rainbow Program’ to be approached easily by hospice patients. The purposes of it are to improve the quality of life of the terminal patients with their dignity, to help them live in abundant and meaningful in their lives, to care them in peaceful in dying process with understanding them in whole personal, and also to palliate the grief and suffering of the bereaved. It was provided by hospice professionals(nurses, medical doctors, social worker, pastors, art therapists) and volunteers those who were educated in hospice for multi-diciplinary team approach to collaborate with each role play I 20-30 minuters of each through visiting their rooms individually and a place of hospice palliative care unit of S hospital in P city. The subjects of it were the terminal patients those who admitted hospice palliative care unit and their familes. with agreement in hospice palliative care in their terminal disease. The characteristics of it were multi-disciplinary team approach, whole personal care, individual care and total care according to their needs in their condition. The contents of it were pain control, symptom control, counseling patient, counseling family, hair cutting, hair shampooing, bed bath, recreation, taking a walk, event of culture(screen, recital, festival of praises, exhibition and so on), pastoral counseling, ritual service in bed, praying, service in bed, sing a worship praise, listening to the music, sharing remembrance of life, individual visiting music service(sing and praying), meditation Bible, art therapies(dance and drawing), social worker's counselling, confessing and sharing love and thanksgiving. The experimental group subjects participated in Wholistic Hospice Nursing Program which takes 120 minutes per session, total 10 sessions(total 1,200 minutes) altogether. In conclusion, this Wholistic Hospice Nursing Intervention can be used actively for whole personal well-being of the patients in hospice palliative in hospice palliative care unit and also applied in hospice practice as an useful model of multi-disciplinary team approach by hospice professionals.

  • This study aims to investigate hospice care awareness, user satisfaction with intensive care unit facilities and the difference in hospice care awareness level. A survey of 160 people including the intensive care patients in capital district areas and their family members was conducted from March $10^{th}$ until $July31^{st}$ of 2013. As an analytical method, the SPSSWIN18.0 program was used and the sample size followed Cohen's sample extracting formula of G*Power 3.1, an analytical program for test power. The mean, standard deviation and regression analysis have been statistically carried out. The results indicate that the care awareness of hospices have meaningful relations with age, religion and marital status. As for the satisfaction level with hospices, the facilities in the countryside are preferred to those in the cities, and the hospice rooms with Korean floor heating system is preferred to rooms with bed. The implication of this study is expected to become a basic reference to the improvement of the hospice system, seen from the result of survey with highly favorable response for care units in hospices.

The Awareness of Cardiovascular Risk Factors and Its Correlates in Patients with Coronary Artery Diseases (관상동맥질환자의 위험요인 인식 및 관련요인)

  • Purpose: Cardiovascular risk factor modification is important for patients with coronary artery disease to prevent poor progression of the disease. Without an understanding and an awareness of risk factors, patients with coronary artery disease are not able to reduce their risk by the lifestyle modification. The aims of this study were to assess patient's awareness of risk factors and to identify predictors of awareness of risk factors. Methods: A descriptive correlational study using a cross-sectional survey method was performed. The sites of the study were three hospitals in Seoul, Korea. The sample consisted of 214 subjects. The awareness of risk factors and other subjects' characteristics were measured by a questionnaire developed for this study. Results: A range of subjects (gender: 72.0%; hyperlipidemia: 51.9%; hypertension: 40.7%; obese: 37.4%; age: 35.5%; smoking: 22.4%; diabetes: 19.2%; family history: 18.2%) were not able to accurately identify the risk factors. Among predictors, gender (${\beta}$=-.17) and diabetics (${\beta}$=-.25) had statistically significant influences on awareness of risk factors. Conclusion: Our findings highlight the lack of awareness of risk factors for coronary artery disease. The findings have important implications for nursing practice in terms of guiding educational strategies for the modification of risk factors for coronary heart disease.

CHILDHOOD ONSET SCHIZOPHRENIA IN DEVELOPMENTAL ASPECT (소아 정신분열병의 발달학적 측면)

  • This review is a clinical and research update of recent literature related to childhood onset schizophrenia (with an onset of psychosis by age 12). Childhood onset schizophrenia(COS) is a rare disorder, but that may represent a more homogeneous patient population in which to search for risk or etiologic factors of schizophrenia. These overview data show that COS shares the same clinical and neurobiological features as later onset forms of the disorder. Compared with later onset schizophrenia, however, this subgroup of patients appear to have more severe premorbid neurodevelopmental abnormalities, more cytogenic abnormalies, poor outcome, and potentially greater family histories of schizophrenia and associated spectrum disorders. Future studies of this subgroup may provide important clues as to the genetic basis for schizophrenia and how gene products influence certain feature of the disease, such as age of onset and mode of inheritance.

A study of association between Cold Hypersensitivity and Orthostatic Hypotension(OH) (냉증과 기립성 저혈압의 연관성에 관한 연구)

  • Purpose The purpose of this study was to verify the association between Cold Hypersensitivity and Orthostatic Hypotension(OH). Method This study was carried out on 39 women who visited outpatient department of Dept. of Gynecology. Bundang CHA oriental Hospital. College of Medicine. Pochun CHA univ. from December 2001 to November 2002. We divided them into two groups (Orthostatic Hypotension Group 24, Normal BP Group 15) and investigated general characteristics, weight, height, past history. family history through out the patient's note. OH was assessed 1 minute after the patients rose from a supine position by using tilting table. For the diagnosis of cold hypersensitivity, thermographic measurements were performed on two pairs of areas(palm-upper arm and back of hand-upper arm. And for that of the feet, other two pairs of areas(anterior thigh-top of the feet and posterior thigh-heel). And then by which verify the association between Cold Hypersensitivity and Orthostatic Hypotension(OH) Results As a results, 16 out of 24 patients, the OH group had cold hypersensitivity and 4 out of 15 Patients the normal BP group had cold hypersensitivity. It means that there was association between Cold Hypersensitivity and Orthostatic Hypotension(OH). Conclusion Above these results, we can reach the conclusion that OH is considered one of the causes of cold hypersensitivity.

Short-term Glycemic Control and the Related Factors in Association with Compliance in Diabetic Patients (당뇨병 환자의 치료순응도에 따른 단기간 혈당조절정도와 관련 요인)

  • Objectives : Generally, it seemed that the therapeutic result in diabetic patients was changed by compliance. This study was conducted on the basis of assumption that the therapeutic result id diabetic patients could control according to compliance. This study was conducted to analyze the related factors in association with compliance to drug, diet and exercise therapy. Methods : 224 diabetic patients in Kyungpook National University Hospital were selected through the interviews and HbA1c values from 1 Jan. to 28 Feb.1997. The drug compliance was tested by regularity of drug administration, the diet compliance was tested by restriction of food, exactly allocation, balance of nutrient, measuring food and the exercise compliance was tested by regularity of exercise per day. We assessed compliance by percentage, $x^2-test$ and generalized logit regression model(method:enter). Results : The significant variable was the satisfaction to medical personnels in drug, the knowledge to disease in diet, the participation of the diabetic education in exercise therapy and the satisfaction to medical personnels in HbA1c. Using the generalized logit model(method : enter) in compliance change, the significant variables were the satisfaction to medical personnels and the complication in drug; the significant variables were the age at the first diagnosis, the family history, the concern of health, the knowledge of disease, the self-exertion for therapy and the complication in diet: the only significant variable was the gender in exercise therapy. Conclusions : The degree of glycemic control in diabetic patients was influenced by compliance. In order to improve patient's compliance, we must foster the knowledge on the diseases, lead participation for diabetic education. Because the satisfaction to medical personnels was the important variables, we must build up good relationship between doctors and patients.

  • This Study is to propose the resonable management for cancer patients by identifying correlation among the treatment, their length of hospital stay and medical charges. Research subjbject is 144 patient that breast cancer, uterine cancer and Family Medical patients of inpatients in K University Hospital in Korea during six month, from January 1, 2010 to June 30, 2010. The analysis shows that the emergecy is superior range thag outpatient because each of them has 97.9%, 2.1% by looking at the path to admission. And the age of 40-49 is the higest group by age. When matrix components of breast cancer patients, principal component is composed of two axes. Factors associated with the first component appeared correlations between all variables without the age. Following results, this study is considered similar types of disease or treatments are need to introduce the DRG.

A Study on the Ward Rounding System of Medical Record Administrator for Improving the Completeness of the Medical Records (의무기록 완성도에 대한 병동순회 의무기록사제도의 개입효과)

  • Background : With the CQI concepts, which emphasize doing the right things right the first time, we tried to enhance the timely completion of medical records by changing the review process from retrospective method to concurrent one. Methods : Against the current retrospective QA activity, Medical record administrator did the concurrent QA of the inpatient medical records with the deficiency sheets. One general surgery ward was chosen as a trial one. The deficiency rate of the medical records of the discharged patients was compared before and after the enforcement of the system. Job analysis of the medical record departments was done about four tertiary care hospitals located in Seoul to estimate the cost and the time consumed by current system. Results : There was a little improvement in the completion rate of the medical records after the trial. The new system was effective. And job analysis showed that much money and time were wasted by current retrospective feedback system. Conclusion : Though the result was not so satisfactory, it should be considered that this test was a voluntary one and the interns and residents were not forced to complete the medical records during this trial period. If there be any strong motivation to complete the medical record in time, this system is sure to be succeed. As the DRG system requires the concurrent review of the medical records to confirm severity of the patient's illness and to assure the timely discharge, it is desirable to enforce this method with the DRG system together. DRG coding and reducing deficiency rate of the medical records can be accomplished simultaneously.

A Clinical study on the Sasang Constitution and Obesity (사상체질(四象體質)과 비만(肥滿)에 관한 임상적(臨床的) 연구(硏究))

  • Objectives To study on the relation between Sasang constitution and obesity and that of obese patient's clinical symptom. Methods The subjects were 129 female patients to visit to diet. The subjects were 129 female patients to diet who answered questionnaire QSCC II and were diagnosed by the Sasang constitution from May to December in 2001 at Nara oriental medical clinic. Results 1. Of obese patients 79.1% were Taeumin, 11.6% Soeumin, and 9.3% Soyangin. 2. A relation of a grade of obesity and Sasang constitution. 2-1) 45.8% of the obese patients were high level obesity who had over 35% of body fat. Out of these people, 91.5% were Taeumin. 2-2) 40.3% of the obese patients weight hip ratio was between 0.84 to 0.89 and 84.6% of these people were proved to be Taeumin. 2-3) 39.2% of the people who had a minimum weight hip ratio of 0.90 were significantly Taeumin, 6.7% Soeumin and 16.6% Soyangin. 2-4) 65.7% of Taeumin were obese patients. 3. Obesity treatment with constitution. 3-1) 59.8% of Taeumin were Pyo-zng patients. 3-2) As treatment periods tended to increase, Taeumin and Soeumin patients showed a large decling of body fat, whereas Soeumin patients showed the opposite reaction. 3-3) The weight hip ratio was led by Taeumin with 0.03 and then came Soeumin and Soyangin with a 0.02 ratio. As treatment was taken into process longer Taeumin patients showed a great progress in decreasing their weight hip ratios. 4. Costitutional related symptom with Obesity. 4-1) Taeumin was followed by Soyangin and then Soeumin in family history. 4-2) Soeumin was followed by Taeumin, then Soyangin when complain of physical fatigue was taken into consideration. 4-3) The ordinary exercise was led by Soyangin, Taeumin then Soeumin. 4-4) People who receive stress turned out to be 72.9% much higher than the people who don't. 4-5) Overeating due to stress was found in 83.3% of Soyangin.

BRCA1 Gene Mutation Screening for the Hereditary Breast and/or Ovarian Cancer Syndrome in Breast Cancer Cases: a First High Resolution DNA Melting Analysis in Indonesia

  • Specific patterns of the hereditary breast and ovarian cancer (HBOC) syndrome are related to mutations in the BRCA1 gene. One hundred unrelated breast cancer patients were interviewed to obtain clinical symptoms and signs, pedigree and familial history of HBOC syndrome related cancer. Subsequently, data were calculated using the Breast and Ovarian Analysis of Disease Incidence and Carrier Estimation Algorithm (BOADICEA) risk prediction model. Patients with high score of BOADICEA were offered genetic testing. Eleven patients with high score of BOADICEA, 2 patients with low score of BOADICEA, 2 patient's family members and 15 controls underwent BRCA1 genetic testing. Mutation screening using PCR-HRM was carried out in 22 exons (41 amplicons) of BRCA1 gene. Sanger sequencing was subjected in all samples with aberrant graph. This study identified 10 variants in the BRCA1 gene, consisting of 6 missense mutations (c.1480C>A, c.2612C>T, c.2566T>C, c.3113A>G, c.3548 A>G, c.4837 A>G), 3 synonymous mutations (c.2082 C>T, c.2311 T>C and c.4308T>C) and one intronic mutation (c.134+35 G>T). All variants tend to be polymorphisms and unclassified variants. However, no known pathogenic mutations were found.
