  • An endobronchial metastasis is defined as a subsegmental or a more proximal central bronchial metastasis of a nonpulmonary neoplasm in the bronchoscopically visible range. However, the frequencies of endobronchial metastasis range from 2 to 50% of pulmonary metastases from extrathoracic neoplasms by a different definition of an endobronchial metastasis. Primary neoplasms of an endobronchial metastasis including breast cancer, colon cancer, renal cell carcinoma, and ovarian cancer are relatively common. However, an endobronchial metastasis arising from thyroid cancer, parotid gland tumor, bone tumor, bladder cancer, and stomach cancer has only rarely been reported in the literature. Here we report a case of an endobrochial metastases from a hepatocellular carcinoma.

Imaging of Facial Nerve With 3D-DESS-WE-MRI Before Parotidectomy: Impact on Surgical Outcomes

  • Objective: The intra-parotid facial nerve (FN) can be visualized using three-dimensional double-echo steady-state water-excitation sequence magnetic resonance imaging (3D-DESS-WE-MRI). However, the clinical impact of FN imaging using 3D-DESS-WE-MRI before parotidectomy has not yet been explored. We compared the clinical outcomes of parotidectomy in patients with and without preoperative 3D-DESS-WE-MRI. Materials and Methods: This prospective, non-randomized, single-institution study included 296 adult patients who underwent parotidectomy for parotid tumors, excluding superficial and mobile tumors. Preoperative evaluation with 3D-DESS-WE-MRI was performed in 122 patients, and not performed in 174 patients. FN visibility and tumor location relative to FN on 3D-DESS-WE-MRI were evaluated in 120 patients. Rates of FN palsy (FNP) and operation times were compared between patients with and without 3D-DESS-WE-MRI; propensity score matching (PSM) and inverse probability of treatment weighting (IPTW) were used to adjust for surgical and tumor factors. Results: The main trunk, temporofacial branch, and cervicofacial branch of the intra-parotid FN were identified using 3D-DESS-WE-MRI in approximately 97.5% (117/120), 44.2% (53/120), and 25.0% (30/120) of cases, respectively. The tumor location relative to FN, as assessed on magnetic resonance imaging, concurred with surgical findings in 90.8% (109/120) of cases. Rates of temporary and permanent FNP did not vary between patients with and without 3D-DESS-WE-MRI according to PSM (odds ratio, 2.29 [95% confidence interval {CI} 0.64-8.25] and 2.02 [95% CI: 0.32-12.90], respectively) and IPTW (odds ratio, 1.76 [95% CI: 0.19-16.75] and 1.94 [95% CI: 0.20-18.49], respectively). Conversely, operation time for surgical identification of FN was significantly shorter with 3D-DESS-WE-MRI (median, 25 vs. 35 min for PSM and 25 vs. 30 min for IPTW, P < 0.001). Conclusion: Preoperative FN imaging with 3D-DESS-WE-MRI facilitated anatomical identification of FN and its relationship to the tumor during parotidectomy. This modality reduced operation time for FN identification, but did not significantly affect postoperative FNP rates.

  • Background and Objectives: Salivary gland tumors are relatively rare and constitute 3% to 4% of all head and neck neoplasms. Their relative infrequency, inconsistent classification, and highly variable biologic behavior make some difficulty, but some general features can be drawn regarding the incidence, pathology, and pattern of behavior of the various benign and malignant tumors of salivary glands. The present study aims to provide a clinical characteristics of salivary gland tumors. Materials and Methods: We analyzed retrospectively clinical features of the 138 patients who were treated surgically at Chonbuk National University Hospital from 1992 through 2002. Results: We found 107 benign and 31 malignant tumors. Among the patients, 58 were males and 80 were females. The most common age group was the fifties. The most common site of both benign and malignant was parotid gland in major salivary glands and palatal region in minor salivary glands. The most common presenting symptom was palpable mass in both benign and malignant tumor. Histopathologically, the most common type was pleomorphic adenoma in benign tumor and mucoepidermoid carcinoma in malignant tumor. All cases were treated surgically and the most common postoperative complications was transient facial nerve weakness. Conclusion: 138 cases of salivary gland tumors were presented with respect to their clinical features.

  • Background and Objective: Even though major salivary gland tumor is a rare disease, the diversity of histopathologic characteristics makes treatment decisions difficult. The aim of this study is to analysis the clinical experience of our major salivary gland tumor and to suggest a guideline of treatment. Materials and Method: Sixty-eight major salivary gland tumors and tumor-like glandular enlargements treated at Sanggye Paik Hospital during the past seven years between June, 1995 and January, 2002 are analyzed for histopathologic diagnosis, treatment modality, clinical manifestation, local control, and treatment-related morbidity, recurrence rate, retrospectively. Results: In twenty-eight patients the swellings were diagnosed as non-tumorous condition. The clinical features, diagnostic and surgical management of fourty salivary neoplasms involving the parotid and submandibular glands are correlated with their histological features. Conclusion: We have concluded that salivary gland neoplasms are needed multimodal treatment, because of their highly variable biologic behavior in each tumor type. Thirty-four cases were benign and six cases were malignant. Most of benign cases were pleomorphic adenoma and they showed wide age-distribution. In six malignant cases, there were acinic cell carcinoma, adenocarcinoma, carcinoma ex-plemorphic adenoma, myoepithelioma, and adenoid cystic carcinoma.

비강 내 발생한 고립성 섬유종 1예 (A Case of Solitary Fibrous Tumor in Nasal Cavity)

  • The solitary fibrous tumor (SFT) is a mesenchymal neoplasm that is described as spindle-shaped tumor cells on a collagenous background originating from pleural tissues. Recently, extrapleural SFT has been reported in nearly all sites, including the sublingual gland, parotid gland, nasal cavity and paranasal sinuses. Complete surgical excision is primary treatment for SFT, but diagnosing SFT is not often made until immunohistochemical evaluation after surgical resection. We report that the patient, 45-year-old male, was considered as a case of inflammation polyp arising from left nasal cavity with initial biopsy, however, it has turned out to be SFT after surgical treatment.

타액선 상피성 종양 387례에 대한 연구 (Histopathological and Clinical Studies of the 387 Cases of Salivary Gland Epithelial Tumors)

  • There have been few large scale surveys of salivary gland tumors, because the salivary gland tumors comprised less than 3 percent of all neoplasms of the head and neck. Also there is a problem that the incidences of specific types of tumors have the variation according to the geographic regions. ]'nother Korean reports about salivary gland tumor, they only treated about 100 cases only. Objectives : The purpose of this study is to identify the frequency and clinical characteristics of salivary gland tumors and to compare with other Korean reports. Materials and Methods : We had performed to analyze 387 cases of salivary gland tumors from 1983 to June 1997, were reviewed in histopathological and clinical aspects. The diagnoses of individual tumors were based on the World Health Organization classification. Tumors were analyzed according to the histologic type site, age and sex. Results : The majority (79.1%) of cases occurred in the major salivary glands and 20.9% in the minor. The parotid gland was the most frequent site with 52.4% and submandibular glands with 26.3%. Benign tumors were 79.3% of the tumors and malignant tmon were 20.7%. The most frequent tumor was pleomorphic adenoma occupied 63.8% of the total and frequency percent of histologic type of 98 malignant tumors showed 35.7% of mucoepidermoid carcinomas, 24.5% of adenoid cystic carcinomas. Overall gender with female predominance with 1 : 1.4 of male to female ratio. The average age of patients was 42.8 years. The most frequent chief complaint was a palpable mass. The duration of the symptom was shorter in the malignant salivary gland tumors (26.7 months) than in the benign salivary gland tumors (38.2 months). Conclusion : Comparing to the previous Korean reports, the present study confirms that the incidence of salivary gland tumor was lower in the parotid gland, and was higher in the submandibular and minor salivary glands.

타액선 악성종양의 경부 림프절 전이의 위험 요소 (Risk Factors of Nodal Metastasis in Salivary Gland Cancer)

  • Background and Objectives: Nodal metastasis is one of the prognostic factors in salivary gland cancer. The purpose of this study is to identify risk factors of nodal metastasis considered as a predictor of poor prognosis in patients with salivary gland cancer. Material and Methods: The authors retrospectively studied 82 patients with salivary gland cancer who underwent surgery from 1992 to 2002. We analyzed age, sex, tumor size, histologic type and 5-year survival rate to compare patients with and without nodal metastasis. Results: Mean age was 55.4 years, with a male-female ratio of 1 : 1.28. The overall 5-year survival rate was 85.5%. Among the 82 patients, 14 patients had nodal metastasis. There was no nodal metastasis in low grade malignancy. In patients without nodal metastasis, mean age was 52.5 years and the overall 5-year survival rate was 94.2%. In patients with nodal metastasis, mean age was 69.4 years and the overall 5-year survival rate was 42.9%. In patients with less advanced cancer (T1-T2 stage), the nodal metastasis was 7.5% and with advanced cancer (T3-T4 stage), 33.3%. Conclusion: Nodal metastasis significantly decreases survival in patients with salivary gland malignancy. High grade malignancy, large tumor size and old age are important risk factors of nodal metastasis. Nodal metastasis is more common in submandibular gland cancer compared with parotid gland cancer.

악성 혼합종의 임상적 고찰 (Malignant Mixed Tumor of Salivary Glands: A Clinical Study)

  • For malignant neoplasms of salivary tissues. two of the better determinants of progosis are histologic classification and size of the neoplasm. Proper management of these tumors requires an accurate diagnosis by the pathologist and correct interpretation by the surgeon. Malignant mixed tumors account for between 3 and 13 precent of all cancers of the salivary glands and 2 percent of all tumors in these locations. The typical history of these tumors is that of slowly growing mass demonstrating a sudden increase in growth. The duration of onset of the tumor mass and the diagnosis of malignancy has been demonstrated to be 10 to 18 years. The risk of malignat transformation of a benign mixed tumor increases with the duration of the tumor. We analyzed retrospectively 13 cases of malignant mixed tumor who visited from Jan. 1985 to Dec. 1992. Mean age of the patients was 56.5 years. The origin of tumors were parotid gland 7 cases, submandibular gland 2 cases, and minor salivary gland 4 cases(palate 3 cases, tonsil pillar 1 case). According to the criteria of the AJCC on staging, stage I was 1 case, stage II 1 case, stage III 2 cases, and stage IV 9 cases. Histopathologically, carcinma ex pleomorphic adenoma were 12 cases and the true malignant mixed tumor was 1 case. The major treatment modalities were curative surgery, and radiation therapy followed. In conclusion, aggressive therapy of combined surgery and postoperative radiation therapy is required for these lesions, and patients with known or suspected benign tumor should be encouraged to undergo surgery early on in their disease to avoid malignant degeneration at a later dete.
