  • The frequency with which administration of chemotherapy for gynecological cancer treatment is used has increased along with the use of surgery and radiotherapy Among the various side effects of chemotherapy, stomatitis causes a problem of function and sensation in the oral cavity. This oral discomfort can be categorized into two components ; perceived oral symptoms and observed oral symptoms. If the oral problem continues, it may cause infection, bleeding and nutritional deficiencies. As a result of this condition, compliance with the treatment process can be affected as well as the prognosis for the cancer patients. But as the oral discorrfort usually appears after chemotherapy, it is often not reported to the health care personnel as a patient problem. Without problem identification of the oral discomfort and ability to assess the problem, effective intervention cannot be planned. Therefore, this study was conducted to identify the pattern and the degree of oral discomfort due to cancer chemotherapy and thus to provide data for identification of the patient problem and for nursing assessment. The design of this study was a longitudinal de-scriptive study The subjects were in - patients who received chemotherapy under the diagnosis of gynecological cancer between Mar. 15, 1994 and May 15, 1994 at a general hospital in Seoul, Korea. The number of subjects was 64 and they were divided into two groups, one of 41 (A : 5FU & Neoplatin), the other of 23(B : Neoplatin, Cytoxan, Adriamycin), according to the treatment regimen. The data were collected for 24 days using self-re-port instruments. The instruments were the 「Perceived Oral Symptom Assessment Tool」 and 「Observed Oral Symptom Assessment Tool」 developed by this researcher. Data were analyzed using the SPSS-PC program, ANOVA, t-test, paired t-test and the Pearson Correlation Coefficient were applied. The results of this study are as follows : 1. In A regimen the peak time for perceived oral symptom scores was the fifth day after chemotherapy, and the tenth day for observed oral symptom scores. Both of the problems started on first day of chemotherapy and were not resolved completely until the 24th day after treatment. 2. In B regimen, the peak time for perceived oral symptom scores was on the seventh day after chemotherapy, and the eighth day for observed oral symptom scores. It was noted that perceived oral symptom scores were higher than observed oral symptom scores consistently for 24 days. Both also started on first day of chemotherapy, and were not resolved completely until the 24th day after chemotherapy. 3. There were no differences statistically in perceived oral symptom scores between A and B regimen. The loss of appetite and xerostomia caused the most severe discomfort in both of these two groups. 4. The were no differences statistically in observed oral symptom scores between the A and B regi moil. In the A regimen, the highest observed symptom scores were the lips, gingiva, tongue and buccal membrane in that order. But in the B regimen, the highest observed symptom scores were tongue, lips, buccal membrane and gingiva in that order. 5. In A regimen, the patients who had gingival edema and dentures had significantly higher perceived oral symptom scores. And those who had gingival edema and bleeding, foul odor and aphthous stomatitis had significantly higher observed oral symptom scores. 6. In B regimen, the patients who had the experience of stomatitis in the last course of chemotherapy had significantly higher perceived oral symptom scores. Those who had gingival edema had significantly higher observed oral symptom scores. 7. In the A regimen there was no correlation between lab values for lymphocytes and albumin with perceived oral symptom scores and observed oral symptom scores. In the B regimen, there was a significant negative correlation between lymphocytes and albumin with the observed oral symptom scores, but not between perceived oral symptom scores and lymphocytes and albumin values. In conclusion, the nurse should expect that the patient undergoing chemotherapy will complain severely about subjective discomfort and before objective physical change is observed. Also the patients who have chronic oral problems such as dentures, gingival edema and bleeding, foul odor, aphthous stomatitis will complain of severe oral discomfort due to chemotherapy.

  • The purpose of this study was determine the effects of professional tooth cleaning and plaque control instruction(PT & PCI) on reduction of peri-implantitis. A total 80 implant patients were investigated using O'Leary plague index(PI), L${\ddot{o}}$e & Silness gingival index(GI), implant bone loss(BL). While the PT & PCI was conducted by using 'Watanabe method' after scaling for the experimental group, the engine polishing and a plaque control instruction was operated by rolling method after scaling for the control group. The collected data were analyzed with t-test, ANOVA, paired t-test and stepwise multiple regression. PI and GI of the experimental group significantly decreased than the control group(p<0.05). BL, also tended to decreased in the experimental group than the control group(p=0.155). Multiple regression analysis, the factors that was highly correlated with PT & PCI on the PI and GI. According to the implant characteristics, implant location and duration were closely related to PI(p<0.05). Therefore, the PT & PCI is one of the effective methods to reduce the failure caused by the peri-implantitis. Based on this conclusion, that it would be meaningful if the proposed PT & PCI is applied to the oral health management programs of the implant patients.

  • The purposes of this study are to develop the STEAM program grounded on curriculum and to investigate educational effects of the developed program on students' attitude of science and science self-efficacy by application to elementary informal science education environment. In order to develop this program, the literature reviews were conducted and then STEAM education program based on the theme 'light' is developed. The developed program was revised and complemented through preliminary applications and consulting with experts, and applied to 65 students. A single group pre-post paired t-test was conducted through the students' attitude of science and science self-efficacy test. The semi-structure interviews were used to gather focused and additional data. The results of this study were as follows: firstly, STEAM education program was developed with the theme 'light' for elementary students in order to increase their interest related to real life. Secondly, the results indicated that the program was statistically significant on the attitude of science for the group of third and fourth graders. However, the effects of science self-efficacy did not appear a significant result for the third and fourth graders. They expressed one possible reason. The theme of light was not familiar with them because the theme was scheduled to teach in the second semester of the fourth graders. Some of students in this group did have a chance to learn the theme. Thirdly, the program was very effective for the fifth and sixth graders on their attitude of science and science self-efficacy. In conclusion, STEAM education program developed with the theme of light is contributed to elementary students' attitude of science in the informal science education. Students' learning experiences of relevant concepts can influence on students' science self-efficacy. It could be very important factor to consider students' grade level and previous learning experiences when the educational programs develop.

  • Blood flow restriction(BFR) exercise is defined as low intensity and short term exercise using pneumatic pressure belts at the top of limbs, which affects the physiological functions of the body. The purpose of this study was to investigate the effects of walking exercise with BFR on inflammatory index, isokinetic muscle function, and thigh circumference in obese women. Eleven obese women(> BMI $25kg/m^2$ & > body fat 30%) wore pneumatic pressure belts on both femurs and performed walking exercise twice per day, 3 days/wk for 4 weeks (walking 2 min; resting 1 min). Data analysis was carried out using paired t-test. Body weight, BMI, and body fat significantly decreased after exercise(p<.05), and right thigh circumference significantly decreased(p<.05). The concentration of plasma IL-6 significantly increased(p<.05) after exercise. TNF-${\alpha}$ level was not statistically different but tended to slightly increase. CRP slightly decreased, although it did not reach statistical significance after exercise. Muscle strength significantly increased in the $60^{\circ}/sec$ of right/left side extension, left side flexion, and $180^{\circ}/sec$ of left side extension after training(p<.05). These results suggest that 4 weeks of blood flow restriction walking exercise has positive effects on inflammatory index and isokinetic muscle function. Therefore, we consider that blood flow restriction exercise can be used for treatment of obesity, related chronic diseases, and metabolic syndrome. Further, blood flow restriction exercise for a short time has similar effects as a high intensity resistance program.
