적외선 체열촬영을 이용한 통증 치료효과의 평가 (Evaluation of the Therapeutic Effects in Pain Management Using Infrared Thermal Imaging)

  • Background: Infrared Thermal Imaging (ITI) is an effective tool for the diagnosis of disease and evaluation of the therapeutic effects following pain treatment. Patients who were treated for pain in pain clinic described the intensity of pain and the degree of change of their pain using a visual analogue scale (VAS). In this study, the usefulness of ITI following multimodal methods for pain management were compared with the change of VAS. Methods: 1119 patients were evaluated. The patients were treated with stellate ganglion block, epidural block or trigger points injection. Before treatment, the temperature difference (${\Delta}T$) of the involved area and the corresponding area on the opposite side of the body was measured using ITI and VAS was assessed. After treatment, the temperature difference (${\Delta}T$) between the normal and involved areas, the change of ${\Delta}T$ (${\Delta}dT$), VAS and the change of VAS (${\Delta}VAS$) were measured. Statistic correlations between ${\Delta}dT$and ${\Delta}VAS$ were calculated in all groups. Results: Correlation of the ${\Delta}dT$ and ${\Delta}VAS$ was significant by contingency coefficient test. (SGB group, C = 0.358, Epi group, C = 0.377, TPI group, C = 0.374, P < 0.05) Conclusions: ITI is a reliable tool for the assessment of therapeutic effects following multidimensional management of painful disease.

경추부 견인이 경추부 통증 환자의 증세 및 통증에 미치는 영향 (The Effect of Cervical Traction on Pain & Symptom for Patients with Cervical Pain)

  • The purpose of this study was to investigate the influence of cervical pain and radiating pain after cervical traction for patients with cervical pain. This evaluation was made 81 persons who cervical pain or radiating pain. The result of this study were as following ; 1. There were statistically significant decrease in cervical pain and radiating pain after cervical traction. 2. A type group(only neck pain ; n=5) and B type group(neck to elbow radiating pain ; n=11), there were pain decreased but there were not significant difference (p>0.05), C type group(neck to shoulder radiating pain group ; n = 14) and D type group(neck to hand radiating pain group ; n = 50), there were pain decreased before test then after test by VAS and significant difference(p<0.05).

악관절 질환 환자에 대한 초기치료의 효과: 상담 및 투약

  • ㆍPurpose: This study was performed to investigate the initial conservative treatment for TMD patients using careful counselling and medication prospectively. ㆍMaterials and Methods: Careful counselling and medication were performed in 51 TMD patients and 27 patients had follow-up check 2 months or more. Diagnosis of TMD was based on medical history and, physical and radiographic examination. TMD included masticatory disorder, internal derangement, degenerative joint disease, inflammatory joint disorder. and problems resulting from extrinsic trauma. All patients had chief complaints of TMJ pain, mouth-opening limitation. joint noise, and/or referred pain. We counselled and explained to the patient about the pathogenesis, etiologic factors, diagnosis and treatment plan for abut 10 minutes. We prescribed nonsteroidal anti-inflammatorv analgesic(Somalgen) and amitriptyline 10mg per day for 2 weeks. We informed the patient of the attention sheet and taught self-exercise of jaw. The patient were assessed by answering the questionnaire of subjective evaluation of TMD & maxillofacial pain. Questionnaire of an activity limitation. Questionnaire of a jaw function, and Questionnaire for the evaluation of TMD. ㆍResults: In questionnaire for the evaluation of TMD, 88.5% of 26 patients answered that the treatment was efficacious. 71.4% of 21 patients answered no problem in everyday life. There were significant differences between pretreatment and final follow-up in the evaluation of the subjective pain in the following sections: opening widely, chewing, resting, morning, masticatory muscle, and temporal portion(SAS program, paired T-test, P = 0.05). ㆍConclusions: Considerate counselling and proper medication could be significantly effectve in the initial treatment of TMD.

Comparative Analysis of the Independent Medical Examination Reports and Legal Decisions in Pain Medicine

  • Background: An independent medical examination (IME) is a critical process for awarding reparation for injury. However, conducting an IME in pain medicine is very difficult, not only because pain is a subjective symptom, but also because there are no proper objective methods to demonstrate it. This study was conducted to compare IME reports and the court decisions on the disability status of the patients. Methods: We analyzed 79 IME reports and 25 corresponding court decisions on the disability status of patients. The diagnoses, causal relationships between the patients' status and the trauma, McBride's degree of disability, the American Medical Association's impairment ratings, the estimated annual cost for future treatment, and the necessity of care-giving were compared and analyzed. Results: The diagnoses in the 79 cases were complex regional pain syndrome (CRPS) type I (58 cases), CRPS type II (7 cases), peripheral neuropathy (5 cases), myofascial pain syndrome (4 cases), herniated intervertebral disc (2 cases), and fibromyalgia (1 case). The types of accidents were road traffic accidents (50 cases), military injuries (14 cases), industrial accidents (11 cases), and others (4 cases). The IME reports and the court decisions stated considerably different McBride's degrees of disability (P = 0.014). However, there was no significant difference in the estimated cost for future treatment between the IME reports and the court decisions (P = 0.912). Conclusions: IME reports should be accurate, fair, and based on objective findings. Feedback on IMEs from the court decisions is helpful for reference use.

Effect of audio distraction with thermomechanical stimulation on pain perception for inferior alveolar nerve block in children: a randomized clinical trial

  • Background: Pain control is a crucial aspect of pediatric dentistry for patient management. Thermo-mechanical devices (BuzzyTM Pain Care Labs, USA) work on the concept of vibration and cooling and have shown promising results in pain control during local anesthesia in pediatric dentistry. On the other hand, audio distraction has also been used for pain management. The amount of pain endured is determined by the patient's perception and attentiveness. Thus, if audio function is added to the thermomechanical device it might increase its efficiency. Hence, the present study aimed to compare pain on injection using a thermo-mechanical device with and without audio during inferior alveolar nerve block (IANB) injection in children aged 5-10 years old. Methods: Twenty-eight children aged between 5 and 10 indicated for IANB were included in this randomized study. Children who were undergoing the dental procedure were divided into 2 groups, with 14 children in each group. The study group was the thermo-mechanical device with audio distraction; the control group was the thermo-mechanical device without audio distraction. IANB was administered. Subjective pain evaluation was performed using the Wong-Baker Faces Pain Rating Scale (WBFPR) and objective pain evaluation was done using the Faces, Leg, Activity, Consolability, Cry (FLACC) scale. Results: The outcome depicted a significant reduction in pain on injection for both objective and subjective evaluations in the thermo-mechanical device with an audio distraction group. Conclusions: Less pain on injection was observed, when a thermo-mechanical device was used with audio distraction for IANB procedures.

통증신경과학 교육을 결합한 필라테스 안정화 운동이 만성요통 환자의 통증 정도, 장애지수, 심리적 요인에 미치는 영향 (Effects of Pilates Stabilization Exercise combined with Pain Neurosicence Education on Pain, Disability Index, and Physichosocial Factor in Patients with Chronic Back Pain)

  • Background: This study conducted a comparative evaluation of the effects of Pilates stabilization exercise combined with pain neuroscience education (PNE) in patients with chronic low back pain. The evaluation was based on their visual analogue scale (VAS) scores, Korean Oswestry disability index (KODI) scores, and fear avoidance belief questionnaire (FABQ) scores. Methods: A total of 36 participants were recruited and randomly assigned to either a Pilates stabilization exercise group (PSE, n=18) or Pilates stabilization exercise combined with pain neuroscience education group (PPNE, n=18). Both the PSE and PPNE groups participated in 50-minute sessions of Pilates stabilization exercise, three times per week for six weeks. The VAS, KODI, and FABQ scores of the participants were measured before and after the intervention. Results: There were significant improvements in the VAS of the PSE and PPNE group, with significant difference found between them. Both groups showed a significant decline in KODI scores following the exercise interventions, with significant difference observed between the two groups. FABQ scores were significantly decline in both groups, with significant difference found between them. Conclusion: In this study, Pilates stabilization exercise combined with pain neuroscience education was found to be more effective than Pilates stabilization exercise alone in reducing the VAS, KODI, and FABQ scores of patients with chronic low back pain. Thus, Pilates stabilization exercise combined with pain neuroscience education can be used in clinical practice to treat and prevent chronic lower back pain.

간호사를 대상으로 한 통증사정 교육프로그램의 효과 (Evaluation of Pain Assessment Education Program for Nurses)

  • Purpose: This study was performed to evaluate the effectiveness of a pain assessment education program developed for nurses. Methods: Research design of this study was nonequivalent control group quasi-experimental study. Subjects for this study were 56 nurses for control group, and 53 nurses for experimental group. The experimental group participated in pain assessment education program. Data were collected before and 6 months after the program and analyzed using the SPSS 12.0 program. Results: The results of this study were as follows: In pretest, there were no significant differences in general characteristics, knowledge of pain, attitude toward pain and pain assessment behaviors. In post test, the experimental group had significantly higher scores of pain knowledge scores and pain assessment behaviors than control group. However, attitude toward pain remained unchanged. Conclusion: According to the results, pain assessment education program was effective in improving pain knowledge and pain assessment behavior.

경견부 근막통증증후군에 대한 침치료 효과와 압통역치의 변화 (Effects of Acupuncture on Upper Back Myofascial Pain and Pain Pressure Threshold)

  • Objective : This study was performed to confirm the effects of acupuncture on myofascial pain syndrome(MPS) through the change of visual analogue scale(VAS) and pain pressure threshold(PPT) and the usefulness of pressure algometer on the evaluation of pain. Methods : We perfomed this study with 20 outpatients complaining of upper back pain. Before acupuncture therapy(AT), immediately after AT and 2-3 days after AT, we respectively checked visual analogue scale(VAS) and pain pressure threshold(PPT) through pressure algometer, with patients seated and relaxed. The PPT was checked at major trigger point of upper trapezius, levator scapulae, supraspinatus, infraspinatus, rhomboideus minor. and the patients were needled at the same points and maintained for 15 minutes. Results : VAS of immediately after AT was mild higher than that of before AT, but not significantly different. and VAS of 2-3 days after AT was significantly lower than before AT and immediately after AT. PPT of immediately after AT was lower than before PT, but not significantly different. PPT of 2-3 days after AT was significantly higher than that of before AT and immediately after AT. Also PPT was significantly correlated with VAS. Conclusion : PPT of omen was signicantly lower than that of men. and there was no significant difference by age. PPT was increased according to pain duration. Effectiveness of acupuncture on myofascial pain syndrome through PPT and VAS is showed at 2-3 days after AT rather than immediatly after AT. and pressure algometer is useful for the evaluation of Acupuncture therapy on myofascial pain syndrome.

근골격계 만성통증 환자의 통증신념과 통증정도, 대처양상 및 통증생활방해정도와의 관계 (Relationships among the Pain Belief, Pain Coping, and Pain Disability of Patients with Chronic Musculoskeletal Pain)

  • Purpose: The purpose of this study was to understand the degree of pain belief in musculoskeletal patients, and to identify the correlation with chronic pain, pain coping and pain disability. Methods: A total of 203 inpatients or outpatients with chronic pain in orthopedics agreed voluntarily to participate in this study and answer a questionnaire. Data were analyzed using SPSS/WIN 17.0 program with descriptive statistics, t-test, ANOVA, Scheffe test, Pearson's correlation coefficient, and simple linear regression. Results: The degree of pain belief in this study showed statistically significant differences depending on their age, education, job, health status, and pain duration. Among the variables correlating with pain belief in this study, there were positive correlations between pain and pain disability, pain and passive coping, pain belief and passive coping, pain belief and pain, pain belief and pain disability. The strongest correlation was passive coping and pain disability. Conclusion: From the results of this study, we concluded that it is necessary to develop the nursing intervention which can help reducing negative pain belief in patients with chronic musculoskeletal pain. Also we need to enhance the ways of coping to active or chronic pains for controlling them effectively.

The Long-Term Safety and Efficacy of Intrathecal Therapy Using Sufentanil in Chronic Intractable Non-Malignant Pain

  • This report describes the long term safety and efficacy of intrathecal therapy using Sufentanil for the management of chronic intractable neuropathic pain in 12 chronic pain patients. Standardized psychological screening was used to determine treatment suitability. Evaluation data included the Visual Analog Scale (VAS), Wong-Baker Faces Scale, Brief Pain Inventory (BPI), Disability of Arm, Shoulder, and Hand (DASH), McGill Quality of Life Questionnaire, and complications (granulomas, toxicity, withdrawal, or deaths). SPSS version 18 was used for data analysis. Pre- and post- treatment BPI measures and pain scale scores showed a statistically significant difference. There were no complications directly related to drug toxicity, nor drug withdrawals, granulomas, or deaths. Intrathecal therapy with Sufentanil therapy offers a good treatment alternative for those cases that have failed both surgery and standard pain treatment. Strict patient selection based on psychological screening, control of co-morbidities, a proper pain management may contribute to successful outcome.
