TRAUMATIC TMJ INJURY (외상성 악관절 손상)

  • Mandibular trauma is developed due to traffic accident, fall down, industrial injury, and others. TMJ disorder is usually also developed after facial traumatic injury. Many authors suggested that disc displacement or tearing, acute synovitis, TMJ ankylosis, traumatic arthritis, or effusion are developed after facial trauma. It is still very controversible what is the best treatment of TMJ injury such as condylar fracture and meniscal injury. In TMJ injury, synovial inflammation is developed and pain mediators such as prostaglandin E2 or leukotriene B4 are released from the synovial membrane. This can be a cause of TMJ disorder. I present a variety of experimental study about the condylar fracture and meniscal injury and enzyme-immunoassay of synovial fluid after mandibular trauma that have been studied since 1992 and establish the treatment criteria of traumatic TMJ injury. I think that the treatment option of condylar fracture depends upon the surgeon's criteria exclusively. There are no significant differences between conservative and surgical treatment. If the aggressive functional physical therapy and long-term followup be performed, the favorable functional recovery of TMJ can be obtained. And I think that the initial surgical management of meniscus of TMJ is unnecessary in condylar fracture. And also arthrocentesis can be available to release the patient's subjective symptoms and improve the healing of injured TMJ.

A Case of Colchicine-Induced Neutropenia in Liver Cirrhosis Patient (간경변 환자에서 콜히친 사용에 의한 호중구성 백혈구감소증 1예)

  • Colchicine is an alkaloid that has been used for treating acute gouty arthritis, psoriasis, scleroderma and Behcet's syndrome. Colchicine decreased liver fibrosis in rats with carbon tetrachloride induced cirrhosis and in patients with many liver diseases. Therapeutic oral doses of colchicine may cause nausea, vomiting, abdominal pain and diarrhea. The adverse effect of colchicine associated with the dose is bone marrow suppression, and especially neutopenia. Neutropenia has often been reported in patients have taken an overdose of colchicine. We describe a 64-year-old female liver cirrhosis patient with neutropenia that was induced by a therapeutic dose of colchicine.

  • A 49-year-old man with drowsy mentality was diagnosed with acute type A aortic dissection; he underwent an emergency operation. When selective antegrade cerebral perfusion was initiated, the right regional cerebral oxygen saturation ($rSO_2$) decreased as compared to the left one. Adequate blood flow was perfused through the branch of the artificial graft, after distal anastomosis, but the right $rSO_2$ did not recover. Angiography revealed another intimal tear on the right common carotid artery. A stent was then inserted. The right $rSO_2$ promptly increased to the same level as that of the left one. The patient was discharged without any neurologic complications.

  • Hyperventilation is produced by several distinct causes: anxiety, respiratory alkalosis, increased blood catecholamine levels, and a decrease in the level of the ionized calcium in the blood. The dental fears about acute pain, bleeding, needle, drill and dental surgery lead to the severe anxiety and increased blood catecholamine level. Therefore, the most dental patient should be cared gently as the stress reduction protocol. In spite of the gentle care, a hyperventilation were occurred during the surgical extraction of impacted third molar with pericoronitis. We suggest that the dental patients with anxiety disorder must be attention for the manifestation of hyperventilation, especially in the psychologic disabled patient.

  • Craniomaxillofacial injuries produce complex wound environments involving various tissue types and treatment strategies. In a clinical setting, care is taken to properly irrigate and stabilize the injury, while grafts are molded in an attempt to maintain physiological functionality and cosmesis. This often requires multiple surgeries and grafts leading to added discomfort, pain and financial burden. Many of these injuries can lead to disfigurement and resultant loss of system function including mastication, respiration, and articulation, and these can lead to acute and long-term psychological impact on the patient. A main causality of these issues is the lack of an ability to spatially control pre-injury morphology while maintaining shape and function. With the advent of additive manufacturing (three-dimensional printing) and its use in conjunction with biomaterial regenerative strategies and stem cell research, there is an increased potential capacity to alleviate such limitations. This review focuses on the current capabilities of additive manufacturing platforms, completed research and potential for future uses in the treatment of craniomaxillofacial injuries, with an in-depth discussion of regeneration of the periodontal complex and teeth.

Anti-inflammation Effect of Low Intensity Laser Therapy in Collagen-induced Arthritis in Rats

  • Arthritis of the knee is the most common type of joint inflammatory disorder and it is associated with pain and inflammation of the joint capsule. The aim of present study was to investigate the endogenous effect of low intensity laser acupuncture on collagen-induced arthritis in rats. Forty Sprague-Dawley rats were randomly divided into normal group, arthritis group, low laser group with 10 rats in each group. Arthritis in rats was induced by subcutaneous injection of type II collagen combined with complete Freund's adjuvant. Here we investigated the effects of low intensity laser therapy in experimentally induced rat knee arthritis. To evaluate preventive and therapeutic effects of low intensity laser acupuncture on collagen-induced arthritis rats. In collagen induced arthritic rats, there was significant increase in rat paw volume and decrease in body weight increment, whereas low intensity laser therapy groups, showed significant reduction in paw volume and normal gain in body weight. The altered biochemical parameters(blood urea, serum creatinine, total proteins and acute phase proteins) in the arthritic rats were significantly brought back to near normal by the low intensity laser therapy. Therefore, low intensity laser acupuncture may be a useful treatment in the prevention and treatment of collagen-induced arthritis.

A Refeeding Syndrome in the Treatment of Anorexia Nervosa Complicated by Spontaneous Gastric Rupture (자발적 위천공을 동반한 신경성 식욕부진 환자의 재급식 증후군 치험 1예)

  • We report a rare case of gastric perforation in a 13-year-old boy with anorexia nervosa. He was admitted to our hospital with the chief complaint of body weight loss. He had lower abdominal pain after 2 days. An abdominal CT revealed diffuse peritonitis. At laparotomy, the stomach was dilated and perforated. Postoperatively, the patient suffered from malnutrition. We monitored electrolytes, minerals, and fluids closely before and during the initiation of feedings to prevent morbidity and mortality associated with refeeding syndrome. We present an extremely rare complication that relates to this phenomenon, describing an acute gastric dilatation that led to gastric necrosis and perforation through an unusual mechanism in an extremely anorectic teenager during hospitalization for refeeding.

  • Five puppies, 14 days old in the same litter showed acute symptoms such as painful crying, anorexia, abdominal pain and depression with fatal terminations. Necropsy of a puppy revealed hemorrhagic and necrotic foci in the lungs and kidney. The histological lesions were characterized with fibro-necrotizing foci in the lungs, nonsuppurative encephalitis and intranuclear inclusions of liver cells. A cytopathogenic agent was isolated from the thoracic fluid of a dead puppy by cell culture with primary dog kidney cells. The puppies inoculated with the field isolate showed the same clinical signs and lesions as those of the spontaneous cases. Viral particles were observed in suspension of the isolated agent by electron microscopy. The primary dog kidney cells infected with the field isolate showed fluorescent foci against anti-CHV monoclonal antibody after FA stain. On these findings of the disease it was diagnosed as CHV infection. The report signifies the first description of an epizootic of CHV infection in Korea.

  • A fifteen-year-old boy was transported to our hospital emmergency room due to sudden onset of chest pain, hemoptysis and dyspnea. He was diagnosed as persistent ductus arteriosus that had developed acute bacterial endocarditis with pulmonic valve vegetation and pulmonary embolism with pulmonary infarction. After conservative antibiotic therapy (vancomycin + aminoglycoside), we operated this patient successfully - patch closer of the ductus and pulmonary valve valvuloplasty - under the cardiopulmonary bypass.

  • According to the oriental medicine study on G.B.S, we obtained the result as follows : 1. G.B.S was inclined in flacid paralysis and pain and numbness(痺) in oriental medicine. 2. Etiology factors of G.B.S was classified exogenous and endogenous pathogenic factors. The formers was warmth and heat(濕熱), summer heat and dampness(暑濕), dampness and heat(濕熱), and cool and dampness(寒濕), the latter was the deficiency in both the spleen and the stomach(脾胃虛弱), deficiency of Yin(vital essence) in both the liver and kidney(肝腎陰虛) the factor of dampness and heat(濕熱) was most numerous. 3. Pathological mechanism of G.B.S was close connected with the five viscera - the spleen(脾), the stomach(胃), the liver(肝), the kidney(腎), the lung(肺) 4. Differentiation of Symptom-Complexes(辨症) in the G.B.S was consumption type of nutrient fluid due to heat symptom in the lung(肺熱傷津), fullness type of dampness and heat(濕熱侵淫), defiency type in both the spleen and the kidney(脾腎不足), deficiency type in both the spleen and the stomach(脾胃虛弱), deficiency type in the liver and the kidney(肝腎兩虛) 5. Acupuncture treatment for G.B.S was mainly Yangmoung channels of both the hand and the foot.(手足陽明經)
