위암 환자의 고통, 불안, 우울 및 영적 요구 (Distress, Depression, Anxiety, and Spiritual Needs of Patients with Stomach Cancer)

  • Purpose: This study was to investigate relationships among distress, depression, anxiety, and spiritual needs of hospitalized patients with stomach cancer. Methods: The participants were 120 in-patients with stomach cancer for surgery or chemotherapy at C University in Seoul from December 2010 to February 2011. To measure emotional and spiritual states was used Distress management version 1 (National Comprehensive Cancer Network, NCCN), the Hospital anxiety and Depression Scale (HADS), and the Spiritual Needs Scale. The data were analyzed using SPSS 19.0, specifically descriptive statistics, t-test, ANOVA, Scheffe's test, and Pearson's correlation coefficients. Results: Distress showed positive correlations with anxiety (r=.49, p<.001), and depression (r=.44, p<.001). Anxiety showed positive correlations with depression (r=.59, p<.001). While, depression showed negative correlations with spiritual needs (r=-.25, p<.001). Conclusion: This study's findings show that hospitalized patients with stomach cancer experienced distress, anxiety, depression and high spiritual needs. Distress, anxiety, and depression of patients with stomach cancer were positively correlated with each other. While the level of depression was negatively correlated with the level of spiritual needs, indicating the higher the level of depression, the lower the spiritual needs. Therefore, nursing interventions for emotional and spiritual support need to be developed for stomach cancer patients.

폐결핵 환자의 흡연행위에 영향을 미치는 요인 (Factors Influencing Smoking Behavior in Patients with Pulmonary Tuberculosis)

  • Purpose: This study was to investigate the factors affecting smoking behavior in patients with pulmonary tuberculosis. Methods: The participants were 130 pulmonary tuberculosis patients at a national tuberculosis hospital. Using a descriptive survey design, data were collected from January to March in 2016 and were analyzed using binominal logistic regression. Results: As a result of a correlation analysis of the data, depression had a significant positive correlation with smoking (r=.19, p=.030), stress (r=.54, p<.001), respectively. And depression had a significant negative correlation with smoking-related self-efficacy (r=-.20, p=.023). Smoking-related self-efficacy, smoking (r=-.79, p<.001), and stress (r=-.23, p=.008) had a significant negative correlation with each other, respectively. The factors affecting the smoking behavior were smoking-related self-efficacy (OR=1.46, p<.001), sex (OR=67.36, p=.001), occupation (OR=17.51, p=.014), and depression (OR=1.16, p=.024). Those factors explained 84.7% (Negelkerke's $R^2=.847$) of pulmonary tuberculosis patients' smoking behavior. Conclusion: Developing and applying a prevention eduction for reducing depression and enhancing smoking-related self-efficacy may become a venue toward good prognosis of the patients with pulmonary tuberculosis.

산업장 근로자의 건강증진 행위 (A Study on the Industrial Workers' Health Promoting Behavior)

  • The purpose of this study was to identify the major factors affecting performance of health promoting behavior and Pender's health promotion model was examined. The subjects were 508 workers employed in 4 manufacturing industries, data was collected from May 19th to 24th, 1997 using questionaires and it was used convenient sampling method. Data were analyzed by SPSS Win 6.1 program. The results of this study were as follows 1. The average score of health promoting behavior was 2.47. 2. The Health promoting behavior in the relationships between health promoting behavior and demographic factors, there is statistically significant difference in age(F=2.56, P=.0378), religion(F=6.34, P=.0001), working type(F=4.56, P=.0036)variables. 3. The performance score of health promoting behavior was statistically positively correlated with Definition of health(r=.2471. P=.000), Self-efficacy(r=.1385, P=.002), Internal health locus of control(r=.126, P=.000), External health locus of control(r=.2550, P=.000), Chance health locus of control(r=.3023, P=.000), but negatively with Perceived health status(r=-.2076, P=.000). 4. The major factors were Chance health locus of control. Definition of health, working hours, self-efficacy, External health locus of control. and explained for $39.58\%$ of Health promoting behavior performance score.

보건소 간호사의 직무몰입 분석 (Analysis of the Public Health Nurses' Job Involvement)

  • This study was carried out to explore the direction of job involvement of public health nurses and explore the way for improving services of public health center in Korea. The subjects were consisted of 164 nurses who were working at public health centers in Chonbuk. The data were collected by self-reporting questionnaire from Jan. 15 to Jan. 27, 1996. The instrument used in this study was Likert-type scale which Job Involvement Scale developed by Kanungo. The questionnaires of organizational and job characteristic were made through reviewing literatures. The data were analyzed by frequency, percentage, t-test, one-way ANOVA, Scheffe test, and Pearson's correlation coefficient with SPSSPC+ program. Major findings were as follows : 1. Mean scores for job involvement were 3.0879 on a 5 point scale. 2. In the personal variables, professional experience(t=-2.18, p=.031), position(t=2.34, p=.021), and age(F=-1.94, p=.038) were statistically significant in job involvement. 3. The variables to job characteristic were statistically significant in job involvement: job challenge (r=.4785, p=.000), role ambiguity (r=-.3141, p=.000), task significance (r=.2714, p=.000), and role conflict(r=-.2166, p=.003). 4. The variables to organizational characteristic were statistically significant in job involvement : formalization(r=.3184, p=.000) and human centered organizational characteristic (r=.2450, p=.001).

임상간호사의 조직몰입과 관련변인 분석 (Analysis of the Clinical Nurses' Organizational Commitment and Relating Variables.)

  • Organizational commitment is characterized by a strong belief in and acceptance of the organization's goal and values ; a willingness to exert considerable effort on behalf of the organization ; a strong desire to maintain membership in the organization. The purpose of this study was to identify the variables which influence the organizational commitment and to test the relationship between the clinical nurses' organizational commitment and turnover intention. The subjects were 412 nurses who were working on general wards at three General Hospitals in Chounbuk. The data were collected by self-reporting questionnaire from Sept. 20 to Sept. 31, 1995. The instruments used in this study were Likert-type scale which were Organizational Commitment Scale by Mowday et al, and Turnover Intention Scale by Mobley et al. The questionnaire of organizational characteristics was made through reviewing literature. The data were analyzed by frequency, percentage, t-test, one-way ANOVA, Scheffe test, Pearson's correlation coefficient, and stepwise multiple regression with $SPSS-PC^+$ program. Major findings are as follows : 1. Mean scores for organizational commitment were 3.008 and turnover intention 3.167 on a 5 point scale. 2. Age and clinical experience were positively related to organizational commitment(r=.4806, p=.000; r=.4792, p=.000). 3. In the personal variables, hospital type(F=21.374, p=.000), education level(F=5.481, p=.001), position(F=30.867, P=.000), and marital status(t=-5.63, p=.000) of the nurses showed significant differences in organizational commitment. 4. The variables to the organizational characteristic were statistically significant in organizational commitment : formalization(r=.3458, p=.000), human-centered organizational characteristic(r=.4302, p=.000), performance- centered organizational characteristic(r=-.1502, p=.000), payment(r=.2234, p=.000), promotion(r=.2710, p=.000), benefit & service(r=.2325, p=.000), and resource inadequacy(r=-.2172, p=.000). 5. For the purpose of identifying the predicting variables in organizational commitment, stepwise multiple regression is conducted. The results show that age, human-centered organizational characteristic, formalization, hospital type, resource inadequacy, promotion, and benefit & service were significant variables and explained 46% of the variance. 6. Organizational commitment was significantly negatively correlated with turnover intention(r=-.6442, p=.000). As a result of stepwise multiple regression analysis, organizational commitment is the most powerful variable predicting turnover intention and explained 41.5% of the variance. In conclusion, this study shows that the higher the organizational commitment level, the lower the turnover intention. Therefore, for effective turnover management, it is important to improve the organizational commitment among clinical nurses. In order to enhance the organizational commitment by the clinical nurses, it will be useful to consider significant organizational characteristics variables identified in this study.

간호대학생의 임상실습 중 언어폭력, 성폭력 및 우울, 자아존중감이 진로정체감에 미치는 영향 (Effects of Verbal Abuse, Sexual Violence, Depression, and Self-Esteem on Career Identity in Nursing Students during Clinical Training)

  • 본 연구는 간호대학생의 진로정체감에 영향을 미치는 요인을 파악하기 위한 서술적 조사연구이다. 연구 대상자는 C도와 두 곳의 G도 등에 소재하는 종합대학의 간호대학생 177명을 편의표집 하였으며, 자료 수집은 2016년 10월 15일부터 11월 5일까지 이루어졌다. 자료분석은 SPSS/WIN 21.0을 사용하여 t-검정, ANOVA, Pearson's 상관분석 및 다중회귀(multiple regression)분석을 하였다. 간호대학생의 진로정체감은 자아존중감과 유의한 양의 상관관계가, 우울(r=-.529, p<.001))과는 음의 상관관계가 있는 것으로 나타났다. 또한 간호대학생들의 언어폭력은 성폭력(r=.615, p<.001), 우울(r=.540, p<.001)과 유의한 양의 상관관계, 성폭력과 우울(r=.299, p<.001)은 유의한 양의 상관관계, 우울과 자존감(r=-.390, p<.001)은 유의한 음의 상관관계가 있는 것으로 나타났다. 대상자의 진로정체감에 영향을 미치는 변수로는 우울(${\beta}=-.377$, p<.001), 자아존중감(${\beta}=.443$ p<.001), 학년(${\beta}=.178$, p=.001), 소속병원유무(${\beta}=-.140$, p=.008)로 나타났으며 이 변수는 대상자의 진로정체감에 대해 52.2%의 설명력을 나타냈다. 이는 간호대학생들의 진로정체감을 확립하기 위해 우울을 낮추고 자아존중감을 높이는 중재프로그램의 개발이 필요하며, 아울러 우울을 야기할 수 있는 폭력경험을 줄이는 방안으로 실습 전 폭력예방 및 대처프로그램의 참여율을 높이는 것이 필요함을 시사한다.

국시를 앞둔 응급구조과 학생의 스트레스 영향 요인에 관한 연구 (An Analysis Study on stress factor of Emergency medical Students during preparing Examination for Korea Registered licence)

  • 본 연구는 국가고시를 앞둔 응급구조과 학생들의 스트레스에 영향을 미치는 요인을 분석하기 위해 시도되었다. 연구 기간 및 대상은 2010년 10월 1일부터 11월 1일까지 K광역시에 소재한 대학의 응급구조학과 3학년 학생 87명 이었다. 연구도구는 대상자의 일반적 특성 12문항, 스트레스 요인 20문항, 스트레스 적응방법 34문항으로 총 66문항으로 구성되어 있다. 수집된 자료는 SPSS PC프로그램을 이용하여 산술 통계, t-test, ANOVA와 Pearson's Correlation을 이용하여 분석하였다. 본 연구대상자의 스트레스 정도를 분석한 결과 영역별로는 교과과정이 3.67로 가장 높았으며 과제물이 3.39, 공부환경 3.37, 시험 3.35, 취업 3.04 순으로 전체 평균 점수는 3.36이였다. 대상자의 스트레스 적응방법의 전체 평균은 2.85이며, 장기적 적응방법의 평균은 3.14, 단기적 적응방법의 평균은 2.55로 대상자들은 스트레스적응방법 중 장기적 적응방법을 더 많이 사용하는 것으로 나타났다. 일반적 특성 중 스트레스 정도와 유의한 차이를 보인 특성은 성별(t=4.366, p=.040), 성적(F=2.527, p=.047)이었으며 스트레스 영역별 요인과 스트레스 정도와의 상관관계 분석결과 시험(r=.809, p=.000), 교과과정(r=.657, p=.000), 과제(r=.798, p=.000), 공부환경(r=.738, p=.000), 취업(r=.619, p=.000)에서 유의하게 상관성이 높은 것으로 나타났다. 따라서 본 연구 결과에서 나타난 관련요인을 파악하여 스트레스 관리의 효율적인 대처방안 프로그램의 개발과 더 다양하고 구체적인 후속 연구가 필요하다고 사료된다.

일 상급종합병원 간호사의 임종간호스트레스와 임종간호수행 간의 관계에 미치는 공감역량과 극복력의 매개효과 (Mediating Effects of Empathy and Resilience on the Relationship between Terminal Care Stress and Performance for Nurses in a Tertiary Hospital)

  • 목적: 본 연구는 상급종합병원 간호사의 임종간호수행 증진 프로그램 마련을 위해 임종간호 스트레스와 임종간호수행의 관계를 규명하고, 공감역량과 극복력의 매개효과를 분석하고자 하는 것이다. 방법: 본 연구는 2017년 8월 15일부터 8월 30일까지 B 광역시 소재 D대학교병원 간호사 218명을 대상으로 자료수집이 이루어졌다. 자료 분석은 IBM SPSS/WIN 21.0 software를 사용하였으며 t-test, ANOVA, Pearson's correlation coefficients, 회귀분석으로 분석하였다. 결과: 임종간호 스트레스는 공감역량(r=-0.345, P<0.001), 극복력(r=-0.223, P=0.001), 임종간호수행(r=-0.260, P<0.001)과 음의 상관관계였으나 공감역량(r=0.467, P<0.001)과 극복력(r=0.358, P<0.001)은 임종간호수행과 양의 상관관계를 보였다. 임종간호 스트레스와 임종간호수행 간의 관계에서 공감역량은 완전매개 효과(${\beta}=0.409$, P<0.001), 극복력은 부분매개효과(${\beta}=0.294$, P<0.001)를 보였다. 결론: 본 연구결과를 바탕으로 간호사의 공감역량과 극복력을 향상시킬 수 있는 프로그램 마련이 임종간호 수행 증진에 큰 도움이 될 것이다.

STRUCTURE OF THE FLAT COVERS OF ARTINIAN MODULES

  • The aim of the Paper is to Obtain information about the flat covers and minimal flat resolutions of Artinian modules over a Noetherian ring. Let R be a commutative Noetherian ring and let A be an Artinian R-module. We prove that the flat cover of a is of the form $\prod_{p\epsilonAtt_R(A)}T-p$, where $Tp$ is the completion of a free R$_{p}$-module. Also, we construct a minimal flat resolution for R/xR-module 0: $_AX$ from a given minimal flat resolution of A, when n is a non-unit and non-zero divisor of R such that A = $\chiA$. This result leads to a description of the structure of a minimal flat resolution for ${H^n}_{\underline{m}}(R)$, nth local cohomology module of R with respect to the ideal $\underline{m}$, over a local Cohen-Macaulay ring (R, $\underline{m}$) of dimension n.

COVID-19로 인한 교내 임상실습 영향요인 (Factors Influencing the Satisfaction of In-School Clinical Practice Due to COVID-19)

  • 본 연구는 COVID-19로 인해 실습을 할 수 없는 상황에 교내실습으로 대체 한 후 참여한 간호대학생의 실습만족도에 영향을 미치는 요인을 파악하고자 한다. 연구대상자는 G도에 소재한 일개 대학교에서 2021년 9월 13일부터 12월 3일까지 시행된 임상실습 대체 교내실습에 참여한 간호대학생 144명을 대상으로 자료수집하였다. 자료분석은 ANOVA, Pearson's 상관계수, 위계적 다중회귀분석으로 분석하였다. 대상자의 실습만족도는 일반적 특성으로 자기효능감(r=.540, p=.000), 셀프리더십(r=.553, p=.000), 비판적사고능력(r=.382, p=.000), 전공만족도(r=.248, p=.003)간에 유의한 상관관계가 있는 것으로 나타났다. 대상자의 임상수행능력에 영향을 미치는 변인은 자기효능감 20.4%(𝛽=.266, p=.047), 셀프리더십 51.2% (𝛽=.271, p=.047)로 나타났으며, 실습만족도에 대한 변인들의 설명력은 33.8%였다. 따라서 간호대학생의 실습만족도를 높이기 위해서는 셀프리더십과 자기효능감을 높일 수 있는 교육과정 개발하고 적용하는 것이 필요하다.
