The purpose of this study was to investigate the effect of characteristics of hospital choice and hospital service quality on service value, customer satisfaction and hospital revisiting intent and a survey involving patients and their caregivers who used the hospital before was conducted for empirical analysis. Thus, the theory and factors concerning the characteristics of hospital choice, hospital service quality, service value, customer satisfaction and hospital revisiting intent and relevant preceding research were explored after examining the preceding research, the research model was developed accordingly. A total of 459 collected questionnaires were analyze after carrying out the survey on a national scale. The findings of this study have significance since it identified the motivation that affect the choice of hospital and provide information. Also, this study could be utilized as reference data for hospitals to survive in the fierce competition, as it carried out factual survey on considerations when making choice of hospital. Furthermore, due to the low expectation of patients and caregivers for hospital administration, the study indicated that it's linked to a significantly low chance of customer satisfaction and provide the cause. This study was differentiated with other research on hospital choice since it selected security as a factor of hospital choice, though it's not discussed in the preceding research and moreover, conducted the empirical analysis. And the results of empirical analysis revealed that customers felt that service value was improved when the information security was strengthened. In other words, the strengthening of hospital information security could be hospital choice motivation, which means this study provided practical implication.
This study aims at deriving any useful information necessary to strengthen the competitiveness for growth through empirical analyses on a general hospital located in a province in order to countermeasure the opening and competition of medical markets. The characteristics of user were identified on the basis of disease groups under KCD in the research method. In addition, the analysis on the expenses of diagnosis and treatment was divided into the treatment progress and degree of hospital resource utilization. And the regression was carried out to identify the impacts of characteristics of inpatient users on the degree of hospital resource utilization. As a result of major research, the inpatient users of the general hospital located in the provincial area in consideration of inpatient users were formed around the inpatient disease groups representative for Korea. And it was understood that most of residents within a distance of 40 minute by the public transportation were using. Although there are restrictions that the analyses were carried out for a general hospital in a province, the findings of this study can be considered a generalized model for the management of provincial general hospitals when we consider the current circumstance that the diagnosis and treatment are carried out by provincial medical institutions primarily for patients in general within the province, along with implications that any general hospital in Korea should be equipped with the facilities, equipments and human resources under the Medical Treatment Act.
Purpose: The purpose of the study is to emphasize the importance of out-of-hospital cardiac arrest resuscitation. This resuscitation by paramedic is very effectively performed under the medical direction of the doctors. Methods: The cardiac arrest victim was 4 month old infant. Informed consent from the parents of the infant was received. CPR combined with 150J defibrillation was performed to the 4 month old infant. Results: We reported that the 4 month old infant survived the cardiac arrest. Out-of-hospital cardiac arrest infant survived after 150J automated external defibrillator and CPR performance. Conclusion: Specific operative protocol is important because the paramedic can apply the proper manual defibrillator effectively. It is important to extend the work scpoe of the EMT.
The purpose of this study was to find out the restructuring strategy for five provincial hospitals through the business analysis and survey of the service area. Through the survey of the service area of 5 hospitals. service area was classified into three groups, such as underserved area, adequate area, and overserved area. The strategy for the restructuring the clinical departments was set up based on the result of the business analysis and characteristics of the service area. The result of the study was as follows; 1) Whether or not a provincial hospital has specialized in specific area according to the circumstances and the needs of the community was the major factor influencing on the operating result of the hospital. 2) Provincial hospitals at the underserved area has to invest according to the changes of the occupancy rate and increasing pattern of the number of patients while maintaining the status as a general hospital. 3) Provincial hospitals at the adequate area has to lower the grade from the general hospital down to the hospital first and has to upgrade the competency through the restructuring the clinical departments and investment in specific area. 4) Provincial hospitals at the overserved area has to lower the grade from the general hospital down to the hospital first and has to seek ways to change the hospital fundamentally into geriatric hospital, pneumoconiosis hospital or psychiatric hospital etc. Provincial hospitals incapable to compete with private hospitals and clinics has to lower the grade from the general hospital down to the hospital first, to specialize in specific area and to restructure some clinical departments into rental base or self-operating basis. In case such methods are judged not so good solution, provincial hospitals has to find out ways such as shut-down of several departments or operating under the attending system.

Purpose: The purpose of this study was to compare the level of job stress, burn-out and job satisfaction between intensive care unit nurses and general unit nurses. Methods: The subjects of this study were 100 intensive care unit nurses and 100 general unit nurses in university's hospitals. The data were collected using self-report questionnaires. The data were analyzed by descriptive statistics, -test, ANCOVA, t-test, ANOVA, and Pearson correlation coefficient by using the SPSS WIN 20.0 program. Results: There were significantly negative correlation in the score of job stress and job satisfaction in both groups of nurses. Job stress of intensive care unit nurses was significantly differences according to department satisfaction. Burn-out of intensive care unit nurses was significantly differences according to position, department satisfaction. Job satisfaction of intensive care nurses unit was significantly differences according to department satisfaction. Job stress of general unit nurses unit was significantly differences according to department satisfaction. Burn-out of general unit nurses was significantly differences according to clinical experience, position, department satisfaction. Job satisfaction of general unit nurses was significantly differences according to clinical experience, department satisfaction. Conclusions: The appropriate rewards for intensive care nurses and general unit nurses to decrease their job stress will be needed in hospital settings. In addition, a plan for systemic nurse training program is needed to provide high quality nursing education for each unit nurses effectively.

Purpose: We investigated the association between defoliant exposure and survival to discharge after out-of-hospital cardiac arrest. Methods: This is a retrospective case-control study based on the cardiopulmonary resuscitation (CPR) registry. The electronic medical records of out-of-hospital cardiac arrest victims from 6/9/2008 to 12/31/2016 were analyzed statistically. The case patients group had a history of defoliant exposure while the control group did not. Among the 401 victims studied, a total of 110 patients were male out-of-hospital cardiac arrest patients. Baseline characteristics and the parameters involved in cardiac arrest were analyzed and compared between the two groups after propensity score matching. The primary outcome was survival to discharge, and secondary outcomes were sustained return of spontaneous circulation (ROSC) and survival to admission. Results: After propensity score matching a total of 50 patients (case=25, control=25) were analyzed. Primary outcome (survival to discharge) was not significantly different between case and control groups [(OR, 1.759; 95% C.I., 0.491-6.309) and (OR, 1.842; 95% C.I., 0.515-6.593), respectively]. In the subgroup analysis, there were also no significant differences between the control group and subgroups in primary and secondary outcomes according to defoliant exposure severity. Conclusion: There is no statistically significant association between defoliant exposure and survival of out-of-hospital cardiac arrest.
Purpose: Advanced airway maintenance improves the quality of cardiopulmonary resuscitation (CPR) for out-of-hospital cardiac arrest (OHCA) patients. In this study, we evaluate the factors associated with advanced airway management while performing CPR for out-of-hospital cardiac arrest patients by 119 emergency medical technicians (EMTs). Methods: The observational analysis method was used ro retrospectively collect data from 119 rescue run sheets. This study was conducted in a fire station in Seoul, Korea. The subjects of this study were defined as OHCA patients who received CPR from July 2016 to June 2018. We divided the subjects into two groups according to whether advanced airway maintenance was performed or not, and then compared and analyzed both groups. We performed logistic regression analyses for characteristics that differed significantly between groups. Results: Out of 188 OHCA patients, 146 (77.7%) had received advanced airway management. Statistically significant differences in the logistic analysis were found regarding the total number of EMT professionals (adjusted odds ration [aOR]: 1.955; 95% confidence interval [CI]: 1.227-3.115; p=0.005) and scene-time (aOR:1.119;95%CI:1.019-1.228;p=0.019). Conclusion: Advanced airway maintenance while performing CPR for OHCA patients by EMT associated primarily with ensuring an adequate numbers of EMT professionals and sufficient scene time.

In order to improve the hospital service quality, some hospitals try to reduce the outpatients' waiting time in the hospital. One of the dissatisfied service items at the hospital is the long waiting time to take the prescribed medicine. In most cases, the smaller the number of pharmacists, the longer could be the waiting time. The suggestion of criteria for optimal allocation of appropriate number of pharmacists must be very important to manage the hospital pharmacy. In this paper, we suggest the method to figure out appropriate number of pharmacists through the real situation study at the Sampling Medical Center Pharmacy. We present the simulation study results using the simulation package ARENA and the analysis of statistical distribution of the arriving prescriptions. The result of this research could be applied to the other service business to figure out the optimal allocation of available human resources and to do the job analysis for better service quality.
The incidence of hospital infection has been seriously increased in the general hospital in recent years. This study was performed on hospital air and materials in a General Hospital in Seoul from June to December in 1980. The results were as follows: 1. Air sampling was done in multiple strategic areas by exposing standard petridishes for 5 minutes. There was a significant difference of airborne microbe between places. ($F._{99}$ = 3.2, p < 0.01). 2. The mean colony count was 8.6$\pm$6.2 colonies / plate / 5 minutes. 3. Gram stains of colony in air sampling were Gram (+) cocci 66.5%. Gram (+) rod 18.4%, Gram (-) cocci 1.3%, Gram (-) rod 8.7% Fungus 4.5%. 4. For the evaluation of sterilization of steam sterilizer and ethylene oxide gas sterilizer, biological monitoring were done by commercial spore strip. Positive culture was obtained in 2 out of 41 tests on 3 steam sterilizers, and in 3 out of 13 tests on ethylene oxide gas sterilizer. 5. Product sampling and culture were done for 2 kinds of disinfectants and 30 sets of various operation package or dressing materials. Positive culture was obtained in one disinfectants.
Objective : Clinical study of acupuncture and moxibustion therapy on $ch'{\check{o}}nch'u(ST25)$ for treatment of constipation Methods : Comparison study after the acupuncture and the moxibustion treatment to the 41 people who were visiting patients of Kwangdong Oriental Medicine Hospital between 10/1/2000 and 4/17/2001. Results : 21 patients turned out to be in the defecation group and 9 patients in the non-defecation group out of 30 patients who were treated by acupuncture among 41 patients(Defecation effect 70%). 7 patients turned out to be in the defecation group and 4 patients in the non-defecation group out of 11 patients who were treated by moxibustion among 41 patents(Defecation effect 63.64%). In the states of defecation in the group performed the acupuncture, the good excrements like soft and watery-soft consist in 30.4%. In the group performed moxibustion therapy occupied 45.5%. Comparatively the good excrements was shown. Conclusion : The patients who are excessive were more effective than those who are deficient in defecation by acupuncture treatment and the patients who are deficient were more effective than those who are excessive in defecation by moxibustion treatment.
