Objectives: This study was conducted to measure the decline in the health-related quality of life (HRQoL) associated with some diseases in South Korean adults. Methods: The EQ-5D health states in the 2005 National Health and Nutrition Examination Survey (NHNES) and the Korean EQ-5D valuation set were used to obtain the EQ-5D indexes of the study subjects. Each disease group was defined when the subjects reported to the NHNES that they were diagnosed with the corresponding disease during the previous 1 year by physicians. Since the distributions of the EQ-5D indexes in each subgroup were negatively skewed, median regression analysis was used to estimate the effects of specific diseases on the HRQoL. Median regression analysis produced estimates that approximated the median of the EQ-5D indexes and there are more robust for analyzing data with many outliers. Results: A total of 16,692 subjects (6,667 patients and 10,025 people without any disease) were included in the analysis. As a result of the median regression analysis, stroke had the strongest impact on the HRQoL for both males and females, followed by osteoporosis, osteoarthritis, rheumatic arthritis, and herniation of an intervertebral disc. While asthma had a significant impact on the HRQoL only in men, cataract, temporo-mandibular dysfunction, and peptic ulcer significantly affected the HRQoL only in women. Conclusions: Stroke and musculoskeletal diseases were associated with the largest losses of the HRQoL in Korean adults.

Fatty acids, important components of a normal diet, have been reported to play a role in bone metabolism. Osteoclasts are bone-resorbing cells that are responsible for many bone-destructive diseases such as osteoporosis. In this study, we investigated the impact of a medium-chain fatty acid, capric acid, on the osteoclast differentiation, function, and survival induced by receptor activator of NF-${\kappa}B$ ligand (RANKL) and macrophage colony-stimulating factor (M-CSF). Capric acid inhibited RANKL-mediated osteoclastogenesis in bone marrow-derived macrophages and suppressed RANKL-induced $I{\kappa}B{\alpha}$ phosphorylation, p65 nuclear translocation, and NF-${\kappa}B$ transcriptional activity. Capric acid further blocked the RANKL-stimulated activation of ERK without affecting JNK or p38. The induction of NFATc1 in response to RANKL was also attenuated by capric acid. In addition, capric acid abrogated M-CSF and RANKL-mediated cytoskeleton reorganization, which is crucial for the efficient bone resorption of osteoclasts. Capric acid also increased apoptosis in mature osteoclasts through the induction of Bim expression and the suppression of ERK activation by M-CSF. Together, our results reveal that capric acid has inhibitory effects on osteoclast development. We therefore suggest that capric acid may have potential therapeutic implications for the treatment of bone resorption-associated disorders.
Objectives : The purposes of this study were to examine the health-related quality of life(HRQoL) and to identify its related factors for a group of rural adults. Methods : The study subjects were 1,901 adults who were aged 40-70 years and who were living in Naju City, Jeollanamdo. The sociodemographic characteristics, health-related behavior, health status and global self-rated health were collected for statistical analysis. The health-related quality of life was measured by the Euroqol EQ-5D instrument. The differences on the EQ-5D index between the groups were assessed with t-test, ANOVA and hierarchical multiple regression analysis. Results : Overall, the mean value of the EQ-5D index was $0.884{\pm}0.140$ and this score was significantly different according to the socioeconomic characteristics, the health-related behavior, the health status and the global self-rated health. According to the results from the hierarchical multiple regression analysis, the HRQoL was significantly reduced for females, older subjects, and other subjects with no spouse and the subjects with osteoporosis, obesity, mental distress or poorer global self-rated health. Conclusions : The HRQoL for rural adults was related to the socioeconomic characteristics, the health status and the global self-rated health, A better understanding of the factors related to the HRQoL would help to improve the rural adults' quality of life.

The leaves of Acanthopanax species have traditionally been used as a tonic and a sedative as well as in the treatment of rheumatism and diabetes. Chiisanoside is the major active lupane triterpenoid of Acanthopanax leaves. To investigate the bioactivities of chiisanoside, which act on bone metabolism, the effects of chiisanoside on the function of osteoblastic MC3T3-E1 cells were studied. Chiisanoside $(0.02{\sim}20\;{\mu}M)$ significantly increased the growth of MC3T3-E1 cells and caused a significant elevation of alkaline phosphatase (ALP) activity, collagen content, and nodules mineralization in the cells (P < 0.05). The effect of chiisanoside (2 ${\mu}M$) in increasing ALP activity was completely prevented by the presence of tamoxifen, suggesting that the effect of chiisanoside might be partly estrogen receptor mediated. Moreover, cotreatment of p38 inhibitor SB203580 or JNK inhibitor SP600125 inhibited chiisanoside-mediated ALP upregulation, suggesting that the induction of differentiation by chiisanoside is associated with increased activation of p38 and JNK mitogen-activated protein kinases. Our data indicate that the enhancement of osteoblast function by chiisanoside may result in the prevention for osteoporosis.

Considerable beneficial effects on osteoporosis from soy intake in postmenopausal women are being reported (Murkies et al.1998, Arjmandi et al. 1996). Isoflavones(IF), one of phytoestrogenic substances in the soybean, have been suggested for the effect. Our preliminary study showed that even the young chicks fed IF-rich diet tended to have higher bone ash content than those fed IF-poor diet. IF have been reported to decrease bone ash content than those fed IF-poor diet, IF have been reported to decrease bone loss by reducing bone resorption. Soybean meal has been one of the most important protein sources in poultry diets. We assume that the IF intake through dietary soybean meal could give meaningful influences on the birds. This study was carried out to determine effects of dietary IF from soybean sources on bone development in young chicks. Soy protein concentrate (SPC, IF-poor)and an IF concentrate(Phyto-Nutramin) were formulated together with purified-type ingredients to provide three different levels(25, 240 and 480 mg/kg) of total IF. Control diet(240 mg/kg) was prepared with isolated soyprotein(ISP, IF-rich). The diets were fed to 96, 3-day old, layer-type, male chicks, divided into 4 treatments with 3 replications for 3 weeks. Chicks fed the ISP diet had better gain/feed ratios than fed the SPC diets. Birds fed the diets with higher levels of IF tended to show higher values in serum total and ionized Ca% and tibial bone density, length % ash, stiffness and strain. This trend however, appeared less significant at the end of third week. No noticeable differences in sizes of comb and testicle and serum alkaline phosphatase activities were observed among the dietary groups. These results suggest that dietary isoflavones from soybean sources could be associated with chemical composition and physical properties of bone in sizes of comb and testicle and serum alkaline phosphatase activities were observed among the dietary groups. Purpose: The objectives of this study were to assess pain relief according to the time after kyphoplasty and to evaluate the factors affecting pain relief after kyphoplasty in the treatment of a single-level osteoporotic vertebral compression fracture. Methods: A retrospective review of 34 kyphoplasties for single-level osteoporotic vertebral compression fractures was performed. Pain relief was assessed by using the visual analogue scale (VAS) at preoperatively and postoperatively at 6 weeks, 3 months, and 6 months. Associated factors, including age, sex, pain duration, bone mineral density, and cement leakage, were evaluated using the patients' medical records. Statistical analyses were conducted using the paired t-test to assess pain relief and using the independent t-test and Pearson's correlation coefficient to evaluate the relationship between those factors and pain relief. Results: Preoperatively, the mean VAS score was 7.06. Postoperatively, it declined to 3.66 (p=0.001), 2.81 (p=0.001) and 2.24 (p=0.001) at 6 weeks, 3 months and 6 months, respectively. Also, statistically significant pain relief was observed during the periods from 6 weeks to 3 months (p=0.001) and from 3 months to 6 months (p=0.001). However, reduction of the VAS score showed no significant correlation with age, sex, pain duration, bone mineral density, or cement leakage (p>0.05). Conclusion: Our study suggests that a kyphoplasty may be effective in osteoporotic vertebral compression fracture patients with acute pain and that after the kyphoplasty, pain is reduced remarkably for 6 weeks and then continuously for 6 months.
The objective of the present study was to elucidate the effect of bisphosphonates, anti-osteoporosis agents, on glucose uptake in retinal capillary endothelial cells under normal and high glucose conditions. The change of glucose uptake by pre-treatment of bisphosphonates at the inner blood-retinal barrier (iBRB) was determined by measuring cellular uptake of $[^3H]3$-O-methyl glucose (3-OMG) using a conditionally immortalized rat retinal capillary endothelial cell line (TR-iBRB cells) under normal and high glucose conditions. $[^3H]3$-OMG uptake was inhibited by simultaneous treatment of unlabeled D-glucose and 3-OMG as well as glucose transport inhibitor, cytochalasin B. On the other hand, simultaneous treatment of alendronate or pamidronate had no significant inhibitory effect on $[^3H]3$-OMG uptake by TR-iBRB cells. Under high glucose condition of TR-iBRB cells, $[^3H]3$-OMG uptake was increased at 48 h. However, $[^3H]3$-OMG uptake was decreased significantly by pre-treatment of alendronate or pamidronate compared with the values for normal and high glucose conditions. Moreover, geranylgeraniol (GGOH), a mevalonate pathway intermediate, increased the uptake of $[^3H]3$-OMG reduced by bisphosphonates pre-treatment. But, pre-treatment of histamine did not show significant inhibition of $[^3H]3$-OMG uptake. The glucose uptake may be down regulated by inhibiting the mevalonate pathway with pre-treatment of bisphosphonates in TR-iBRB cells at high glucose condition.

Pueraria lobata is a perennial legume plant, widely distributed in the countries of East Asia. It is a medicinally important leguminous plant and produces various isoflavones such as puerarin, daidzein etc which have potential for preventing several chronic diseases including osteoporosis, cardiovascular disease and cancer. In this study, we tried to induce hairy roots in vitro from Korean wild arrowroot P. lobata and investigated the effects of hormones and light conditions. Initially leaf and stem segments were infected with Agrobacterium rhizogenes and incubated in different conditions. Hairy roots were induced from only stem segments and the induction was best at dark condition and the presence of IBA during incubation. Secondary roots were also significantly much more induced at the dark condition than at the 16 hours light condition. Among plant growth regulators of auxin, IBA was best for secondary root formation while 2,4-D, IAA and NAA produced callus or less hairy roots. The presence of the foreign gene rolC transferred by A. rhizogenes that plays a major role in hairy root induction was confirmed by PCR. The accumulation of isoflavones such as puerarin and daidzin was also confirmed. These results will facilitate mass production of hairy root and can be used for the production of functional substances from wild arrowroots.
The purpose of this study was to examine the effects of dietary protein and exercise on bone mineral density and bone mineral content of growing male rats. Forty male, Sprague-Dawley rats(age 21 days) were assigned to four groups that underwent 9 weeks of experimental treatment. Animals were assigned to one of two exercise treatments (treadmill running or sedentary). The exercise and nonexercise group were fed a diet containing casein or soy with rich isoflavones (3.4mg/g protein). The exercise group ran on a rodent treadmill(speed of 15m/min for 30min) three days per week during the 9-week study period. All rats were fed an experimental diet and deionized water ad libitum for 9 weeks. Total bone mineral density (BMD), total bone mineral content (BMC), total body calcium, spine BMD and BMC, and femur BMD and BMC were determined by using dual energy x-ray absorptiometry (FIXI-mus, GE Lunar Radiation Cooperation, Madison, WI, USA). The soy diet group appears to have a significantly higher total BMD/weight and total BMC/ weight, spine BMD/weight, spine BMC/weight, femur BMD/weight and femur BMC/weight compared to the casein group in nonexercise and exercise. The exercise group had significantly greater total BMD/weight and BMC/ weight, spine BMD/weight and BMC/weight, femur BMD/weight and BMC/weight compared to the nonexercise group when the protein source was casein. The exercise combined soy group had significantly greater total BMD/weight and BMC/weight, spine BMD/weight and BMC/weight, femur BMD/weight and BMC/weight, compared to the exercise combined casein group. The results indicate that exercise had a positive influence on bone mineral density and bone mineral content and soy significantly affect on bone mineral density and bone mineral content for the 9 weeks experimental period. It can be concluded that exercise combined with a soy diet is most beneficial for acquisition of spine bone mineral density in young growing male rats. This convincing evidence suggests that a change in life style such as increasing exercise and consumption of soy protein is a practical strategy for significantly reducing the incidence of osteoporosis.
This paper presents the status of nutrition education for older adults in Korea, and examines considerations in developing effective nutrition education programs for the elderly based on literature reviews. Finally, strategies of nutrition education for older adults in Korea are examined. Status of nutrition education were examined by surveying 90 senior centers, and 46 public health centers providing nutrition services. Most senior centers(96%) provided health education programs, however, nutrition was only a part of health programs. Among the 41 public health centers which responded to the survey, 73.1% provided nutrition education for older adults. The frequently covered topics were prevention & management of hypertension/stroke, diabetes, nutritional management during later adulthood, and osteoporosis. Common barriers in planning and implementing elderly education were; lack of educational materials for older adults, reliance on lectures, difficulty in following-up. To develop effective nutrition education, four stages consisting of needs assessment, planning and implementation of programs, and evaluation should be carefully done. Needs assessment might be done using quantitative or qualitative assessment. Factors influencing nutrition behavior of older adults can be systematically examined using a theoretical approach such as the PRECEDE-PROCEED framework. Qualitative methods, such as focus group interviews, also provide insightful information regarding the needs of older adults. In planning nutrition education programs, physical and pshychological changes associated with aging should be considered. Literature regarding elderly education suggest that active participation or participatory learning is also effective for older adults. Educational materials are developed following the principle of KISS and pre-tested. Program evaluation has been rarely done in practice, although it provides valuable feedback to the program. Strategies for developing nutrition education for Korean elderly include; performing needs assessment, developing a standard program by topics in a logical and systematic way, developing programs for subgroups of elderly, applying diverse education methods developing educational materials for the elderly, evaluating programs using simple tools, and delivering a nutrition program as a part of health promotion program. Finally, the interaction and communication between researchers and practitioners is strongly recommended to ensure better nutrition education and services to the elderly.
