Biomechanical Effect on Knee Adduction Moment by Lateral Wedge Insole in Transfemoral Amputee (외측웨지인솔이 대퇴절단자의 무릎내전모멘트에 미치는 영향)

  • The prevalence of knee osteoarthritis was higher people with lower limb amputation. This was identified that transfemoral amputees have a greater external knee adduction moment than ablebodied subjects by biomechanical studies. Therefore, they need rehabilitative intervention for prevention and reduction of knee osteoarthritis. The purpose of this study was to determine the effect of lateral wedge insole used in the treatment of knee osteoarthritis. This study was participated in fourteen unilateral transfemoral amputees and we were analyzed the difference gait variables between without lateral wedge insole and with $5^{\circ}$ and $10^{\circ}$ lateral wedge insole during gait. Our results showed that step length ratio was more symmetrical and, hip adduction and ankle inversion angle were more close to normal value, and knee adduction moment was decreased as the wedge angle increases. We proposed that these data would be utilized conservative treatment of knee osteoarthritis in lower limb amputees.

Prevalence and Related Factors of Knee Osteoarthritis in Rural Women (농촌여성의 무릎 골관절염 유병률 및 관련요인)

  • Objectives: This study was performed to investigate the prevalence of knee osteoarthritis according to the criteria of diagnosing knee osteoarthritis in rural women and the factors related with this disease. Methods: The data obtained from 200 women older than 40 years of age residing in 5 Ri's in Goryeong-gun. Gyeongsanbuk-do by random cluster sampling from September to October 2002. Knee osteoarthritis was determined positive according to the Kellgren and Lawrence classification and knee pain. Results: Among these subjects, 71.0% showed more than grade 2 in radiologic finding and the rate of knee pain according to the survey was 67.0%. The rate of subjects meeting the criteria of knee osteoarthritis was 54.0%. According to univariate analysis, the prevalence of knee osteoarthritis increased with age and those farming people and people working in household industry was significantly high at 58.9% compared with others. The prevalence of knee osteoarthritis showed a significant relationship with the family history and past history of knee injury and knee surgery(p<0.01), and diabetes mellitus(p<0.05). The score of ADL was significantly different in the subjects with knee osteoarthritis compared with normal group(p<0.05). When the presence of knee osteoarthritis and the period of the life style of seating down on the floor were compared, a significant difference was present between the osteoarthritis group and normal group. As for metabolic factors, the blood sugar level, bone density, and body mass index(BMI) were significantly different in the osteoarthritis group compared with normal group. When multiple logistic regression analysis was performed with the presence of knee osteoarthritis as the dependent variable, the prevalence of knee osteoarthritis was significantly affected by older age, subjects farming or working in household industry, the history of knee injury, the history of surgery, higher blood sugar level, and higher BMI. Conclusions: These subjects need an intervention through self-care programs such as exercise for preventing osteoarthritis, weight control programs, other exercise programs strengthening knee joints, and guidelines when working in vinyl houses.

Proprioception After Total Knee Replacement (슬관절 전치환술 이후의 고유수용성 감각 변화)

  • Purpose : The purpose of this article was to review the literature on change of knee position sense after total knee replacement intervention in knee rehabilitation. Methods : This review outlines scientific findings regarding different literature data in knee rehabilitation, and discusses proprioceptive change of knee joints after total knee replacement intervention. Result : Severe pain and diminished joint sensation may precipitate degenerative changes of knees joint, and a strong association between decreased proprioception and function has been identified in elderly patients with knee osteoarthritis. Total knee replacement(TKR) of the knee joint are effective surgical procedures in osteoarthritis patients, resulting in decreased pain, and improved physical function and quality of life in patients. After total knee replacement, joint position sensation is partially recovered, which may improve functional stability of the knee, but aspects of physical functioning are not fully restored to preoperative status. Conclusions : Thus, recovery rate in proprioception after TKR also is considered to be important because it may be a significant risk factor in failure of total knee replacement and knee rehabilitation intervention.

A Clinical Case Study of Osteoarthritis of Knee Joint treated with Traditional Korean Medical Therapy and Core Exercise (한방 치료와 코어 운동을 병행한 퇴행성 슬관절염 환자 1례에 대한 증례보고)

  • Objectives : The objective of this study is to report the effect of Traditional Korean medical therapy and core exercise on osteoarthritis of knee joint by observing a clinical case study. Methods : Patient with a diagnosis of osteoarthritis of knee joint, was treated by acupuncture, herb medicine, physical theraphy and core exercise. Results : After treatment, Visual Analogue Scale(VAS), Korean version of the Western Ontario and McMaster Universities(KWOMAC), Pain Rating Score(PRS) were reduced from 5 to 2, from 25 to 16, from 55 to 27, and CENTAUR test results were improved. Conclusions : This study showed that Traditional Korean medical therapy and core exercise has meaningful effect on osteoarthritis of knee joint, and more researches should be followed.

Extracorporeal Shock Wave Therapy with Meridian and Acupoint Theory for Knee Osteoarthritis: Systematic Review (경혈경락이론에 근거한 체외충격파 치료가 무릎 관절염에 미치는 영향: 체계적 문헌 고찰)

  • Objectives To determine the evidence of effectiveness and safety of extracorporeal shock wave therapy (ESWT) with meridian and acupoint theory for knee osteoarthritis. Methods By March 3, 2021, five foreign electronic databases and six Korean medical electronic databases were reviewed with the key words 'extracorporeal shock wave' and '(acupoint OR acupuncture point)'. This key words was set up to increase the sensitivity of the search. After the search, knee osteoarthritis study was selected based on the title and abstract and then included after full-texts were read. Results Five randomized controlled trials were eligible in our inclusion criteria. The meta-analysis of three studies showed positive results for the using ESWT with meridian and acupoint theory for knee osteoarthritis compared with the control group on efficancy rate, visual analog scale and lysholm knee score. but there remains a conundrum regarding the safety of ESWT in the treatment of knee osteoarthritis. Conclusions Most of studies showed ESWT with meridian and acupoint theory were statistically effective to knee osteoarthritis. However there are limitations that the number of selected studies was small, risk of bias was unclear. So use of ESWT with meridian and acupoint theory for knee arthritis has limited evidence compared to usual care.

A Clinical Study of Knee Joint Osteoarthritis Patients on the Effect of Korean Traditional Medicine Treatment with Acupuncture, Cupping, Physical Therapy and Ganghwalijetong-yeum(Qianghuochutong-yin): Case Series (퇴행성 슬관절염 환자에 대한 침, 부항, 물리치료, 강활제통음의 한방 복합 치료 효과: 사례군 연구(예비연구))

  • Objective This study investigated the effect of Korean Traditional medicine treatment with Acupuncture, Cupping, Physical Therapy and Ganghwalijetong-yeum(Qianghuochutong-yin) on Knee joint Osteoarthritis patients. Methods 5 Knee joint Osteoarthritis patients were treated with Ganghwalijetong-yeum(Qianghuochutong-yin), acupunture, cupping, physical therapy and exercise for Knee joint Osteoarthritis. K-WOMAC(Western Ontario and McMasters Universitis) Index, VAS(Visual Analogue Scale) were checked to evaluate the effectiveness of the treatment. Results K-WOMAC of patients treated with Ganghwalijetong-yeum(Qianghuochutong-yin) is improved from $67.00{\pm}24.41$ to $44.80{\pm}21.18$ and there was statistical significance(p<0.05). VAS of patients treated with Ganghwalijetong-yeum(Qianghuochutong-yin) is also improved from $4.20{\pm}1.79$ to $2.30{\pm}1.10$ but there was no statistical significance(p>0.05). Conclusions According to the study, Korean Traditional complex therapy with Ganghwalijetong-yeum(Qianghuochutong-yin) might improve Knee joint Osteoarthritis.

The Effects of the BeHaS Exercise Program on Balance and Walking Ability in Middle-aged Women with Knee Osteoarthritis (베하스(BeHaS)운동프로그램이 슬골관절염을 가진 중년여성의 균형 및 보행능력에 미치는 효과)

  • Purpose: The purpose of this study was to determine the effects of the BeHaS exercise program on balance and walking ability in middle-aged women with knee osteoarthritis. Method: Participants were 36 patients (experimental group: 18, control group: 18) with knee osteoarthritis. Women with knee osteoarthritis in the experimental group participated in the BeHaS exercise program for 65 to 90 minutes per session twice a week for 12 weeks. Data were collected by a structured interview and measurement tool from September 1, 2005 to March 30, 2006. Descriptive statistics, such as frequency, percentages, means, SD and t-test, Chi square test were used with the SPSS Win 12.0 to analyze the data. Results: There was a significant difference between the two groups for dynamic balance, but differences in balance and walking ability were only partially supported. Conclusion: These findings indicate that the BeHaS exercise program could be a good nursing intervention for health maintenance and promotion of patients with knee osteoarthritis. Further research is needed in order to confirm these findings in elderly people with osteoarthritis.

Biomechanical Properties of the Anterior Walker Dependent Gait of Patients with Knee Osteoarthritis (무릎관절 골관절염 환자의 보행기 보행에서 생역학적 특성)

  • Purpose: Osteoarthritis occurs in many different joints of the body, causing pain, stiffness, and decreased function. The knee is the most frequently affected joint of the lower limb. The aim of this study was to investigate the differences of biomechanics between independent gait and anterior walker dependent gait of patients with osteoarthritis of the knee. Methods: Lower limb joint kinematics and kinetics were evaluated in 15 patients with knee osteoarthritis when walking independently and when walking with an anterior walker. Participants were evaluated in a gait laboratory, with self-selected gait speed and natural arm swing. Results: When walking with a dependent anterior walker, participants walked significantly faster (p<0.01), using a longer stride length (p<0.01), compared to independent gait. When walking with a dependent anterior walker, participants exhibited significantly greater knee flexion/extension motion (p<0.01) and lower knee flexion moment (p<0.05) compared to independent gait. When walking with a dependent anterior walker, participants showed significantly greater peak ankle motion (p<0.01), ankle dorsiflexion/plantarflexion moments (p<0.01), and ankle power generation (p<0.05) compared to independent gait. Conclusion: These biomechanical properties of gait, observed when participants walked with a dependent anterior walker, may be a compensatory response to impaired knee function to allow sufficient power generation for propulsion. Therefore, rehabilitative strategies for patients with osteoarthritis of the knee are needed in order to improve not only knee function but also hip and ankle function.

The Effects of Exercise with Taping on Muscle Strength and Bone Mineral Density, IGF-1 in Blood of Elderly Women (퇴행성슬관절염 여성노인의 운동과 테이핑이 근력과 골밀도, 혈중 IGF-1에 미치는 영향)

  • Purpose : This study was to explore the effects of knee joint taping exercise on muscle strength, bone mineral density, pain and IGF-1 in blood of elderly women with knee Osteoarthritis. Methods : Thirty elderly women with knee osteoarthritis were divided into three groups: the taping with exercise group (n=10), the regular exercise group (n=10) and control group (n=10). Participants' muscle strength, bone mineral density, pain and IGF-1 in blood were measured three times : before exercise, after 6 weeks, and after 12 weeks. Results : Participants in both exercise (taping & non-taping) groups showed improvement in muscle strength, bone mineral density, pain and IGF-1 in blood after 6 and 12 weeks compared to before exercise. In particular, the taping exercise group had a greater effect on muscle strength than the regular exercise group. Conclusion : Both exercise programs considerably improved muscle strength, bone mineral density, reduced pain and IGF-1 in blood in elderly women with knee Osteoarthritis. The knee joint taping exercise is perhaps a better exercise to improve muscle strength than the regular exercise in treating elderly women with knee Osteoarthritis

An Integrated and Complementary Evaluation System for Judging the Severity of Knee Osteoarthritis Using CNN (CNN 기반 슬관절 골관절염 중증도 판단을 위한 통합 보완된 등급 판정 시스템)

  • Knee osteoarthritis (OA) is a very common musculoskeletal disorder worldwide. The assessment of knee osteoarthritis, which requires a rapid and accurate initial diagnosis, is determined to be different depending on the currently dispersed classification system, and each classification system has different criteria. Also, because the medical staff directly sees and reads the X-ray pictures, it depends on the subjective opinion of the medical staff, and it takes time to establish an accurate diagnosis and a clear treatment plan. Therefore, in this study, we designed the stenosis length measurement algorithm and Osteophyte detection and length measurement algorithm, which are the criteria for determining the knee osteoarthritis grade, separately using CNN, which is a deep learning technique. In addition, we would like to create a grading system that integrates and complements the existing classification system and show results that match the judgments of actual medical staff. Based on publicly available OAI (Osteoarthritis Initiative) data, a total of 9,786 knee osteoarthritis data were used in this study, eventually achieving an Accuracy of 69.8% and an F1 score of 76.65%.
