Long-term Retrospective Clinical Study Comparing Submerged Type with External Hex Connection and Non-submerged Type with Internal Morse Taper Connection Implants

  • Purpose: This study was aimed to compare the survival and success rates, and long-term crestal bone loss according to the use of 2 connection types of dental implants (submerged-USII and non-submerged-SSII; Osstem $Implant^{(R)}$) by analyzing the change in alveolar bone height after 1 year under load and during final follow-up period. Materials and Methods: Between December 2004 and August 2008, patients with two types of Osstem implants (USII and SSII) were retrieved retrospectively. A total of 92 patients with 284 implants (USII=60, SSII=224) was finally selected. Their mean follow-up period was 7.5 years. The mesial and distal alveolar crestal bone changes were measured using radiographic images and the average was calculated at 1 year after loading and during final follow-up period. Result: Among the 284 implants, 4 USII and 7 SSII implants were removed, indicating 93.3% and 96.9% survival rates. Of the survived implants, mean crestal bone loss 1 year after loading was 0.39 mm for USII and 0.19 mm for SSII (P=0.018). During the final follow-up, mean crestal bone loss was 0.63 mm and 0.35 mm for USII and SSII, respectively, without statistical significance (P=0.092). According to the criteria for the success and failure of the implant by Albreksson and colleagues, final success rate was estimated as 86.7% for USII and 91.5% for SSII, respectively. Conclusion At 1 year after loading, the average crestal bone loss was significantly different between USII and SSII; however, both types met the criteria for implant success. During the final follow-up, both groups showed insignificant bone resorption patterns and did not show any pathological clinical symptoms. Therefore, both implants exhibited high long-term stability.

  • Purpose: This study was performed to compare and evaluate the effect of recipient site depths and diameters of the drills on the primary stability of implant in pig's ribs. Materials and methods: An intact pig's rib larger than 8 mm in width and 20 mm in height; RBM(resorbable blasting media) surface blasted ${\phi}3.75mm$ and 8.0 mm long USII Osstem Implants (Osstem Co., Korea) were used. To measure the primary stability, $Periotest^{(R)}$ (Simens AG, Germany) and $Osstell^{TM}$ (Model 6 Resonance Frequency Analyser: Integration Diagnostics Ltd., Sweden) were used. They were divided into 6 groups according to its recipient site formation method: D3H3, D3H5, D3H7, D3.3H3, D3.3H5, D3.3H7. Each group had, as indicated, 10 implants placed, and total 60 implants were used. The mean value was obtained by 4-time measurements each on mesial, distal, buccal, and lingual side perpendicular to the long axis of the implant using $Periotest^{(R)}$ and $Osstell^{TM}$. For statistical analysis one-way ANOVA was used to compare the mean value of each group, and the correlation between placement depths and the primary stability, and that of measuring instruments was analyzed using SPSS 12.0. Results: The primary stability of the implants increased as the placement depths increased (p<0.05), and showed a proportional relationship (p<0.01). The primary stability increased when the diameter of the recipient site was smaller than that of the implant but with no statistical significance. There was a strong correlation between $Osstell^{TM}$ and $Periotest^{(R)}$ (p<0.01). Conclusion: These results suggest that increasing the placement depth of implants enhances the primary stability of implant.

Evaluation of the correlation between insertion torque and primary stability of dental implants using a block bone test

  • Purpose: Implant stability at the time of surgery is crucial for the long-term success of dental implants. Primary stability is considered of paramount importance to achieve osseointegration. The purpose of the present study was to investigate the correlation between the insertion torque and primary stability of dental implants using artificial bone blocks with different bone densities and compositions to mimic different circumstances that are encountered in routine daily clinical settings. Methods: In order to validate the objectives, various sized holes were made in bone blocks with different bone densities (#10, #20, #30, #40, and #50) using a surgical drill and insertion torque together with implant stability quotient (ISQ) values that were measured using the Osstell Mentor. The experimental groups under evaluation were subdivided into 5 subgroups according to the circumstances. Results: In group 1, the mean insertion torque and ISQ values increased as the density of the bone blocks increased. For group 2, the mean insertion torque values decreased as the final drill size expanded, but this was not the case for the ISQ values. The mean insertion torque values in group 3 increased with the thickness of the cortical bone, and the same was true for the ISQ values. For group 4, the mean insertion torque values increased as the cancellous bone density increased, but the correlation with the ISQ values was weak. Finally, in group 5, the mean insertion torque decreased as the final drill size increased, but the correlation with the ISQ value was weak. Conclusions: Within the limitations of the study, it was concluded that primary stability does not simply depend on the insertion torque, but also on the bone quality.

RBM 표면 테이퍼형태 임프란트의 단기간 후향적 임상 평가 (Short-Term Retrospective Clinical Study of Resorbable Blasting Media Surface Tapered Implants)

  • Purpose: The aim of the present study was to evaluate the clinical outcome of resorbable blasting media surface tapered implant. Methods: 169 Osstem$^{(R)}$ GS III dental implants in 73 patients who received implant treatments at Seoul National University Bundang Hospital, were included in this study. The incidence of biological and prosthetical complications has been carefully analysed for each implant. Results: The short-term implant survival rate was 97.63%, success rate 94.7%. The prevalence of biological complications was 15.38% and the prevalence of prosthetic complications was 13.04%. The mean value of crestal bone loss was $0.28{\pm}0.57$ mm. The relationship between loading periods and marginal bone loss was small and not statistically significant. In mandible, marginal bone loss was larger than in maxilla, no statistically significant. Also, length and diameter of implant had no relationship with marginal bone loss. Conclusion: We suggest that this implant system could achieve successful and stable results.

수산화인회석 코팅 임프란트의 골치유과정에 관한 실험적 연구 (Experimental study about the bony healing of hydroxyapatite coating implants)

  • Introduction: Hydroxyapatite(HA) coating implant can accelerate osseointegration, however, there are many controversies. Materials and Methods: This study examined the early osseointegration of two types of hydroxyapatite coated implants. Twelve adult male miniature pigs (Medi Kinetics Micropigs, Medi Kinetics Co., Ltd., Busan, Korea) were used in this study. In the implants placed in the mandible, a histomorphometric evaluation was performed to evaluate the bone-implant contact (BIC) ratio. Results: The BIC ratio increased with time. TS III HA and Zimmer HA were not significantly different (P>0.05). At 8 weeks, the BIC of Zimmer HA was higher than TS III HA, but there was no significant difference (P>0.05). Conclusion: HA coated implants will accelerate early osseointegration.

Comparative Biomechanical Study of Self-tapping and Non Self-tapping Tapered Dental Implants in Artificially Simulated Quality 2 Bone

  • Purpose: Modifications of implant design have been related to improving initial stability. The purpose of this study was to investigate their respective effect on initial stability between two tapered implant systems (self-tapping vs. non-self-tapping) in medium density bone using three different analytic methods. Materials and Methods: Self-tapping implant (GS III$^{(R)}$; Osstem Implant Co., Busan, Korea) and non-self-tapping implant (Replace Select$^{(R)}$; Nobel Biocare, G$\H{o}$teborg, Sweden) were investigated. In Solid rigid polyurethane blocks of artificially simulated Quality 2 bone, each of the 5 implants was inserted according to the manufacturer's instructions for medium-bone drilling protocol. Evaluation of initial stability was carried out by recording the maximum insertion torque (IT) and performing the resonance frequency analysis (RFA), and the pull-out test. Results: The IT and RFA values of self-tapping implant were significantly higher than those of non self-tapping implant (P=.009 and P=.047, respectively). In the pull-out values, no significant differences were found in implants between two groups (P=.117). Within each implant system, no statistically significant correlation was found among three different outcome variables. Conclusions: These findings suggest that design characteristics of implant geometry significantly influence the initial stability in medium bone density.

Osstem USII plus 임플란트의 단기간 성공률 및 변연골 흡수량 평가 (Success rate and marginal bone loss of Osstem USII plus implants; Short term clinical study)

  • 연구 목적: 임플란트 경부까지RBM 표면 처리를 한 Osstem$^{(R)}$ USII plus system (Osstem Co., Seoul, Korea)의 임상적 유용성을 평가하기 위하여 연세대학교 치과대학 병원에 내원한 환자 중 이 system을 이용하여 치료 받은 32명의 환자에게 식립된 88개의 임플란트를 대상으로 기능 부하 후 1년까지의 임상적, 방사선학적 결과를 후향적으로 분석하였다. 연구 재료 및 방법: 진료 기록부를 통해 전신 조건, 성별, 연령, 식립 위치, 식립 된 임플란트의 직경 및 길이 등을 조사하여 그에 따른 분포 및 생존율의 차이와 함께 이들 항목이 변연골 흡수량에 영향을 미치는지 조사하였다. 방사선사진 분석으로 임플란트 식립 시, 보철물 장착 후 6개월 내지 1년간의 변연골 흡수량을 측정하여 분석하였다. 각 항목과의 연관성 분석을 위하여 독립 t-검정(independent sample t-test)을 이용하였으며(${\alpha}$=.05), 보철물 장착 전과 보철물 장착 후 12개월 동안의 변연골 흡수량 비교를 위하여 일원 반복측정 분산 분석(one-way repeated-measures ANOVA)을 이용하였다(${\alpha}$=.05). 결과:총 32명에게 식립된 88개의 임플란트 중에서 실패한 것은 없었으며 누적 생존율은 100%로 나타났다. 임플란트 식립 시부터 보철물 장착까지의 변연골 흡수량은 0.24 mm이었으며, 보철물 장착 후 12개월까지의 변연골 흡수량은 평균 0.19 mm로 임플란트 식립 시부터 보철물 장착 12개월까지의 총 변연골 흡수량은 0.43 mm였다. 상악에 식립된 경우와 하악에 식립된 경우의 변연골 흡수량을 비교했을 때는 통계적으로 유의한 차이를 보이지 않았으나 구치부에 식립된 경우가 전치부에 식립된 경우에 비해 변연골 흡수량이 크게 나타났으며 이는 통계적으로 유의한 차이가 있었다. 결론: 이상의 결과를 토대로 보철물 장착 12개월까지의 기간 동안 임플란트 고정체의 경부까지 RBM 표면 처리를 한 외측 연결형의 국산 임플란트의 단기간 임상적인 성공률은 만족스러운 결과를 보였으며 변연골 흡수량도 임플란트 성공 기준에 부합하였다.

반복하중에 따른 수종 임플란트의 피로파절에 관한 연구 (Fatigue fracture of different dental implant system under cyclic loading)

  • 연구목적: 임플란트는 수직교합 하중에는 비교적 잘 견디나 측방하중에 대해서는 약한 역학적 성질을 갖고 있으므로 임플란트의 재료 특성과 기하학적 형태에 따른 응력 분석 연구의 필요성이 제기되고 있다. 연구재료 및 방법: 외부육각구조를 갖는 28개의 임플란트를 7개씩 4군으로 나누어 그 제품에 적합한 UCLA gold abutment를 이용해, 제3형 금합금으로 보철 물을 제작하였고, A군 (3i, FULL $OSSEOTITE^{(R)}$-Implant), B군 (Nobelbiocare, Branemark $System^{(R)}$Mk III Groovy RP), C군 (Neobiotec, $SinusQuick^{(TM)}$ EB), D군 (Osstem, US-II)으로 분류하였다. 고정체와 지대주나사, 지대주를 연결한 후 수직적으로 절단하여 연마한 후 미세경도계를 이용하여 10군데에서 경도측정을 실시하였고, 동적하중 피로시험기를 이용하여 60-600 N범위로 파절시까지 동적 하중을 가하였다. 주사전자현미경을 이용하여 지대주나사 및 고정체의 파절 양상과 파절 위치 등을 관찰하였고, 유한요소분석을 통해 고정체와 지대주 나사에 나타나는 응력 분포와 파절면을 비교 분석하였다. 결과: 1.고정체 경도는 A, B, C, D군에서 각각 245.3, 289.7, 281.3, 300.4 Hv로 D군이 가장 높았고, A군이 가장 낮았다. 지대주 나사의 경도는 A, B, C, D군에서 각각 340.00, 317.62, 306.5, 306.2 Hv로 A군이 가장 높고, D군이 가장 낮았다. 2. 모든 실험군에서 임플란트 고정체의 파절은 응력이 집중되는 고정체 3-4번째 나사산 홈 (valley) 부위 또는 내면의 사공간부와 일치하는 부위에서 발생되었고, 피로수명은 A, B, C, D군에서 각각 31585, 47311, 30141, 105371로 D군이 가장 높았으며, A, B, C군과는 유의한 차이가 있었다(P<.05). 3. 파절양상은 B군과D군에서는 고정체와 나사 모두에서 수직 (longitudinal)파절과 수평 (transverse)파절이 동시에 일어나는 복합 (complex mode) 파절이 관찰 되었고, A와 C군에서는 고정체에서 수평 (transverse mode) 파절만이 관찰되었다. 4. 유한요소분석 결과 인장응력이 가장 높은 고정체 표면부에서 피로 균열이 시발되어 압축응력이 가장 높은 반대편 부위로 피로균열이 전파되었으며, 최대 유효 응력값은 C군이 가장 높았고, B군에서 가장 낮았다. 결론: 피질골 높이와 일치하는 임플란트 고정체 부위에서 최대 인장 주응력이 발생되며, 고정체 사공간부 (dead space)가 최대 인장 주응력이 작용하는 지그 표면과 일치할 때 피로파절이 발생되었다. 따라서 악골에 식립된 임플란트의 신뢰성을 향상시키고 수명을 증대시키기 위해서는 가능한 임플란트 주위의 골소실이 일어나지 않도록 해야 할 것이나 골흡수가 일어나 사공간부 수준까지 진행된다면 임플란트의 파절 빈도가 증가될 수 있으므로 이에 대한 대처가 필요할 것으로 사료된다.

임프란트 식립시 초기 안정성이 골유착에 미치는 영향 (THE INFLUENCE OF THE INITIAL STABILITY AFTER DENTAL IMPLANT INSTALLATION ON THE OSSEOINTEGRATION)

  • Purpose: The long-term experience of using osseointegrated implants for prosthetic rehabilitation of the edentulous patients shows that high success rates can be predictably achieved. Primary implant stability has been identified to be a prerequisite to achieve osseointegration. In this study, we set up the amount of removed bone so that it differed on implant installation site for each group. The influence of each initial stability on secondary stability and osseointegration was compared with time lapse using resonance frequency analysis and histomorphometric analysis. Materials and methods: A total 27 US $II^{(R)}$ (Osstem, Korea) implants were placed in the mandibular edentulous area of 3 beagle dogs. The implant site was prepared by the conventional technique with drills, and three experimental groups were divided into under-drilling group, normal-drilling group and over-drilling group. The Implant Stability Quotient (ISQ) was measured at intervals of immediately, 4, 8, 12 weeks after placement using $Osstell^{(R)}$ mentor RFA. After the animals were sacrificed, histomorphometric evaluation was executed for measuring BIC and BD. Results: 1) The under-drilled group showed most high ISQ value for whole experiment period. 2) Bone-to-implant-contact(BIC) showed the tendency to be increased gradually as the experiment period passed except the 8 weeks of the normal group. 3) The under-drilled group showed most high bone density(BD) level for whole experiment period, and it was expressed the aspect to be increased gradually according to an experiment period passage in the average of all group. 4) Resonance frequency analysis and histomorphometric analysis are presumed by generally proportional. Conclusions: As this research result, it seems that there are some correlation between resonance frequency analysis and histomorphometric analysis. As are accomplished osseointegration stably so that more superior at the region which the overpressure comes to add, it will be applicable method in clinical field.

$Smartpeg^{TM}$의 고압멸균소독 및 재사용이 임플랜트 안정성 지수(ISQ) 측정에 미치는 영향 (THE EFFECT OF AUTOCLAVE STERILIZATION AND REUSE OF $SMARTPEG^{TM}$ ON THE IMPLANT STABILITY QUOTIENT (ISQ) MEASUREMENT)

  • Statement of problem: Resonance frequency analysis is evaluated as the non-invasive and objective method for the evaluation of implant stability and has been increasingly used. It is necessary to evaluate the factors affect the ISQ measurement stability. Purpose: This study was performed to evaluate the effect of the autoclave sterilization and reuse of $Smartpeg^{TM}$ on ISQ measurement. Material and methods: $SmartPeg^{TM}$ (Integration Diagnostics Ltd., $G\ddot{o}teborg$, Sweden) of autoclave group (A) was autoclave sterilized 9 times and $Smartpeg^{TM}$ of reuse group (R) was reused 9 times. Ten $SmartPeg^{TM}s$ were allocated to each group and after each autoclave sterilization and reuse, implant stability quotient (ISQ) values were measured 3 times from the two directions a and b at a right angle. $Osstell^{TM}$ mentor (Integration Diagnostics Ltd. $G\ddot{o}teborg$, Sweden) was used and type 1 (article no. 100353) $Smartpeg^{TM}$ was selected according to $Smartpeg^{TM}$ reference list. Osstem Implant US II future (Osstem Co., Seoul, Korea) in $4.0mm{\times}11.5mm$ was embedded in the self-curing acrylic resin ($Orthojet^{(R)}$, Lang Dental, U.S.A.). Data was statistically analyzed by one-way ANOVA $({\alpha}=.05)$ and scheffe test was done where a significant difference exist. Correlation test was also done between ISQ value and the number of autoclave sterilization or reuse. Results: 1. In autoclave group, the means and sd. of ISQ value before autoclave sterilization were $84.97{\pm}0.41,\;84.93{\pm}0.74$ at direction a and b. There was significant differences between autoclave groups at direction a and b (P=.000). 2. In reuse group, the means and sd. of ISQ value before reuse were $85.40{\pm}0.62,\;85.50{\pm}0.57$ at direction a and b. There was no significant difference between reuse groups at direction a and b (P>.05). 3. There was a weak positive correlation between the number of reuse and ISQ value at direction a and b (${\gamma}=.207$ and .246, P<.01). Conclusion: Within the limitations of this study, the following conclusions were drawn. Till ninth reuse of $Smartpeg^{TM}$, the ISQ measurement stability did not be affected. After twice autoclave sterilization of $Smartpeg^{TM}$ the ISQ measurement stability was affected.
