A STUDY HO THE PREDICTION OF DURATION OF ORTHODONTIC TREATMENT IN MALOCCLUSION (부정교합 치료기간의 예측에 관한 연구)

  • To predict duration of orthodontic treatment of malocclusion, 304 male and female patients whose orthodontic treatment was finished in department of orthodontics, Infirmary of dental college, Chosun University were studied. The duration of treatment was studied according to types of malocclusion, beginning ages of treatment, Hellman's dental ages, the determination of tooth extraction, types of tooth extraction and types of orthodontic appliance. The following results were obtained. 1. The duration of treatment was 18 months in Class I malocclusion, 20 months in Class II malocclusion and 24 months in Class III malocclusion. 2. The more early treatment was done, the more duration of treatment was needed according to beginning age of treatment and Hellman's dental ages. 3. Treatment of tooth extraction case was needed for 78 days more than that of non-extraction case. 4. The duration of treatment with unilateral extraction of premolars was the shortest (598 days) and that with single arch extraction of premolars was the longest (685 days) according to types of tooth extraction. 5. The duration of treatment by removable appliance was the shortest (237 days) and the combination of removable appliance, headgear and full banded appliance was the longest (1425 days) according to types of orthodontic appliance. 6. The duration of treatment was 18 months in typical orthodontic treatment with 4 extraction of premolar and full banded appliance.

  • Traditionally, goals of orthodontic treatment fall into four categories: good tooth alignment, improvement of skeletal profile and good soft tissue, and rehabilitation of the normal function. Electromyography (EMG) has proven as useful tool in evaluating masticatory function among the maxillomandibular relationship and could be useful in accompanying orthodontic treatment objectives. But in pursuit of clinical application, it is necceassry to systemize clinical guideline for diagnosis by EMG records in evaluating the effects of orthodontic, orthopedic, or surgical treatment. This study present 3 orthodontic cases, which treatment results, ie neuromuscular balances of masticatory muscle, could be moitered by EMG procedure.

Factors of Influencing Satisfaction with Orthodontic Treatment in Orthodontic Patients (치열교정치료시 교정환자의 만족도에 영향을 미치는 요인)

  • The purpose of this study was to research into the satisfaction with orthodontic treatment and the psychologically satisfactory condition to be expected, and to offer information in order to enhance medical-treatment service, by surveying the influential factors upon satisfaction with treatment targeting patients who receive orthodontic treatment. Targeting 382 patients who are under treatment at S dental clinic in Gyeonggi Province, B dental clinic in Daegu Metropolitan City, and Y dental clinic in Busan Metropolitan City, which specialize in orthodontic treatment, it carried out the direct interview survey of using the structured questionnaire from January 20, 2009 to February 28. The collected data was analyzed by using SPSSWIN 17.0 K. The following conclusions were obtained. 1. In the satisfaction with orthodontic treatment according to aesthetic factor, the group with bad aesthetic health condition was higher than the group with good health condition(p<0.05). As for the factor of tooth alignment, a case of good health condition was indicated to have higher(p<0.05) satisfaction than a case of being bad. 2. As for satisfaction according to necessity for orthodontic treatment, the satisfaction was high in a case of recognizing necessity much and of understanding medical-treatment contents much, and in a case of being well adjusted to the progression of orthodontic treatment. As for a psychological change after orthodontic treatment, the satisfaction was high in a case with a positive change(p<0.001). 3. The factors of influencing satisfaction with orthodontic treatment had significant influence in order of necessity for orthodontic treatment(p<0.001), adjustment to progression of orthodontic treatment(p<0.001), psychological change after orthodontic treatment(p<0.01), expectation level of dental health after orthodontic treatment(p<0.01), and understanding about treatment contents(p<0.01). As a result of research, the satisfaction with orthodontic treatment in patients with orthodontics is indicated to be relatively high. Thus, the careful consideration is considered to be probably necessary on qualitative part in medical treatment and on patients' expectation by grasping patents' mentally psychological condition.

A STUDY ON THE LEVEL OF THE SALIVARY ORAL MICROORGANISMS IN FIXED ORTHODONTIC PATIENT (고정성 교정장치 장착 환자의 타액내 구강미생물군의 수준에 관한 연구)

  • This study was done to evaluate the effect of fixed orthodontic patients on the level of oral streptococci, Streptococcus mutans, lactobacilli, yeasts in saliva. 35 patients wearing bands were compared with age-matched 35 non-banded control group by conlony counting method on the specially designed culture medium. The following results were obtained ; 1. The colony forming unit(CFU) of total streptocci per militer of saliva in subjects with or without orthodontic treatment showed no significant statistical difference between them(p>0.05). 2. The colony forming unit(CFU) of total Streptococcus mutans per mililiter of saliva in subjects with orthodontic treatment showed significantly higher than those without orthodontic treatment(p<0.05). 3. The colony forming unit(CFU) of total lactobacilli per mililiter of saliva in sujects with or without orthodontic treatment showed no significant statistical difference between them but higher tendency in those with orthodontic treatment(p=0.052). 4. The colony forming unit(CFU) of total yeasts per mililiter of saliva in subjects with or without orthodontic treatment showed no significant statistical difference between them(p>0.05).

Alveolar ridge expansion-assisted orthodontic space closure in the mandibular posterior region

  • Orthodontic closure of old, edentulous spaces in the mandibular posterior region is a major challenge. In this report, we describe a method of orthodontic closure of edentulous spaces in the mandibular posterior region accelerated by piezoelectric decortication and alveolar ridge expansion. Combined piezosurgical and orthodontic treatments were used to close 14- and 15-mm-wide spaces in the mandibular left and right posterior areas, respectively, of a female patient, aged 18 years and 9 months, diagnosed with skeletal Class III malocclusion, hypodontia, and polydiastemas. After the piezoelectric decortication, segmental and full-arch mechanics were applied in the orthodontic phase. Despite some extent of root resorption and anchorage loss, the edentulous spaces were closed, and adequate function and esthetics were regained without further restorative treatment. Alveolar ridge expansion-assisted orthodontic space closure seems to be an effective and relatively less-invasive treatment alternative for edentulous spaces in the mandibular posterior region.

STUDY OF RECOGNITION OF MALOCCLUSION AND ORTHODONTIC TREATMENTS (부정교합과 교정치료에 대한 인식변화에 관한 연구)

  • In this study, in order to survey the effect on public recognition in malocclusion and orthodontic treatment by brochures made by Korean orthodontic association, sample were divided into control and experimental group and a questionnaire was inguired and the results were as follows: 1. In the experimental group, frequency of recognition for the need of orthodontic treatment was high, and this indicates dental health behavior and recognition can be inspired by adequate giving of information. 2. Between the control and the experimental group, patterns of the negative opinion was different, and especially the demand for specific orthodontic treatment can be influenced by the content of brochures. 3. The brochures used in this study can call attention to develope the orthodontic aspect for the malocclusion, hereafter more systemic and active measures for development of public dental behavior and recognition should be needed.

THE EFFECTS OF OCCLUSION ON THE STABILITY AFTER ORTHODONTIC TREATMENT (교정치료후 안정성에 미치는 교합의 영향)

  • We tend to consider only static occlusion such as molar relationship, canine key, and interdigitation at finishing stage. Of course, this static occlusion is important for post-orthodontic stability. But we should remember that mandible is always on the move during its various functions. If no pressure or too much pressure is put on during its functions, untoward tooth movement could occur. And tooth mobility, periodontitis, wear facet, bruxism, and far worse temporomandibular disorder could occur. After many studies have been done on what is a desirable occlusal scheme to strengthen post-orthodontic stability, today, "mutually protective occlusion" is recommended. If an orthodontist does not have understanding about this occlusal scheme during orthodontic treatment, the following conditions will be resulted after orthodontic treatment. I. Centric discrepancy 1. centric prematurity 2. sunday bite 3. molar fulcrum II. Eccentric discrepancy 1. posterior interference 2. anterior interference If we have deep understanding about these discrepancies that can happen after orthodontic treatment and their causes, corrections, and especially preventions against them, post-orthodontic stability could be strengthened and further temporomandibular disorder could be prevented.

State of Orthodontic Treatment and Facial Plastic Surgery among Adult Patients (일부 성인의 교정치료 및 안면 성형 수술에 대한 실태)

  • This study was designed to investigate the status of orthodontic treatment and facial plastic surgery among 389 patients who visited dental clinics located in Seoul and Gyeonggi Province. After the collection of data, an analysis was carried out using a spss statistical program. The analysis results are as follows. the level of satisfaction with orthodontic treatment from patients with experience of the orthodontic treatment turned out to be high(p<0.01). Women considered 3.01 to 4 million won to be reasonable cost for orthodontic treatment (p<0.05), which is higher cost recognized by men, and patients with experience of orthodontic treatment recognized 19 to 24 months as the proper orthodontic treatment period(p<0.01). In addition, women showed higher figures than men in their awareness on the need for orthodontic treatment checkups(p<0.001), and facial plastic surgery experience (p<0.01). With respect to the part of the face on which they hoped to have plastic surgery, eye plastic surgery was higher in women (p<0.01), and nose surgery turned out to be higher in patients with experience of orthodontic treatment(p<0.05). Meanwhile, women considered the eye as the most important part among other facial areas (p<0.001), and they recognized that dentist should perform plastic surgery on both jaws among facial areas(p<0.01). In conclusion, the dental medical staff should provide consultations and operate treatment systems in consideration of gender differences and needs of dental patients. In the case of orthodontic treatment for the improvement of facial appearance, especially double jaw surgery which is closely related to the oral function, professional dental medical staff needs to perform the operation, considering the special oral environment. In addition, it is determined that provision of relevant medical information and promotion of dental service is required to provide patients with correct knowledge about orthodontic treatment and facial plastic surgery.

Investigation of the association between orthodontic treatment and temporomandibular joint pain and dysfunction in the South Korean population

  • Objective: This study investigated the relationship between orthodontic treatment and temporomandibular disorders (TMD) in South Korean population. Methods: This study obtained data from the 2012 Korean National Health and Nutrition Examination Survey. The final sample size was 5,567 participants who were ${\geq}19$ years of age. Logistic regression analysis was performed to evaluate the relationship between orthodontic treatment and TMD. Results: Participants who underwent orthodontic treatment showed higher educational level, lower body mass index, reduced chewing difficulty, and reduced speaking difficulty. The adjusted odds ratios (ORs) and their 95% confidence intervals (CIs) for orthodontic treatment and TMD were 1.614 (1.189-2.190), 1.573 (1.162-2.129) and 1.612 (1.182-2.196) after adjusting for age, sex and psychosocial factors. Adjusted ORs and their 95% CIs for orthodontic treatment and clicking were 1.778 (1.289-2.454), 1.742 (1.265-2.400) and 1.770 (1.280-2.449) after adjusting for confounding factors. However, temporomandibular joint pain and functional impairment was not associated with orthodontic treatment. Conclusions: Temporomandibular joint pain and dysfunction was not associated with orthodontic treatment.

The comparison of the appearance perception, satisfaction, and self-esteem following the adolescent orthodontic treatment demand (청소년의 교정치료 희망여부에 따른 외모인식도와 외모 만족도 및 자아존중감 비교)

  • Objectives : The purpose of the study is to investigate the relationship between appearance perception, appearance satisfaction, and self-esteem in adolescents who demand the orthodontic treatment. Methods : A total sample of 403 high school students filled out the questionnaire and data were analyzed by using SPSS ver 20.0. for n (%), chi-square test, t-test, and a logistic regression. Results : Girl students (68.3%) tended to have a higher demand for orthodontic treatment than male students (31.7%). Higher appearance perception(t=6.967, p<0.001), lower appearance satisfaction(t=-5.477, p<0.001) and lower self-esteem(t=-7.333, p<0.001) were higher in group of demand for orthodontic treatment. The logistic regression analysis revealed that a gender, self-perception of their teeth, appearance perception, and self-esteem were significantly associated with orthodontic treatment demand. Conclusions : The result of this study will provide basic data for the consideration of psychosocial factors in orthodontic treatment in adolescents.
